Abstract

Immigrants to the US from Mexico, Central and South America may have mental health problems caused by exposure to violence in their native country, according to a survey of 638 Latino immigrants in Los Angeles, California. About half had been exposed to political violence in their native country. Thirty-six percent had symptoms of depression and 18% had symptoms of post-traumatic stress disorder.

207. Evolution of disintegration properties of granite residual soil with microstructure alteration due to wetting and drying cycles.

Abstract

Despite extensive investigations of the disintegration behavior of rock, much less is known about the disintegration behavior of residual soil derived from the weathering of the parent rock. The engineering geology of RS is widely recognized as being associated with the parent rock properties as well as the weathering conditions. In the case of granite residual soil, how climate changes affect its disintegration behavior is currently not well understood. This paper performed laboratory disintegration tests on natural and remolded GRS as well as the residual soil subjected to various wetting–drying (W–D) cycles. The soil microstructure alterations due to W–D cycles are also investigated through scanning electron microscope and mercury intrusion porosimetry. The W–D cycles transform the microstructure of natural GRS toward the pattern for remolded soil by damaging the cementation among soil particles, expanding the pore diameter, and forming macropores and fissures. The deterioration of cementation weakens the particle association; thus, the soil is more disintegrative, and the generation of fissures facilitates this water–soil interaction by allowing infiltrating water to pass through. Several parameters are proposed to quantify the microstructure alterations and are found to correlate well with the disintegration rates of soil subjected to W–D cycles. This paper quantifies the microstructure evolution as induced by W–D cycles and enhances the understanding of the mechanism for the disintegration of residual soil. [ABSTRACT FROM AUTHOR]

Abstract

Exercise training can improve heart function in patients who have a heart transplant. Researchers randomly assigned 27 patients who had received a heart transplant in the past two weeks to participate in a structured exercise program at a cardiac rehabilitation clinic or to receive unstructured therapy at home. Patients attending the clinic received training from a physical therapist. Stress tests at one and six months after transplantation showed that the patients attending the clinic had improved heart function compared to those at home.

Abstract

Flow hydraulics play important roles in soil erosion and loss of soil nutrients. A better understanding of the relationship between flow hydraulics and nutrient losses will improve chemical transport modeling. The laboratory experiment was conducted to determine flow hydraulics' effects on total nitrogen (TN) loss. The impacts of rainfall intensities (0.6, 1.1, 1.61, 2.12, and 2.54 mm·min−1) and slope gradients (10°, 15°, and 20°) on TN loss were also studied. Selected soils were derived from purple sandy shales, which are the main parent materials in Wangjiaqiao watershed, southern China. Results show that negative linear relationships (R2 = 0.71) were observed between the rate of Manning roughness coefficient to average flow depth and the unit area runoff-associated TN transport rate. There was a good linear relationship between the unit area sediment-associated TN transport rate and Reynolds numbers (R2 = 0.90), flow velocity (R2 = 0.87), and stream power (R2 = 0.73), while Froude numbers, Darcy–Weisbach and Manning friction coefficients were not good hydraulic indicators of the sediment-associated TN loss of purple soil. The equation including stream power and flow velocity may have a better correlation coefficient (R2 = 0.94).

234. The entry of underrepresented minority students into US medical schools: an evaluation of recent trends

Abstract

Objectives. Recent challenges to affirmative action suggest the need to reassess the status of the admission of underrepresented minority students to US medical schools. Methods. The Association of American Medical Colleges provided US medical school enrollment data and characteristics. Five measures of underrepresented minority enrollment and an overall performance scale were constructed for each school. Multivariate regression identified significant overall performance predictors. Predicted and observed values were compared. Results. Underrepresented minority enrollment increased by 43% after 1986, peaked at 2014 in 1994, did not increase in 1995, and decreased by 5% in 1996. Enrollment was associated with increasing federal research funding and with percentage of underrepresented minorities in the source population (P [is less than] .001). The 1996 decline was almost entirely limited to public medical schools. Those in California, Texas, Mississippi, and Louisiana accounted for 18% of 1995 enrollment but 44% of the 1996 decline. Conclusions. Recent gains in medical school enrollment of underrepresented minorities are being reversed, particularly at public institutions. Implications exist for the health of poor, minority, and underserved communities, which are more likely to be cared for by underrepresented minority physicians. Abstract

BackgroundParents play an important role in their children's oral health behaviors, provide oral health access, initiate prevention, and coping strategies for health care.AimThis paper develops a short form (SF) to assist parents to evaluate their children's oral health status using Patient-Reported Outcome Measurement Information System (PROMIS) framework that conceptualized health as physical, mental, and social components.DesignSurveys of parents were conducted at dental clinics in Los Angeles County, together with an on-site clinical examination by dentists to determine clinical outcomes, Children's Oral Health Status Index (COHSI), and referral recommendations (RRs). Graded response models in item response theory were used to create the SF. A toolkit including SF, demographic information, and algorithms was developed to predict the COHSI and RRs.ResultsThe final SF questionnaire consists of eight items. The square root mean squared error for the prediction of COHSI is 7.6. The sensitivity and specificity of using SF to predict immediate treatment needs (binary RRs) are 85% and 31%.ConclusionsThe parent SF is an additional component of the oral health evaluation toolkit that can be used for oral health screening, surveillance program, policy planning, and research of school-aged children and adolescents from guardian perspectives.

