Encouraging results have been reported in terms of feasibility, safety, and oncologic, outcomes even for major (≥ 3 segments) or complex for location-specific (right posterior segments) laparoscopic liver resections. Despite this, technically challenging issues and advanced laparoscopic skills required to perform it have limited its use in few highly specialized centers. The aim of this study was to assess the learning curve for major-complex totally laparoscopic liver resections (TLLR) performed by a single HPB surgeon.From October 2008 to February 2012, a total of 70 TLLR were performed; 24 (33.3%) were major-complex resections. This series was divided in 2 groups according to time of operation: group A (12 cases early series) and group B (12 cases late series); perioperative outcomes were retrospectively analyzed and compared.Comparing the 2 groups, a statistically significant improvement was found in terms of operative time (P=0.017), blood loss (P=0.004), number of cases requiring a Pringle maneuver (P=0.006), and blood transfusion (P=0.001) from case number ten onward.This study shows that a minimum of 10 cases are required to obtain a significant improvement in perioperative outcome for surgeons with specific training on hepatobiliary surgery and advanced laparoscopic surgical procedures. More studies are required to clarify the minimum standard of training to perform safely this kind of advanced laparoscopic liver surgery on a large scale.

The closure of atrial septal defects through right-sided limited posterior thoracotomy has been well established in selected subsets. We present a case of large ostium secundum atrial septal defect, pulmonary valvar stenosis, absent right superior vena cava, and isolated left superior vena cava draining to right atrium via coronary sinus. The child successfully underwent total correction through limited posterior thoracotomy with necessary modifications of intraoperative steps.

Background We experienced a rare case of benign arterial stricture of the right posterior hepatic artery (RPHA) caused by atherosclerosis in a patient with hilar cholangiocarcinoma. Case presentation A 75-year-old man was referred to our hospital for the detailed investigation of serum hepatobiliary enzyme elevation. The patient had a history of hypertension, type 2 diabetes mellitus, and an operative history of coronary artery bypass grafting 10 years before. Endoscopic retrograde cholangiography found strictures of the right and left hepatic ducts with involvement of right anterior and posterior bile ducts. Adenocarcinoma was evident by brush cytology. We diagnosed these findings as hilar cholangiocarcinoma and planned left trisectionectomy including bile duct reconstruction. Although the tumor and RPHA were not adjacent, preoperative multidetector computed tomography revealed a stricture of the RPHA that was 5.6 mm in length. We suspected that atherosclerosis caused the stricture, and we performed digital subtraction angiography and intravascular ultrasonography that showed stricture of the RPHA accompanied by thick plaques in the arterial wall. We placed a bare-metal stent in the RPHA and then performed left trisectionectomy. Since this patient developed bile leakage postoperatively, percutaneous drainage was performed. The bile leakage was successfully controlled, and the patient was discharged 3 months after surgery. Unfortunately, 4 months after hepatectomy, he was re-hospitalized with multiple pyogenic liver abscesses. We performed intensive multimodal treatment for the liver abscesses and stabilized the disease; however, we eventually lost this patient due to liver failure 14 months after surgery. Conclusion To the best of our knowledge, there is no previous literature on atherosclerosis of the RPHA, which was evident preoperatively in our case. Because arterial complications may lead to critical biliary complications in patients who undergo left trisectionectomy, we first performed prophylactic arterial stent placement. We speculate that existing chronic microscopic injury of the peribiliary plexus might have caused the liver abscesses. We successfully diagnosed atherosclerosis of the RPHA preoperatively. However, further investigation of patients is warranted to determine if left trisectionectomy is contraindicated in these patients.

The contents of working memory (WM) can affect the subsequent visual search performance, resulting in either beneficial or cost effects, when the visual search target is included in or spatially dissociated from the memorized contents, respectively. The right dorsolateral prefrontal cortex (rDLPFC) and the right posterior parietal cortex (rPPC) have been suggested to be associated with the congruence/incongruence effects of the WM content and the visual search target. Thus, in the present study, we investigated the role of the dorsolateral prefrontal cortex and the PPC in controlling the interaction between WM and attention during a visual search, using repetitive transcranial magnetic stimulation (rTMS). Subjects maintained a color in WM while performing a search task. The color cue contained the target (valid), the distractor (invalid) or did not reappear in the search display (neutral). Concurrent stimulation with the search onset showed that relative to rTMS over the vertex, rTMS over rPPC and rDLPFC further decreased the search reaction time, when the memory cue contained the search target. The results suggest that the rDLPFC and the rPPC are critical for controlling WM biases in human visual attention.

On June 5, 1968, having won the Democratic Party presidential primary in California, Senator Robert F. Kennedy delivered a victory speech to supporters at the Ambassador Hotel in downtown Los Angeles. Just after 12:15 am (Pacific daylight savings time), a lone assassin shot Kennedy 3 times at point-blank range. One of the bullets struck Kennedy in the right posterior auricular region. Within the ensuing 26 hours, Kennedy was transported to 2 hospitals, underwent emergency surgery, and eventually died of severe brain injury. Although this story has been repeated in the press and recounted in numerous books, this is the first analysis of the senator’s injuries and subsequent surgical care to be reported in the medical literature. The authors review eyewitness reports on the mechanism of injury, the care rendered for 3 hours prior to the emergency craniotomy, the clinical course, and, ultimately, the autopsy.

