http://isami2010.di.uminho.pt/, Ambient Assisted Living is based on a set of technologies with the aim to provide an enhanced support to people’s daily life. One important field deals with the offer of new solutions for healthcare. Those solutions intend to improve the population quality of life and reduce costs associated with healthcare. Before that happens, new platforms, hardware and software, must be available in order to acquire and store the required signals, to process and extract information from those signals, and to detect a set of features required to fire alarms and/or electronic assistance. This paper presents a platform (hardware and software) that was designed to acquire data from a subject, or from a device, and send it to a remote hub. It was designed taking into account its size, power autonomy, and quality of sfor one day.

In the present work apples from eleven regional varieties originating from Portugal were studied in terms of their content in phenolic compounds and dietetic fibres (soluble and insoluble). In some cases, apples from two different production modes were analysed: conventional and biological. Some of the most commercialized apples in Portugal were also studied for comparison purposes. In addition, a sensorial evaluation of some of the varieties was performed. From the present work was possible to conclude that the regional varieties studied contain higher amounts of dietetic fibre than those of commercial varieties and other fruits. With respect to the content in total phenolic compounds, the apples of regional varieties showed values clearly higher than those quantified in commercial varieties, and being concentrated mainly on the peel, then on the seeds, and finally on the pulp. From the sensorial analyses was possible to infer that the regional varieties which were more appreciated were Bravo de Esmolfe and Camoesa de Alcongosta

