To investigate the association of genetic markers in ESR1 and ESR2 with craniofacial measurements. Cross-sectional study. School of Dentistry of Ribeirão Preto, University of São Paulo. A total of 146 biologically unrelated, self-reported Caucasian Brazilians with no syndromic conditions were included. Sagittal and vertical measurements (ANB, S-N, Ptm'-A', Co-Gn, Go-Pg, N-Me, ANS-Me, S-Go and Co-Go) from lateral cephalograms were examined for craniofacial evaluation. DNA was extracted from saliva and genetic markers in ESR1 (rs2234693 and rs9340799) and in ESR2 (rs1256049 and rs4986938) were analysed by real-time polymerase chain reaction. Hardy–Weinberg equilibrium was evaluated using the Chi-square test within each marker. The associations between craniofacial dimensions and genotypes were analysed by linear regression and adjusted by sex and age. The established alpha was 5%. Individuals carrying CC in ESR1 rs2234693 had a decrease of –3.146 mm in ANS-Me (P = 0.044). In addition, rs4986938 in ESR2 was associated with S-N measurement (P = 0.009/ ß = –3.465). This marker was also associated with Go-Pg measurement, in which the CC genotype had a decrease of –3.925 mm in the length of the mandibular body (P = 0.043). The present study suggests that in ESR1 and ESR2 are markers for variations in the craniofacial dimensions. However, further research should confirm the results. [ABSTRACT FROM AUTHOR]

Introduction: Ossifying fibroma is a benign fibro-osseous lesion characterized by the formation of a cellular fibrous connective tissue stroma with cementum-like material and/or osseous components. The juvenile variant is reported to be an aggressive lesion in paediatric patients. Its poor symptomatology contrasts with its highly agressive behaviour in maxillary or mandibular region. Objective: The aim of the present study is to report a case of an atypical ossifying fibroma in association of multiple deciduous teeth. Case report: A nine years-old girl was referenced to the Oral and Maxillofacial Surgery Service at Positivo University by the Paediatric Service after radiographic confirmation of a round lesion in lower left mandible region. Imaginological examination revealed a well-defined, round, with multiple radiopaque foci associated with the roots of deciduous molars and with root of first permanent mandibular molar of the left side. Lesion embedded the root and crown of the unerupted second left premolar, suggesting that the tumour arouse from it. After incisional biopsy, histopathological analysis suggested the trabecular pattern of juvenile ossifying fibroma. Surgery was conducted through an intra-oral approach. Incision was drawn in gingiva from the left lateral inferior incisor region until the unerupted second molar region, finished with a relaxing incision. A round tumour in association with the unerupted permanent premolars, which were also extracted, was removed in its entire extent by enucleation. Histopathological analysis confirmed the previous diagnosis of juvenile trabecular ossifying fibroma. Peripheral ostectomy was conducted to diminish chances of lesion relapse. Introduction: Displaced dental implants into the maxillary sinus are relatively rare. Treatment consists of implant surgical removal. Objective: To present two case reports of the removal of implants displaced into maxillary sinus by a conservative technique. Case report: Report 1: A 44-year-old female sought for treatment complaining about headaches and recurrent sinusitis. In the anamnesis she reported she had been undergone placement of nine maxillary implants, fifteen years ago, with total loss of those elements. At the imaging exams it was possible to observe two displaced implants into the bilateral maxillary sinus, besides radiopaque image on the left maxillary sinus, compatible with maxillary sinusitis. Based on this data, the proposed treatment plan was the removal of the displaced implants through Caldwell Luc approach and sinusotomy in the left side, under general anesthesia. The patient has a follow-up of 6 months without complaints. Report 2: A 42-year-old male was referred to the Surgery and Maxillofacial Traumatology Department for implant removal from the right maxillary sinus. Patient underwent implant installation in the area around 9 months ago. In the computarized tomography it was possible to diagnose the implant migration into the right maxillary sinus. the proposed treatment plan was the removal of the displaced implant via Caldwell Luc Access by repositioning the bony window and sinusotomy. Masseter traumatic myositis chondro-ossificans (TMCO) is a rare pathological condition that causes severe mandibular function restriction. The aim of the present study is to report a TMCO case after direct masseter muscle injury and correlate it to bone and cartilage biomarkers up-regulation. Caucasian male patient, 38 years old, seeks treatment nine days after trauma with severe mouth opening limitation. Physical