207. Ivacaftor for the treatment of patients with cystic fibrosis and the G551D mutation: A systematic review and cost-effectiveness analysis

Abstract

Background: Ivacaftor (Kalydeco®, Vertex Pharmaceuticals) is the first of a new class of drugs that target the underlying protein defect in cystic fibrosis (CF). It is aimed at patients with the G551D (glycine to aspartate change in nucleotide 1784 in exon 11) mutation; 5.7% of patients with CF in the UK have this mutation. Objectives: To review the clinical effectiveness and cost-effectiveness of ivacaftor for the treatment of CF in patients aged ≥ 6 years who have the G551D mutation. Methods: Ten databases, including MEDLINE and EMBASE, were searched from inception to July 2012. Studies that evaluated ivacaftor for the treatment of adults and children (≥ 6 years) with at least one G551D mutation were eligible. There were insufficient data to conduct a formal meta-analysis. The manufacturer of ivacaftor, Vertex Pharmaceuticals, submitted a deterministic patient-level simulation model for the assessment of the lifetime cost-effectiveness of ivacaftor. We modified the model where values were not UK-specific or not recent, or where better estimates could be found. The only change to the model structure was the addition of lung transplantations. We changed utility values, annual decline in percentage predicted forced expiratory volume in 1 second (FEV1), and the baseline exacerbation rate, and used data from the CF Registry to estimate the relation between costs, age and percentage predicted FEV1. Estimates of treatment effect of ivacaftor came from the clinical effectiveness review. We modelled three scenarios for the longer-term effects of ivacaftor. We also modelled an 'optimistic' scenario for patients aged < 12 years with little lung damage. We conducted a budget impact analysis to estimate the total cost to the NHS of introducing ivacaftor in England. A reusable agent-based generic model is presented for a specific class of Ambient Intelligence pplications: those cases addressing human wellbeing and functioning from a human-like understanding. The model incorporates ontologies, knowledge and dynamic models from human-directed sciences such as psychology, social science, neuroscience and biomedical sciences. The model has been formally specified, and it is shown how for specific applications it can be instantiated by applicationspecific elements, thus providing an executable specification that can be used for prototyping. Moreover, it is shown how dynamic properties can be formally specified and verified against generated traces.

Abstract

Task performance of humans that act under demanding circumstances may vary over time, depending on the characteristics of human, task and environment. To increase the effectiveness and efficiency of task performance, personalised assistance may be provided, in the form of automated personal assistant agents that constantly monitor the task execution and well-being of the human, and intervene when a problem is detected. This paper proposes a generic design for a multi-agent system architecture including such personal assistant agents, which can be deployed in a variety of domains.

Abstract

To share resources in open distributed systems we have developed autonomous mobile programs, which periodically use a cost model to decide where to execute in a network. In addition, self-aware mobile coordination for common patterns of computation over collections are encapsulated by autonomous mobility skeletons. Performance can be improved if an autonomous mobile program can predict the cost of the entire program rather than a single iteration. We propose a cost calculus that predicts the costs for the remainder of a computation at arbitrary program points. We extend our autonomous mobility skeleton cost models to be parametrised on the cost of the remainder of the program, and propose costed autonomous mobility skeletons. An automatic cost analyser which implements the calculus has been constructed to generate Jocaml programs with higher-order functions replaced by costed autonomous mobility skeletons.

Abstract

Allogeneic stem cell transplantation (SCT) following reduced-intensity conditioning (RIC) as treatment modality has curative potential in patients suffering from chronic lymphocytic leukemia (CLL) or mantle cell lymphoma (MCL), illustrating susceptibility of these leukemic cells for the graft-versus-leukemia (GvL) effect. However, effectiveness of this therapy is limited due to low immunogenicity of leukemic cells and the lack of specificity resulting in concurrent development of graft-versus-host-disease. Both CLL and MCL cells lack expression of costimulatory molecules necessary for an efficient immune response and thus may escape from T-cell mediated reactivity. To improve the specificity of the immune response, primary CLL or MCL cells were successfully modified into antigen-presenting cells (APC) by CD40 stimulation in the presence of cytokines. In contrast to primary malignant cells as stimulator cells, these malignant APC were capable of inducing the generation of CLL- or MCL-reactive cytotoxic T cell (CTL) clones from HLA-identical donors. These CTL clones effectively recognized and killed the primary leukemia and other patient-derived targets but not donor-derived targets indicating that these clones were minor histocompatibility antigen-specific. These results allow the development of new cellular immunotherapeutic interventions to further exploit the GvL effect following allogeneic SCT.

Abstract

The management of naval organizations aims at the maximization of mission success by means of monitoring, planning and strategic reasoning. This paper presents an agent-based meta-level architecture for the improvement of automated strategic reasoning in naval planning. The architecture is instantiated with decision knowledge acquired from naval domain experts and is formed into an executable agent-based model, which is used to perform a number of simulation runs. To evaluate the simulation results, relevant properties for the planning decision are identified and formalized. These important properties are validated for the simulation traces.
