We present a case of a giant mullerian duct cyst with components of clear cell adenocarcinoma in a 15-year-old boy. Complete tumor resection was achieved in two steps. The presented case with a combination that has never been described before may change the perspective of treatment approaches for this rare entity.

Resume L’infiltration du nerf grand occipital (NGO) est utile dans le diagnostic et le traitement de la nevralgie d’Arnold, en particulier chez des patients resistants au traitement specifique. Cette technique necessite de bien connaitre le trajet complexe du NGO et les structures de voisinage. Le parcours sinueux du NGO fait apparaitre deux regions de vulnerabilite et /ou de conflits potentiels engendres notamment lors des mouvements de la tete. La premiere sur le trajet proximal, ou le nerf est le plus vulnerable au niveau de deux sites : a son origine et lors de sa premiere boucle autour du muscle oblique caudal de la tete et la deuxieme en distalite a son emergence lors de la traversee du muscle trapeze. Nous presenterons notre technique particuliere d’infiltration sur les deux sites au niveau proximal et les resultats chez nos 55 premiers patients.

To introduce an animal model of hepatocellular carcinoma (HCC) in ACI-rats, and to evaluate the therapeutic effects of Poly-lactide-co-glycolide (Plcg) -microspheres in the transarterial chemoembolization (TACE) in this model, as well the value of this model in the experiments of interventional therapy.Subcapsular implantation of a solid Morris Hepatoma 3 924A (1 mm(3)) in the livers was carried out in 11 male ACI-rats. The tumor volume (V1) was measured by magnetic resonance imaging (MRI) (13 days after implantation). After laparotomy and retrograde placement of catheter into the gastroduodenal artery (14 days after implantation), the following protocols of interventional treatment were performed: (A) mitomycin C+Poly-lactide-co-glycolide (Plcg)-microspheres (n=4); (B) 0.9 % NaCl (control group, n=7). 13 days after these therapies the change of the tumor volume (V2) was determined by MRI again.The success rate of tumor implantation reached to 100 %. The mean tumor volume before TACE (V1) were 0.082 cm(3) in group A and 0.096 cm(3) in group B respectively. The mean tumor volume after TACE (V2) were 0.230 cm(3) in group A and 1.347 cm(3) in group B respectively. The mean V2/V1 were 2.860 in group A and 27.120 in group B respectively. Compared to the control group (group B), groups A showed a significant reduction of tumor growth (P=0.004) in the period of observation.The growth of liver tumor could be obviously prevented by utilizing Plcg-mitomycin-microspheres in TACE in animal model. This rat model of HCC is suitable for the experimental studies of interventional therapy.

Objectives: Previous attempts at meta-analysis and systematic review have not provided clear recommendations for the clinical application of thermal ablation in metastatic colorectal cancer. Many authors believe that the probability of gathering randomised controlled trial (RCT) data is low. Our aim is to provide a consensus document making recommendations on the appropriate application of thermal ablation in patients with colorectal liver metastases. Methods: This consensus paper was discussed by an expert panel at The Interventional Oncology Sans Frontières 2013. A literature review was presented. Tumour characteristics, ablation technique and different clinical applications were considered and the level of consensus was documented. Results: Specific recommendations are made with regard to metastasis size, number, and location and ablation technique. Mean 31 % 5-year survival post-ablation in selected patients has resulted in acceptance of this therapy for those with technically inoperable but limited liver disease and those with limited liver reserve or co-morbidities that render them inoperable. Conclusions: In the absence of RCT data, it is our aim that this consensus document will facilitate judicious selection of the patients most likely to benefit from thermal ablation and provide a unified interventional oncological perspective for the use of this technology. Key Points: • Best results require due consideration of tumour size, number, volume and location. • Ablation technology, imaging guidance and intra-procedural imaging assessment must be optimised. • Accepted applications include inoperable disease due to tumour distribution or inadequate liver reserve. • Other current indications include concurrent co-morbidity, patient choice and the test-of-time approach. • Future applications may include resectable disease, e.g. for small solitary tumours.
