Soil erosion by water and sediment delivery to water bodies are major threats to soil and water resources around the globe. Under climate change, these processes are exacerbated because of an increase in extreme convective rainfalls in a warmer world. Many soil erosion models are based on the Universal Soil Loss Equation (USLE) where the influence of climate is included in the rainfall erosivity factor R. In these models, projections of future rainfall erosivity are often impaired by the low spatio-temporal resolution of precipitation data generated by global or regional climate models. Convection-permitting climate simulations (CPS) have a high and to date unexploited potential for climate impact studies on soil erosion because of their high spatio-temporal resolution and their more accurate representation of heavy rainfall. Here we used the precipitation data generated by the convection-permitting model COSMO-CLM with a resolution of 3 km and 1h to calculate rainfall erosivity for central Europe in the past (1971-2000), present (2001-2019), near future (2031-2060) and far future (2071-2100). Our results show a high similarity between past modelled and measured rainfall erosivity. Furthermore, rainfall erosivity calculated from CPS precipitation data increased by up to 84% in the far future. This increase is much higher than previous estimates based on annual precipitation simulated with an ensemble of regional climate models. Introduction An ED visit for opioid overdose may be a person's only contact with the medical and behavioral health care systems and is an important opportunity to reduce risk of subsequent overdose and death. While ED initiatives to engage people with opioid use disorder (OUD) are being increasingly implemented, there are significant gaps in the receipt of services at the time of the ED encounter. Methods This is a retrospective cohort study of an outreach pilot project providing real-time telehealth delivered buprenorphine initiation and referral to community harm reduction and addiction treatment services via a follow up telephone call to patients after an ED visit for an opioid overdose. Results From January 2020 to April 2021 there were 606 patients with an ED visit for an opioid overdose eligible for a callback. Of the 606 eligible patients, 254/645 (42%) patients could be contacted and accepted service and/or treatment referrals. Fifteen patients were connected same-day to a buprenorphine prescriber for a telehealth encounter and, of connected patients, nine received a buprenorphine prescription. Conclusion A post-ED follow up telephone call protocol is an opportunity to improve treatment engagement and access to buprenorphine for patients at high risk for opioid overdose and death.

Solid organ transplant candidates encountered increased wait times and mortality rates during the coronavirus 2019 (COVID-19) pandemic. Despite improvement in medical management and vaccination efficacy, this patient population remains at increased risk for complications post COVID-19 including organ rejection. We describe the development of antibody mediated rejection with or without cellular rejection in heart transplant (HT) recipients and previous COVID-19 infection or vaccination. Although centers have changed their management of outpatient follow-up for orthotopic heart transplant (OHT) patients, little is known on surveillance of rejection and management of HT recipients after COVID-19 infection. We recommend frequent surveillance for rejection or allograft dysfunction after COVID-19 infection. We have adopted a transplant surveillance protocol for HT recipients with COVID 19 infection, given our recent experience with transplanted patients affected of COVID-19. Introduction The U.S. Naval Ship COMFORT has performed six humanitarian assistance and disaster relief mission since 2007. This paper describes the surgical volume per surgical specialty for five missions spanning 19 countries. Materials and Methods Raw surgical case logs were analyzed for total case volume, total operating days, unanticipated return to operating room, and percentage of pediatric cases ( Results Total surgical volume for the five missions was 5,142. The countries most frequently visited were Columbia and Haiti with seven and five visits, respectively. General surgery, ophthalmology, and plastic surgery have had consistent volume over time. Orthopedic surgery volume has steadily decreased with the exception of the 2018 mission. Conclusion As volume in military treatment facilities declines, alternative sources of surgical volume for military surgeons are being examined. This paper highlights the historical volume which can inform future personnel planning requirements of U.S. Naval Ship COMFORT missions. With the exception of orthopedic surgery, surgical volume has remained consistent over the last decade. For future best practice, historical case data should be used to determine staffing needs on hospital ships and case logs and operating procedures and follow-up protocols should be standardized.

Background Nearly 300 medications contain pharmacogenomic information in their labeling approved by the U.S. Food and Drug Administration. As this number continues to grow, community pharmacists will be called on to use available pharmacogenomic data at the point of dispensing. Objective This qualitative study aimed to describe how pharmacists envision the integration of pharmacogenomic data into the current workflows of community pharmacy practice. Methods Community pharmacists from a regional supermarket chain pharmacy in the greater Pittsburgh area were interviewed using a semistructured interview guide. Participating pharmacists were presented with 3 clinical scenarios, followed by questions, to gain insight into how they envisioned the integration of pharmacogenomic data into community pharmacy workflow. The interview transcriptions were transcribed and coded. The content was analyzed to deduce the final themes. Supporting quotes were selected to illustrate each theme. Results Ten community pharmacists from 3 different pharmacy locations participated in the study. A thematic analysis produced 6 themes: (1) integrating pharmacogenomic data into the dispensing software, (2) receiving an alert for pharmacogenomic information within the dispensing software, (3) accessing pharmacogenomic clinical guidelines to guide drug–decision-making, (4) contacting the prescriber by adding a task to the call queue, (5) placing a mandatory counseling alert on medications that were adjusted using pharmacogenomic data, and (6) counseling the patient on the first refill of a medication that was adjusted using pharmacogenomic data. Conclusion This study describes how pharmacists envisioned the integration of pharmacogenomic data into community pharmacy workflow. The participants sought the integration of pharmacogenomic data into existing dispensing software, alerts for actionable prescribing changes using patient-specific pharmacogenomic data when available, and access to clinical decision support. In addition, the participants preferred to engage prescribers and receive alerts to counsel patients at prescription pick-up. These findings are key to integrating pharmacogenomic data into community pharmacy practice.

Zusammenfassung Elektrische Kontaktöffnungsentladungen sind eine potenzielle Zündquelle für brennbare Gasgemische. Betriebsmittel, in denen gegebenenfalls Kontaktentladungen auftreten, können mit einem sogenannten Funkenprüfgerät der internationalen Norm IEC 60079-11 (6te Ed.) geprüft werden. Nachteile dieser Methode sind die schlechte Reproduzierbarkeit der Ergebnisse und die Verwendung von toxischem Cadmium. Für die Entwicklung einer Alternative wurden bisher die elektrischen Entladungen charakterisiert. Der Übergang von der Entladung zur Zündung wurde dabei noch nicht ausreichend untersucht. In diesem Artikel werden die ersten Ansätze zur Abschätzung des Temperaturfeldes mit Schlieren- und örtlich aufgelösten spektroskopischen Untersuchungen dargestellt.
