Abstract

The study by Dittmann et al. is a beacon of hope for nuclear thyroidology. A hope that someday soon, nuclear thyroidologist will finally have a low radiation exposure, high resolution, high accuracy, PET/CT-based outpatient procedure for diagnostic imaging in DTC, and perhaps also in other thyroid and non-thyroid diseases. Certainly, the authors give this development the warmest of welcomes and can only encourage anyone interested to put in an endeavour to start studies further validating [18F]TFB, preferably in a multi-centre, international manner. Until such studies are complete, we can only wait with longing and yearning for that which for other imaging targets in nuclear medicine has been available for so long already.

Abstract

Hintergrund: Das differenzierte Schilddrüsenkarzinom weist eine sehr gute Prognose auf. Dennoch gibt es Patienten, die refraktär auf die bereits etablier-ten Therapiemöglichkeiten der operativen Therapie, der Radioiodtherapie und der dauerhaften Thyreotropin-Suppression reagieren. Chimäre Antigenrezeptor-T-Zellen (CAR-T-Zellen) haben bereits vielversprechende Ergebnisse in der Therapie hämatoonkologischer Erkrankungen erzielt. Ziel: Diese Arbeit evaluiert das Potential von CAR-T-Zellen, die gegen den Thyreotropin-Rezeptor gerichtet sind (anti-TSHR CAR-T-Zellen), gegen das dif-ferenzierte, TSHR-positive Schilddrüsenkarzinom. Material und Methoden: Zielzellen sind die papilläre Schilddrüsenkarzinom-zelllinie K-1 und die follikuläre Schilddrüsenkarzinomzelllinie FTC-133. Zur Beur-teilung der Abtötung dieser Zellen durch anti-TSHR CAR-T-Zellen wird die Min-derung der Zielzellen in Ko-Kultur unter dem Mikroskop, dem Fluoreszenzmik-roskop (nach GFP-Markierung der Zielzellen) und mittels einer Caspase-3/7-Lumineszenz-Analyse untersucht. Ergebnisse: Für K-1-Zellen besteht ein signifikanter Unterschied zwischen anti-TSHR CAR-T-Zellen und unspezifischen CAR-T-Zellen (T-Wert = 0,01), wäh-rend anti-TSHR CAR-T-Zellen im Vergleich mit unmodifizierten T-Zellen die Sig-nifikanz knapp verfehlen (T-Wert = 0,06). Für FTC-133 Zellen besteht ein signi-fikanter Unterschied zwischen anti-TSHR CAR-T-Zellen und unmodifizierten T-Zellen (T-Wert = 0,02), während anti-TSHR CAR-T-Zellen im Vergleich mit un-spezifischen CAR-T-Zellen die Signifikanz knapp verfehlen (T-Wert = 0,06) (Signifikanzniveau α = 0,05). Fazit: Anti-TSHR CAR-T-Zellen stellen eine potentielle, neue Therapiemodalität zur Ablation gesunder und maligne transformierter Schilddrüsenzellen dar., Background: Differenciated thyroid cancer presents with a very favourable prognosis. However, there are some cases with differentiated thyroid cancer that are refractory to the established treatment consistant of thyroidectomy, radioactive 131-Iodide therapy and Thyrotropin-suppression. Chimeric antigen receptor T-cells (CART) have already shown impressive results in hematological cancers. Objective: This study aims to evaluate the potential of CART targeting the Thyrotropin-receptor (anti-TSHR CART) against differentiated, TSHR-positive thyroid cancer. Material and methods: Target cells are the papillary thyroid cancer cell line K-1 and the follicular thyroid cancer cell line FTC-133. The killing of target cells by anti-TSHR CART is measured as a reduction of target cells in co-culture un-der the microscope, the fluorescence microscope (after GFP-labelling) and with a caspase-3/7-luminescnece analysis. Results: With K-1 cells we could detect a significant difference between anti-TSHR CART and unspecific CART (T-value = 0.01), while anti-TSHR CART in comparison to unmodified T cells just missed significance (T-value = 0.06). With FTC-133 cells we could detect a significant difference between anti-TSHR CART and unmodified T cells (T-value = 0.02), while anti-TSHR CART in com-parison to unspecific CART just missed significance (T-value = 0.06) (signifi-cance level α = 0.05). Conclusion: Anti-TSHR CART pose a potentially new treatment option to ab-late healthy and malignant transformed thyroid cells expressing the Thyrotropin-receptor.

245. Ga-68-PSMA-PET/CT helps to select patients for salvage radical prostatectomy with local recurrence after primary radiotherapy for prostate cancer

Abstract

Objective To investigate the diagnostic performance of gallium-68 prostate-specific membrane antigen positron emission tomography/computed tomography (Ga-68-PSMA PET/CT) in patients with recurrent prostate cancer with regard to the presence of lymph node metastases (LNM) and local recurrences after primary radiotherapy. Patients and methods We retrospectively reviewed 142 patients following salvage radical prostatectomy (sRP), 50 of which had a(68)Ga-PSMA PET/CT performed as a preoperative staging module. Predictive clinical parameters were analysed in a multivariate Cox regression analysis. Sensitivity, specificity, positive (PPV) and negative predictive values (NPV) and the accuracy of(68)Ga-PSMA PET/CT were analysed with regard to LNM and local recurrence. Results In all, 613 lymph nodes were resected in 40 patients and 23 lymph nodes had metastatic deposits in 14 patients. In all patients local recurrence could have been found with(68)Ga-PSMA PET/CT. Sensitivity, specificity, PPV and NPV and accuracy on a per lymph node basis were 34.78% (16.38-57.2%), 100% (99.38-100%), 100%, 97.52% (96.69-98.15%) and 97.55% (96.00-98.62%). For detecting local recurrence, the sensitivity and PPV were both 100% with an accuracy of 100% (92.89-100%). Conclusion Ga-68-PSMA PET/CT should be the standard imaging in biochemical recurrent prostate cancer. With this imaging module one detects first local recurrence and can detect locoregional and distant metastases more precisely than standard CT and bone scan.
