The aim of this experimental study was to clarify whether affective states with different arousal and valence levels influence the perceived funniness and aversiveness shown as a response to humor stimuli. We used the International Affective Picture System, IAPS (Lang, Peter J., Margaret M. Bradley & Bruce N. Cuthbert. 1999. International Affective Picture System (IAPS): Technical manual and affective ratings. Gainesville, FL: The Center for Research in Psychophysiology, University of Florida) and followed a mixed factorial design. Affective state differed between four groups: 1) neutral group; 2) negative valence-moderate arousal group; 3) positive valence-moderate arousal group; and 4) negative valence-high arousal group. We measured pre- and post-perceived funniness and aversiveness in response to humorous material. Participants were 80 psychology students who were randomly assigned to one of four induction conditions. The results showed that, regardless of the type of valence, affective states with moderate arousal levels did not affect humor appreciation. However, the perceived funniness response was significantly lower in the negative valence-high arousal group. Perceived aversiveness responses were not affected by valence or arousal level. Results are discussed in relation to several current theories on the role of affective state in humor appreciation.

Lactic acidosis is a clinical status related to clinical worsening. Actually, higher levels of lactate is a well-established trigger of emergency situations. The aim of this work is to build-up a prehospital early warning score to predict 2-day mortality and intensive care unit (ICU) admission, constructed with other components of the lactic acidosis besides the lactate.Prospective, multicenter, observational, derivation-validation cohort study of adults evacuated by ambulance and admitted to emergency department with acute diseases, between January 1st, 2020 and December 31st, 2021. Including six advanced life support, thirty-eight basic life support units, referring to four hospitals (Spain). The primary and secondary outcome of the study were 2-day all-cause mortality and ICU-admission. The prehospital lactic acidosis (PLA) score was derived from the analysis of prehospital blood parameters associated with the outcome using a logistic regression. The calibration, clinical utility, and discrimination of PLA were determined and compared to the performance of each component of the score alone.A total of 3334 patients were enrolled. The final PLA score included: lactate, pCO2, and pH. For 2-day mortality, the PLA showed an AUC of 0.941 (95%CI: 0.914-0.967), a better performance in calibration, and a higher net benefit as compared to the other score components alone. For the ICU admission, the PLA only showed a better performance for AUC: 0.75 (95%CI: 0.706-0.794).Our results showed that PLA predicts 2-day mortality better than other lactic acidosis components alone. Including PLA score in prehospital setting could improve emergency services decision-making.

The evidence about the use of natriuretic peptides (NP) to predict mortality in the pre-hospital setting is limited. The main objective of this study is to assess the ability of point-of-care testing (POCT) N-terminal portion of B-type natriuretic peptide (NT-proBNP) to predict 2-day in-hospital mortality of acute cardiovascular diseases (ACVD).We conducted a multicentric, prospective, observational study in adults with ACVD transferred by ambulance to emergency departments (ED). The primary outcome was 2-day in-hospital mortality. The discrimination capacity of the NT-proBNP was performed through a prediction model trained using a derivation cohort and evaluated by the area under the curve (AUC) of the receiver operating characteristic on a validation cohort.A total of 1006 patients were recruited. The median age was 75 (IQR 63-84) years and 421 (41.85%) were females. The 2-day in-hospital mortality was 5.8% (58 cases). The predictive validity of NT-proBNP, for 2-day mortality reached the following AUC: 0.823 (95%CI: 0.758-0.889, p 0.001), and the optimal specificity and sensitivity were 73.1 and 82.7. Predictive power of NT-proBNP obtained an AUC 0.549 (95%CI: 0.432-0.865, p 0.215) for acute heart failure, AUC 0.893 (95%CI: 0.617-0.97, p 0.001) for ischemic heart disease, AUC 0.714 (95%CI: 0.55-0.87, p = 0.0069) for arrhythmia and AUC 0.927 (95%CI: 0.877-0.978, p 0,001) for syncope.POCT NT-proBNP has proven to be a strong predictor of early mortality in ACVD, showing an excellent predictive capacity in cases of syncope. However, this biomarker does not appear to be useful for predicting outcome in patients with acute heart failure.

