Abstract

Hepatocellular carcinoma is a form of liver cancer; this malignancy is more common in Asian nations than in the United States. The prognosis for this form of liver cancer is quite poor, and the average survival time may be around six months. Although the liver cancers may be quite small at the time of diagnosis, surgery is often impossible; the cancer may be in an inoperable site or may be scattered in several places. Furthermore, patients with hepatocellular carcinoma are also very likely to have liver cirrhosis, which may also make liver surgery impractical. Researchers have observed that oily substances injected into the main liver artery (for purposes of enhancing X-rays) seemed to be preferentially retained by cancerous over normal tissue. This fact may prove useful in designing treatments for hepatocellular carcinoma. Lipiodol is an oily compound used in enhancing X-rays; since lipiodol contains the chemical element iodine, it is a simple matter to replace the normal iodine-127 in its chemical structure with the radioactive isotope iodine-131. If the liver cancers selectively retain the resulting radioactive oil, it was hypothesized, they may contribute to their own destruction by irradiation. This method of selective irradiation was used in the experimental treatment of 24 patients with hepatocellular carcinoma. The adverse effects were mild and well tolerated by the patients; these effects included fever, nausea, and mild abdominal pain. Imaging of radioactive emissions confirmed that the tumors were sequestering the radioactive oil; the smaller tumors, however, were the most affected. Almost 90 percent of the liver cancers smaller than 4.0 centimeters were reduced in size by the treatment. Only one fourth of the tumors larger than 5.1 centimeters showed a decrease in size. The patients avoided the complications of bone marrow suppression and hypothyroidism because the radioactive iodine used in the treatment could not freely travel through the body. Abstract

A noncontact method to generate and measure torsional waves in rotating shafts by a set of magnetostrictive patches and a solenoid is proposed. The main issues in the present wave experiment are how to generate torsional waves having sufficient power and how not to interfere with shaft rotational motion. Magnetostrictive patches are bonded obliquely to the shaft axis for pure torsional wave generation and an arrayed patch configuration is employed for sufficient power generation and frequency localization. For uninterrupted shaft rotation, a solenoid encircling a rotating shaft wirelessly supplies or detects the ac magnetic field in the patches. The validity and performance of the proposed wave transduction method were checked by several experiments. Abstract

The circular irrigated decapolar nMARQ ablation catheter designed for treating atrial fibrillation (AF) has recently been recalled following two deaths due to esoatrial fistula. Injury to the esophagus has been previously reported in up to 50% of patients using 20-25 W unipolar radiofrequency (RF) energy. Low power of 15 W has been proposed to prevent this complication, but the efficacy of this strategy to avoid AF recurrence is unknown.Consecutive patients with drug-refractory, symptomatic AF were included. Under electroanatomical navigation, the nMARQ catheter was used to isolate all PVs by applying 15 W of unipolar RF simultaneously from up to 10 poles during 40 seconds. Multiple applications were used for each vein, until isolation was achieved. Follow-up was performed after a 2-month blanking period. A total of 50 patients (37 males, age 58 ± 10 years) were included. All PVs were acutely isolated without requiring touch-up by conventional ablation catheters. Pericardial effusion occurred in two patients, of whom one required periocardiocentesis. Right phrenic nerve palsy occurred in another patient, which partially resolved. There were no cases of esophageal fistula or stroke. After a follow-up of 15 ± 4 months, AF recurred in 27/50 (54%) patients.The recurrence rate of AF with 15 W unipolar applications is high. Despite use of low power, complications such as pericardial effusion and phrenic nerve palsy may occur.

Abstract

In contrast to the well-documented motility of monopisthocotyleans within the monogenean group, the mobility of polyopisthocotyleans, such as Microcotyle sebastis, has largely been undocumented with empirical evidence. This study aimed to investigate the locomotion of the polyopisthocotylean parasite of M. sebastis, which infests Korean rockfish (Sebastes schlegelii), thereby providing tangible evidence of early behavior in polyopisthocotyleans. The oncomiracidium of M. sebastisdemonstrated movement facilitated by the hamulus and bilateral cilia, aiding in directional changes and propulsion. In the gill filaments of M. sebastislarvae utilized both the hamulus and their entire body for initial movements, effectively navigating between gill filaments. The hamulus played a crucial role in the locomotion of the early larval stages before the development of three clamp pairs, after which it began to degenerate. This study presents concrete evidence of early mobility in polyopisthocotyleans like M. sebastis, offering insights that could inform aquaculture management strategies, particularly in the mitigation of parasite-induced damages.

Abstract

Typical atrial flutter (AFL) or type I AFL is the most common type of macroreentrant atrial tachycardia. Its prevalence increases with age and is more common in men. Ablation of the cavo-tricuspid isthmus (CTI), a critical part of the circuit, is safe and highly effective. Despite the long-term efficacy of this therapy, a significant proportion of patients undergoing CTI ablation will develop atrial fibrillation (AF) during the follow-up and identifying them can have an important impact on their management. PV isolation and typical flutter ablation during the same procedure may be an effective treatment strategy for patients with clinical documentation of both arrhythmias.

Abstract

Transposable elements (TEs) are mobile genetic sequences that comprise a large portion of vertebrate genomes. The olive flounder (Paralichthys olivaceus) is a valuable marine resource in East Asia. The scope of most genomic studies on the olive flounder is limited to its immunology as their focus is the prevention of mass mortality of this species. Thus, for a broader understanding of the species, its genomic information is consistently in demand. Transcripts sequences were acquired from transcriptome analysis using gill tissues of 12 olive flounders. Distribution of TEs inserted in exonic region of the olive flounder genome was analyzed using RepeatMasker (http://www.repeatmasker.org/). We found 1140 TEs in the exonic region of the genome and long interspersed nuclear elements (LINEs) and long terminal repeats (LTRs) insertions occurred with forward orientation preferences. Transposons belonging to the hAt, Gypsy, and LINE 1 (L1) subfamilies were the most abundant DNA transposons, LTRs, and long interspersed elements (LINEs), respectively. Finally, we carried out a gene ontology analysis to determine the function of TE-fused genes. These results provide some genomic information about TEs that is useful for future research on changes in properties and functions of genes by TEs in the olive flounder genome. Abstract

Context Several surgical complications are related to obesity. Objective This study evaluated the impact of obesity on pancreatic fistula after pancreaticoduodenectomy. Design We retrospectively reviewed the medical records of 159 patients who underwent pancreaticoduodenectomy between October 2002 and December 2008. Setting The patients were divided according to the body mass index as obese (body mass index equal to, or greater than, 25 kg/m2), or normal (body mass index less than 25 kg/m2). Methods Univariate and multivariate analyses were applied. Two-tailed P values less than 0.05 were considered as significant. Results Forty-six patients (28.9%) were obese and 113 patients (71.1%) were normal-weight. Obese group had a significantly higher incidence of pancreatic fistula and a greater amount of intraoperative blood loss. Other surgical complications were not significantly different between the two groups. Multivariate analysis found obesity, small pancreatic duct size (less than, or equal to, 3 mm), intraoperative blood loss, and combined resection as significant factors affecting pancreatic fistula.
