The discovery in 1989 of sildenafil, a highly selective inhibitor of phosphodiesterase-5 (PDE-5), was the result of extensive research on chemical agents targeting PDE-5 that might potentially be useful in the treatment of coronary heart disease. Initial clinical studies on sildenafil in the early 1990s were not promising with respect to its antianginal potential. However, the incidental discovery of its antiimpotence effect led to its approval of for the treatment of erectile dysfunction. Thereafter, several reports of adverse cardiac events in patients on sildenafil raised concerns about its safety in cardiovascular disorders. Novel therapeutic indications are emerging for sildenafil with the recent discovery that PDE-5 is expressed in various other tissues such as the arterial vasculature, including pulmonary and coronary arteries, venous vasculature, skeletal muscles, platelets, and visceral and tracheobronchial muscles. In this review we briefly summarize the pharmacology of sildenafil and the current available evidence on its potential therapeutic applications in cardiovascular disorders.

Body piercing has become a way of life for many individuals. It represents freedom, as well as rebellion, and can provide shock value to the public. Often, it is used as a rite of passage from adolescence into adulthood, and can also provide a boost in self-esteem. However, body piercing may lead to non-infectious complications such as prolonged bleeding and keloid formation, while infectious complications include the transmission of blood-borne infections (e.g. human immunodeficiency virus, hepatitis B, C and D), as well as bacteremia through the site of piercing. Infective endocarditis in individuals with congenital heart defects has been identified after body piercing. Here, the first documented case is reported of mitral valve endocarditis in a previously fit and healthy young female following navel piercing.

Background: Off-pump coronary artery bypass grafting is fast-becoming a procedure of choice for elective revascularization in high-risk patients with multi-vessel coronary artery disease. However, the role of off-pump coronary artery bypass grafting for patients with acute coronary syndromes requiring emergency revascularization still requires validation. We present our experience to show the feasibility of off-pump coronary artery surgery as an emergency revascularization technique., Methods: From April 2001 to September 2003, emergency (operation within 24 hours after hospitalization) coronary artery bypass grafting without cardiopulmonary bypass (CPB) was performed in 66 patients with a mean age of (66.9 +/- 5.4) years (range 49-72 years). They presented acute coronary syndromes with 38 patients on platelet glycoprotein IIb/IIIa receptor antagonists. All patients underwent off-pump coronary artery bypass surgery via sternotomy with the intention of complete coronary revascularization., Results: An average of 2.9 grafts per patient were performed and the posterior descending artery and marginal branches of the circumflex artery were grafted in 83.3% of the patients. There were 4 events of intraoperative cardiac instability, precipitated by occlusion of right coronary artery or positioning of a cardiomegaly heart, leading to immediate conversion to CPB. The mortality rate was 3% (2/66). Two patients suffered postoperative stroke while three needed hemofiltration for acute renal failure. Post surgery elective coronary angiography (n = 46) showed no significant stenosis., Conclusion: Emergency off-pump coronary artery surgery with complete revascularization is feasible in patients with acute coronary syndrome with low morbidity and mortality and excellent early results.

Background: The ideal prosthesis for aortic valve replacement in children and young adults has not been found yet. In recent years there has been a renewed interest in the replacement of aortic valve with the pulmonary autograft owing to its advantages of lack of anticoagulation, potential for growth and excellent haemodynamic performance. The purpose of this study was to review our institutional experience at Alder Hey hospital with the Ross procedure in children and young adults., Methods: From November 1996 to September 2003, 38 patients (mean age, 13.1 +/- 5.7 years) underwent the Ross procedure for various aortic valve diseases using the root replacement technique. Clinical and echocardiographic follow-up was performed early (within 30 days), 3 to 6 months, and yearly after surgery. Medical records of all patients were reviewed retrospectively., Results: There was 1 perioperative death. The patients were followed-up for a median interval of 36 months and up to 7 years. One patient died 3 years after surgery secondary to ventricular arrhythmia with overall mortality of 5.3%. Actuarial survival at 7 years was 94 +/- 2.5% and there was 100% freedom from reoperation for autograft valve dysfunction or any other cause. Balloon dilatation was required in 2 patients for pulmonary homograft stenosis. The haemodynamics at the latest follow-up were also similar to those at the time of discharge after surgery. There was no progression in the degree of aortic regurgitation for 11 patients with trivial and 3 with mild regurgitation., Conclusion: Our experience demonstrates that Ross operation is an attractive option for aortic valve replacement in children and young adults. Not only can the operation be accomplished with a low operative risk but the valve function stays normal over a long period of time with minimal alteration in lifestyle and no need for repeated operations to replace the valve as a result of somatic growth of the children.

We report the case of a 65-year-old man who developed norepinephrine-resistant vasoplegic syndrome after elective off-pump coronary artery bypass surgery (OPCAB). The failure of norepinephrine to improve the patient's hemodynamics prompted us to start treatment with vasopressin; within 30 minutes, the hemodynamics began to improve. After 12 hours, the patient was stable enough to be weaned from the vasopressin. He was discharged from the hospital on the 10th postoperative day. To our knowledge, ours is the 1st report of vasopressin use for vasodilatory shock after OPCAB in the English-language medical literature. Herein, we discuss the pathophysiology and management of vasoplegic syndrome--which is controversial--with special emphasis on the use of vasopressin in this situation.

Background: Gastrointestinal complications after cardiac surgery remain a significant problem despite improvements in pre-operative, operative and post-operative care. The pathophysiology is uncertain, and their prevention remains suboptimal. This prospective, randomised study was designed to define the role of cardiopulmonary bypass (CPB) and cardioplegic arrest in the pathogenesis of gastrointestinal complications following coronary artery surgery., Methods and Results: Three hundred patients were prospectively randomised to (1) on-pump conventional coronary artery surgery [150 patients, 114 men, mean age 64 (45-75 years)] with mild hypothermic (32c) CPB and cardioplegic arrest of the heart or (2) off-pump surgery [150 patients, 113 men, mean age 64 (38-66) years] on the beating heart. The association of perioperative factors with gastrointestinal complications was investigated by univariate analysis. Significant variables were then included into a stepwise logistic regression model to ascertain their independent influence on the occurrence of gastrointestinal complications. There were no significant baseline differences between the groups. Eleven patients in the on-pump group and one patient in the off-pump group had post-operative gastrointestinal complications. Univariate analysis showed that CPB inclusive of cardioplegic arrest, left ventricular ejection fraction <50%, emergency surgery, prolonged aortic cross clamp and CPB time, post-operative low cardiac output syndrome, post-operative inotropic requirement, new onset atrial fibrillation (AF), excessive post-operative blood loss and redo thoracotomy <24 hours were predictors of gastrointestinal complications after coronary artery surgery (all p<0.05). However, stepwise multivariate regression analysis identified CPB inclusive of cardioplegic arrest as the only predictor of post-operative gastrointestinal complications (OR7.4; CI 3.4-17.9)., Conclusions: Cardiopulmonary bypass, inclusive of cardioplegic arrest, is the main independent predictor of post-operative gastrointestinal complications in patients undergoing coronary revascularisation.