Resumen Introduccion La demencia vascular (DV) representa la segunda causa de demencia, con el 15,8-20% de los casos. Dado que los estudios previos del efecto de la ocupacion en DV no muestran resultados concluyentes, nuestro objetivo fue analizar si el tipo de ocupacion varia el riesgo de desarrollar DV en mayores de 55 anos. Material y metodos El proyecto ZARADEMP evaluo inicialmente una muestra comunitaria de 4.803 individuos seguidos durante 4 olas. La ocupacion se clasifico siguiendo la Clasificacion Nacional de Ocupaciones de 2011 y la Clasificacion Internacional Uniforme de Ocupaciones de 2008. Se utilizaron diferentes instrumentos estandarizados: las historias medicas y psiquiatricas (History and Aetiology Schedule) y otros factores de riesgo (Mini-Mental Status Examination, Geriatric Mental State-AGECAT). Para este estudio, se calcularon tasas de incidencia, razon de tasas de incidencia y hazard ratio en modelos multivariados de regresion de Cox, estratificados por sexo. Han sido incluidos 3.883 participantes Resultados En mujeres, el riesgo de DV se duplico en trabajadoras de cuello azul, con un efecto moderado (d de Cohen = 0,54), y se multiplico por 2,7 en labores del hogar respecto a cuello blanco, con un efecto moderado (d de Cohen = 0,77), aunque sin significacion estadistica. No observamos efecto de la ocupacion en el riesgo de DV en varones. Conclusiones Hemos encontrado un efecto moderado, pero no significativo, entre la ocupacion y el riesgo de DV en mujeres. No hallamos efecto de la ocupacion en el riesgo de DV en varones, donde otros factores de riesgo (la edad, tener diabetes o el indice de masa corporal) tienen un efecto mas claro que la ocupacion laboral.

(1) Background: The impact of SARS-CoV-2 has been variable over the time course of the pandemic and in different populations. The aim was to analyze the impact of COVID-19 infection in a known population of hemodialysis (HD) patients and professionals in Spain at different times of the pandemic. (2) Methods: We conducted an observational, descriptive study with a follow-up from 3 March 2020 to 23 April 2022 (776 days), using in average of 414 professionals and 1381 patients from 18 HD units in Spain. The data from the positive PCR or the rapid antigen detection test (RADT) subject were analyzed and segmented into six periods (waves). (3) Results: Of 703 positive COVID-19 tests, 524 were HD patients (74.5%), and 179 were HD professionals (25.5%). Overall, 38% of staff and 43% of patients were affected. Differences were observed in regard to incidence (21% vs. 13%), mortality (3.5% vs. 0%), and symptomatology between the patients and professionals and throughout the pandemic. (4) Conclusions: COVID-19 severity varied during different pandemic waves, with a greater impact seen in the first wave. HD professionals and patients had similar infection rates, but patients had higher mortality rates. Community transmission was the primary route of infection.

Technological development has profoundly marked the evolution of the economy. The constant changes brought about by scientific and technological advances have been decisive in the transition from an analogue to a digital world. In this context, the impact of the fourth industrial revolution (or Industry 4.0.) manifests itself in many ways. Environmental impact is one of these. The energy sector has been evolving and changing just like the economy and society. Therefore, a study of this sector, and of the other related elements, is of interest to better understand the 4.0 concept. The promotion of sustainability at both political and social levels has led to changes in different areas, such as the productive vision, the use of green energies or the implementation of green taxes. Energy as a key factor in Industry 4.0. involves studying it both quantitatively and qualitatively. This is to under-stand the lights and shadows that the concept currently presents. Therefore, this work aims to bring the reality of the energy sector closer to reality, both in its positive and negative aspects, considering the main factors of incidence, in order to show the strengths and weaknesses that can be deduced.

Hypertension (HTN) is one of the leading causes of death in the world. Agri-food by-products are emerging as a novel source of natural antihypertensive agents allowing for their valorization and making food and agricultural industries more environmentally friendly. In this regard, wine making process generates large amounts of by-products rich in phenolic compounds that have shown potential to exert several beneficial effects including antihypertensive properties. The aim of this study was to review the blood pressure-lowering effects of winery by-products. In addition, molecular mechanisms involved in their bioactivity were also evaluated. Among the winery by-products, grape seed extracts have widely shown antihypertensive properties in both animal and human studies. Moreover, recent evidence suggests that grape stem, skin and pomace and wine lees may also have great potential to manage HTN, although more studies are needed in order to confirm their potential in humans. Improvement of endothelial dysfunction and reduction of oxidative stress associated with HTN are the main mechanisms involved in the blood pressure-lowering effects of these by-products.