Background and study aims Evaluation of longitudinal tumor extent is indispensable for curative surgical treatment of extrahepatic cholangiocarcinoma. The aim of this study was to evaluate the usefulness and feasibility of cholangioscopic-guided mapping biopsy using a newly developed peroral digital cholangioscope, SpyGlass DS (SpyDS), for preoperative evaluation of extrahepatic cholangiocarcinoma. Patients and methods Thirteen patients (mean age, 75 years; male 10, female 3) with extrahepatic cholangiocarcinoma who underwent cholangioscopic-guided mapping biopsy using SpyDS for preoperative evaluation were included in this study. Successful cholangioscopic-guided mapping biopsy was defined as the acquisition of specimens sufficient for histopathological diagnosis. Results The mean number of biopsies was 5 per patient. The overall success rate for cholangioscopic-guided mapping biopsy was 88 % (59/67). The success rate for cholangioscopic-guided mapping biopsy from the confluence of the right and left hepatic ducts was 89 %, that from the B4 confluence was 93 %, that from the confluence of the right anterior and right posterior segmental ducts was 86 %, that from the intrapancreatic common bile duct was 67 %, and that from the main lesion was 100 %. The overall diagnostic accuracy of longitudinal tumor extent at the hepatic side, the duodenal side and overall by cholangioscopic findings and mapping biopsy, was 88 % (7/8), 88 % (7/8) and 88 % (7/8), respectively. Assessment according to location of the main lesion revealed that diagnostic accuracy in the patients with distal bile duct carcinoma was 100 % (5/5) and that in patients with perihilar bile duct carcinoma was 66 % (2/3). Complications after the procedure did not occur in any patients. Conclusions Cholangioscopic-guided mapping biopsy using SpyDS is thought to be feasible for preoperative evaluation of extrahepatic cholangiocarcinoma.

Background In Parkinson's disease (PD), skill retention is poor, even when acquisition rate is generally preserved. Recent work in normal subjects suggests that 5 Hz-repetitive transcranial magnetic stimulation (5Hz-rTMS) may induce phenomena of long-term potentiation at the cortical level. Objective/hypothesis We thus verified whether, in PD, 5Hz-rTMS enhances retention of a visuo-motor skill that involves the activity of the right posterior parietal cortex. Methods A group of patients with PD was tested in two two-day sessions, separated by one week (treatment and placebo sessions). The first day of each session, they learned to adapt their movements to a step-wise 60° visual rotation. Immediately after the task, either real 5Hz-rTMS (treatment) or sham (placebo) stimulation was applied over the right posterior parietal cortex (P6). Retention of this motor skill was tested the following day. Results In patients with PD, adaptation achieved at the end of training was comparable in the treatment and placebo sessions and was similar to that of a group of age-matched controls. However, retention indices tested on the following day were significantly lower in the placebo compared to the treatment session in which retention indices were restored to the level of the controls. Importantly, reaction and movement time as well as other kinematic measures were the same in the treatment and placebo sessions. Conclusion These results suggest that rTMS applied after the acquisition of a motor skill over specific areas involved in this process might enhance skill retention in PD.

The posterior right temporo-parietal junction (pRTPJ) is a key brain region representing other's mental status. Despite reports of atypical activation at pRTPJ during mentalizing in individuals with autism spectrum disorder (ASD), the intrinsic functional connectivity (iFC) of the pRTPJ remains under-investigated. We examined whether boys with ASD show altered resting-state iFC of the pRTPJ, and whether atypical iFC of the pRTPJ is associated with social deficits in ASD in a sample of 40 boys with high-functioning ASD (aged 9–17 years, mean age, 12.38 ± 2.17; mean IQ, 105.60 ± 16.06) and 42 typically developing (TD) boys (aged 9–17 years, mean age, 11.64 ± 2.71; mean IQ, 111.29 ± 13.45). Both groups received resting-state fMRI assessment after imaging data quality control for in-scanner head motion and spatial coverage. Seed-based approach was used to investigate iFC of the pRTPJ. TD and ASD boys demonstrated a resting-state pRTPJ iFC pattern comparable to the known spatial involvement of the default-mode network. Boys with ASD showed pRTPJ hyperconnectivity relative to TD boys in the right ventral occipito-temporal cortex. This atypically increased iFC in the ASD group was positively correlated with social deficits assessed by the Chinese version of the Autism Diagnostic Interview-Revised and the Social Responsive Scale. Our findings provide empirical support for functional “dysconnectivity,” that is, atypical functional integration among brain regions, as an integral component of the atypical neurobiology of ASD. Background Mismatch negativity (MMN) is a typical event-related potential component reflecting pre-attentive processing. MMN impairment, especially reduced duration-related MMN (dMMN), has been suggested as a potential predictive biomarker for the onset of schizophrenia. Objective This study attempts to manipulate specific MMN activities using advanced neuroimaging-guided intermittent theta-burst stimulations (iTBS), which will be helpful to uncover the sources of MMN generation and contribute to the development of new clinical treatments. Methods Twenty-four healthy volunteers were recruited and participated two-session modulations consisting of active and sham iTBS. ITBS was precisely delivered over individual right posterior superior temporal cortex (pSTG). Before and after each iTBS session, two MMN components evoked by duration and frequency deviants were quantified respectively. Results A significant interaction of time and iTBS was observed on dMMN amplitudes, but not frequency-related MMN amplitudes. dMMN only decreased after active precise iTBS intervention, but did not after sham iTBS. The post effect of iTBS on dMMN was found in 16 of 20 subjects, suggesting a robust effect even at individual level. Furthermore, sLORETA analysis showed that the lateralization of STG activation was reversed after the active iTBS. Conclusions We applied a precise strategy for neuroimaging-guided iTBS modulation over the right pSTG, which is promising in selectively modulating MMN for specific deviants.

