To evaluate the role of Nesbit's and Byar's type of soft tissues reinforcement interposition flaps (STRIFs) in prevention of urethro-cutaneous fistulas (UCFs) in distal Snodgrass TIP urethroplasties. Between 2003-2005, 28 distal hypospadiacs, ranging from 8 to 14 years of age, without chordee, underwent Snodgrass TIP urethroplasties. The reconstructed neo-urethras were reinforced with preputial dartos fascial flaps, harvested by way of de-epithelialization. The de-epithelialized dorsal prepuce was either button holed (n=14, Nesbit' type of STRIFs) or divided in to two equals halves (n=14, Byar's type of STRIFs) to be ventralized and tucked under the glans wings distally to provide a complete cover to the neo-urethras. Total of four patients, two from each group, developed single, micro-sized coronal urethro-cutaneous fistula three of which healed spontaneously within a period of 6 weeks, and one required purse string closure after 3 months. At one year follow up, all the patients were asymptomatic with good functional and cosmetic out come. Keywords: Hypospadias. Urethro-cutaneous fistulas (UCFs). Introduction: Patients who are unable to perform intermittent self-catheterization may undergo continent catheterizable stomas using the appendix (Mitrofanoff Appendico-vesicostomy). If the appendix is not available, the bowel is used to create a catherizable channel (Monti-Yang-Casale technique). Patients may develop shortening of the submucosal bladder tunnel with secondary leakage after being dry from the initial procedure. We have devised a novel technique for elongation of the submucosal intravesical tunnel in patients with secondary stomal leakage due to acquired shortening of the submucosal bladder tunnel. Methods: An in-situ bladder tube was created from the dome of the bladder with one end attached to bladder. A submucosal tunnel of adequate length (6: 1 ratio over 10F red rubber catheter) was created. The in situ bladder tube was rotated and brought through the tunnel to create flap valve mechanism. The intravesical end of the tube is anastomosed and the other end is attached to Monti-Yang-Casale tube, which is brought out as a catherizable stoma through the umbilicus. In the second patient, modification was used to create an in-situ tube from the posterolateral portion of the bladder, with urethral side attached to the bladder to prevent any rotation of the tube while placing it in the tunnel. Results:. We have used this technique in two patients. The first patient, who had Monti-Yang-Casale procedures, was fully continent after initial surgery and subsequently developed urinary leakage at the stoma. The second patient had a bladder tube combined with the Monti-Yang-Casale as the primary procedure. Both patients are totally continent following the new in-situ continent bladder tube procedure. Conclusions: Revision for incontinence may be surgically challenging and can lead to vascular compromise. The advantages of this technique are: (1) An in-situ continent bladder tube with can be added to the Monti-Yang-Casale procedure to correct recurrent urinary stomal leak. This technique preserves the stoma loop without handling of the bowel mesentery. (2) This method can also be used as a primary method when the Monti-Yang-Casale tunnel is not sufficiently long enough to cause continence, or compromised blood supply to the Monti-Yang-Casale catheterizable tube. The management of UPJ has evolved dramatically over the past 20 years in response to the development of new technology. Endoscopic and laparoscopic procedures have largely replaced open pyeloplasty for the majority of primary UPJ obstruction. Yet laparoscopic pyeloplasty is a technically demanding procedure requiring advanced intracorporeal suturing skills. With the robotic (Da Vinci system), the suturing and laparoscopic tools is handled with a great degree of freedom much more than the laparoscopic instruments which allows the surgeon to treat even anatomical demanding difficult UPJ. Background: A prospective study of all patients admitted to the University of Maiduguri Teaching Hospital (UMTH), with a provisional diagnosis of carcinoma of the urinary bladder in the six year period between January, 2001 and December, 2006. The object of this investigation was to determine the relationship between schistosomiasis and carcinoma of the urinary bladder in Maiduguri, North East of Nigeria. Method: All the patients were assessed clinically, and investigated by urine cytology, cystoscopy and biopsy for histology. Those confirmed at histology to have carcinoma were analyzed for age, gender, occupation, history of Schistosomiasis, tobacco use, pathological type, presence or absence of schistosoma ova in the specimen, stage of the disease, complications, treatment and outcome. Results: All but 13 of the 65 confirmed cases were males, giving a male to female ratio of 4 to 1. Squamous cell carcinoma (SCC) of the bladder (n=46) was significantly commoner than transitional cell carcinoma (TCC) cases (n=15), in those with past history of Schistosomiasis (p< 0.001), with a Relative Risk (RR) of 4.06. Also 65.2% of SCC showed Schistosomiasis in the histology specimen compared with 13.3% of TCC cases (P < 0.001) Four patients had undifferentiated carcinoma, 3 (75%) of which showed Schistosoma ova in their specimen. Patients with SCC were significantly younger (45.26 +/SD13.5years) than those with TCC (P< 0.001). Our patients were mostly farmers of low socio-economic class. Most presented with advanced disease, thus treatment was mostly palliative and the outcome quite poor. Conclusion: This result clearly shows that carcinoma of the bladder in Maiduguri, North East of Nigeria, is mainly SCC and strongly associated with Schistosomiasis. Therefore, a deliberate policy and effort to control Schistosomiasis in this region will lead to a reduction in the incidence of bladder cancer and the attendant morbidity and mortality.
