207. Predictability of formulae for intraocular lens power calculation according to the age of implantation in paediatric cataract

Abstract

AimsTo analyse the predictability of diverse intraocular lens (IOL) power calculation formulae in paediatric patients with congenital cataract.MethodsThe medical records of patients who underwent cataract surgery and posterior chamber IOL implantation (in-the-bag) for congenital cataract before 17 years of age were reviewed retrospectively. Target refractions calculated by Sanders-Retzlaff-Kraff (SRK)/II, SRK/T and Hoffer-Q formulae were compared with the actual refraction. Patients were subgroup according to the age at IOL implantation (age group 0–24 months, 25–60 months, 61–120 months, 121–203 months), and we compared mean prediction error (PE) and mean absolute error (AE) for each formula. Corrected AE was obtained by linear regression analysis.ResultsTotally 481 eyes were included in the final analysis. Both SRK/II and SRK/T yielded the lowest mean AE in the age group 0–24 months and SRK/II yielded the lowest mean AE in the age group 25–60 months. For every formula, the mean PE was positive during the first five years of age, which converged to zero according to age as IOL implantation increases. The tendency for immediate postoperative overcorrection in younger patients (<6 years) could be improved by corrected formulae based on the linear regression equation.ConclusionsPatients with congenital cataract who underwent IOL implantation within 5 years of age showed higher AE than the older ones did. Among the three formulae evaluated, SRK/II consistently provided the best predictive result in these patients. For patients aged >10 years, all three formulae showed favourable predictive abilities.

Abstract

1 Phorbol ester decreases muscle tension in the rat myometrium, and the effect is more potent in late-pregnant myometrium than in nonpregnant myometrium. In the present study, we have examined the contribution of protein kinase C (PKC) isoforms to the phorbol ester-induced inhibition of tension in rat uterine smooth muscle. 2 Thymeleatoxin (THX), a selective activator of conventional-type PKC (cPKC), and 12-deoxyphorbol 13-isobutyrate (DPB), an activator of pan PKC, inhibited the tension induced by high K(+), and inhibitions were significantly increased in pregnant myometrium compared to nonpregnant myometrium. The inhibition by DPB and THX of high K(+)-induced tension was significantly attenuated when PKC was downregulated by long-term pretreatment with THX and inhibited by Go6976, a cPKC inhibitor. 3 Of the cPKCs, PKC alpha is predominantly expressed in the rat myometrium, as detected by Western blot analysis. The expression of PKC alpha gradually increases from the beginning of gestation, reaching a maximum at day 21 of pregnancy. Treatment with DPB induced PKC alpha to translocate from the cytosol to the membrane in the pregnant myometrium. PKC epsilon and PKC zeta, other dominant PKC isoforms in the rat myometrium, decrease during gestation, reaching a minimum in late pregnancy. 4 These results suggest that cPKC may be at least partly involved in the PKC-mediated inhibition of muscle tension in the rat myometrium.

Abstract

This article discusses diversity and inclusiveness in the early church in terms of Mark’s attitudes towards the unknown exorcist in Mark 9:38–40. Exorcisms are well-documented phenomena in early Christianity. Mark’s episode of the unknown exorcist reflects such a historical situation. Through the episode, Mark explains that God’s kingdom expands not merely through certain well-known authoritative figures, but also anonymous, minor figures such as the unknown exorcist. The evangelist explains the diversity and inclusive characteristic of God’s kingdom to the reader. While many studies on the given Marcan passage understand the exorcist as an outsider of God’s kingdom, this research suggests that the evangelist regards the exorcist as an authentic member of God’s kingdom and his exorcism as a mystical way of the growth of the kingdom. The twelve disciples fail to recognize such a secret expansion of the kingdom. They define the boundary of the kingdom rather narrowly. Mark reveals the disciples’ narrow view and engages the reader in reflections on the true characteristic of God’s kingdom. He invites the reader to understand the mysterious expansion and the inclusiveness of the kingdom and participate in that expansion. [ABSTRACT FROM AUTHOR]