Objective To investigate the effects of Uygur sand therapy on the mechanical properties of the femur bone of osteoarthritic rabbits. Methods Sixteen rabbits were injected with papain in the right posterior femoral articular cavity on the first, fourth and seventh day to establish the osteoarthritis (OA) rabbit model. Animals were divided into the experimental group and control group (8 rabbits each). The experimental group was treated with sand therapy, and the control group received no sand therapy treatment. Computed tomography (CT) scanning was used to collect the data of the femur before modeling, after modeling and 14 and 28 days after sand treatment. A 3D model of the femur was generated with the MIMIC software the bone layer was divided according to the different gray values and the change of the bone volume was analyzed. The body mesh is divided, and the material properties are given, then the three-point bending simulation is performed in Ansys. Additionally, the three-point bending test was performed on all the rabbits' femur to obtain the deflection and maximum stress values. And the effects of the sand treatment on the volume and mechanical properties of the bone were analyzed. Finally, the simulation results are compared with the experimental results, and the effects of sand treatment on the volume and mechanical properties of the bone are analyzed. Results (1) there is a tendency in the control group to convert the hard bone into dense bone and soft bone, while in the experimental group, the soft bone is converted into dense bone and hard bone obviously; (2) the morphological parameters of the experimental group are lower than those of the control group, whereas the maximum load, maximum normal stress, maximum shear stress of the experimental group are higher than those of the control group. (3) The mechanical test of three-point bending test was carried out using the three dimensional finite element model of rabbit femur. Conclusion The sand therapy has positive effects on the volume distribution of bone layer and the mechanical properties of the femur of adult osteoarthritic rabbits.

We investigated the role of reference electrode placement (ipsilateral v contralateral frontal pole) on conjunction visual search task performance when the transcranial direct current stimulation (tDCS) cathode is placed over right posterior parietal cortex (rPPC) and over right frontal eye fields (rFEF), both of which have been shown to be causally involved in the processing of this task using TMS. This resulted in four experimental manipulations in which sham tDCS was applied in week one followed by active tDCS the following week. Another group received sham stimulation in both sessions to investigate practice effects over 1 week in this task. Results show that there is no difference between effects seen when the anode is placed ipsi or contralaterally. Cathodal stimulation of rPPC increased search times straight after stimulation similarly for ipsi and contralateral references. This finding does not extend to rFEF stimulation. However, for both sites and both montages, practice effects as seen in the sham/sham condition were negated. This can be taken as evidence that for this task, reference placement on either frontal pole is not important, but also that care needs to be taken when contextualizing tDCS "effects" that may not be immediately apparent particularly in between-participant designs.

Background To compare the surgical outcomes of major laparoscopic liver resection (LLR) and open liver resection (OLR) for hepatocellular carcinoma (HCC). Methods We retrospectively reviewed the medical records of 177 patients who underwent major liver resection for HCC between January 2004 and June 2015. We divided the 177 patients into two groups according to the type of procedure: major LLR (LLR group; n = 67) and major OLR (OLR group; n = 110). Results Procedures in the LLR group were right hepatectomy (30 patients), right posterior sectionectomy (28), left hepatectomy (11), right anterior sectionectomy (6), extended right hepatectomy (6), and central bisectionectomy (2). Tumor size was greater in the OLR group than in the LLR group (6.3 ± 3.8 vs 4.1 ± 2.4 cm; P = 0.016). The mean indocyanine green retention rate at 15 min (P = 0.698) and serum α-fetoprotein (P = 0.186) were similar in both groups. The mean operation time was longer in the LLR group (416.6 ± 166.9 vs 332.5 ± 105.4 min; P = 0.002). Blood loss (P = 0.319), transfusion rate (P = 0.260), and R0 rate (P = 0.255) were similar in both groups. Hospital stay was shorter (11.3 ± 8.3 vs. 18 ± 21.4 days; P = 0.007) and the complication rate was lower (20.5% vs. 38.7%; P = 0.005) in the LLR group. The 5-year overall survival (77.3% vs 60.2%; P = 0.087) and disease-free survival (50.8% vs 40.1%; P = 0.139) rates were comparable in both groups. Conclusion Major LLR of HCC is feasible and oncologically safe when performed by experienced surgeons. Further refinements of the surgical technique are needed to reduce operation time.

Background: People with anorexia nervosa (AN) have difficulties in a wide range of social-emotional processes. Previous work suggests atypical involvement of the prefrontal cortex (PFC), amygdala, insula, and fusiform gyri during social-emotional processing in AN. Methods: Twenty women with AN and twenty healthy comparison (HC) women were presented with happy, fearful, and neutral faces during a functional magnetic resonance imaging study. Group differences were investigated in the following regions of interest: lateral PFC, amygdala, insula, and fusiform gyri. Results: The HC participants showed significantly increased recruitment of the ventrolateral PFC and amygdala in the fearful > neutral contrast relative to the AN participants. The AN participants showed a significantly increased recruitment of a small cluster in the right posterior insula in the happy > neutral contrast. Conclusions: These findings are in line with the hypothesis that people with AN have a blunted response to negative and atypical exaggerated response to positive emotionally provoking stimuli.