Objectivos: Avaliar as indicações para a esplenectomia e colecistectomia, os seus resultados e complicações em crianças com esferocitose hereditária. Material e Métodos: Realizou-se uma revisão retrospectiva das crianças com diagnóstico de Esferocitose Hereditária (EH) seguidas na consulta de Hematologia entre Janeiro de 1992 e Agosto de 2007. Os parâmetros avaliados foram: idade actual; sexo; grau de gravidade da EH avaliado de acordo com os critérios publicados pela British Society for Haematology em 2004; co-hereditariedade de síndrome de Gilbert; doentes submetidos a esplenectomia e colecistectomia; idade à data da cirurgia; método cirúrgico usado (laparotomia/laparoscopia; esplenectomia total/parcial); parâmetros relevantes na decisão de esplenectomia e/ou colecistectomia (anemia, reticulocitose, esplenomegalia, dependência transfusional, litíase vesicular, hematopoiese extramedular e outras) ; parâmetros analíticos pré e pós-cirúrgicos (hemograma, contagem de reticulócitos, bilirrubina total e bioquímica do ferro); atitudes profiláticas relativas a complicações de esplenectomia (vacinação prévia, antibioterapia profi lática e terapêutica) e registo de possíveis complicações (sépsis, trombose, lítiase vesicular) nos doentes submetidos a esplenectomia. Procedeu-se a análise estatística com recurso ao Software SPSS® versão 13. Resultados: O número de doentes com o diagnóstico de EH foi de 52. Destes 19 eram classificados como EH ligeira, 24 como EH moderada e nove como EH grave. Em 15 doentes foi efectuada esplenectomia total (ET) e em três doentes esplenectomia parcial (EP). Os motivos mais relevantes na decisão da esplenectomia foram: anemia com elevada reticulocitose (n=14), esplenomegalia severa (n=14), dependência transfusional (n=4), litíase vesicular (n=5) e pancreatite litiásica (n=2). O incremento médio de hemoglobina pós-esplenectomia foi de 4,95 g/ dl (1,6-10,1). A erradicação de hemólise, entendida como correcção da anemia, reticulocitose e valor de bilirrubina total, após a esplenectomia, verificou-se em 11 casos, três mantiveram reticulocitose moderada (2EP+1ET), dois reticulocitose severa (1ET+1EP). Não foram observadas complicações graves pós-esplenectomia. Em seis doentes observou-se trombocitose sustentada. Cinco das dezoito crianças submetidas a esplenectomia realizaram esplenectomia com colecistectomia concomitante, por lítiase vesicular. Nove doentes realizaram apenas colecistectomia num único tempo cirúrgico. Conclusões: As opções seguidas estão de acordo com as recomendações da literatura, ou seja, a maioria das EH graves foram esplenectomizados precocemente, apenas algumas EH moderadas mais tardiamente, enquanto que os casos ligeiros não foram esplenectomizados. A colecistectomia isolada, realizada em nove doentes, contraria a generalidade da literatura, que recomenda face a indicação para colecistectomia, a realização em simultâneo, de esplenectomia. Esta recomendação fundamenta-se num eventual risco de recorrência de litíase nas vias biliares. ABSTRACT Objective: To evaluate the indications for splenectomy and cholecystectomy, its results and complications in children with diagnosis of Hereditary spherocytosis (HS). Material and methods: The authors retrospectively reviewed all cases of HS followed in the Haematology Department of our hospital between January of 1992 and August of 2007.The parameters evaluated were: actual age, sex, HS classification accordingly to the criteria published by the British Society for Haematology in 2004, co-inheritance of Gilbert Syndrome, patients submitted to splenectomy and cholecystectomy; age at surgery; surgical method (laparotomy/laparoscopy; total/ partial splenectomy); relevant parameters for splenectomy and cholecystectomy (anemia, reticulocits number, transfusion dependence, splenomegaly, gallstones; extramedullary hematopoiesis and others); analytic results before and after surgery (hemogram, reticulocits count, total bilirrubin, iron bioquimics); prophylactic attitude in respect of complications due to splenectomy (previous vaccination, prophylactic and therapeutic antibiotics); assessment of possible complications (sepsis, thrombosis, gallstones) in patients submitted to splenectomy. It was performed a descriptive analysis recurring to the software SPSS, 13rd version. Results: Fifty-two patients were diagnosed with HS. Nineteen patients were classified as mild HS, 24 as moderate HS and nine as severe HS. Total splenectomy (TS) was performed in 15 patients and partial splenectomy (PS) in three patients. The most relevant motives in the decision to splenectomy were: anaemia with high reticulocyte count (n=14), severe splenomegaly (n=14), transfusion dependence (n=4), gallstones (n=5) and pancreatitis by cholelithiasis (n=2).The median haemoglobin increment after splenectomy was 4.95g/dl (1,6-10,1). The eradication of haemolysis, perceived as anaemia, reticulocitosis and bilirrubinemia correction, after splenectomy, was observed in 11 patients, three maintained moderate reticulocytosis (2PS+1TS) and two maintained severe reticulocytosis (1TS+1PS). We did not observe severe complications after splenectomy. In six patients persistent thrombocytosis was observed. Five from the eighteen children submitted to splenectomy realize both splenectomy and cholecystectomy, because of the presence of gallstones. Nine patients were submitted to cholecystectomy in one surgical time. Conclusions: The options followed agreed with the recommendations suggested in the literature: the majority of the severe HS were splenectomized early, some of the moderate forms were splenectomized later and the milder forms were not submitted to splenectomy. The isolated cholecystectomy, realized in nine patients, refute the general literature, that recommends at the same time of the cholecystectomy the realization of splenectomy. This recommendation fundaments in the eventual risk of recurrence of gallstones in biliar routes.

In the present work apples of regional varieties originating from Portugal were studied in terms of some chemical properties along three consecutive harvest years. It was possible to conclude that these regional varieties of apples were generally sweeter and contained more minerals than other varieties described in literature. The data obtained revealed that the content in acidity, protein and minerals depends on the apple variety. The acidity content varied much along the 2004, 2005 and 2006 harvests. For total sugar,or protein contents the variations observed were comparatively smaller. In the 2004 and 2005 harvests the sugar and protein contents do not differ so much differing more in the 2006 harvest. In respect to mineral content, the 2004 harvest registered the higher differences, and this was also the year when the apples were considerably riper.

