Abstract

Typical parameters of electron bunches available now in undulator X-ray FELs enable generation of induced radiation in crystal-based X-ray VFEL. An important peculiarity of an undulator X-ray FEL is electron bunch spatial modulation with the period equal to the wavelength of the produced radiation. Therefore, XFEL can be considered as a modulator of electron bunch density. As a result a facility combining an undulator, which produced a modulated beam, and a crystal, on which the modulated beam is incident, can be considered to be similar to klystron, which is a well-known source for microwaves emission. Abstract

We have investigated the formation of microparticles-based Coulomb crystals in a linear vertical quadrupole Paul trap with a single end-cap electrode. For the first time, we have described the effect of gravity on Coulomb crystals formation. We have observed a new type of stable Coulomb crystals configuration that we referred to as "Christmas tree-like Coulomb crystals". We have provided numerical simulation and experimental research of the "Christmas tree-like Coulomb crystals" and discussed these structures in the perspective of Science and Art performance.

Abstract

This work seeks to make explicit the operational connection between the preparation of two-level quantum systems with their corresponding description (as states) in a Hilbert space. This may sound outdated, but we show there is more to this connection than common sense may lead us to believe. To bridge these two separated realms -- the actual laboratory and the space of states -- we rely on a paradigmatic mathematical object: the Hopf fibration. We illustrate how this connection works in practice with a simple optical setup. Remarkably, this optical setup also reflects the necessity of using two charts to cover a sphere. Abstract

The Laguerre functions $l_{n,\tau}^\alpha$, $n=0,1,\dots$, are constructed from generalized Laguerre polynomials. The functions $l_{n,\tau}^\alpha$ depend on two parameters: scale $\tau>0$ and order of generalization $\alpha>-1$, and form an orthogonal basis in $L_2[0,\infty)$. Let the spectrum of a square matrix $A$ lie in the open left half-plane. Then the matrix exponential $H_A(t)=e^{At}$, $t>0$, belongs to $L_2[0,\infty)$. Hence the matrix exponential $H_A$ can be expanded in a series $H_A=\sum_{n=0}^\infty S_{n,\tau,\alpha,A}\,l_{n,\tau}^\alpha$. An estimate of the norm $\Bigl\lVert H_A-\sum_{n=0}^N S_{n,\tau,\alpha,A}\,l_{n,\tau}^\alpha\Bigr\rVert_{L_2[0,\infty)}$ is proposed. Finding the minimum of this estimate over $\tau$ and $\alpha$ is discussed. 205. Improved convergence of the spectral proper orthogonal decomposition through time shifting

Abstract

Spectral proper orthogonal decomposition (SPOD) is an increasingly popular modal analysis method in the field of fluid dynamics due to its specific properties: a linear system forced with white noise should have SPOD modes identical to response modes from resolvent analysis. The SPOD, coupled with the Welch method for spectral estimation, may require long time-resolved data sets. In this work, a linearised Ginzburg-Landau model is considered in order to study the method's convergence. SPOD modes of the white-noise forced equation are computed and compared with corresponding response resolvent modes. The quantified error is shown to be related to the time length of Welch blocks (spectral window size) normalised by a convective time. Subsequently, an algorithm based on a temporal data shift is devised to further improve SPOD convergence and is applied to the Ginzburg-Landau system. Next, its efficacy is demonstrated in a numerical database of a boundary layer subject to bypass transition. The proposed approach achieves substantial improvement in mode convergence with smaller spectral window sizes with respect to the standard method. Furthermore, SPOD modes display growing wall-normal and span-wise velocity components along the stream-wise direction, a feature which had not yet been observed and is also predicted by a global resolvent calculation. Abstract

