Background: Atrial fibrillation (AF) ablation can lead to oesophageal thermal injuries (ETI). These are thought to be the precursor of the much rarer but frequently fatal atrio-oesophageal fistulas. Many centers performing AF ablation routinely use oesophageal temperature monitoring (ETM). This meta-analysis aims to determine the utility of ETM in preventing ETI in the context of radiofrequency catheter ablation of AF. Methods: A systematic search of PubMed, Embase databases and Cochrane registry was performed comparing ETI between ETM and non-ETM strategies in AF ablation. Data on endoscopically determined ETI, AF recurrence, procedure time and ablation time were extracted. Statistical analyses including subgroup and covariate analyses were performed using random effect model in R platform. Results: ETI were similar in both ETM (n = 864) and non- ETM groups (n = 639) (RR 1.04, 95 % CI 0.34–3.23) across 12 studies. AF recurrence was statistically similar in both groups (IRR 0.92, 95 % CI 0.73–1.17) but showed a lower trend in non-ETM group. Ablation time was numerically lower in the ETM group and procedure time was numerically higher trend in the ETM group; but they were not statistically significant. Covariate analysis found that posterior wall ablation power setting, additional linear ablation, BMI, use of GA or prophylactic PPI after ablation had no significant correlation in the incidence of ETI. Conclusion: ETM was not associated with a reduced incidence of ETI during AF ablation. Evidence supporting the routine use of ETM to reduce the risk of ETI or atrio-oesophageal fistulas is lacking.

(1) Background: Head and neck cancer (HNC) is a major public health challenge worldwide, with oral squamous cell carcinoma (OSCC) being the predominant form. Despite advances in treatment, OSCC remains a major cause of morbidity and mortality due to delayed diagnosis and limited therapeutic efficacy. This study reviews omics technologies to assess new salivary biomarkers for the early detection of OSCC. (2) Methods: A comprehensive literature search in the last 20 years identified four relevant studies focusing on salivary biomarkers in OSCC. (3) Results: Proteomic and genomic analyses revealed significant changes in salivary composition between OSCC patients and healthy controls, suggesting promising diagnostic and prognostic biomarkers. However, studies showed varying degrees of bias, indicating the need for further research and improved standardization. (4) Conclusions: Saliva, with its advantages of ease of collection, minimal invasiveness, and potential for large-scale screening, is an emerging promising substrate for non-invasive biomarker research. Nonetheless, there is a need for improved biomarker sensitivity and specificity; currently, histological examination remains the golden standard.

Este artículo busca ofrecer un análisis clasificatorio y descriptivo de las intervenciones del Cabildo de Buenos Aires en el abasto de carne de la Ciudad, con el objetivo de identificar y explicar las principales características de la obligación o remate del abasto de carne, los asuntos y problemas más frecuentes, su tratamiento y resolución por parte de las autoridades. Nuestra hipótesis sostiene que el monopolio fue el principal mecanismo de abastecimiento alimenticio entre las décadas de 1720 y 1750, pero que, dentro de dicho período, se fueron dando importantes cambios en las prácticas ganaderas y comerciales. Para cumplir con lo planteado, nos basamos en el análisis sistematizado de los Acuerdos del Extinguido Cabildo de Buenos Aires, relevados en el Archivo General de la Nación.

In this paper, we study the long-time dynamics of a Timoshenko system modeling vibrations of beams with non-linear localized damping mechanisms acting on both displacement and angular rotation and subjected to non-linear source terms. Using recent quasi-stability methods, we prove the existence of smooth finite dimensional global attractor, which is characterized as unstable manifold of the set of stationary solutions. Moreover, the existence of exponential attractors is shown. These aspects were not previously considered for the Timoshenko system with localized damping. [ABSTRACT FROM AUTHOR]