Antidepressants are a class of compounds widely used in clinical settings for the treatment of several diseases. In the last years there has been a considerable increase in their consumption, representing an important public health issue in several countries. Because they are substances with narrow therapeutic windows, and since they are capable of interacting with other classes of compounds, monitoring of these compounds is of relevance, minimizing the risk of medical interactions as well as side and toxic effects. In addition, understanding the extent of their use, their detection through routine toxicology tests and development of new methods for detection and monitoring is of extreme importance concerning public health, patient well-being, and implications in clinical and forensic situations. The main objective of this work is to perform a critical review on the biological samples used in the detection and quantification of antidepressants with special focus on the techniques for sample preparation.

Os problemas menstruais constituem aproximadamente 50% das queixas ginecológicas na adolescência, sendo a hemorragia uterina disfuncional responsável por 90 a 95% dos casos. Trata-se de uma entidade clínica muito frequente nos 2 primeiros anos após a menarca, uma vez que os ciclos menstruais tendem a ser anovulatórios. É um diagnóstico de exclusão, devendo outras possíveis causas ser estudadas: discrasias sanguíneas, complicações de gravidez, endocrinopatias, patologia pélvica e medicamentos. Os autores fazem uma revisão da fisiopatologia, avaliação diagnóstica e tratamento, no sentido de uniformizar atitudes terapêuticas. ABSTRACT: The menstrual disorders constitute about 50% of gynaecological complaints in adolescence, and the dysfunctional uterine bleeding accounts for 90 to 95% of cases. This is a very common clinical entity in the first 2 years after menarche, because the menstrual cycles tend to be anovulatory. It is a diagnosis of exclusion, and other possible causes should be studied: coagulation disorders, complications of pregnancy, endocrinopathies, pelvic pathology and medications. The authors present a review of the pathophysiology, diagnostic evaluation and treatment in order to unify attitudes.

Este trabalho apresenta os resultados de um estudo sobre as perdas de protensão, medidas "in loco", de cordoalhas engraxadas em lajes planas. Essas lajes planas, com 17 centímetros de espessura e 350 m2 de área, fazem parte de um edifício para fins hoteleiros de 16 pavimentos. As forças de protensão foram medidas por células de carga colocadas nas proximidades da ancoragem ativa e da ancoragem passiva. As medidas de forças foram feitas desde a protensão até idades de 170 dias após esse instante. Os resultados revelam um coeficiente de atrito μ igual a 0,0528 bastante próximo ao valor apresentado pela norma brasileira NBR 6118: 0,05. Nas perdas pela acomodação da ancoragem os valores apurados foram bastante similares e em termos médios, essa perda correspondeu a 5,17% da força inicial de protensão. Os valores de força medidos mostram também que a perda de tensão na cordoalha, próximo à ancoragem passiva, é menor em relação à perda na ancoragem ativa. Até a idade de 170 dias, verificou-se uma perda média de 14% da força inicial de protensão. This paper presents the results of "in situ" measurements of stress losses in unbonded tendons of post tensioned flat slabs. These flats slabs have approximately 350 m2 in area and are 17 centimeter thick. They are part of a 16 story high hotel building. Load cells were employed to measure the forces in the tendons; they were placed in the vicinity of the active and the passive anchorages. The forces in the tendons were monitored from the time of their straining up to 170 days afterwards. The measurements reveal a frictional coeficiente μ equal to 0.0528, a very close value to the one prescribed by NBR 6118 of 0.05. The stress loss due to the anchorage of the tendons was of 5.17% of the initial stress. The results indicate also that the remaining force near the passive Anchorage is larger in relation to the value measured in the vicinity of the active one. The total stress loss measured so far (170 days) corresponds to 14 % of the initial post-tensioning stress.