examination revealed a circumscribed hardened area connected to masseter muscle on the left side. Cone beam tomography and ultrasonography of masseter region were requested. There was incomplete fracture between the posterior board of inferior jaw and coronoid process as well as calcification within masseter muscle. The proposed treatment was excisional biopsy of calcification, coronoid process removal to enhance mouth opening as well as incomplete condyle fracture monitoring. Material removed was sent for histological analysis in order to confirm diagnosis. Immuhistochemistry was conducted and it was found that chondro-ossification biomarkers such as TGF-b1, Indian Hegdehog (IHH), BMP2, osteopontin (OP) and osteocalcin (OC) were up-regulated. One-year follow-up showed that the patient is stable with increased mouth opening and satisfactory jaw movements. Pathologists and maxillofacial surgeons must be aware of differential diagnosis of TMCO. Understanding cellular mechanisms of muscle tissue after trauma is also important once cellular pathway modifications leads to clinical features that differ from previously described in literature. The purpose of this cohort study was to identify associations between combined oral and bone disease phenotypes and genes present in cell regulatory pathways. The studied pathways play important roles in cellular growth, proliferation, differentiation, and homeostasis. DNA samples extracted from whole saliva of 3,912 individuals were genotyped and these data analyzed according to dental caries experience, periapical lesions, periodontitis, osteoporosis, or temporomandibular joint discomfort. Samples were obtained from the Dental Registry and DNA Repository project at the University of Pittsburgh. Twenty-seven polymorphisms in eight genes related to mTOR or endoplasmic reticulum stress pathways were selected for genotyping. Allele frequencies and Hardy-Weinberg equilibrium were calculated. Analyses were performed comparing genotypes between affected and unaffected individuals for each phenotype, as well as for the associated phenotypes combined. For all analyses, we used the software PLINK with an alpha of 0.002. Borderline associations with multiple variants of several genes were found, suggesting that both pathways may be involved in the susceptibility to multiple conditions affecting the oral cavity and bones. When combining patients that had concomitant dental caries, periodontitis, and periapical pathology, several markers in RHEB showed statistically significant association. Multiple conditions affecting bone and teeth (i.e., dental caries, periodontitis, periapical lesion formation, and osteoporosis) appear to share similar underlying genetic etiological factors, which allow us to hypothesize that instead of individually, they should be studied in conjunction in human populations. Objective Compare the anatomical dimensions of mandibular condyles with the same CBCT measurements. Methods Four landmarks were identified on the condyles of dried skulls and are measured with their corresponding areas in CBCT images: Right Width of Condyle (RW), Left Width of Condyle (LW), Right Length of Condyle (RL), and Left Length of Condyle (LL). Results With respect to direct measurements on the dry skulls, statistically significant differences were found between sexes (p < 0.001). When the real values were compared with the CBCT, it was observed that the bias depends on the magnitude of the measurement. It means that there is an underestimation for smaller values and an overestimation for larger values. Objective: The objective of the study is to evaluate bone repair in rats treated with different alendronate doses. Matherials and methods: Sixty female rats ovariectomized were randomly divided in three groups: group C (control group), group A1 (ALN/1 mg/kg), and A2 (ALN/ 3 mg/kg). Each animal received subcutaneous applications of sodium alendronate at a dose correspondent to group A1 or A2 three times a week, while the control group received 0.9% saline solution. After 4 weeks of application, a critical defect was created in the calvaria of animals of all groups. The defect was filled by particulate autogenous bone. The applications were maintained until euthanasia, which occurred 15 and 60 days after the surgical procedure. The pieces were sent for histological, histomorphometric and immunohistochemical analysis. The data were submitted to statistical analysis with significance level of 0.05. Results: The descriptive histological analysis demonstrated an increase in bone neoformation in both groups treated with alendronate when compared to the control group. The histomorphometric analysis showed an increase in the amount of neoformed bone in A1 and A2 groups when compared to group C, both at 15 days (p = 0.0002) and at 60 days (p = 0.001). In the immunohistochemical analysis, it was possible to observe a difference in immunolabeling just for Mmp2 at the time of 60 days in A1 (p = 0.001) and A2 (p = 0.023) when compared to the control group. Conclusion: Systemic delivery of alendronate, regardless of the dose, increased the amount of bone neoformation. The aim of this study was to investigate the association between condylar bone morphological characteristics with occlusal conditions. Besides the study will compare the tomography images with the real condition in 122 temporomandibular joints from 61 skulls. The occlusal conditions were evaluated by number of teeth missing, measurement of overjet and overbite, in millimeters, and presence or absence of crossbite, openbite and dental rotation. The condylar bone morphological conditions were classified in five types (normal, presence of erosion, presence of osteophytes, flattening and/or deformation). This classification was used in real skulls and in Cone Beam Computed tomography (CBCT) images. The data were submitted to statistical analysis with a level of significance of 0.05. Occlusal variables have no association to morphologic data (p > 0.05). Normal condylar bone was seen in 62 CBCT versus 53 in real skulls while morphological alterations were seen in 60 CBCT versus 67-real condyles. The clinical and tomographic measurements were compared, demonstrating an important difference in the classification demonstrating poor association between detection methods (k − 0.3, p < 0.001). The occlusal conditions appear to have no correlation with the morphological condyle conditions. The CBCT is a reliable diagnostic method, although it may present divergences of findings when compared with clinical raw examination to morphologic condylar conditions. [ABSTRACT FROM AUTHOR]

Objective: The aim of this case-control study was to investigate whether benign migratory glossitis (BMG) is associated with catechol-O-methyltransferase (COMT) and serotonin transportation gene (5HTT) polymorphisms and anxiety.Study Design: The study comprised 43 patients with BMG and 114 patients without a history of BMG. We used the Hamilton Anxiety (HAM-A) rating scale to assess each individual's anxiety. We collected DNA from buccal cells and analyzed polymorphisms of COMT and 5HTT. We conducted statistical evaluations by using SPSS software (SPSS Inc., Chicago, IL) and STATA (StataCorp, College Station, TX). Alpha value was set at 0.05.Results: Overall anxiety level was significantly higher in the case group than in the control group (P < .001). In adjusted multiple logistic regression, the COMT markers were not associated with BMG. Individuals with the CC genotype, in rs3813034 of 5HTT, presented an odds ratio (OR) of 2.85 (95% confidence interval [CI] 1.03-7.82; P = .042). Individuals with the TT genotype, in the rs1042173 of 5HTT, presented an OR of 3.77 (95% CI 1.32-10.74; P = .013). For each incremental increase in the anxiety score, there was an 8% increase in the probability of BMG (ORa=1.08; 95% CI 1.03-1.14; P = .007).Conclusions: Anxiety increases the risk of BMG. Introduction: The bichectomy has been increasingly sought in dental practice, whether due to functional or aesthetic complaints. It is considered an aesthetic and functional surgery, for improving both facial harmony and chewing. Objective: This study aimed to establish a quality of life profile of patients submitted to bichectomy surgery in a private service in Curitiba, PR, Brazil. Material and methods: The OHIP-14 was applied in a single moment in patients who underwent bichectomy in the last 6 to 18 months. Results: Thirty-six individuals of both sexes were evaluated, being 1 man and 35 women, with the median age of 31 (19-53) years. The median OHIP-14 scores were 0.00 (0-24) points. There was no association between OHIP-14 with age, postoperative complications, postoperative time or tobacco smoking (p > 0.05). Conclusion: The individuals were satisfied after performing the surgical procedure, presenting very low scores when compared to the literature. [ABSTRACT FROM AUTHOR]

Objective: The objective of the study was to evaluate the effect of orthognathic surgery on temporomandibular disorder (TMD) in patients with skeletal class III malocclusion.Study Design: Forty-seven patients undergoing surgery were evaluated by a trained examiner using Axis I of the Research Diagnostic Criteria/TMD index, consecutively, at 3 different periods: 1 week before operation (T0), 6 months after operation (T1), and 1 year after operation (T2). Bivariate analyses were performed to compare the evaluation periods (P < .05).Results: The prevalence of TMD in the sampled patients from T0 to T1 decreased from 30 (63.8%) to 22 (46.8%) (P = .021). Even in T2, the prevalence of TMD remained lower than that in T0, at 21 (44.7%) diagnosed patients (P = .049). The reported frequencies of myofascial pain and headache were lower in T1 and T2 than in T0 (P < .001). Decrease in the frequency of joint pain and joint sounds was observed only from T0 to T1 (P = .039 and P = .021, respectively). The mean maximum of mouth