DUNE is an international experiment dedicated to addressing some of the questions at the forefront of particle physics and astrophysics, including the mystifying preponderance of matter over antimatter in the early universe. The dual-site experiment will employ an intense neutrino beam focused on a near and a far detector as it aims to determine the neutrino mass hierarchy and to make high-precision measurements of the PMNS matrix parameters, including the CP-violating phase. It will also stand ready to observe supernova neutrino bursts, and seeks to observe nucleon decay as a signature of a grand unified theory underlying the standard model. The DUNE far detector implements liquid argon time-projection chamber (LArTPC) technology, and combines the many tens-of-kiloton fiducial mass necessary for rare event searches with the sub-centimeter spatial resolution required to image those events with high precision. The addition of a photon detection system enhances physics capabilities for all DUNE physics drivers and opens prospects for further physics explorations. Given its size, the far detector will be implemented as a set of modules, with LArTPC designs that differ from one another as newer technologies arise. In the vertical drift LArTPC design, a horizontal cathode bisects the detector, creating two stacked drift volumes in which ionization charges drift towards anodes at either the top or bottom. The anodes are composed of perforated PCB layers with conductive strips, enabling reconstruction in 3D. Light-trap-style photon detection modules are placed both on the cryostat's side walls and on the central cathode where they are optically powered. Abstract

We study generalised quasilinear (GQL) approximations applied to turbulent plane Couette flow. The GQL framework is explored in conjunction with a Galerkin reduced-order model (ROM) recently developed by Cavalieri & Nogueira (Phys. Rev. Fluids 7, L102601, 2022), which considers controllability modes of the linearised Navier-Stokes system as basis functions, representing coherent structures in the flow. The velocity field is decomposed into two groups: one composed by high-controllability modes and the other by low-controllability modes. The former group is solved with the full nonlinear equations, whereas the equations for the latter are linearised. We also consider a modified GQL framework, wherein the linearised equations for the low-controllability modes are driven by nonlinear interactions of modes in the first group, which are characterised by large-scale coherent structures. It is shown that GQL-ROMs successfully recover the statistics of the full model with relatively high controllability thresholds and sparser nonlinear operators. Driven GQL-ROMs were found to converge more rapidly, providing accurate description of the statistics with a larger number of linearised modes with respect to standard GQL approximations. The results presented here show that further model reductions are attainable with GQL-ROMs, which can be valuable to extend these models to larger Reynolds numbers.

208. On the Low-Frequency Dynamics of Turbulent Separation Bubbles

Abstract

The low-frequency modal and non-modal stability characteristics of an incompressible, pressure-gradient-induced turbulent separation bubble (TSB) are investigated with the objective of studying the mechanism responsible for the low-frequency contraction and expansion (breathing) commonly observed in experimental studies. The configuration of interest is a TSB generated on a flat test surface by a succession of adverse and favourable pressure gradients. The base flow selected for the analysis is the average TSB from the direct numerical simulation of Coleman et al. (J. Fluid Mech., vol. 847, 2018). Global linear stability analysis reveals that the flow is globally stable for wavenumbers. The mode closest to the stability threshold appears to occur at zero frequency and low, non-zero spanwise wavenumber. Resolvent analysis is then employed to examine the forced dynamics of the flow. At low frequency, a region of low, non-zero spanwise wavenumber is also discernible, where the receptivity appears to be driven by the identified weakly damped global mode. The results from resolvent analysis are compared to the unsteady experimental database of Le Floc'h et al. (J. Fluid Mech., vol. 902, 2020) in a similar TSB flow. The alignment between the optimal response and the first spectral proper orthogonal decomposition mode computed from the experiments is shown to exceed 95 %, while the spanwise wavenumber of the optimal response is consistent with that of the low-frequency breathing motion captured experimentally. This indicates that the fluctuations observed experimentally at low frequency closely match the response computed from resolvent analysis. Based on these results, we propose that the forced dynamics of the flow, driven by the weakly damped global mode, serve as a plausible mechanism for the origin of the low-frequency breathing motion commonly observed in experimental studies of TSBs.

