Background: Smoking is a strong environmental factor leading to adverse outcomes in Crohn's disease, but a more benign course in ulcerative colitis. Several single nucleotide polymorphisms (SNPs) are associated with smoking quantity and behaviour.Aim: To assess whether smoking-associated SNPs interact with smoking to influence the clinical course of inflammatory bowel diseases.Methods: Genetic and prospectively obtained clinical data from 1434 Swiss inflammatory bowel disease cohort patients (821 Crohn's disease and 613 ulcerative colitis) were analysed. Six SNPs associated with smoking quantity and behaviour (rs588765, rs1051730, rs1329650, rs4105144, rs6474412 and rs3733829) were combined to form a risk score (range: 0-12) by adding the number of risk alleles. We calculated multivariate models for smoking, risk of surgery, fistula, Crohn's disease location and ulcerative colitis disease extent.Results: In Crohn's disease patients who smoke, the number of surgeries was associated with the genetic risk score. This translates to a predicted 3.5-fold (95% confidence interval: 2.4- to 5.7-fold, P7 had a significantly shorter time to first intestinal surgery. The genetic risk score did not predict surgery in ulcerative colitis or occurrence of fistulae in Crohn's disease. SNP rs6265 was associated with ileal disease in Crohn's disease (PPConclusions: SNPs associated with smoking quantity is associated with an increased risk for surgery in Crohn's disease patients who smoke. Our data provide an example of genetics interacting with the environment to influence the disease course of inflammatory bowel disease.

Objective: The objective was to describe the methodology used to develop new response criteria for adult DM/PM and JDM.Methods: Patient profiles from prospective natural history data and clinical trials were rated by myositis specialists to develop consensus gold-standard ratings of minimal, moderate and major improvement. Experts completed a survey regarding clinically meaningful improvement in the core set measures (CSM) and a conjoint-analysis survey (using 1000Minds software) to derive relative weights of CSM and candidate definitions. Six types of candidate definitions for response criteria were derived using survey results, logistic regression, conjoint analysis, application of conjoint-analysis weights to CSM and published definitions. Sensitivity, specificity and area under the curve were defined for candidate criteria using consensus patient profile data, and selected definitions were validated using clinical trial data.Results: Myositis specialists defined the degree of clinically meaningful improvement in CSM for minimal, moderate and major improvement. The conjoint-analysis survey established the relative weights of CSM, with muscle strength and Physician Global Activity as most important. Many candidate definitions showed excellent sensitivity, specificity and area under the curve in the consensus profiles. Trial validation showed that a number of candidate criteria differentiated between treatment groups. Top candidate criteria definitions were presented at the consensus conference.Conclusion: Consensus methodology, with definitions tested on patient profiles and validated using clinical trials, led to 18 definitions for adult PM/DM and 14 for JDM as excellent candidates for consideration in the final consensus on new response criteria for myositis.