opening decreased from T0 to T1 (P < .001) and increased again at T2 (P < .001).Conclusions: Orthognathic surgery promoted reduction in the frequencies of myofascial pain and headache reported by patients with skeletal class III malocclusion. Objectives: TGF-β1 is a cytokine that may induce both osteoneogenesis through Runx-2 or fibrosis via the transcription of α-smooth muscle actin (α-SMA). Because it has been previously known that alendronate increases the level of TGF-β1 and that under the usual condition of bone metabolism the estrogen may prevent the fibrotic effect of TGF-β1, the aim of this study was to evaluate if alendronate alters the cellular differentiation process post calvarial surgery in estrogen-deficient specimens.Materials and methods: A transosseous defect that was 5 mm in diameter was created on the calvarium of each of 32 female rats with previous ovarian-salpingo-oophorectomy. All defects were treated with autografts, and 16 rats received the administration of 1 mg/kg of alendronate three times a week until euthanasia on the 15th and 60th day post surgery. Histomorphometric and immunohistochemical analyses of the expression of TGF-β1, estrogen receptor alpha nuclear (α-ER), α-SMA, BMPR1B, and Runx-2 were performed, and ELISA was used to measure the level of estrogen.Results: All animals demonstrated low levels of estrogen post ovarian-salpingo-oophorectomy. The histological results demonstrated larger bone matrix deposition in specimens treated with alendronate on the 15th day post surgery. The result was associated with a higher co-expression of TGF-β1, BMPR1B, and Runx-2 when compared with the control group. In addition, on the 60th day post surgery, the increase of bone matrix deposition from 15th to 60th day was discrete in specimens treated with alendronate compared with the control group. This result coincided with the intense simultaneous expression of TGF-β1, α-ER, and α-SMA, whereas the expression of BMPR1B and Runx-2 decreased.Conclusion: The prolonged administration of alendronate altered the cranial repair in ovarian-salpingo-oophorectomized specimens due to the simultaneous occurrence of low estrogen and the presence of TGF-β1+/α-ER+ inducing the presence of α-SMA+, whereas BMPR1B and Runx-2 were suppressed.Clinical relevance: The prolonged administration of alendronate alters osteoneogenesis and induces an unusual microenvironment in the bone that seems to imitate the physiological tissue damage that culminates in the loss of the functional layer of endometrium. Ameloblastoma is an aggressive odontogenic tumor which typically occurs between third and fourth decade of life that often needs resective approach. Immediate reconstruction may show better results. The treatment of multicystic ameloblastoma in the mandible being a rare case that occurred in the late second decade of life, which was surgically removed along with the affected teeth with safety margins and the region was immediately reconstructed using a vascularized graft, removed from the fibula. Its integration, in combination with osseointegrated dental implants and fixed implant-supported prostheses, restored chewing function and esthetics. After 6 years from fibular graft and 24 months of dental implants, an excellent outcome was observed, with oral health and normal functions properly restored and the immediate reconstruction of the mandible in resective cases, associated with oral rehabilitation with dental implants, may be considered a suitable treatment option. Objective: The aim of this study was to evaluate alterations in condylar positioning through submentovertex projection (Hirtz Radiographic Technique) in patients who underwent orthognathic surgery for maxillary advancement and mandibular setback with stable internal fixation. Methods: A prospective longitudinal clinical study of 40 surgical patients presenting dentofacial deformity admitted in the Oral and Maxillofacial Surgery Department of Federal University of Paraná (UFPR) in the period between March 2013 and December 2015. We performed two submentovertex digital radiographs, one 7 days before surgery and the other one 30 days after the procedure. Cephalometric tracings were made using Radiocef® Studio 2 Software and measured the intercondylar and condylar angles (right and left). Results: There was a decrease in the intercondylar angle ( p < 0.001) and an increase in condylar angles both the right and the left side ( p < 0.001) when compared with the pre and postoperative period. There was a larger increase in condylar angle on the right side in males ( p = 0.007). Conclusion: There is a tendency of decreasing of the intercondylar angle after orthognathic surgery, regardless of the alteration in the condylar angles, creating a new position of the condyle in the glenoid fossa. Patients with asymmetry may present greater alterations in the positioning of the opposite condylar to the deviation of the mandibular midline. [ABSTRACT FROM AUTHOR]