The “5-to-7-year shift” refers to the remarkable improvements observed in children’s cognitive abilities during this age range, particularly in their ability to exert control over their attention and behavior—that is, their executive functioning. As this shift coincides with school entry, the extent to which it is driven by brain maturation or by exposure to formal schooling is unclear. In this longitudinal study, we followed 5-year-olds born close to the official cutoff date for entry into first grade and compared those who subsequently entered first grade that year with those who remained in kindergarten, which is more play oriented. The first graders made larger improvements in accuracy on an executive-function test over the year than did the kindergartners. In an independent functional MRI task, we found that the first graders, compared with the kindergartners, exhibited a greater increase in activation of right posterior parietal cortex, a region previously implicated in sustained attention; increased activation in this region was correlated with the improvement in accuracy. These results reveal how the environmental context of formal schooling shapes brain mechanisms underlying improved focus on cognitively demanding tasks.

Current literature suggests that right hemisphere lesions produce predominant spatial-related dyslexic error in English speakers. However, little is known regarding such lesions in Chinese speakers. In this paper, we describe the dyslexic characteristics of a Chinese-English bilingual patient with a right posterior cortical lesion. He was found to have profound spatial-related errors during his English word reading, in both real and non-words. During Chinese word reading, there was significantly less error compared to English, probably due to the ideographic nature of the Chinese language. He was also found to commit phonological-like visual errors in English, characterized by error responses that were visually similar to the actual word. There was no significant difference in visual errors during English word reading compared with Chinese. In general, our patient's performance in both languages appears to be consistent with the current literature on right posterior hemisphere lesions. Additionally, his performance also likely suggests that the right posterior cortical region participates in the visual analysis of orthographical word representation, both in ideographical and alphabetic languages, at least from a bilingual perspective. Future studies should further examine the role of the right posterior region in initial visual analysis of both languages.

Some patients have P6a running on the ventral side (Ventral-P6a), relative to the right hepatic vein (RHV). Forty-one patients who underwent left trisectionectomy or central bisectionectomy for biliary cancer were enrolled. We compared the anatomical features using 3D images and surgical outcomes between patients with Ventral-P6a (n = 17) and those with P6a running on the dorsal side relative to the RHV (Dorsal-P6a; n = 25). Purpose To evaluate images of non-contrast-enhanced 3D MR portography within a breath-hold (BH) using compressed sensing (CS) compared to standard respiratory-triggered (RT) sequences. Materials and methods Fifty-nine healthy volunteers underwent MR portography using two sequences of balanced steady-state free-precession (bSSFP) with time-spatial labeling inversion pulses (Time-SLIP): BH bSSFP-CS and RT bSSFP. Two radiologists independently scored the diagnostic acceptability to delineate the portal branches (MPV: main portal vein; RPV: right portal vein; LPV: left portal vein; RPPV: right posterior portal vein; and P4 and P8: portal branch of segment 4 and segment 8, respectively) and the overall image quality on a four-point scale. We assessed noninferiority of BH bSSFP-CS to RT bSSFP. For quantitative analysis, vessel-to-liver contrast (Cv-l) was calculated in MPV, RPV and LPV. Results BH bSSFP sequence was successfully performed with a 30-second acquisition time. The diagnostic acceptability scores of BH bSSFP-CS compared with RT bSSFP were statistically noninferior: MPV (95% CI for score difference of Reader 1 and Reader 2, respectively: [− 0.16, 0.06], [− 0.05, 0.02]), RPV ([− 0.00, 0.11], [− 0.01, 0.08]), LPV ([− 0.03, 0.10], [− 0.10, 0.03]), RPPV ([− 0.03, 0.10], [− 0.05, 0.05]), P4 ([− 0.13, 0.34], [− 0.28, 0.21]) and P8 ([− 0.21, 0.11], [− 0.25, − 0.02]). However, the overall image quality of BH bSSFP-CS did not show noninferiority ([− 0.61, − 0.24], [− 0.54, − 0.17]). Lone standing abutments or pier abutments have usually been restored with a conventional fixed partial denture (FPD) using a rigid connector between the pontics and the retainers. However, such fixed and rigid restorations have been associated with a higher rate of debonding, microleakage, caries, etc., The aim of the modern day prosthodontics is the preservation of what remains as given by M.M Devan. As a result, a non rigid connector (NRC) may be the connector of choice in situations of pier abutment. A NRC acts as a stress breaker so that the anterior and posterior segments can move independent of each other. This case report presents a simple method of rehabilitating a patient with a pier abutment in the upper right posterior region of the mouth. Rehabilitation was done by FPD with an inverted key-keyway type NRC. Follow up was done up to 11 months.

Background: To analyze the differences in perioperative outcomes between laparoscopic formal right hepatectomies (RH) and laparoscopic anatomical posterosuperior (PS) resections, including segmentectomies in PS segment SVII and right posterior sectionectomies (segment VI and VII resection). Methods: A retrospective analysis of all patients undergoing laparoscopic formal RHs and anatomical PS resections, including segmentectomies in PS segment SVII and right posterior sectionectomies (segment VI and VII resection), between January 2010 and August 2017 was performed. The two groups were compared in terms of patients’ characteristics, intraoperative parameters, and short-term outcomes. Results: Sixty-eight patients were included of which 32 RHs and 36 anatomical PS resections. In the PS resection group, 18 had a segmentectomy of segment VII and 18 had a bisegmentectomy of both segments VI and VII. Patients’ preoperative data were comparable. This paper reports for the first time in Colombia, two myasis cases caused by Cochliomyia hominivorax in stray dogs. Clinical examination of both patients revealed fever and a large wound at the palpebral level on the right side, and multiple perforating wounds in the right posterior limb where larvae were found. The treatment applied was the manual removal of larvae in the dogs, and subsequent eye enucleation in one dog due to the total loss of the upper right eyelid and the severe mechanical damage that the cornea received. Treatment was reinforced with repellents applied around the wounds and antibiotics. This finding of Cochliomyia hominivorax in stray dogs in Colombia raises concerns about the potential of dogs serving as a reservoir for human infection in the region.