Background. The aim of this study was to assess the role of prehospital point-of-care N-terminal probrain natriuretic peptide to predict sepsis, septic shock, or in-hospital sepsis-related mortality. Methods. A prospective, emergency medical service-delivered, prognostic, cohort study of adults evacuated by ambulance and admitted to emergency department between January 2020 and May 2021. The discriminative power of the predictive variable was assessed through a prediction model trained using the derivation cohort and evaluated by the area under the curve of the receiver operating characteristic on the validation cohort. Results. A total of 1,360 patients were enrolled with medical disease in the study. The occurrence of sepsis, septic shock, and in-hospital sepsis-related mortality was 6.4% (67 cases), 4.2% (44 cases), and 6.1% (64 cases). Prehospital National Early Warning Score 2 had superior predictive validity than quick Sequential Organ Failure Assessment and N-terminal probrain natriuretic peptide for detecting sepsis and septic shock, but N-terminal probrain natriuretic peptide outperformed both scores in in-hospital sepsis-related mortality estimation. Application of N-terminal probrain natriuretic peptide to subgroups of the other two scores improved the identification of sepsis, septic shock, and sepsis-related mortality in the group of patients with low-risk scoring. Conclusions. The incorporation of N-terminal probrain natriuretic peptide in prehospital care combined with already existing scores could improve the identification of sepsis, septic shock, and sepsis-related mortality.

Background: Education and occupational complexity are main sources of mental engagement during early life and adulthood respectively, but research findings are not conclusive regarding protective effects of these factors against late-life dementia. Objective: This project aimed to examine the unique contributions of education and occupational complexity to incident dementia, and to assess the mediating effects of occupational complexity on the association between education and dementia across diverse cohorts. Method: We used data from 10,195 participants (median baseline age = 74.1, range = 58∼103), representing 9 international datasets from 6 countries over 4 continents. Using a coordinated analysis approach, the accelerated failure time model was applied to each dataset, followed by meta-analysis. In addition, causal mediation analyses were performed. Result: The meta-analytic results indicated that both education and occupational complexity were independently associated with increased dementia-free survival time, with 28%of the effect of education mediated by occupational complexity. There was evidence of threshold effects for education, with increased dementia-free survival time associated with ‘high school completion’ or ‘above high school’ compared to ‘middle school completion or below’. Conclusion: Using datasets from a wide range of geographical regions, we found that both early life education and adulthood occupational complexity were independently predictive of dementia. Education and occupational experiences occur during early life and adulthood respectively, and dementia prevention efforts could thus be made at different stages of the life course.

Health benefits among the members of state-level societies may vary depending on sex, social privilege, and whether the individual resides in an urban or rural setting. Human skeletal remains are prone to express individual life experiences and, ultimately, well-being. This research elaborates on these correlates by contextualizing the physiological stresses among Classic Maya hinterland populations in comparison to their urban peers. Comparisons are made using the frequencies and expression of enamel hypoplasia, caries, porotic hyperostosis, infectious osteomyelitis/subperiosteal reaction and osteoporosis in 842 adult skeletons of both sexes from 63 peripheral and centric, inland, lowland settlements. The results suggest problematic inland weaning diets and higher infectious load among rural populations. While comparisons between urban and rural lifeways show inconsistent load differences, our results indicate repeated distinctions between the sexes. We cautiously interpret this pattern as an indication of a physically demanding regime of rural life compared to a more sedentary routine among urban peers and gendered lifestyles in general. We conclude that apart from these distinctions (and potential sample biases), the health costs versus benefits impacted rural lifestyles in a complex and non-uniform fashion during the first millennium a.d., rejecting clear-cut hierarchical conceptualizations while inviting more nuanced causal explorations.