Abstract

We develop a field-nonlinear theory of sub-Doppler spectroscopy in a gas of two-level atoms, based on a self-consistent solution of the Maxwell-Bloch equations in the mean field and single-atom density matrix approximations. This makes it possible to correctly take into account the effects caused by the free motion of atoms in a gas, which lead to a nonlinear dependence of the spectroscopic signal on the atomic density even in the absent of a direct interatomic interaction (e.g., dipole-dipole interaction). Within the framework of this approach, analytical expressions for the light field were obtained for an arbitrary number of resonant waves and arbitrary optical thickness of a gas medium. Sub-Doppler spectroscopy in the transmission signal for two counterpropagating and co-propagating waves has been studied in detail. A previously unknown red shift of a narrow sub-Doppler resonance is predicted in a counterpropagating waves scheme, when the frequency of one wave is fixed and the frequency of the other wave is varied. The magnitude of this shift depends on the atomic density and can be more than an order of magnitude greater than the known shift from the interatomic dipole-dipole interaction (Lorentz-Lorenz shift). The found effects, caused by the free motion of atoms, require a significant revision of the existing picture of spectroscopic effects depending on the density of atoms in a gas. Coherent elastic neutrino-nucleus scattering (CE$\nu$NS) offers a valuable approach in searching for physics beyond the Standard Model. The Ricochet experiment aims to perform a precision measurement of the CE$\nu$NS spectrum at the Institut Laue-Langevin nuclear reactor with cryogenic solid-state detectors. The experiment plans to employ an array of cryogenic thermal detectors, each with a mass around 30 g and an energy threshold of sub-100 eV. The array includes nine detectors read out by Transition-Edge Sensors (TES). These TES based detectors will also serve as demonstrators for future neutrino experiments with thousands of detectors. Abstract

As one of the most widespread social dynamics, cooperative behavior is among the most fascinating collective phenomena. Several animal species, from social insects to human beings, feature social groups altruistically working for a common benefit. This collaborative conduct pervades the actions and opinions of individuals, yielding strategic decision-making between political, religious, ethnic, and economic social puzzles. Here, we explore how cooperative behavior phenomena impact collective opinion dynamics and entropy generation in social groups. We select a random fraction $f$ of community members as collaborative individuals and model the opinion dynamics using a social temperature parameter $q$ that functions as a social anxiety noise. With probability $q$, regular individuals oppose their companions about a social decision, assuming group dissent. Collaborative agents experience a reduced effective social noise $\mu q$, where $0 < \mu < 1$ is the social anxiety noise sensibility parameter that enhances social validation. We perform numerical simulations and mean-field analysis and find the system undergoes nonequilibrium order-disorder phase transitions with expressive social entropy production. Our results also highlight the effects of an individual social anxiety attenuation level in enhancing group consensus and inducing exuberant collective phenomena in complex systems.

Abstract

Primary central nervous system (CNS) lymphomas account for 13-20% of the posttransplant lymphoproliferative disorders (PTLD) and rank among the most aggressive conditions. Reduction of immunosuppressive therapy should be mandatory to treat PTLD, but this is rarely used as the only therapy option. Chemotherapy regimens for PTLD involving the CNS most commonly include high-dose rituximab and high-dose methotrexate and/or cytarabine. The efficiency only of discontinuation of immunosuppressive therapy for PTLD does not exceed 5—10%, but there are no literature data on its efficiency for PTLD involving the CNS. The paper describes a clinical case of achieving long-term remission in a female patient with Epstein-Barr virus (EBV)-positive diffuse large B-cell lymphoma involving the central nervous system, associated with immunosuppression after kidney transplantation from a related donor, in the absence of chemotherapy during immunosuppressive therapy discontinuation and transplantectomy.

