Abstract

Background: Transcranial Doppler (TCD) is used to monitor cerebral vasospasm after subarachnoid hemorrhage (SAH), but its diagnostic ability is reported to be limited. Therefore, the purpose of this study was to investigate the relationship between the diagnosability of TCD and the localization of the vasospasm. Methods: This retrospective study included 20 patients who presented with symptomatic vasospasm after SAH. All 20 patients underwent daily TCD examinations and cerebral angiography after the onset of delayed cerebral ischemia. We defined positive findings on TCD as a maximum flow velocity >200 cm/s or as a mean flow velocity >120 cm/s at the horizontal part of the middle cerebral artery (MCA). We also examined the site of vasospasm on cerebral angiography. Results: Fourteen patients had true-positive findings on TCD examination, and cerebral angiography showed diffuse vasospasm involving the horizontal segment of the MCA. However, 6 patients had false-negative findings on TCD examination, and cerebral angiography showed vasospasm localized at the distal part of the MCA (the insular and/or cortical segments). The patients with proximal vasospasm were significantly younger than those with distal vasospasm. Blood flow velocity at initial TCD and the increase in velocity at the onset of vasospasm were lower and smaller, respectively, in the distal vasospasm group. Conclusions: In patients with cerebral vasospasm localized at the distal part of the MCA, flow velocity at the horizontal segment of the MCA did not increase to the level we defined as positive. To avoid such false negatives, a slight increase in velocity on TCD should be considered as positive in distal vasospasm cases, especially in older patients.

Abstract

Intelligent equipment is necessary to ensure stable, high-quality, and efficient production of facility agriculture. Among them, intelligent harvesting equipment needs to be designed and developed according to the characteristics of fruits and vegetables, so there is little large-scale mechanization. The intelligent harvesting equipment in Japan has nearly 40 years of research and development history since the 1980s, and the review of its research and development products has specific inspiration and reference significance. First, the preferential policies that can be used for harvesting robots in the support policies of the government and banks to promote the development of facility agriculture were introduced. Then, the development of agricultural robots in Japan was reviewed. The top ten fruits and vegetables in the greenhouse were selected, and the harvesting research of tomato, eggplant, green pepper, cucumber, melon, asparagus, and strawberry harvesting robots based on the combination of agricultural machinery and agronomy was analyzed. Next, the commercialized solutions for tomato, green pepper, and strawberry harvesting system were detailed and reviewed. Among them, taking the green pepper harvesting robot developed by the start-up company AGRIST Ltd. in recent years as an example, the harvesting robot developed by the company based on the Internet of Things technology and artificial intelligence algorithms was explained . This harvesting robot can work 24 h a day and can control the robot's operation through the network. Then, the typical strawberry harvesting robot that had undergone four generations of prototype development were reviewed. The fourth-generation system was a systematic solution developed by the company and researchers. It consisted of high-density movable seedbeds and a harvesting robot with the advantages of high space utilization, all-day work, and intelligent quality grading. The strengths, weaknesses, challenges, and future trends of prototype and industrialized solutions developed by universities were also summarized. Finally, suggestions for accelerating the development of intelligent, smart, and industrialized harvesting robots in China's facility agriculture were provided.

Abstract

The purpose of this paper is to present improved estimates of the intertemporal elasticity of substitution (IES) for Japan assuming a constant relative risk aversion (CRRA) utility function. The estimates of the IES we obtain range from 0.2 to 0.5 when we use quarterly consumption data and the Continuous Updating Estimator (CUE). We find that the IES is weakly identified when we employ the two-step GMM estimator, while the CUE can identify the IES. Moreover, we also find that using consumption data of different frequencies leads to quite different estimates of the IES.

Abstract

A study of the effects of nitric oxide (NO) models on the prediction of NO formation in a gas-fired regenerative furnace with highly preheated air was undertaken. Three chemical kinetic processes for NO formation/depletion, i.e., thermal NO, prompt NO, and NO reburning, are included. In the thermal NO model, the sensitivity encountered when using two different approaches, namely the equilibrium approach and the partial equilibrium approach, for determining the O radical concentration was studied. The effects of the third reaction in the thermal NO mechanism, NO reduction (reburning) mechanism, and different types of probability density functions (PDFs) on the NO predictions have also been tested. The sensitivity of the excess air ratio on the NO generation rate in the furnace has been investigated. Finally, the impact o[ the temperature on the NO formation rate in the regenerative furnace was discussed. [S0195-0738(00)00304-6]

Abstract

Objective: To review our use of non-invasive positive pressure ventilation (NPPV) for acute cardiogenic pulmonary oedema (ACPO) in the routine clinical management, especially in terms of the timing of endotracheal intubation (ETI) and outcome. Methods: We retrospectively reviewed 61 patients diagnosed with ACPO admitted to our emergency room (ER) or intensive care unit (ICU) and who received NPPV. The reasons for ETI were reviewed, and the intervals between the estimated appropriate time for ETI and the actual time of ETI and in-hospital mortality were recorded. Results: The mortality rate of patients receiving NPPV was 8.2% (five out of 61). Forty-eight patients (78.7%) were successfully weaned off NPPV without ETI, and 13 (21.3%) required ETI. Five of the 13 intubated patients died, but there was no significant difference in the duration of NPPV before ETI between those who survived and those who died. The interval between the estimated appropriate time for ETI and the actual time of ETI was significantly shorter in patients who survived than in those who died (1.9 ± 3.8 hours versus 8.6 ± 5.4 hours, p=0.02).