Methods: 25 human fresh livers were obtained after autopsy and studied by corrosion cast method. Polymeric granules of butyl butyrate were dissolved in acetone and 20% homogenous solution was made. Solution was injected into portal vein and the injected liver was placed in 10 % formal saline for 24 hours at room temperature (20°C) for polymerization of infused butyl butyrate solution. Maceration of liver tissue achieved by wholeorgan immersion in 1.8 N KOH solution at 68°C for 24 hrs. Each cast thus obtained was preserved in glycerin and details were studied. Results: The length of the right portal vein varies 0.5 to 1.8 cm (1.2 cm). The right portal vein bifurcated into second order branches right anterior portal vein (RAPV) & right posterior portal vein (RPPV) in 87 % of the cases, while trifurcated in rest of 13 % of cases. The angle between the anterior and posterior division ranged from 58°-95 °. Anterio-superior (P8) branch shown three type of ramification Bifurcation type (72 %), P8one pedicle type (8 %) and P8trifurcation type (20%). Anterio-inferior (P5 max ) branch shown the three type of ramification pattern P5 -common type (72 %), P5 – P8 anterior type (28 %) but P5 – P8 posterior type was not observed. Right Posterior Portal Vein has shown three types of ramification pattern Type I-Fan shaped (64%),Type II (28 %) & Type III-Trifurcation type (8%). Conclusions: The findings of present study on hepatic vasculature have immense importance in the field of hepato-biliary surgeries like hepatic resection, segmentectomy and liver transplantation.

However, tongues reconstructed with RFFF occasionally need revision surgery because inadequate defect measurement at primary surgery can lead to bulkiness and limited movement of reconstructed tongue. In this case, the patient underwent partial glossectomy and RFFF reconstruction for treatment of tongue cancer five years prior. We could not make a lower denture for the patient, because the alveolo-lingual sulcus of tongue was almost lost. So we performed vestibuloplasty with a modified Kazanjian method on the lingual vestibule of the mandibular right posterior area, and defatting surgery to debulk the flap. After surgery, we observed that the color and texture of the revised tongue changed to become similar with adjacent tissue. The patient obtained a more functional and esthetic outcome. Accordingly, we present a case report with a review of relevant literature.

Bipolar depression (BD) is a common psychiatric illness characterized by deficits in emotional and cognitive processing. Abnormalities in the subregions of the insula are common findings in neuroanatomical studies of patients with bipolar disorder. However, the specific relationships between morphometric changes in specific insular subregions and the pathogenesis of BD are not clear. In this study, structural magnetic resonance imaging (MRI) was used to investigate gray matter volume abnormalities in the insular subregion in 27 patients with BD and in 27 age and sex-matched controls. Using DARTEL (diffeomorphic anatomical registration through exponentiated lie algebra) for voxel-based morphometry (VBM), we examined changes in regional gray matter volumes of the insula in patients with BD. As compared with healthy controls, the BD patients showed decreased gray matter volumes in the right posterior insula and left ventral anterior insula and increased gray matter volumes in the left dorsal anterior insula. Consistent with the emerging theory of insular interference as a contributor to emotional-cognitive dysregulation, the current findings suggest that the insular cortex may be involved in the neural substrates of BD.

Depersonalisation disorder (DPD) is characterised by a sense of unreality about the self and the world. Research suggests altered autonomic responsivity and dysfunction in prefrontal and temporal lobe areas in this condition. We report the first structural magnetic resonance imaging study of 20 patients with DPD and 21 controls using the FreeSurfer analysis tool employing both region-of-interest and vertex-based methods. DPD patients showed significantly lower cortical thickness in the right middle temporal region according to both methods of analysis. The vertex-based method revealed additional differences in bilateral temporal lobes, inferior frontal regions, the right posterior cingulate, and increased thickness in the right gyrus rectus and left precuneus. Clinical severity scores were negatively correlated with cortical thickness in middle and right inferior frontal regions. In sum, grey matter changes in the frontal, temporal, and parietal lobes are associated with DPD. Further research is required to specify the functional significance of the findings and whether they are vulnerability or disease markers.

