- Abstract
Purpose: Neuroendocrine neoplasms (NENs) represent heterogeneous tumors arising from neuroendocrine cells in different organs. Despite growing interest in the nutritional aspects of NEN management, research in this area is limited. Aim of this review is to summarize the current state of knowledge, highlight research gaps, and underscore the significance of nutrition in the comprehensive care of NEN patients. Methods: We conducted an extensive bibliographic search focusing on studies (including retrospective and prospective studies, systematic reviews, case series, and guidelines) exploring the relationship between nutritional assessments, dietary interventions, micronutrient deficiencies, and their impact on NEN outcomes. Results: Significant gaps exist in current research, particularly in understanding the specific nutritional needs of NEN patients and how tailored nutritional interventions can improve clinical outcomes. Evidence suggests that a high-fat Western diet may promote the growth of NEN, while a Mediterranean diet may help lower insulin levels and strengthen the immune system, potentially preventing tumor development. The ketogenic diet and intermittent fasting may also have positive impacts. Addressing common micronutrient deficiencies, such as vitamin D and niacin, is crucial to mitigate disease progression. There’s a crucial need for future studies to include a comprehensive nutritional assessment incorporating patient-reported outcomes, to fully capture the impact of nutritional strategies. Conclusion: Nutritional management, an important but under-researched facet of NEN treatment, significantly improves patients’ quality of life and survival. Integrating nutrition into personalized cancer care is essential, highlighting the role of nutritional strategies in optimizing patient outcomes.

- Abstract
The development of compact fusion reactors relies on high temperature superconductors to generate the required high field. In this frame Rare-Earth Barium Copper Oxide tapes appear as the most promising solution. However, these tapes will experience a radiation harsh environment due to the high flux and fluence of energetic neutrons and of the secondary particles produced by the neutrons interaction with the materials. Realistic 3D Monte Carlo simulations represent the required tool to reliably estimate the radiation environment of the tapes, providing the starting point for experimental investigation of their radiation hardness and performance under working conditions. In this paper a comprehensive analysis of damage and energy deposition including secondary particles on a 3D model of a VIPER cable is carried out, starting from a 3D model of an ARC-class reactor.

The design of new tokamak machines relying on the use of high temperature superconductors (HTS) is promoting the study of HTS properties at the operating conditions required by fusion applications. In particular, the interest in the damage induced by neutron irradiation on REBa
2 Cu3 O7-δ (REBCO, RE = Y or lanthanide series), one of the most used family of HTS, has recently risen and several studies have been devoted to radiation hardness tests performed with ion irradiation or fission neutrons. In this work, the effect of neutron irradiation on YBCO films and commercial REBCO tapes was investigated using, for the first time, neutrons produced by the D-T fusion reaction. The experiment was carried out at ENEA-Frascati Neutron Generator (FNG) where a deuteron beam is accelerated up to 300 keV and directed on a tritiated target to produce a nearly isotropic 14.1 MeV neutron field via the T(d,n)α fusion reaction. Different YBCO films deposited through metal-organic decomposition (MOD) route on single crystals (SrTiO3 and LaAlO3 ) and REBCO commercial tapes, grown by pulsed laser deposition, were irradiated. Samples exposed to three fluences were compared with a maximum neutron fluence of 1.2·1014 cm−2 . The properties of HTS materials were assessed before and after irradiation by means of different techniques. From these measurements, no significant effect on the considered properties was recognized indicating the robustness of films up to the explored irradiation fluences.- Published

