A minimally invasive posterolateral approach designed to avoid the lateral misplacement of midcervical pedicle screws was reported, but there is no technical report that describes this technique without C-arm fluoroscopy. We report the results of a 2.5 years follow-up of a 62-year-old female patient with C4 metastatic breast cancer. The patient suffered from severe neck pain and impending quadriplegia for 2 months after radiation therapy. We performed C-arm-free minimally invasive cervical pedicle screw fixation (MICEPS). The patient was suc-cessfully treated with surgery, and her neck pain was well controlled. She had neither neurological deficits nor neck pain at the final (2.5-year) follow-up. C-arm-free MICEPS is a useful technique; in addition, the sur-geons and staff have no risk of radiation exposure, there is a reduced need for postoperative imaging, and a decreased revision rate can be expected with C-arm-free MICEPS.

Background: Locomotive syndrome (LS) is a condition of decreased mobility caused by disorders of the locomotive organs. Lumbar spinal stenosis (LSS) is a LS disorder. The Japanese Orthopaedic Association score (JOA score) and the Zurich Claudication Questionnaire (ZCQ) are international evaluation tools for LSS. However, the relationship between LS and JOA score or ZCQ is unknown. This study aimed to clarify the correlations between LS progression and the values/parameters of the JOA score or ZCQ and to determine the critical cutoff point of the JOA score or ZCQ that indicates LS progression. Methods: We recruited preoperative LSS patients (n = 82). Patients' mean age was 73.4 years. The study participants were evaluated using the 25-question Geriatric Locomotive Function Scale (GLFS), JOA score, and ZCQ (which consists of symptom severity and physical function), and the patients' health-related quality of life was assessed using EuroQoL-5 dimension (EQ-5D) utility values and the EuroQoL-visual analog scale (EQ-VAS). We investigated the correlations between the 25-question GLFS and each clinical variable and evaluated the critical cutoff point of each international evaluation tool to detect LS. Results: There was a statistically significant correlation between 25-question GLFS and each clinical evaluation tool. LSS patients with LS showed significantly worse scores in the evaluation tools than LSS patients without LS. Moreover, we found that critical cutoff points of 17.5 on JOA score, 3.1 on ZCQ-symptom, and 2.3 on ZCQ-function could detect LS. Conclusions: A statistically significant correlation exists between the 25-question GLFS and the JOA score or ZCQ. It might be important to perform decompression surgery for LSS patients before they reach the cutoff values of the several clinical evaluation tools to avoid LS progression. 第36回北方圏国際シンポジウム「オホーツク海と流氷2022」 北方圏国際シンポジウム実行委員会 2022年2月21日-23日 紋別市民会館・道立オホーツク流氷科学センター・紋別市立博物館・北海道大学北極域研究センター, While the diverse environmental changes have been seen globally in recent years, the purpose of this joint research is to extract issues for the appropriate Arctic tourism development with a view of improving community resilience that is expected to respond to those global changes. In this context, therefore, this research adopted a framework of Adventure tourism (AT), which has been actively discussed in the Arctic in these days and also has the latest trend of indigenous people’s perspective on nature. And then we conducted a basic comparative analysis based on some hearing surveys in the Inari region in Finish Lapland and the Akan region in Hokkaido, Japan. Although their histories and backgrounds of regional tourism development are different, the common significant issues that belong to each of the eight issue categories for the Arctic tourism development revealed in the first year of this research were extracted. Furthermore, we extracted these issues from the perspectives of community, natural environment in region, and indigenous culture as the result of comprehensive analysis of both region’s endeavors and possibility of attracting AT customers with their remarkable characteristics., 昨今グローバルに多様な環境変化が見られる中、その変化への対応が期待される地域のレジリエンス向上を見据えた適切な北極域観光振興の課題抽出が本研究の目的である。そのため、近年北極域でも活発に議論され、とくに先住民族の自然観を最新のトレンドとするアドベンチャーツーリズム（AT）の枠組みを使用し、フィンランド・ラップランドのイナリ地域と国内・北海道の阿寒地域の観光の取組み状況をヒアリング調査した上で、本研究は基礎的な比較分析を行った。地域観光の振興の歴史や背景はそれぞれ異なるが、両地域の取組みと顕著な特徴をもつAT顧客の誘致の可能性を総合して分析した結果、本研究が初年度に明らかにした北極域観光振興における8つの課題に属する共通の重要課題が、コミュニティや地域の自然環境、そして先住民族文化の視点からそれぞれ抽出された。
