Abstract

Endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) and fine needle biopsy (FNB) are effective techniques that are widely used for tissue acquisition. However, it remains unclear how to obtain high-quality specimens. Therefore, we conducted a survey of EUS-FNA and FNB techniques to determine practice patterns worldwide and to develop strong recommendations based on the experience of experts in the field. Methods: This was a worldwide multi-institutional survey among members of the International Society of EUS Task Force (ISEUS-TF). The survey was administered by E-mail through the SurveyMonkey website. In some cases, percentage agreement with some statements was calculated; in others, the options with the greatest numbers of responses were summarized. Another questionnaire about the level of recommendation was designed to assess the respondents' answers. Results: ISEUS-TF members developed a questionnaire containing 17 questions that was sent to 53 experts. Thirty-five experts completed the survey within the specified period. Among them, 40% and 54.3% performed 50-200 and more than 200 EUS sampling procedures annually, respectively. Some practice patterns regarding FNA/FNB were recommended. Conclusion: This is the first worldwide survey of EUS-FNA and FNB practice patterns. The results showed wide variations in practice patterns. Randomized studies are urgently needed to establish the best approach for optimizing the FNA/FNB procedures.

Abstract

Study focused to investigate the use of specific lexical items to comment on sports in social media applications. For this, the application of Facebook was taken to observe comments of Pakistani cricket supporters. The study is designed to first identify, then discuss actual and situational meanings of semantic structures that have been solely employed for cricket matches. It was possible with the adoption of the model of analyzing the semantics of lexis given by Geeraert's (2009). The project is designed on descriptive (survey). Corpus from the commentary section on Facebook was taken that comprised 4 chats on Facebook and (24) chats on sub-pages of Facebook. They were all coded from 1-24 as (C1 to C-24). The following chats have been carefully extracted to fulfill the purpose of the study in the platform groups of Facebook. They were visualized under principles to find the semantics of lexis through the proposed model (2009) of Geeraert. The findings of the project revealed that Pakistani supporters of Cricket have employed peculiar words and structures that verified specific interactions in the sports domain. Following phrases and words were possible to get interpreted with defined context. This has been concluded in the present study that chats on the Facebook application by Pakistani supporters of cricket have used the majority of sentences that were aligned to sports' register and could only be understood as well employed by fans of cricket. This contribution tends to suggest in the end that following the framework of the following study, many studies on the relationship of sports with the use of language can be presented by research scholars in the future from different parts of the World with the employment of theory on the register of language by Halliday (1978). Abstract

Simulations of ocean currents using numerical circulation models are becoming increasingly realistic. At the same time, these models generate increasingly large volumes of model output data. These trends make analysis of the model data harder for two reasons. First, researchers must use high-performance data-analysis clusters to access these large data sets. Second, they must post-process the data to extract oceanographically-useful information. Moreover, the increasing model realism encourages researchers to compare simulations to observations of the natural ocean. To achieve this task model data must be analyzed in the way observational oceanographers analyze field measurements; and, ideally, by the observational oceanographers themselves. The OceanSpy package addresses these needs.

249. The Superficial "Swing-Down" Quadriceps Tendon Autograft Is a Viable Option for Medial Patellofemoral Ligament Reconstruction: A Systematic Review.

Abstract

Purpose: The purpose of this systematic review is to ascertain the reported clinical outcomes and complication profiles of medial patellofemoral ligament (MPFL) reconstruction performed using a superficial "swing-down" quadriceps tendon autograft.Methods: Three databases (PubMed, EMBASE, and MEDLINE) were searched from January 1, 2000, to April 06, 2020. Keywords used in the search included: "medial patellofemoral ligament" and "reconstruction". After screening based on inclusion and exclusion criteria, patient demographics, graft type, outcomes, and complications were extracted. Methodological Index for Non-Randomized Studies criteria were used to assess the quality of each included study.Results: Eleven studies were included, comprising data from 226 patients with mean follow up ranging from 12 to 38 months. All reconstructions used a superficial "swing-down" quadriceps tendon autograft fixed at 20-30 degrees of knee flexion where reported. Eight of eleven studies reported significant improvement in patient reported outcomes such as Kujala score, Lysholm score, and Tegner score. The mean preoperative Kujala score ranged from 35.8 to 82.1 (167 patients), while the mean postoperative Kujala score ranged from 88.4 to 94.8 (197 patients). The mean of the preoperative Lysholm score ranged from 43.3 to 79.3 (77 patients), while the mean of the postoperative Lysholm score ranged from 81.9 to 90.9 (99 patients). The I2 statistic for Lysholm and Kujala scores was 94% and 97%, respectively. Across data reported on 194 patients, there was no incidence of patellar redislocation, patellar fracture, or graft site morbidity.Conclusion: The quadriceps tendon produced improved clinical outcomes with low rates of recurrent postoperative patellar dislocation. These data suggest that the quadriceps tendon remains a suitable alternative for MPFL reconstruction and should remain part of the arsenal of knee ligament surgeons.Level Of Evidence: Systematic review of Level III and IV studies. 250. Controls on Upper Ocean Salinity Variability in the Eastern Subpolar North Atlantic During 1992–2017

Abstract

The upper ocean salinity in the eastern subpolar North Atlantic undergoes decadal fluctuations. A large fresh anomaly event occurred during 2012–2016. Using the ECCOv4r4 state estimate, we diagnose and compare mechanisms of this low salinity event with those of the 1990s fresh anomaly event. To avoid issues related to the choice of reference salinity values in the freshwater budget, we perform a salt mass content budget analysis of the eastern subpolar North Atlantic. It shows that the recent low salt content anomaly occurs due to the circulation of anomalous salinity by mean currents entering the eastern subpolar basin from its western boundary via the North Atlantic Current. This is in contrast to the early 1990s, when the dominant mechanism governing the low salt content anomaly was the transport of the mean salinity field by anomalous currents. On decadal time scales, the eastern subpolar North Atlantic shifts between a salty and fresh upper ocean. Between 2012 and 2016, there was a large event which freshened this region more than at any other time in over a century. A similar event occurred in the early 1990s, but with a smaller magnitude. We use a numerical model of the ocean to figure out why these events occurred. Our study shows that there were two different mechanisms at play. The recent event occurred because a lot of fresh water came in from the west by the mean currents. The 1990s event occurred because ocean currents shifted and brought fresh water from outside the region.