218. Multiple myeloma: Maintenance therapy after autologous hematopoietic stem cell transplantation, depending on minimal residual disease

Abstract

Aim. To determine the efficiency of maintenance therapy with bortezomib in patients with multiple myeloma (MM) who have achieved complete remission (CR) after autologous hematopoietic stem cell (auto-HSCT), depending on the presence of minimal residual disease (MRD). Subjects and methods. In January 2014 to February 2016, fifty-two MM patients (19 men and 33 women) aged 24 to 66 years (median 54 years), who had achieved CR after auto-HSCT, were randomized to perform maintenance therapy with bortezomib during a year. On day 100 after auto-HSCT, all the patients underwent immunophenotyping of bone marrow plasma cells by 6-color flow cytometry to detect MRD. Relapse-free survival (RFS) was chosen as a criterion for evaluating the efficiency of maintenance therapy. Results. After auto-HSCT, MRD-negative patients had a statistically significantly higher 2-year RFS rate than MRD-positive patients: 52.9% (95% confidence interval (CI), 35.5 to 70.5%) versus 37.2% (95% CI, 25.4 to 49.3%) (p=0.05). The presence of MRD statistically significantly increased the risk of relapse (odds ratio 1.7; 95% CI, 1.2 to 3.4; p=0.05). Two-year cumulative risk of relapse (using the Kaplan-Meier) after auto-HSCT did not statistically significantly differ in MRD-negative patients receiving (n=15) and not receiving (n=10) maintenance therapy with bortezomib (p=0.58). After completion of maintenance treatment, 42% of the MRD-positive patients achieved a negative status. In the MRD-positive patients who had received maintenance therapy, the average time to recurrence was 5 months longer than that in the naïve patients: 17.3 versus 12.3 months. Conclusion. The MRD status determined in MM patients who have achieved CR after auto-HSCT is an important factor for deciding on the use of maintenance therapy.

It discusses the possible causes of their development and methods for their prevention and treatment. Controlled natural hypocoagulation, in which the dose of an administered deficient factor decreases to such an extent that in order to maintain the safe level of hypocoagulation (plasma factor activity is 15—20%; activated partial thromboplastin time is 1.5—2 times normal values), is proposed as one of the treatment options.

Breast implant-associated anaplastic large-cell lymphoma will be identified as a separate nosological entity in the 2017 adapted WHO classification due to differences in its clinical presentations, pathogenesis, and prognosis with those of nodal and cutaneous anaplastic large-cell lymphomas. The paper gives a review of the literature and describes the authors’ own clinical case of common breast implant-associated anaplastic large-cell lymphoma involving breast tissue, axillary lymph nodes, anterior chest muscles, and bone marrow. The treatment policy chosen by the authors could achieve complete remission.

Abstract

Aim. To identify a parameter predicting a collection of at least 2·106 CD34+ hematopoietic stem cells (HSC)/kg body weight per leukapheresis (LA) procedure. Subjects and methods. The investigation included 189 patients with hematological malignancies and 3 HSC donors, who underwent mobilization of stem cells with their subsequent collection by LA. Absolute numbers of peripheral blood leukocytes and CD34+ cells before a LA procedure, as well as a number of CD34+ cells/kg body weight (BW) in the LA product stored on the same day were determined in each patient (donor). Results. There was no correlation between the number of leukocytes and that of stored CD34+ cells/kg BW. There was a close correlation between the count of peripheral blood CD34+ cells prior to LA and that of collected CD34+ cells calculated with reference to kg BW. Conclusion. The optimal absolute blood CD34+ cell count was estimated to 20 per µl, at which a LA procedure makes it possible to collect 2·106 or more CD34+ cells/kg BW.