Right posterior sector (RPS) grafts have been used to overcome graft size discrepancies, the major concern of living donor liver transplantation. Previous studies have reported the volumetry-based selection of RPS grafts without anatomical exclusion. We reviewed our data and established selection criteria for RPS grafts. The procurement of RPS grafts [conventional (n = 3) and extended (n = 5)] was performed for 8 of 429 recipients at our center. Extended RPS grafts contained the drainage area of the right hepatic vein. The mean graft weight (GW) according to 3-dimensional computed tomography volumetry was 488 g, and the GW/standard liver weight (SLW) ratio was 42.6%. The mean actual GW was 437 g, and the GW/SLW ratio was 38.4%. One donor exhibited standard bifurcation of the right portal vein (PV) and the left PV, and 2 donors exhibited trifurcation of the left PV, the right anterior portal vein (APV), and the posterior PV. The remaining 5 donors exhibited APV branching from the left PV, which is the most suitable anatomy for RPS grafts. Two recipients died of sepsis or small-for-size graft syndrome. One underwent retransplantation because of an intractable bile leak and fibrosing cholestatic hepatitis. Intractable bile duct (BD) stenosis developed in 4 of the 6 survivors. In conclusion, with the significant complications and potential concerns associated with RPS grafts, these grafts should be used very rarely and with extreme caution. Donors with the standard bifurcation of the PV and the posterior BD running through the dorsal side of the posterior PV are not suitable candidates for RPS grafts. Introduction: Recently, biologic agents have been added to bone graft materials in sinus-augmentation surgeries to accelerate the rate of regeneration. One such agent is recombinant human bone morphogenetic protein-2 (rhBMP-2) using an absorbable collagen sponge (ACS) as a carrier. The aim of this study was to explore whether FoF was associated with variability in both leg and trunk movements during gait in community-dwelling elderly. Ninety-three elderly people participated in this study. Each participant was categorized into either Fear or No-Fear group on the basis of having FoF. The participants walked 15m at their preferred speed. The wireless motion recording sensor units were attached to L3 spinous process and right posterior surface of heel during gait. Gait velocity, stride time and stride length were calculated. Variability in lower limb movements was represented by coefficient of variation (CV) of stride time. Trunk variability was represented by autocorrelation coefficients (AC) in three directions (vertical: VT, mediolateral: ML and anteroposterior: AP), respectively. Gait parameters were compared between groups, and further analyses were performed using generalized linear regression models after adjustment of age, sex, fall experience, height, weight, and gait velocity. Although gait velocity, mean stride time and stride length did not differ significantly between groups, stride time CV and all ACs were significantly worse in the Fear group after adjustment for variables, even including gait velocity (stride time CV: p=0.003, β=-0.793; AC-VT: p=0.011, β=0.053; AC-ML: p=0.044, β=0.075; AC-AP: p=0.002, β=0.078). Our results suggest that fear of falling is associated with variability in both leg and trunk movements during gait in community-dwelling elderly. Further studies are needed to prove a causal relationship.

Complete mediastinal plunging thyroid gland is a rare entity, accounting for 1% of all mediastinal tumours, particularly posterior mediastinal tumour much more rare. We would like to present, a 30-year-old lady presented to ENT department with neck swelling and its further investigation shown to be giant posterior mediastinal mass. Thyroid function tests were normal. CT scans of the neck and chest revealed a large right mediastinal mass compressing the trachea from the right side and extending to the superior part of the posterior mediastinum with enlarged right thyroid gland in the cervical position. Midline extended sternotomy was done for complete surgical excision of the mass along with right hemi thyroidectomy. It was well circumscribed, capsulated, multinodular firm inconsistency, and vascular. Histopathology revealed thyroid tissue negative for malignancy. Giant plunging thyroid in the mediastinum is very rare. It should be differentiated from other mediastinal mass. The plunging goiter in posterior mediastinum is surgically challenging. Transsternal surgical removal is the treatment of choice in such cases.

Objectives: The aim of this case report is to explain further about radiograph on impacted right maxillary third molar and canine using CBCT 3D. Case Report: A 18 year old female patient came with swelling on her upper right posterior gum. The condition was associated with pus contained in the gum. Physical examination revealed no exact abnormalities condition on her teeth. Panoramic radiograph examination showed radiopaque image surrounded by radiointermediate image in maxillary sinus. The result of 3D CBCT radiograph exhibited radioopaque image resembling tooth 18 surrounded by unilocular radiointermediate filling 2/3 maxillary sinus with well-defined border. Conclusion: CBCT is a supported examination which may help in establishing diagnosis more accurately.

The authors report the case of a 75-year-old man with an atrophic right posterior mandible who underwent bone augmentation through guided bone regeneration with a preshaped titanium mesh adapted on a stereolithographic model of the patient's jaw. The graft volume was simulated with a light-curing resin. The actual site was grafted with a mixture of autogenous and equine-derived bone. Five months later, the mesh was retrieved, three cylindrical implants were positioned, and a bone biopsy was collected for histomorphometric analysis. A provisional prosthesis was delivered three and a half months later. Definitive rehabilitation was accomplished after one additional month. The graft allowed for effective bone formation (newly formed bone, residual biomaterial, and medullar spaces were, respectively, 39%, 10%, and 51% of the core volume). The patient has functioned successfully throughout six and a half years of follow-up. Using the preshaped titanium mesh in association with the enzyme-treated equine bone substitute provided effective bone regeneration. Radiofrequency (RF) assisted perforation of the atrial septum was performed successfully in three infants using a 0.035΂ RF wire deployed through a Williams right posterior catheter. Balloon atrial septoplasty was performed over the 0.035΂ RF wire in two of them, shortening the procedural time.

While neuroimaging data can provide valuable phenotypic information to inform genetic studies, the opposite is also true: known genotypes can be used to inform brain connectivity patterns from fMRI data. Here, we propose a framework for genetically informed group brain connectivity modeling. Subjects are first stratified according to their genotypes, and then a group regularized regression model is employed for brain connectivity modeling utilizing the time courses from a priori specified regions of interest (ROIs). With such an approach, each ROI time course is in turn predicted from all other ROI time courses at zero lag using a group regression framework which also incorporates a penalty based on genotypic similarity. Simulations supported such an approach when, as previously studies have indicated to be the case, genetic influences impart connectivity differences across subjects. The proposed method was applied to resting state fMRI data from Schizophrenia and normal control subjects. Genotypes were based on D-amino acid oxidase activator (DAOA) single-nucleotide polymorphisms (SNPs) information. With DAOA SNPs information integrated, the proposed approach was able to more accurately model the diversity in connectivity patterns. Specifically, connectivity with the left putamen, right posterior cingulate, and left middle frontal gyri were found to be jointly modulated by DAOA genotypes and the presence of Schizophrenia. We conclude that the proposed framework represents a multimodal analysis approach for incorporating genotypic variability into brain connectivity analysis directly.