International audience; This work aims at studying the manufacturing and technology peculiarities of Apulian red figure (4th − 3rd century BCE) production from Arpi. In particular, the relationship between the red figure production from Arpi with coeval pottery of different classes retrieved in the same contexts in Arpi is investigated, as well as the connection between the red figure production from Arpi and same class vases from different Apulian sites. This goal has been achieved through widening the archaeometrical studies so far performed to red figures finds from two more tomb complexes in Arpi – i.e. tomb of the Niobids vase and 4/2004 tomb – and also organically reorganizing the so far obtained experimental data on Arpi findings of various ceramic classes. The systematic characterization of red figure vases was obtained by Inductively Coupled Plasma Mass Spectrometry (ICP-MS), Scanning Electron Microscopy (SEM) with Energy Dispersive X-ray Spectroscopy (EDS), Powder X-ray diffraction (PXRD) and Raman Spectroscopy. Analytical data allow to define the elemental and mineralogical composition of ceramic pastes and identify pigments and coatings.The results allow to validate the archaeological hypotheses that Arpi red figure production is different from the one from other Apulian areas and that pictorial experiences were shared among tombs, domus and ceramic production in Arpi.

The association between IGF-1 levels and mortality in humans is complex with low levels being associated with both low and high mortality. The present meta-analysis investigates this complex relationship between IGF-1 and all-cause mortality in prospective cohort studies. A systematic literature search was conducted in PubMed/MEDLINE, Scopus, and Cochrane Library up to September 2019. Published studies were eligible for the meta-analysis if they had a prospective cohort design, a hazard ratio (HR) and 95% confidence interval (CI) for two or more categories of IGF-1 and were conducted among adults. A random-effects model with a restricted maximum likelihood heterogeneity variance estimator was used to find combined HRs for all-cause mortality. Nineteen studies involving 30,876 participants were included. Meta-analysis of the 19 eligible studies showed that with respect to the low IGF-1 category, higher IGF-1 was not associated with increased risk of all-cause mortality (HR = 0.84, 95% CI = 0.68–1.05). Dose–response analysis revealed a U-shaped relation between IGF-1 and mortality HR. Pooled results comparing low vs. middle IGF-1 showed a significant increase of all-cause mortality (HR = 1.33, 95% CI = 1.14–1.57), as well as comparing high vs. middle IGF-1 categories (HR = 1.23, 95% CI = 1.06–1.44). Finally, we provide data on the association between IGF-1 levels and the intake of proteins, carbohydrates, certain vitamins/minerals, and specific foods. Both high and low levels of IGF-1 increase mortality risk, with a specific 120–160 ng/ml range being associated with the lowest mortality. These findings can explain the apparent controversy related to the association between IGF-1 levels and mortality. 223. Radiation Environment and Damage of HTS Magnets in an ARC-Like Reactor

- Abstract
Compact fusion reactors require high temperature superconductors (HTS) for the production of high field magnets capable of confining the plasma and achieving enough fusion power to reach commercial interest. The generation of high energy neutrons in the plasma, together with the consequent proliferation of secondary particles, threatens the long term performance of the HTS materials due to the heat load and radiation damage. For this reason it is crucial to develop tools for the comprehensive simulation of radiation environment and effects throughout the reactor structure. Here we present Monte Carlo simulations of an ARC-like reactor yielding the neutron and secondary particles spectra, the power deposition, and the materials damage mapped over the whole system, focusing on the effects on the toroidal field and poloidal field HTS coils. We find that the peak of the heat deposition in the magnets is located near the divertor, and that a strongly non-homogeneous distribution of damage urges the geometrical optimization of neutron shields.

- Abstract
Early oral feeding is the preferred mode of nutrition for surgical patients. Avoidance of any nutritional therapy bears the risk of underfeeding during the postoperative course after major surgery. Considering that malnutrition and underfeeding are risk factors for postoperative complications, early enteral feeding is especially relevant for any surgical patient at nutritional risk, especially for those undergoing upper gastrointestinal surgery. The focus of this guideline is to cover both nutritional aspects of the Enhanced Recovery After Surgery (ERAS) concept and the special nutritional needs of patients undergoing major surgery, e.g. for cancer, and of those developing severe complications despite best perioperative care. From a metabolic and nutritional point of view, the key aspects of perioperative care include the integration of nutrition into the overall management of the patient, avoidance of long periods of preoperative fasting, re-establishment of oral feeding as early as possible after surgery, the start of nutritional therapy immediately if a nutritional risk becomes apparent, metabolic control e.g. of blood glucose, reduction of factors which exacerbate stress-related catabolism or impaired gastrointestinal function, minimized time on paralytic agents for ventilator management in the postoperative period, and early mobilization to facilitate protein synthesis and muscle function.