The serology of the pig has been investigated firstly in terms of specific antigen/antibody reactions and secondly on a more general basis of the protein constituents of porcine serum. The antigen/antibody reactions studied have been those involved in red blood cell typing and serum immunoglobulin allotyping. These two approaches have been correlated by using red cell typing reactions and immuno-diffusion in gel, immuno-electro- phoretic, specific staining, passive haemagglutination, haemagglutination inhibition, gel filtration, ion exchange and starch gel electrophoretic techniques. In order to achieve this correlation, it has been necessary to partially separate and identify a large number of pig serum components on the basis of molecular size, electrical charge and specific staining characteristics. These components not only include those involved in well established systems of pig serum protein biochemical polymorphism such as haemopexin, transferrin, serum amylase, prealbumin, caeruloplasmin and slow oc g-globulin But also additional constituents such as fast and slow lipoprotein, variable?: -globulin, complement components, haptoglobin and the porcine immunoglobulins Ig M, Ig A, Ig G^^ and Ig Gg. A provisional investigation of the porcine A-0 blood group system has also been initiated. More specifically the following conclusions have been established. Antibodies reacting with pig red blood cell typing factors by the direct agglutination, haemolytic and indirect sensitation (Ooombs) test have been demonstrated in the 19 S, 7 S and intermediate fractions on Sephadex G200 fractionation. These reactivities have been partially correlated with porcine Ig M, Ig G and only tentatively with Ig A. The heterogeneity of distribution of these reactivities within the 7 S peak of G200 fractionation suggests the possi?bility of novel sub-classes of Ig G on a molecular size and/or steric hindrance to gel filtration basis. Iso-immune precipitins against pig immunoglobulin allo?types have been produced using a double emulsion (water-oil- water-WOW) type of adjuvant incorporating whole pig serum. The reactions are demonstrated by double diffusion (Ouchterlony) and immuno-electrophoretic procedures in agar gel. The late hyperimmune precipitin has Ig G^ characteristics. Antibodies with two specificities anti factor 1 and anti factor 2 have been investigated. Allotype factor 1 occurs on Ig G molecules with a full range of electrophoretic mobility from to y g* Factor 2 has a more restricted distribution on Ig G-^ molecules. These factors are therefore termed porcine Ig G - allotype factor 1 and porcine Ig G-, - allotype factor 2 or P. Ig G - Al and P. Ig G-j - A2. In the pig A-0 blood type system the failure of neonatal piglets? cells to type as A positive with conventional typing techniques has been shown to be a property of their serum, or iii.of their cells and serum, hut not of their cells alone. It is not caused hy a quantitative lack of A blood group substance in the serum. In adult pigs a provisional investigation has established that all pigs typing as 0 negative by direct test possess an antibody accessible but non-haemolisable cryptantigenic form of 0 on their red cells.

- Abstract
Modern mitochondria have preserved few traits of the original bacterial endosymbiont. Unexpectedly, we find that certain representatives of heteroloboseans, jakobids and malawimonads possess homologues of four core components of the type 2 secretion system (T2SS) so far restricted to eubacteria. We show that these components are localized to the mitochondrion, and their behaviour in functional assays is consistent with the formation of a mitochondrial T2SS-derived protein secretion system. We additionally identified 23 protein families exactly co-occurring in eukaryotes with the T2SS. Seven of these proteins could be directly linked to the core T2SS by functional data and/or sequence features, whereas others may represent different parts of a broader functional pathway, possibly linking the mitochondrion with the peroxisome. Its distribution in eukaryotes and phylogenetic evidence indicate that the whole mitochondrial T2SS-centred pathway is an ancestral eukaryotic trait. Our findings thus have direct implications for the functional properties of the early mitochondrion.

BACKGROUND: For the prevention of attacks of hereditary angioedema (HAE), the efficacy and safety of subcutaneous human C1-esterase inhibitor (C1-INH [SC]; HAEGARDA, CSL Behring) was established in the 16-week Clinical Study for Optimal Management of Preventing Angioedema with Low-Volume Subcutaneous C1-Inhibitor Replacement Therapy (COMPACT). OBJECTIVE: To assess the long-term safety, occurrence of angioedema attacks, and use of rescue medication with C1-INH(SC). METHODS: Open-label, randomized, parallel-arm extension of COMPACT across 11 countries. Patients with frequent angioedema attacks, either study treatment-naive or who had completed COMPACT, were randomly assigned (1:1) to 40 IU/ kg or 60 IU/kg C1-INH(SC) twice per week, with conditional uptitration to optimize prophylaxis (ClinicalTrials.gov registration no. NCT02316353). RESULTS: A total of 126 patients with a monthly attack rate of 4.3 in 3 months before entry in COMPACT were enrolled and treated for a mean of 1.5 years; 44 patients (34.9%) had more than 2 years of exposure. Mean steady-state C1-INH functional activity increased to 66.6% with 60 IU/kg. Incidence of adverse events was low and similar in both dose groups (11.3 and 8.5 events per patient-year for 40 IU/kg and 60 IU/kg, respectively). For 40 IU/kg and 60 IU/kg, median annualized attack rates were 1.3 and 1.0, respectively, and median rescue medication use was 0.2 and 0.0 times per year, respectively. Of 23 patients receiving 60 IU/kg for more than 2 years, 19 (83%) were attack-free during months 25 to 30 of treatment. CONCLUSIONS: In patients with frequent HAE attacks, long-term replacement therapy with C1-INH(SC) is safe and exhibits a substantial and sustained prophylactic effect, with the vast majority of patients becoming free from debilitating disease symptoms. Advanced LIGO's second observing run (O2), conducted from November 30, 2016 to August 25, 2017, combined with Advanced Virgo's first observations in August 2017 witnessed the birth of gravitational-wave multi-messenger astronomy. The first ever gravitational-wave detection from the coalescence of two neutron stars, GW170817, and its gamma-ray counterpart, GRB 170817A, led to an electromagnetic follow-up of the event at an unprecedented scale. Several teams from across the world searched for EM/neutrino counterparts to GW170817, paving the way for the discovery of optical, X-ray, and radio counterparts. In this article, we describe the online identification of gravitational-wave transients and the distribution of gravitational-wave alerts by the LIGO and Virgo collaborations during O2. We also describe the gravitational-wave observables which were sent in the alerts to enable searches for their counterparts. Finally, we give an overview of the online candidate alerts shared with observing partners during O2. Alerts were issued for 14 candidates, six of which have been confirmed as gravitational-wave events associated with the merger of black holes or neutron stars. - Abstract
This research was undertaken by the National Institute of Health Innovation (NIHI), University of Auckland, to provide insight into consumers’ awareness of, and views around, energy labelling of alcoholic beverages. This report summarises the first phase of the twopart research project, namely the qualitative research to determine the influence of energy labelling on the likely purchase and consumption of alcoholic beverages. The second phase relates to the conduct of a four-arm parallel group clinical trial testing the effects of different types of energy labels placed on alcoholic beverages on purchase behaviour. The results of the trial are reported in a separate document.