In December 2009 to March 2016, a total 134 patients aged 30 to 81 years (median 57 years) underwent a pretreatment FISH-study of bone marrow (BM) with centromeric and locus-specific DNA probes to identify amp1q21, t(11;14), t(4;14), t(14;16), t(14;20), t(6;14), trisomies of chromosomes 5, 9, 15, del13q14, del17p13/TP53, and t(8q24)/cMYC. Induction therapy with bortezomib-containing cycles was performed. Autologous stem cell transplantation was carried out in 48 patients. The median follow-up of patients was 19.3 months (3.2—77.4 months). Disease progression was diagnosed in 69 (51.5%) patients; 12 patients also underwent FISH study during disease progression. Results. At the onset of MM, amp1q21 was detected in 53 (39.6%) patients. The overall 5-year survival rate in patients with amp1q21 was almost 2 times lower than that in those without amp1q21 (43.5 and 79.4%, respectively; p=0.07). The overall 5-year survival rate in patients with one extra copy of 1q21 (only 3 copies) was 67.3%, that in those with 2 or more extra copies of 1q21 (only 4—7 copies) was 20.9% (p=0.0016). Nine (75%) of the 12 patients examined during disease progression were found to have amp1q21: 2 cases were detected in the period of progression to have amp1q21 in its absence at disease onset; 7 cases had amp1q21 both at MM onset and progression; however, the number of copies of 1q21 was unchanged. Conclusion. Аmp1q21 is one of the most common chromosomal abnormalities in patients with new-onset MM and may appear in the course of disease progression. The presence of аmp1q21 is an important prognostic factor and must have to be included in the diagnostic study both at disease onset and progression.

Abstract

Aim. To analyze the efficiency and reproducibility of the ALL-2009 protocol within the Russian prospective multicenter study based on different principles of cytostatic effects (non-intensive, but continuous cytotoxic treatment and a small number of allogeneic hematopoietic stem cells). Subjects and methods. The ALL-2009 (NCT01193933) study conducted in April 2009 to December 2016 included 194 patients (95 males and 99 females) aged 15 to 55 years (median age 28 years) with Ph-negative B-cell acute lymphoblastic leukemia (ALL). There was early pre-B-cell ALL in 54 patients, common ALL in 101, pre-B ALL in 39, initial leukocytosis in 9.4·109/l (0.4-899.0), lactate dehydrogenase in 901 IU (31-13 059), an initial central nervous system lesion in 17 (8.7%), mediastinal injury in 3 (1.5%), and splenomegaly in 111 (57.2%). The results of standard cytogenetic analysis are known in 113 (60.4%) patients. Normal karyotypes were detected in 49 (54.5%) out of the patients; t(4;11) in 9 (5.4%), t(1;19) in 2 (1.2%), and other karyotypic abnormalities in 53 (46.9%). Thirteen (7.8%) patients underwent allogeneic hematopoietic stem cell transplantation in first complete remission (CR); their proportion did not differ in the federal and regional centers. Results. The frequency of CR achievement was the same in the federal and regional centers and generally amounted to 87.5%. Early (8.8%) and CR (9.6%) mortality rates remained high despite the low aggressiveness of cytotoxic action, necessitating the improvement of auxiliary treatment. The five-year overall survival (OS) rates vary considerably in the federal and regional centers (72.6 and 43.8%), the relapse-free survival (RFS) (70.2 and 53.4%) and recurrence risk (23.1 and 36.5%) are comparable. This suggests that the non-intensive, but continuous exposure principle built in the ALL-2009 protocol makes it possible to reproduce the envisaged treatment program and to achieve satisfactory results. Conclusion. The ALL-2009 protocol allows both the federal and regional centers to obtain the long-term results comparable with those of current foreign studies: OS (54.2%), RFS (56.5%); and relapse risk (35.4%). Multivariate analysis has identified age (over 30 years), initial leukocytosis (30·109/l and more) and t(4;11) among the main clinical prognostic factors. Gene mutation detection evaluated in a small number of patients (8/36) is not a poor prognostic sign. There is a need for further investigations with centralized evaluation of the mutation status of leukemic cells and the clearance of minimal residual disease.

Abstract

Double-hit lymphoma (DHL) is a rare aggressive B-cell lymphoma with concomitant c-MYC, BCL2 or BCL6 gene rearrangements, which is characterized by the high frequency of extranodal lesions and by resistance to chemotherapy. The median survival does not exceed 18 months in patients with this disease. The majority of DHL is represented by с-MYC/BCL2 cases. The combination of c-MYC/BCL6 occurs rarely (5—8%). The paper describes a case of DHL with concomitant c-MYC and BCL6 gene rearrangements, which mimics diffuse large B-cell lymphoma, leg-type.