239. Responsiveness of PROMIS® to change in chronic obstructive pulmonary disease.

Abstract

BackgroundChronic obstructive pulmonary disease (COPD) is a progressive chronic disease characterized by airflow obstruction that leads to shortness of breath and substantial negative impacts on health-related quality of life (HRQL). The course of COPD includes periodic acute exacerbations that require changes in treatment and/or hospitalizations. This study was designed to examine the responsiveness of Patient-Reported Outcomes Measurement Information System® (PROMIS®) measures to changes associated with COPD exacerbation recovery.MethodsA longitudinal analysis using mixed-effects models was conducted of people who were enrolled while stable (n = 100) and those who experienced an acute exacerbation (n = 85). PROMIS (physical function, pain interference, pain behavior, fatigue, anxiety, depression, anger, social roles, discretionary social activities, Global Health, dyspnea severity and dyspnea functional limitations) and COPD-targeted HRQL measures were completed at baseline and at 12 weeks.ResultsWe administered PROMIS measures using computer adaptive testing (CAT), followed by administration of any remaining short form (SF) items that had not yet been administered by CAT. Examination of the difference between group differences from baseline to 12 weeks in the stable and exacerbation groups revealed that the exacerbation group changed (improved) significantly more than the stable group in anxiety (p < .001 to p < .01; f2 effect size [ES] = 0.023/0.021), fatigue (p < .0001; ES = 0.036/0.047) and social roles (p < .001 to p < .05; ES = 0.035/0.024). All effect sizes were small in magnitude and smaller than hypothesized. Depression was also statistically significant (p < .05, SF only) but the ES was trivial. For all other PROMIS domains, the differences were not significant and ES were trivial.ConclusionsThis longitudinal study provides some support for the validity of the PROMIS fatigue, anxiety, and social roles domains in COPD, but further evalua

ObjectiveTo describe preferences for survey instruments on health-related quality of life (HRQOL) and subjective well-being (SWB) among adults with spinal cord injury (SCI), and compare perspectives on the instruments between the United States and the United Kingdom.DesignWe conducted 20 in-depth interviews.SettingParticipants were interviewed in their homes, some in person and some via Skype.ParticipantsA convenience sample of 20 adults with SCI (10 in the US and 10 in the UK) were recruited via print and on-line advertisements.InterventionsNot applicable.Main outcome measuresInterviewees reviewed six instruments and rated how important it was for their medical providers to know answers to each survey. Two coders analyzed verbatim transcriptions independently using an inductive approach. Keyword-in-context (KWIC) analysis identified the most frequently used words by interviewees to discuss the merits of each instrument.ResultsParticipants in both samples identified the Fatigue Severity Scale (FSS) as "vital" that their medical providers know about it. This was followed by the Spinal Cord Injury Independence Measure III, and a stand-alone Eudaimonic Well-Being question. The KWIC analysis showed that the most distinctive words used to discuss FSS were "fatigue" and "pain."ConclusionsUnderstanding what HRQOL and SWB measures are valued by adults living with SCI can lead to selection of informative instruments, which could help clinicians to complement and tailor established care and rehabilitation protocols for individual needs. Participants identified fatigue as a significant issue, and the FSS as a vitally important instrument to share with medical providers.

241. Data-Driven Correction Reduced Order Models for the Quasi-Geostrophic Equations: A Numerical Investigation

Abstract

This paper investigates the recently introduced data-driven correction reduced order model (DDC-ROM) in the numerical simulation of the quasi-geostrophic equations. The DDC-ROM uses available data to model the correction term that is generally used to represent the missing information in low-dimensional ROMs. Physical constraints are added to the DDC-ROM to create the constrained data-driven correction reduced order model (CDDC-ROM) in order to further improve its accuracy and stability. Finally, the DDC-ROM is tested on time intervals that are longer than the time interval over which it was trained. The numerical investigation shows that, for low-dimensional ROMs, both the DDC-ROM and CDDC-ROM perform better than the standard Galerkin ROM (G-ROM) and the CDDC-ROM provides the best results.

242. Using 3D scanner to study gully evolution and its hydrological analysis in the deep weathering of southern China

Abstract

This study integrated 3D scanner measurement techniques with hydrological analysis to study the evolution of gullies developed in the deep weathering (granite) in the subtropical climate of southern China. Two gullies were repeatedly measured using a 3D laser scanner over a 7.5-year period from 2009 to 2016. The measurements with sub-centimeter accuracy indicated that gully head retreat was the dominant process in the gully development, with a mean rate of 0.46 and 1.10 m yr−1 for gully A and B respectively. To explore the relationship between gully head retreat (GHR) and hydrologic variables, rainfall with three different thresholds, rainfall erosivity and runoff depth were calculated using different methods. Statistical analysis showed a significant correlation between GHR rates and cumulative erosive rainfall depth with a daily rainfall threshold >25 mm. Hydrological analysis and field observations suggested that gully head retreats mainly result from mass movement triggered by gravity and soil saturation, although water erosion might also play a role in destabilizing the already unstable gully slopes. This study has implications for restoration of gullies in the area, which requires techniques that go beyond a mere reduction of runoff contribution to gully systems. Further studies are needed to gather more data on the evolution and sediment delivery of gullies, to prove the hypothesis of gravity-driven gully headcut development, and to explore more effective gully restoration techniques.