This research by the National Institute of Health Innovation (NIHI), University of Auckland measures the impact of energy labelling on alcoholic beverages, on consumer purchase or consumption behaviour in a New Zealand population. Energy labelling information on alcoholic beverages is not currently required in New Zealand, unless a nutrient claim is made. The first phase of the project provided qualitative insight into consumers’ awareness of and views around energy labelling of alcoholic beverages (available on the Health Promotion Agency’s website; Walker, McCormack, Jiang, Lang, & Ni Mhurchu, 2018). This report summarises the second phase of the project, namely quantitative research to determine the likely influence of energy labelling on the purchase or consumption of alcoholic beverages.

Background and purpose: Acquired neuromyotonia can occur in patients with thymoma, alone or in association with myasthenia gravis (MG), but the clinical prognostic significance of such comorbidity is largely unknown. The clinico‐pathological features were investigated along with the occurrence of neuromyotonia as predictors of tumour recurrence in patients with thymoma‐associated myasthenia.Methods: A total number of 268 patients with thymomatous MG were studied retrospectively. Patients with symptoms of spontaneous muscle overactivity were selected for autoantibody testing using immunohistology for neuronal cell‐surface proteins and cell‐based assays for contactin‐associated protein 2 (CASPR2), leucine‐rich glioma inactivated 1 (LGI1), glycine receptor and Netrin‐1 receptor antibodies. Neuromyotonia was diagnosed according to the presence of typical electromyography abnormalities and/or autoantibodies against LGI1/CASPR2.Results: Overall, 33/268 (12%) MG patients had a thymoma recurrence. Five/268 (2%) had neuromyotonia, four with typical autoantibodies, including LGI1 (n = 1), CASPR2 (n = 1) or both (n = 2). Three patients had Netrin‐1 receptor antibodies, two with neuromyotonia and concomitant CASPR2+LGI1 antibodies and one with spontaneous muscle overactivity without electromyography evidence of neuromyotonia. Thymoma recurrence was more frequent in those with (4/5, 80%) than in those without (28/263, 10%, P < 0.001) neuromyotonia. Neuromyotonia preceded the recurrence in 4/5 patients. In univariate analysis, predictors of thymoma recurrence were age at thymectomy [odds ratio (OR) 0.95, 95% confidence interval (CI) 0.93–0.97], Masaoka stage ≥IIb (OR 10.73, 95% CI 2.38–48.36) and neuromyotonia (OR 41.78, 95% CI 4.71–370.58).Conclusions: De novo occurrence of neuromyotonia in MG patients with previous thymomas is a rare event and may herald tumour recurrence. Neuronal autoantibodies can be helpful to assess the diagnosis. These observations provide pragmatic risk stratification for tumour vigilance in patients with thymomatous MG.