Abstract

Aim. To determine plasma extracellular low-molecular-weight DNA (elmwDNA) as an indicator of apoptosis in patients with chronic obstructive pulmonary disease (COPD) in remission versus healthy donors, patients with chronic non-obstructive bronchitis (CNOB), and their first-degree relatives (FDRs). Subjects and methods. The investigation recruited 110 participants, including 17 healthy donors, 31 patients with COPD, and 20 patients with CNOB in remission, 19 healthy FDRs of patients with COPD, and 23 healthy FDRs of those with SNOB. The plasma levels of elmwDNA were determined in the study participants. Nucleic acids were isolated by phenol/chloroform extraction, precipitated with ethanol, and treated with RNase; elmwDNA was analyzed by electrophoresis. Results. In patients with COPD, the mean level of elmwDNA was 7.8±2.0 ng/ml, which was 3.9 and 3.0 times statistically significantly lower than that in healthy donors and patients with SNOB, respectively; while the level of elmwDNA in the latter did not differ statistically significantly from that in healthy donors. In both the blood relatives of patients with COPD and FDRs of those with SNOB, the mean level of elmwDNA was not significantly different from that in healthy donors. The content of elmwDNA tended to increase in COPD patients aged 60—80 years as compared to those aged 45—59 years; that in both age groups was, however, significantly lower than in healthy donors of the same age. Conclusion. The level of elmwDNA in plasma and, accordingly, apoptosis in the lung are reduced in patients with COPD in remission, whereas that is unchanged in those with SNOB. In patients with COPD, blood elmwDNA release is unrelated to heredity and varies little with age. The determination of elmwDNA is recommended for use in patients with COPD to assess apoptosis.

Background and Objectives: Severe asthma seen in substantial proportion of patients has been a poorly controlled disease, with recurring symptoms and exacerbations despite the use of inhaled glucocorticoids (ICS), long-acting beta-agonists (LABAs-2), leukotriene receptor antagonist (LTRA), and theophyllines. A potential alternative approach is addition of a second bronchodilator with an alternative mode of action, the anticholinergic tiotropium bromide. Hence, the present study was undertaken to evaluate clinical effectiveness of tiotropium bromide 18 mcg as add-on therapy in severe asthma patients. Methodology: A single-blinded randomized study was carried in patients with severe asthma. The patients were randomized into two groups. The study group received an addition of 18 mcg of tiotropium while both groups were continued on LABA and high dose ICS combination, theophyllines, and LTRA. Improvement in lung function (forced expiratory volume in 1 s [FEV1], forced vital capacity [FVC], and peak expiratory flow rate [PEFR]), clinical symptoms, and quality of life were assessed by mini asthma quality of life questionnaire (mini-AQLQ) at 4, 8, and 12 weeks. Results: Seventy patients were included in the study, of which 63 completed the study period. Baseline characteristics were well matched between the groups. At the end of 12 weeks, the tiotropium group showed a mean improvement in FEV1 and FVC of 450 ml and 560 ml, respectively, as compared to the control group (P < 0.0001). PEFR showed mean difference of 16.8 L/min in the study group at the end of 12 weeks as compared to the control group and it was statistically significant (P < 0.0001). Quality of life as assessed by mini-AQLQ was statistically significant in study group as compared to control group (3.42 vs. 3.07; P < 0.0001). In study group, about 21 patients (65.62%) had no exacerbations compared to eight patients (25.8%) in the control group (P = 0.045). Conclusions: The addition of once-daily tiotropium to severe asthma treatment regimen including high-dose ICS plus LABA, theophyllines, and LTRA significantly improves lung functions and quality of life over 12 weeks of therapy.