Introdução: A deficiência selectiva de Imunoglobulina A (IgA) é a imunodeficiência primária mais frequente. Está associada a diversas patologias com diferentes apresentações, nomeadamente patologia infecciosa, alérgica, gastrointestinal, doenças auto-imunes e neoplásicas. Objectivo: Analisar as características clínicas e laboratoriais de crianças com défice de IgA seguidas no Hospital de Crianças Maria Pia. Material e Métodos: Análise retrospectiva de processos de crianças e adolescentes com défice selectivo de IgA, no período compreendido entre 1 de Janeiro de 2000 e 31 de Dezembro de 2005. Foram obtidos dados relativos ao sexo, idade do diagnóstico, patologia associada e parâmetros laboratoriais nomeadamente, doseamento de IgM, IgG e IgE, subclasses de IgG, presença de anticorpos antinucleares (ANAs), factor reumatóide (FR) e anticorpos específicos. Resultados: Foram avaliadas 50 crianças, sendo 56%(28) do sexo masculino. A mediana da idade de diagnóstico foi de 6 anos. As manifestações clínicas associadas foram infecções recorrentes das vias aéreas superiores em 50%(25), asma em 34%(17), rinite alérgica em 22%(11), pneumonia em 18%(9), giardíase em 6%(3), doença celíaca em 8%(4), e doenças auto-imunes em 8%(4). Na avaliação laboratorial, observaram-se níveis de IgG aumentados em 42%(21) dos casos. Discussão e Conclusão: Os resultados apresentados estão de acordo com outras séries publicadas. Este trabalho salienta a grande variabilidade de apresentação clínica do défice de IgA. O seu conhecimento permitirá um diagnóstico precoce e orientação adequada. ABSTRACT Introduction: Selective immunoglobulin A (IgA) deficiency is the most frequent primary immunodeficiency. It is associated with diverse pathologies with different presentations, namely infectious, allergic, gastrointestinal, auto-immune and neoplastic disorders. Objective: Analyse clinical and laboratorial characteristics of children with IgA deficiency attending Maria Pia Children’s Hospital. Matherial and Methods: Retrospective analysis of IgA deficient children and adolescent clinical files, in the period from the1st January 2000 until the 31st December 2005. We collected information on sex, age at diagnosis, associated diseases and laboratory data, namely IgM, IgG and IgE, IgG subclasses, presence of antinuclear antibodies, rheumatoid factor, and specifi c antibodies. Results: Fifty children were evaluated, 56%(28) of them were males. The median of age was 6 years. The associated clinical manifestations were recurrent upper airway infections in 50%(25), asthma in 34%(17), allergic rhinitis in 22%(11), pneumonia in 18%(9), Giardia infection in 6%(3), celiac disease in 8%(4), and autoimmune diseases in 8%(4). On laboratory evaluation, high IgG levels were observed in 42%(21) of cases. Discussion and Conclusion: The results presented are similar to other published series. This work points out that IgA deficiency has a great variability of clinical presentation. This knowledge will allow a rapid diagnosis and an adequate orientation.

The present paper describes the study done on the dosimetric characteristics of the Varian Clinac 600C dynamic wedges (DW) and their comparison with the physical wedges (PW) in terms of the differences affecting the dose distributions, beam spectra, energy fluence and angular distributions. The geometry of the 4 MV photon beam and the dose distributions in a water phantom were simulated with GEANT3 and DPM Monte Carlo code systems. The DW was modelled through the constant movement of the upper jaws. The depth dose distributions and lateral profiles for the DW, PW and open fields were measured and compared with the Monte Carlo simulations and the global agreement was found to be within 3%. It was also found that the effects of a DW on beam spectral and angular distributions are much less significant than those produced by a PW. For example, in our study we found out that the 450PW, when compared with the corresponding open field, can introduce a 30% increase in the mean photon energy due to the beam hardening effect and that it can also introduce a 4.5% dose reduction in the build-up region because of the reduction of the contaminated electrons by the PW. For the DW neither this mean-energy increase nor such dose reduction was found. The PW, when compared to the DW, significantly alters the photon-beam spectrum and these dosimetric differences are significant and further investigation must be performed to quantify the impact in clinical use of these beams.

