Abstract

Ranolazine utilization in the management of refractory angina has been established by multiple randomized clinical studies. However, there is growing evidence showing an evolving role in the field of cardiac arrhythmias. Multiple experimental and clinical studies have evaluated the role of ranolazine in prevention and management of atrial fibrillation, with ongoing studies on its role in ventricular arrhythmias. In this review, we will discuss the pharmacological, experimental, and clinical evidence behind ranolazine use in the management of various cardiac arrhythmias.

Abstract

Amiodarone is reported to cause a wide continuum of serious clinical effects. It is often challenging to detect Amiodarone-induced pulmonary toxicity (AIPT). Typically, the diagnosis is made based on the clinical settings and may be supported by histopathology results, if available. We describe a 57-year-old patient who developed severe rapidly progressive respiratory failure secondary to AIPT with acute bilateral infiltrates and nodular opacities on chest imaging. Interestingly, Amiodarone was discontinued 3 weeks prior to his presentation. He had normal cardiac filling pressures confirmed by echocardiography. To our knowledge, this is the first case of isolated acute lung injury induced by Amiodarone, three weeks after therapy cessation, with adequate clinical improvement after supportive management and high dose steroid therapy.

ABSTRACTBackgroundData comparing MANTA to other vascular closure devices (VCDs) after TAVR is limited.MethodsWe performed a meta-analysis of all published studies reporting the outcomes of MANTA vs. other VCDs in TAVR patients. Outcomes included major and minor vascular complications, major and minor bleeding, VCD failure, blood transfusion, additional surgical or endovascular treatment, flow-limiting dissection, hematomas, pseudoaneurysm, change in hemoglobin, and length of stay (LOS). Definitions used were according to the “Valve Academic Research Consortium-2 consensus document” (VARC-2).ResultsWe included five observational studies with a total of 1,410 patients (MANTA n = 601, other VCDs = 809). Three studies compared MANTA to the Proglide device (Abbot Vascular, CA, USA), and two studies compared MANTA to the Prostar XL device (Abbott Vascular, IL, USA). The prevalence of moderate to severe calcification was higher in the MANTA group (31% vs. 21%, p= 0.01) compared with other VCDs group. During the index hospitalization, there was no difference in all major or minor complications between MANTA and other VCDs. In a sensitivity analysis, comparing MANTA to Proglide, the risk of major and minor vascular complications, major and minor bleeding, and VCD failure was similar for both devices.ConclusionIn the TAVR population, although more used in calcified vessels, the safety profile and efficacy of the new collagen-based VCD, MANTA, is similar to currently available suture based VCD’s Proglide and Prostar XL.

250. Temporal Trends and Outcomes of Transcatheter versus Surgical Aortic Valve Replacement in Patients with Prior Myocardial Infarction

Abstract

ABSTRACTBackground:There is a paucity of data regarding the comparative safety and effectiveness of transcatheter aortic valve replacement (TAVR) versus surgical aortic valve replacement (SAVR) in patients with prior myocardial infarction (MI).Methods:We queried the NIS database (2012–2016) to identify patients with a prior history of MI who underwent isolated AVR. We conducted a propensity matching analysis comparing patients with TAVR versus SAVR based on patient demographics, comorbidities and hospital characteristics. The primary outcome was in-hospital mortality.Results:Among 24,090 hospitalizations with prior MI undergoing isolated AVR, 58.5% underwent TAVR. Over the study period, there was an increase in the proportion of patients undergoing TAVR and a decrease in patients undergoing SAVR (Ptrend<0.001). After propensity matching, in-hospital mortality was not different between TAVR and SAVR (odds ratio 1.07; 95% confidence interval 0.85–1.33). Compared with SAVR, TAVR was associated with lower rates of cardiac arrest, cardiogenic shock, acute kidney injury, respiratory complications, postoperative bleeding, blood transfusion, acute stroke, vascular complications and shorter length of stay. TAVR was associated with a higher incidence of complete heart block and pacemaker implantation.Conclusion:In this nationwide observational study, there has been a steady increase in the number of TAVRs performed in patients with prior MI. Compared with SAVR, TAVR was associated with similar in-hospital mortality and a lower rate of complications and length of stay.