The number of patients receiving statins increases every year and due to the fact that they should take statins during their lives, the problem of their safety use comes to the forefront. The paper analyzes the safety of using the medications of this group and discusses the diagnosis of myopathies induced by statins and the occurrence of immune-mediated statin myopathies. It considers a personalized approach to prescribing statins, analyzes Russian and foreign experience in using pharmacogenetics to reduce the risk of myopathies, publishes the results of the authors’ experience in clinically introducing pharmacogenetic testing at hospitals, and analyzes the long-term results of determining the polymorphism of the SLCO1B1 gene for the prediction of the risk of adverse events when using statins and estimating patient compliance to prescribed treatment.

Abstract

With time, any surgical specialty or procedure invariably undergoes change. New techniques and technologies become available and the next generations of surgeons innovate to modify established methods. The surgical management of retinal detachment has come a long way from the days of Custodis and Schepens. Scleral buckling (SB), which was once the “gold standard” when reattachment of the retina was desired, no longer enjoys the coveted standing of yore. Despite being in use for more than six decades, the many “modifications” of this surgical procedure are rather superficial - although some may argue that the procedure itself is superficial - and the original surgical principles described have remained constant. In this article, we focus on the utility of buckling, the dynamic physical forces at work when a buckle attempts to reattach the retina and the role of SB in this era of microincision surgeries.

Abstract

We demonstrate the gradual resolution of foveal herniation with optical coherence tomography (OCT) images after epiretinal membrane (ERM) removal. A young male presented with diminished vision in the left eye (OS). Examination of OS revealed an ERM and thickening of the foveal region. OCT delineated the ERM clearly. It also showed a central defect in the ERM through which the inner retinal layers had prolapsed into the vitreous cavity leading to foveal herniation. The patient underwent vitrectomy and surgical removal of the ERM. After removing the source of macular traction, over a period of 4 months, gradual reduction in height of the elevated central foveal tissue was observed. At 6 months, the foveal bulge had reduced remarkably and remained stable. The resolution of foveal herniation after ERM removal is a slow process. The OCT images convey that it may take few months for the foveal bulge to decrease in height. When the outer retinal layers are normal, visual recovery, though delayed, is appreciable.

Damage reduction in agricultural production has been successfully implemented across the insurance industry, where the spatial and sectoral differentiation in insurance rates enables early and efficient distribution of insurance amounts to the prevention of damage. Differentiation in insurance rates depending on the risk of land use gives the chance to agricultural producers to increase significantly the volumes of insurance risks, and the insurance company to perform reinsurance activities aimed at forestalling a possible emergency situation.

We consider the optimization model of planning the volume and range of products manufactured by the enterprise, taking into account the non-linear nature of the objective function. Currently planning is based on models with linear objective function that does not take into account economic law of decreasing marginal utility and leads to significant errors in terms of the saturation of the market sales. The feature of the proposed model is the nonlinear character of the objective function. The nonlinearity in contrast to the known models are taken into account in development of product strategies and enterprise decreasing marginal utility. Aim: compare the effectiveness of treatment with nebivololum or sotalolum in patients with hypertensive disease (HD) and paroxysmal atral fibrillation (AF) taking into account quantitative evaluation of the regulatory adaptive status (RAS). Materials and methods. 50 patients with HD of stages II-III and paroxysmal AF took part in the research, they were randomized into two groups for treatment with nebivololum (5.6±1.6 mg/day n=25) or sotalolum (157.0±38.3 mg/day, n=25). As part of combination therapy, patients were administered lisinoprilum (14.3±3.7 mg/day and 14.4±3.9 mg/day), when required alsoatorvastatinum (18.8±4.4 mg/day, n=11 and 16.0±5.1 mg/day, n=12), acetylsalicylic acid (91.2±14.1 mg/day, n=11and 92.1±16.8 mg/day, n=11), respectively. Initially and 6 months after therapy, the following was done: quantitative assessment of RAS (by cardio-respiratory synchronism test), echocardiography, triplex scanning of brachiocephalic arteries, treadmill test, six-minute walk test, all-day monitoring of blood pressure and electrocardiogram, subjective assessment of quality of life. Results. Both drug regimens comparably improved structural and functional condition of the heart, controlled arterial hypertension, effectively suppressed paroxysms of AF, improved the quality of life. At the same time, the use of nebivololum increased the RAS and increased exercise tolerance, to a lesser degree than the use of sotalolum. Conclusion. In patients with HD of stages II-III and paroxysmal AF the use of nebivololum as part of combination therapy may be preferable to sotalolum due to its positive impact on the RAS.