Dental amalgam is the most thoroughly researched and tested restorative material, which has been used successfully in dentistry for more than a century. Despite its poor esthetic and lack of adhesion, it is still one of the most widely used restorative materials in dental practice. This case report describes a novel clinical technique of fabricating an inlay using silver amalgam for restoring a large cervical abrasion cavity. A patient with large and deep cervical abrasion cavity in relation to mandibular right posterior region was reported. The impression of the abrasive cavity was done using rubber base impression material. Later, cast was fabricated, amalgam was condensed into the cavity, contoured and inlay was prepared. Fit of the inlay was checked on the patient, thereafter it was finished and polished and cemented using luting glass ionomer cement.

Summary This study examined the influence of narrative instructions on the occlusal contact area, occlusal contact point and masticatory muscle activities in normal subjects. Twelve healthy men and 12 healthy women with no more than one missing tooth per quadrant participated. Surface EMG was recorded from the masseter and temporal muscle. As a control measurement, intercuspal position was maintained to produce a habitual clenching record (NCR) while the occlusal contact area and occlusal contact point was recorded by means of silicone material. Subsequently, the occlusal contact area was recorded with the narrative instruction for minimum clenching record (MCR), light clenching record (LCR) and strong clenching record (HCR). INTRODUCTIONRight posterior segmental graft (RPSG) is an alternative procedure for living-donor liver transplantation (LDLT). Although the first case of RPSG was reported in 2001, it has not been disseminated because of the lack of popularity, technical concerns, and surgical difficulties.PRESENTATION OF CASEA 37-year-old man with primary sclerosing cholangitis. His spouse was the only transplantation candidate, although she was ABO incompatible. Preoperative investigations revealed that left-lobe graft was insufficient for the recipient and that right-lobe graft was accompanied by donor risk. In RPSG, estimated graft-to-recipient weight ratio (GRWR) and estimated ratio of liver remnant were reasonable. In the donor operation, the right hepatic vein (RHV) and demarcation line were confirmed, and intraoperative cholangiography was performed. The cut line was carefully considered based on the demarcation line and RHV. The RPSG was harvested. Actual GRWR was 0.54. Unfortunately, this recipient showed a poor course and outcome after LDLT.DISCUSSIONSegmental branches of vessels and biliary duct may be not suitable for reconstruction, and surgeons must exercise some ingenuity in the recipient operation. Segmental territory based on inflow and that based on outflow never overlap completely, even in the same segment. The selection of RPSG based only on liver volume may be unfeasible. Liver resection should be carefully considered based on preoperative imaging, and demarcation line and RHV during surgery.CONCLUSIONRPSG is a useful tool for LDLT. However, detailed studies before surgery and careful consideration during surgery are important for RPSG harvest.

Endoscopic ultrasound (EUS) can demonstrate the detailed anatomy of the liver from the transgastric and transduodenal routes. Most of the liver segments can be imaged with EUS, except the right posterior segments. The intrahepatic vascular landmarks include the major hepatic veins, portal vein radicals, hepatic arterial branches, and the inferior vena cava, and the venosum and teres ligaments are other important intrahepatic landmarks. The liver hilum and gallbladder serve as useful surface landmarks. Deciphering liver segmentation and anatomy by EUS requires orienting the scan planes with these landmarkstructures, and is different from the static cross-sectional radiological images. Orientation during EUS requires appreciation of the numerous scan planes possible in real-time, and the direction of scanning from the stomach and duodenal bulb. We describe EUS imaging of the liver with a curved linear probe in a step-by-step approach, with the relevant anatomical details, potential applications, and pitfalls of this novel EUS application.

Background/Aim The Respiratory system can be affected by exposure to cold. It is well known that acute cold exposure induces asthmatic attacks. However, the influence of chronic cold environment exposure on lung perfusion and the pulmonary circulation was not studied in any previous study. Therefore this study was designed to investigates the effects of chronic cold exposure on lung perfusion using radionuclide study. Methods New Zealand White rabbits were used in these experiments. The rabbits were kept in the cold room (4 °C) for 7 weeks. Lung perfusion scintigraphy was performed at the end of this period. Each rabbit was injected with 74 MBq (2 mCi) technetium-99m macroaggregated of albumin ( 99m Tc MAA). Perfusion studies were done using Gamma camera equipped with a low energy, high resolution, parallel hole collimator interfaced with a computer. Static images were obtained 5 min after administration of the radiotracer. Static images were acquired include anterior/posterior (Ant/Post), right anterior oblique/left posterior oblique (RAO/LPO), right lateral/left lateral (RLat/LLat), right posterior oblique/left anterior oblique (RPO/LAO). Results Rabbits chronically exposed to cold had lesser lung perfusion than controls using radionuclide perfusion study. The lung counts of chronic cold exposure (4 °C) for 7 weeks on rabbit lung perfusion for 5 min was 64±4%. ( n =6, ⁎⁎⁎ P Conclusions Our results indicate that chronic cold exposure decreased pulmonary circulation and lung perfusion in normal subjects. Therefore chronic cold exposure might worsen some diseases that are affected by cold such as asthma.