RESUMO Introdução: A aterosclerose tem início na infância e progride durante a adolescência. Nas últimas décadas verifica-se uma tendência para o aumento da adiposidade corporal e distúrbios metabólicos associados. A sua prevenção, detecção e correcção precoces podem reduzir a incidência de doença cardiovascular na idade adulta. As recomendações actuais para o rastreio de dislipidémia apontam para um rastreio selectivo baseado na identificação de vários factores de risco. Objectivo: Conhecer o perfil lipídico, a prevalência de sobrepeso e obesidade, a história familiar de doença cardiovascular e os hábitos alimentares entre 88 adolescentes, observados durante exame global de saúde dos 11-13 anos, no Centro de Saúde da Sr.ª da Hora - Matosinhos. Material e Métodos: O IMC foi avaliado e classificado segundo as curvas de crescimento do CDC- Centres of disease control- National Centrer for Health Statistics dos Estados Unidos. A classificação do perfil lipídico baseou-se nos critérios do National Cholesterol Education Program (NCEP) para o Colesterol Total e Colesterol LDL e do Lipid Research Clinics Prevalence Study (LRC) para os triglicerideos e colesterol HDL. We studied the effects of tracheal tube cuffs filled with air, saline or alkalinised lidocaine on haemodynamic changes during tracheal extubation and postoperative laryngotracheal morbidity in children. We randomly allocated 164 children aged 3-13 years undergoing general anaesthesia to one of four groups; tracheal tube cuffs filled with air (n = 41); saline (n = 41); alkalinised lidocaine 0.5% (n = 41); or alkalinised lidocaine 1% (n = 41). Intracuff pressure was monitored and maintained below 20 cmH2 O. The mean (SD) increases in systolic blood pressure after tracheal extubation compared with before extubation were 10.9 (10.8) mmHg, 7.3 (17.7) mmHg, 4.1 (10.5) mmHg and 1.9 (9.5) mmHg in the air, saline, 0.5% and 1% alkalinised lidocaine groups, respectively (p = 0.021). The mean (SD) increases in diastolic blood pressure after tracheal extubation compared with before extubation were 3.9 (9.7) mmHg, 7.9 (14.6) mmHg, 0.7 (10.4) mmHg and 3.6 (6.9) mmHg in the air, saline, 0.5% and 1% alkalinised lidocaine groups, respectively (p = 0.019). The mean (SD) increases in heart rate after tracheal extubation compared with before extubation were 14.2 (7.6) beats.min-1 , 15.5 (13.1) beats.min-1 , 5.2 (9.6) beats.min-1 and 4.1 (6.6) beats.min-1 in the air, saline, 0.5% and 1% alkalinised lidocaine groups, respectively (p < 0.001). The incidence of sore throat 8 h after tracheal extubation was 22.0% in the air-filled group, 9.8% in the saline group, 4.9% in the 0.5% alkalinised lidocaine group and 2.4% in the 1% alkalinised lidocaine group, p = 0.015. We conclude that filling the tracheal tube cuff with alkalinised lidocaine-filled reduces the haemodynamic response to tracheal extubation and postoperative laryngotracheal morbidity in children. 249. Comparison of engine power correction factors for varying atmospheric conditions

This work evaluates proposed methods to correct engine power output as a function of atmospheric conditions. The analysis was made through experiments carried out in a vehicle on the road, under different temperature, pressure and air humidity conditions. The vehicle had a four-cylinder gasoline-fuelled engine, with multi-point fuel injection system, variable intake pipe length and variable intake valve camshaft position. The vehicle was tested at sea level and at 827 m above sea level, corresponding to atmospheric pressures between 1027 and 926 mbar. Air temperature varied from 22,8 to 33,8 °C at the test locations. The measured performance parameter in the tests was the vehicle acceleration time. The acceleration times from 0 to 400 m, 0 to 1000 m, 40 to 100 km/h and 80 to 120 km/h were all recorded, leaving from an initial vehicle speed of 40 km/h. The engine power curve obtained in laboratory under a standard ambient condition was corrected to the conditions of the road tests by the correction factors proposed by the methods under evaluation, and the corresponding acceleration times were calculated and compared with the measurements from the road tests. The evaluated methods for power correction were the following: DIN 70020, SAE J 1349, JIS D 1001 and ISO 1585. The SAE J 1349 method provided the best approach between the experimental and calculated acceleration times.