Molecular motors, kinesin and myosin, are dimeric consisting of two linked identical monomeric globular proteins. Fueled by the free energy generated by ATP hydrolysis, they walk on polar tracks (microtubule or filamentous actin) processively, which means that only one head detaches and executes a mechanical step while the other stays bound to the track. One motor head must regulate the chemical state of the other, referred to as "gating", a concept that is still not fully understood. Inspired by experiments, showing that only a fraction of the energy from ATP hydrolysis is used to advance the kinesin motors against load, we demonstrate that the rest of the energy is associated with chemical transitions in the two heads. The coordinated chemical transitions involve communication between the two heads - a feature that characterizes gating. We develop a general framework, based on information theory and stochastic thermodynamics, and establish that gating could be quantified in terms of information flow between the motor heads. Applications tokinesin-1 and Myosin V show that information flow, with positive cooperativity, at external resistive loads less than a critical value, Fc. When force exceeds Fc, effective information flow ceases. Interestingly, Fc, which is independent of the input energy generated through ATP hydrolysis, coincides with the force at which the probability of backward steps starts to increase. Our findings suggest that transport efficiency is optimal only at forces less than Fc, which implies that these motors must operate at low loads under in vivo conditions. [ABSTRACT FROM AUTHOR]

El complejo del Leopardus colocolo, según una reciente actualización taxonómica, ha sido dividido en cinco especies. Una de estas es el Leopardus braccatus conocida como gato del pantanal, distribuido en Bolivia, Brasil y Paraguay. A pesar de su distribución, esta es una especie poco estudiada. Presentamos aquí un nuevo registro de esta especie, siendo el primero en el bosque seco chiquitano, un ecosistema amenazado que previamente no se consideraba su hábitat. Este hallazgo se llevó a cabo como parte de un programa de monitoreo de felinos con cámaras trampa, donde adicionalmente seis especies de félidos fueron detectadas en un área de 41km2, lo que indica una alta diversidad de felinos en esta ecorregión. Además, se identificaron amenazas directas para el gato del pantanal, como la actividad agrícola y ganadera, los perros domésticos y la cacería ilegal. La relevancia de este nuevo registro radica en su ubicación en el límite occidental-sur de la distribución conocida para Bolivia, así también como sus características morfológicas similares a la especie L. munoai. Esto subraya la necesidad de realizar investigaciones adicionales para comprender mejor su distribución y conservación. Estudios futuros podrían explorar también las similitudes morfológicas y de distribución entre L. braccatus y L. munoai.

Objetivo: avaliar os níveis de estresse ocupacional de enfermeiros intensivistas de Boa Vista, Roraima. Método: estudo descritivo-correlacional, realizado entre novembro/2022 e janeiro/2023 com 36 enfermeiros intensivistas de um hospital público. Resultados: 80,6% dos enfermeiros apresentaram alta demanda (16,2 ± 1,0) e 63,9% apresentaram alto controle (19,1 ± 1,0), havendo uma distribuição igualitária quanto ao apoio social. Referente aos escores de estresse, a maioria dos participantes apresentaram alta demanda psicológica (16,2) e alto controle (19,1), o que se traduz como um trabalho ativo. Os enfermeiros que não receberam apoio e reconhecimento por parte dos superiores apresentaram escore médio mais baixo nas dimensões Demanda Psicológica (2,12 unidades mais baixo) e Controle (3,26 unidades mais baixo). Conclusão: as vivências dos participantes no cuidado crítico durante a pandemia influenciaram na percepção do trabalho, e que, mesmo tendo um trabalho ativo considerado menos nocivo, a longo prazo pode se tornar negativo.

Background: Since ancient times, poisoning, even serious poisoning, has been known to occur during nature walks. Intentional or unintentional ingestion of toxins of animal origin is one of the possible causes of poisoning. Bufadienolide poisoning is a critical case. This is because of its high potency and its ability to cross the blood-brain barrier. Due to the rarity of these poisonings in humans in Central Europe, their identification is often difficult. The following is a case report of a poisoning by toad eggs in an Italina child, that presented vertigo, fussiness and sleepiness. A method of toxin identification using the prince of pharmacotoxicology, liquid chromatography (LC) coupled with tandem mass spectrometry (MS/MS), and an innovative reasoning were used. This method can be applied to other poisoning cases.