Hydropeaking, by artificially generated flow peaks, influences hydro-sedimentary dynamics on rivers and, consequently, affects bed material entrainment and transport. This study examines the onset of motion of sediment particles in four sections of a Pyrenean gravel-to-cobble bed river exposed to frequent hydropeaking (once per day, on average). Five criteria of particle entrainment have been used to assess the prediction of the initiation of grain motion at-a-section scale. Theoretical entrainment conditions were validated using real observations of mobility by means of tracers. It was found that the maximum flow discharged by the hydropower plant mostly affects the furthest downstream section, located almost 17 km downstream, in which the finer fractions of the bed are entrained. The mobile grain sizes include up to coarse gravels (≈ 30 mm). Differences in sediment supply (imposed by tributaries), the value of the bed slope and the structure of the coarse surface layer decisively control the downstream variability of incipient particle motion between sections. Results from a 17 km study segment indicated that hydropeaking generate partial transport, that is, a partially size-selective transport that occurs downstream from the hydropower plant and winnows the sand and small gravel further downstream, increasing armouring and depleting fine sediments. La VI edición de la Semana Universitaria y Cervantina en honor al cronista e hijo predilecto de la Villa de Quero: Alfonso Ruiz Castellanos ha tenido lugar durante los días 19 al 24 de abril, bajo la marca de calidad Querote22, con dos congresos, uno científico de transferencia en la lucha ante el reto demográfico y el otro abre el catálogo de cursos de verano y extensión universitaria de la UCLM en 2022. Fruto de esta ya acreditada y singular semana se presenta este trabajo coordinado por los profesores Dr. Víctor Raúl López y Dr. Domingo Nevado que quiere dar visibilidad al evento a través de la herramienta, en la lucha sobre el reto demográfico, del desarrollo endógeno junto a la imagen percibida de Cervantes. Recogemos las ponencias del Congreso de Investigación para el Reto Demográfico (ISEN_RED22), junto con una selección de investigaciones noveles de los estudiantes de la UCLM presentadas al ISEN_RED22 sobre despoblamiento y el medio rural. Posteriormente son las ponencias del Congreso Cervantino Nacional las que muestran el avance sobre la imagen percibida de Cervantes. Por último, podremos disfrutar de los dos relatos premiados en el IV Certamen de Relatos.

An accurate estimation of the mean annual sediment yield from basins contributes to optimizing water resources planning and management. More specifically, both reservoir sedimentation and the damage caused to infrastructures fall within its field of application. Through a simple probabilistic combination function implemented in hydrometeorological models, this sediment yield can be estimated on a planning and management scale for ungauged basins. This probabilistic combination methodology requires the use of probability distribution functions to model design storms. Within these functions, SQRT-ET max and log-Pearson type III are currently highlighted in applied hydrology. Although the Gumbel distribution is also relevant, its use has progressively declined, as it has been considered to underestimate precipitation depth and flow discharge for high return periods, compared to the SQRT-ET max and log-Pearson III functions. The quantification of sediment yield through hydrometeorological models will ultimately be affected by the choice of the probability distribution function. The following four different functions were studied: Gumbel type I with a small sample size, Gumbel type I with a large sample size, log-Pearson type III and SQRT-ET max. To illustrate this, the model with these four functions has been applied in the Alto Palmones basin (South Iberian Peninsula). In this paper, it is shown that the application of Gumbel function type I with a small sample size, for the estimation of the mean annual sediment yield, provides values on the conservative side, with respect to the SQRT-ET max and log-Pearson type III functions., The authors would like to thank Deryck J. This chapter explores the commonalities and differences in hope between Spanish (N = 206) and South African (N = 100) samples based on data collected with the Hope Barometer in November 2018. Furthermore, we investigate similarities and differences in the sources of hope between the two samples, as reflected in the activities that people engage in to fulfil their hopes and to attain the hoped-for targets (hope activities). Finally, we examine these activities as predictors of hope. Since hope is an important predictor of flourishing, we also analyze the predictive power of hope and its dimensions on flourishing in both samples, using two different measures and conceptualizations of hope (perceived hope and dispositional hope). Finally, we explore the role of sociodemographic indicators as predictors of hope and flourishing. Our results indicated that South African participants had higher levels of hope than the Spanish sample. We also found differences and commonalities in terms of endorsement of specific hope activities. The results indicated that perceived hope was a strong predictor of flourishing in both samples, supporting the idea that perceived hope may be a universal motivational need applicable across cultures. These findings highlight the need to carry out more cross-cultural studies on hope and paves the way for further cross-cultural understanding of this important human resource.