Abstract

Soil respiration (Rs) is an important terrestrial CO2 efflux and receives significant attention at different scale levels. However, the sampling density is limited and global Rs databases are biased towards natural ecosystems. Urbanization is among the most important current land-use trends and its role will likely grow in the future. Urban soils store considerable amount of carbon and are very heterogeneous and dynamic, which affects Rs. Our understanding of the Rs spatial variability is limited, especially for the regions with heterogeneous bioclimatic conditions and high urbanization level. The methodological constraints of direct Rs measurements in the field limit the number of observations. As an alternative approach to approximate the spatial variability of Rs, we used basal respiration (BR) as an indirect measurement. We implemented digital soil mapping technique to map BR as a proxy of Rs in a heterogeneous and urbanized Moscow Region. Topsoil and subsoils BR maps were developed for the region and spatial variability per land-use and soil type was analyzed. BR averaged for the urban areas was lower than in forests and meadows, however, urban areas became the hotspots of BR’s spatial variability in the region. Considerable contribution of subsoil layers to the total BR was also found with the maximal 30% contribution in urban soils. Although the absolute levels of respiration remained uncertain, the spatial patterns of BR are likely to correspond well with Rs patterns, determined by soil type, land use and allocation of urban areas.

Abstract

A 54-year-old male sustained ocular trauma with a projectile. Examination of the right eye revealed an intraocular foreign body (IOFB) adjacent to the optic nerve head, vitritis, vitreous hemorrhage, and translucent perivascular sheathing of the retinal vessels in all quadrants suggesting frosted branch angiitis (FBA). The patient underwent vitrectomy with removal of the IOFB and silicone oil tamponade under steroid cover. With continued use of systemic and topical steroids after surgery, complete resolution of FBA and improvement in vision were noted in a week. Prompt resolution of FBA after IOFB removal points toward a strong association between the presence of IOFB and FBA.

SettingCommunity based tuberculosis (TB) prevalence surveys in ten sites across India during 2006-2012.ObjectiveTo re-analyze data of recent sub-national surveys using uniform statistical methods and obtain a pooled national level estimate of prevalence of TB.MethodsIndividuals ≥15 years old were screened by interview for symptoms suggestive of Pulmonary TB (PTB) and history of anti-TB treatment; additional screening by chest radiography was undertaken in five sites. Two sputum specimens were examined by smear and culture among Screen-positives. Prevalence in each site was estimated after imputing missing values to correct for bias introduced by incompleteness of data. In five sites, prevalence was corrected for non-screening by radiography. Pooled prevalence of bacteriologically positive PTB was estimated using Random Effects Model after excluding data from one site. Overall prevalence of TB (all ages, all types) was estimated by adjusting for extra-pulmonary TB and Pediatric TB.ResultsOf 769290 individuals registered, 715989 were screened by interview and 294532 also by radiography. Sputum specimen were examined from 50 852 individuals. Estimated prevalence of smear positive, culture positive and bacteriologically positive PTB varied between 108.4-428.1, 147.9-429.8 and 170.8-528.4 per 100000 populations in different sites. Pooled estimate of prevalence of bacteriologically positive PTB was 350.0 (260.7, 439.0). Overall prevalence of TB was estimated at 300.7 (223.7-377.5) in 2009, the mid-year of surveys. Prevalence was significantly higher in rural compared to urban areas.ConclusionTB burden continues to be high in India suggesting further strengthening of TB control activities.