Background Surgical strategies are being adapted to face the COVID‐19 pandemic. Recommendations on the management of acute appendicitis have been based on expert opinion, but very little evidence is available. This study addressed that dearth with a snapshot of worldwide approaches to appendicitis. Methods The Association of Italian Surgeons in Europe designed an online survey to assess the current attitude of surgeons globally regarding the management of patients with acute appendicitis during the pandemic. Questions were divided into baseline information, hospital organization and screening, personal protective equipment, management and surgical approach, and patient presentation before versus during the pandemic. Results Of 744 answers, 709 (from 66 countries) were complete and were included in the analysis. Most hospitals were treating both patients with and those without COVID. There was variation in screening indications and modality used, with chest X‐ray plus molecular testing (PCR) being the commonest (19·8 per cent). Conservative management of complicated and uncomplicated appendicitis was used by 6·6 and 2·4 per cent respectively before, but 23·7 and 5·3 per cent, during the pandemic (both P, The COVID‐19 pandemic required reorganization of surgical services, affecting patients with common surgical diseases including acute appendicitis. No evidence is available on the topic. This study found global variation in screening policies, use of personal protective equipment and intraoperative directives. We assessed the effects of oral immunonutrition (OI) on the inflammatory infiltration of the tumor microenvironment (TME) of patients undergoing surgery for gastric cancer.We analyzed patients at the time of their first gastric cancer diagnosis. We collected surgical tissue specimens (stomach), and performed an immunohistochemical analysis to evaluate the inflammatory infiltration of the TME. Patients receiving OI were compared with patients receiving standard oral nutritional supplements and no nutritional support.We enrolled 12 patients with gastric cancer in the study. The median body weight loss was 5.6%. Four patients received OI, five patients received standard nutritional supplement, and three patients received no nutritional supplementation. No difference in age, body mass index (BMI), and body weight loss was observed between the three groups. The OI group showed a tendency of increased number of T-lymphocyte cluster of differentiation (CD) 8+ compared with the other groups, as well as the number of CD83+ and CD68+. The absence of F4/80+ cells was documented only in the TME of the OI group, where a linear positive correlation was present between lymphocytes CD4+ and CD8+ (R = 0.48), and between CD4+ and CD83+ (R = 0.89), although not statistically significant. In the OI group, we observed a positive correlation (not significant) between the number of lymphocytes CD8+ and macrophages CD68+ (R = 0.70; P = 0.30). A strong significant correlation was documented between CD68+ and CD40+ (R = 0.99; P = 0.01), but this correlation did not reach the significance among the patients of the other two groups (R = 0.60; P = 0.116).The administration of OI in patients with gastric cancer might determine changes in inflammatory patterns of the TME.

Recent advances in the nutritional management of patients with digestive cancers suggest that modulation of protein intake may significantly contribute to achieve nutritional and clinical benefits. By reviewing the available evidence, a quantitatively and qualitatively optimal protein intake could be derived.High protein diets (i.e., 1-1.5 g/kg body weight/day) appear key to maintain the adequate nutritional status, and may also contribute to achieve clinical benefits. This target appears particularly relevant in patients with digestive cancers at risk or already malnourished, or in older patients. During active cancer treatments, protein intake should be closer to the upper limit of the recommended intake. Also, high protein intake should be maintained beyond the periods of catabolic crisis associated with active treatments. In contrast with general reasoning, animal proteins should represent the majority of the recommended intake. Based on the available evidence, the intake of no specific amino acid can be strongly recommended to enhance anabolic potentials or the immune modulating effects of high protein diets.High protein intake, mostly based on animal proteins, should be recommended to patients with digestive cancers. However, this target should be translated into clinical prescription after considering the clinical and metabolic needs of the patients. The quest for the optimal protein intake of patients with cancer at different time points of their clinical journey is still open.
