Abstract

Hyperhomocystemia has been reported to be associated with cardiovascular disease, especially stroke. The resistive index (RI) estimated by carotid ultrasound is an established variable for estimating the risk of cerebral infarction. The aim of this study was to evaluate the relationship between homocysteine concentration and carotid RI, a marker of cerebral vascular resistance in essential hypertensive patients. We measured serum total homocysteine and carotid RI in 261 patients. Multiple linear regression analysis was used to determine the association of homocysteine with carotid RI and intima media thickness (IMT). Age, sex, BMI, systolic blood pressure (SBP), homocysteine, total cholesterol, high density lipoprotein-cholesterol (HDL-C), uric acid, CRP, HbA1c, estimated glomerular filtration rate, and use of antihypertensive agents were included as independent variables. Age, sex, use of antihypertensive agents, HDL-C and homocysteine levels were shown to be significant predictors of carotid RI, but not IMT. Multiple regression analysis in men older than 65 years showed homocysteine and SBP were associated significantly with carotid RI. In elderly male patients, homocysteine was the strongest predictor of carotid RI (B=0.0068, CI = 0.0017-0.0120, P = 0.011) in the multivariate model. In conclusion, hyperhomocysteinemia is associated with carotid RI, a surrogate marker of cerebral vascular resistance, especially in elderly men. [ABSTRACT FROM AUTHOR]

Abstract

1回透析の除水量 (UF) が慢性血液透析 (HD) 患者の左室肥大 (LVH) 進展の要因になりうるか否かについてretrospectiveに検討した. 透析導入後1年以内 (平均6ヵ月) に心エコーを施行し, 以後2年間にわたり左室壁厚と左室心筋重量 (LVM) の変化を観察しえたHD患者29例を対象に, UFにより1kg未満 (group 1, n=10), 1kg以上2kg未満 (group 2, n=13), 2kg以上 (group 3, n=6) の3群に分類し, 各群での左室形態の変化を比較した. 透析時間は全例1回5時間で, 週3回透析を施行し, 観察期間中透析方法の変更はなかった. 心エコーは透析前に施行し, Mモード心エコー図から心室中隔厚, 左室後壁厚, 左室拡張末期径, 左室収縮末期径, LVM, Fractional shortening (FS) を測定した. なお, LVHに影響する因子として平均血圧, Htを測定するとともに, 体重管理の状況を評価するためdry weightの経過も観察した.その結果, LVMはgroup 1ではcontrol (C) 期 (212±66g), 1年目 (216±79g), 2年目 (205±102g) と経過中不変であり, group 2ではC期 (202±62g), 1年目 (224±84g), 2年目 (234±103g) とC期に比し1年目に有意の増加 (p

Abstract

Apoptosis (programmed cell death) is observed in vascular smooth muscle cells (VSMC) in atherosclerotic lesions and stenotic lesions after injury, and modulates the cellularity of these lesions. It is recognized that cell growth and apoptosis are two linked processes. Platelet-derived growth factor (PDGF) induces VSMC proliferation and migration in vitro. We studied the effect of PDGF on apoptosis in VSMC. Cultured rat VSMC were treated with PDGF-AA or PDGF-BB. PDGF-BB induced cell death in cultured VSMC in a time- and dose-dependent manner, but PDGF-AA did not. Gel electrophoresis of genomic DNA and in situ DNA labeling confirmed that the cell death induced by PDGF-BB is apoptosis. PDGF-BB treatment reduced bcl-2 mRNA and bcl-xl mRNA expression, in contrast, induced bcl-xs mRNA expression, linked with the induction of apoptosis in cultured VSMC.