Background: In hepatocellular carcinoma, anatomical resection is important because of portal spread. In right anterior sectionectomy (RAS) and right posterior sectionectomy (RPS), the right hepatic vein (RHV) may not correspond with the intersectional plane if an inferior RHV (IRHV) is present. The aim of this study was to evaluate the influence of the IRHV on the exposure of the RHV retrospectively. Methods: One hundred ninety-one patients underwent RAS or RPS by the Glissonean pedicle transection method. The calibres of the RHV and IRHV were measured and assessed the extent of exposure of RHV. Results: One hundred seventeen patients underwent RAS and 74 underwent RPS. The calibre of the RHV averaged 8.0 mm and that of the IRHV, 6.2 mm. Exposure of the RHV was divided into three groups: no exposure 31 (16.2%) (with IRHV, 20 patients; without IRHV, 11 patients), upper half exposure 49 (25.7%; with IRHV, 24; without IRHV, 25) and full exposure 111 (58.1%) (with IRHV, 16; without IRHV, 95). The effect of the IRHV on exposure of the RHV was substantial (P < 0.001). Conclusions: The IRHV can affect the course of the RHV and its exposure. Therefore, in RAS and RPS, it is important to evaluate the existence of the IRHV.

For liver surgery, it is crucial to preoperatively examine the course of the right posterior bile duct. While MR cholangiopancreatography (MRCP) can only visualize the bile ducts, 3D balanced turbo-field-echo (BTFE) sequence clearly depicts the bile ducts and portal veins as well as drip infusion CT cholangiography (DIC-CT), without contrast media. We evaluated whether BTFE could substitute for DIC-CT. Thirty patients undergoing MRCP and BTFE on 1.5-T MR and DIC-CT were evaluated. Two readers retrospectively evaluated the branching pattern (supra-type: A–C or infra-type: D–E) and scored the degree of confidence and motion artifacts using a 3-point scale for the three 2-mm-thick reconstructed images. The bile duct diameter did not differ between DIC-CT and MRCP (p = 0.07). Five patients (17%) had intrahepatic biliary dilatation (>3 mm). The A, B, C, D, and E types were diagnosed in 21, 6, 1, 1, and 1 patient, respectively (28 supra-types and 2 infra-types) on DIC-CT. For DIC-CT, MRCP, and BTFE, the mean motion artifact scores were 3.0/3.0, 2.7/2.6, and 2.9/2.8, respectively. The mean diagnostic confidence scores were 2.9/2.9, 2.4/2.4, and 2.9/2.8, respectively, with no difference between DIC-CT and BTFE. The concordance between DIC-CT and BTFE was high (infra- or supra-type: κ = 1.00/1.00, A–E: κ = 0.86/0.66), but it was poor between DIC-CT and MRCP (infra- or supra-type: κ = 0.35/−0.05, A–E: κ = 0.33/0.41) for both readers. Similar to DIC-CT, the BTFE MR sequence had high diagnostic accuracy regarding the branching pattern of the intrahepatic bile duct, especially for the supra/infraportal type.

Aim. The frequency of the Right Posterior Sectional Bile Duct (RPSBD) hump sign in cholangiogram when it crosses over the right portal vein known as Hjortsjo Crook Sign and the bile duct anatomy are studied. Knowledge of the implication of positive sign can facilitate safe resection for both bile duct and portal vein. Methods. Prospectively, we included 237 patients with indicated ERCP during a period from March 2010 to January 2015. Results. The mean age (±SD) and male to female ratio were 38.8 (±19.20) and 1 : 1.28, respectively. All patients are Arab from Middle Eastern origin, had biliary stone disease, and underwent diagnostic and therapeutic ERCP. Positive Hjortsjo Crook Sign was found in 17.7% (42) of patients. The sign was found to be equally more frequent in Nakamura’s RPSBD anatomical variant types I, II, and IV in 8.4% (20), 6.8% (16), and 2.1% (5), respectively, while rare anatomical variant type III showed no positive sign. Conclusion. Hjortsjo Crook Sign frequently presents in RPSBD variation types I, II, and IV in our patients.

Large maxillofacial defects from malignant tumor treatment are rarely rehabilitated by surgical reconstruction alone. Ameloblastic carcinoma, a rare aggressive odontogenic malignant tumor, requires wide surgical excision to gain a tumor-free margin. In the post-surgical defect, prosthetic rehabilitation is the treatment of choice to restore function and esthetics. Moreover, an intra-oral prosthesis such as an obturator restores speech, mastication and deglutition. Retention of the obturator is a major problem while rehabilitating large defects. The existing anatomical structures from the defect with the help of magnet attachments are suitable to enhance retention, stability and support of the prostheses. This case report presents a patient with an intraoral and extra-oral combination defect following surgical resection of ameloblastic carcinoma and describes the prosthetic techniques and design considerations for a magnet-retained obturator and mid-facial prosthesis. An implant-retained mid-facial prosthesis was fabricated. The retention of combined prostheses was obtained from the remaining right posterior teeth only. The patient had an unfavorable defect due to the large size and presence of scar contracture that vertically tends to dislodge the obturator. Magnet attachments were used to combine the facial and oral prosthesis, minimize the vertical dislodging forces and enhance retention. In addition, the retention was also gained from the scar band at lower border of mid-facial defect that avoided the need for more implants surgery. Magnet attachment with anatomical structure of the mid-facial defect provides an acceptable means of retention in large extraoral-intraoral combinations defects, improving the function, esthetic and the patients’ quality of life.
