204. Case report: small cell transformation and metastasis to the breast in a patient with lung adenocarcinoma following maintenance treatment with epidermal growth factor receptor tyrosine kinase inhibitors.

Abstract

Background: Breast metastasis from lung cancer has been reported, but not from SCLC that is transformed from lung adenocarcinoma during maintenance treatment with epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI). Transformation to small cell lung cancer(SCLC), although uncommonly seen, has been associated with resistance to EGFR-TKI therapy in lung adenocarcinomas.Case Presentation: We describe a case of a 49-year-old man with lung adenocarcinoma harboring L858R point mutation at the exon 21 of the epidermal growth factor receptor (EGFR). During the maintenance treatment with EGFR-TKI, the patient presented with a right breast mass, which was accompanied by elevated serum neuron specific enolase (NSE) level. The histological examination of biopsies from the breast mass and enlarging lung mass revealed SCLC that was less sensitive to standard SCLC treatment. The breast tumor was positive for thyroid transcription factor-1 (TTF-1), consistent with a lung primary cancer.Conclusion: This is the first case report of small cell transformation and metastatic to the breast in a patient with lung adenocarcinoma following EGFR-TKI treatment. Repeat biopsy is important for evaluation of evolving genetic and histologic changes and selection of appropriate treatment. and serum NSE measurement may be useful for detection of small cell transformation in cases with resistance to EGFR-TKI therapy. Abstract

Currently, repeated routine subcutaneous injections of insulin are the standard treatment for insulin-dependent diabetic patients. However, patients’ poor compliance for injections often fails to achieve the stable concentration of blood glucose. As a protein drug, the oral bioavailability of insulin is low due to many physiological reasons. Several carriers, such as macromolecules and liposomes have been used to deliver drugsin vivo. In this review article, the gastrointestinal barriers of oral insulin administration are described. Strategies for increasing the bioavailability of oral insulin, such absorption enhancers, enzyme inhibitors, enteric coatings are also introduced. The potential absorption mechanisms of insulin-loaded nanoparticles across the intestinal epithelium, including intestinal lymphatic route, transcellular route and paracellular route are discussed in this review. Natural polymers, such as chitosan and its derivates, alginate derivatives, γ-PGA-based materials and starch-based nanoparticles have been exploited for oral insulin delivery; synthetic polymers, such as PLGA, PLA, PCL and PEA have also been developed for oral administration of insulin. Abstract

This paper reports a development of a two-phase coupled Eulerian-Lagrangian FEM for analyzing the short-and long-term response of offshore spudcan footing. The proposed method is composed of three integral components which are effective stress Eulerian analysis for the shrot-term installation behavior of spudcan, mesh-to-mesh solution variable mapping and coupled-flow Lagrangian analysis for the post-installation, long-term working behaviour of spudcan. By combining the advantages of Eulerian and Lagrangian finite element methods, this coupled Eulerian-Lagrangian FEM is tailored to solve offshore foundation problems which often see rapid and large-scale undrained soil flow during installation followed by much longer periods of embedment in the ground during operation. Abstract

Cordycepin has been widely used in oriental countries to maintain health and improve physical performance. Compound nerve action potential (CNAP), which is critical in signal conduction in the peripheral nervous system, is necessary to regulate physical performance, including motor system physiological and pathological processes. Therefore, regulatory effects of cordycepin on CNAP conduction should be elucidated. In this study, the conduction ability of CNAP in isolated frog sciatic nerves was investigated. Results revealed that cordycepin significantly decreased CNAP amplitude and conductive velocity in a reversible and concentration-dependent manner. At 50 mg/L cordycepin, CNAP amplitude and conductive velocity decreased by 62.18 ± 8.06% and 57.34% ± 6.14% compared with the control amplitude and conductive velocity, respectively. However, the depressive action of cordycepin on amplitude and conductive velocity was not observed in Ca2+-free medium or in the presence of Ca2+ channel blockers (CdCl2/LaCl3). Pretreatment with L-type Ca2+ channel antagonist (nifedipine/deltiazem) also blocked cordycepin-induced responses; by contrast, T-type and P-type Ca2+ channel antagonists (Ni2+) failed to block such responses. Therefore, cordycepin decreased the conduction ability of CNAP in isolated frog sciatic nerves via L-type Ca2+ channel-dependent mechanism. [ABSTRACT FROM AUTHOR]

249. Application of Shear Strength of Solid Waste and Multilayer Liner in Landfills.

Abstract

The scope of this paper is to present the shear behaviors of municipal solid waste and interfaces of composite liner in landfills. The shear strength of solid waste was investigated by triaxial compression and direct shear tests. Triaxial tests are conducted using standard sand sample randomly reinforced by fabric to investigate the influence of fabric length, fabric content on their strength behavior. Test results showed that the sample strength increases with increasing of fabric length when the fabric length is smaller than a certain length threshold. Above this threshold, the fabric length has little effect on the sample strength. Similarly, the sample strength, especially the cohesion, increases dramatically with increasing of fabric content when the fabric content is below a content threshold. Above it, the fabric content has little influence on the strength. Comparison between the fabric length and content showed that the fabric content is more important, as satisfactory sample strength can be obtained even with a small fabric length when the fabric content is above a certain limit. All results of triaxial compression and direct shear tests show that the stress-strain or stress-displacement response for the solid waste has a steady hardening behavior without reaching peak stress condition due to the progressive contribution of the fibrous materials in the waste. It is suggested that the strength parameter of solid waste by triaxial compression test is obtained at the strain of 15%. The direct shear tests were performed to investigate the interface shear strengths for composite liners. The composite liner consists of nonwoven geotextile, HDPE geomembrane and compacted clay. The weaker interface in the composite liner can be transferred from the interface of geomembrane and nonwoven geotextile to the interface strength of geomembrane and saturated clay with increase of the normal pressure. Abstract

The present study focuses on the adsorption of a polyelectrolyte, the component of which is a polymer of maleic anhydride sodium salt, on the surface of nanometric zinc oxide particles and the colloidal stability of aqueous dispersions. FTIR spectroscopic data provided evidence in support of hydrogen bonding and chemical interaction in the case of the polyelectrolyte-ZnO system. The adsorption isotherms showed the influences of polyelectrolyte concentration, pH and ionic strength on the adsorption. With the increase of pH the saturated adsorbed amount decreased, while the thickness of adsorption layer increased. The saturated adsorbed amount increased with increasing salt concentration, and decreased with further increasing salt concentration. It should be noted that in the presence of a CaCl2 solution the adsorbed amount and the adsorption layer thickness were greater than those in the NaCl solution of the same concentration. The change of the absorbance of zinc oxide dispersions showed that the absorbance decreased slowly at high pH. There was a maximum point for the absorbance of the zinc oxide dispersions in the presence of various polyelectrolyte concentrations. The change of dispersion stability resulted from the change of macromolecular chains conformation at the interface.
