Objectives: To identify predictors of poor overall survival (OS) amongst patients with penile squamous cell carcinoma (pSCC) with clinical inguinal lymphadenopathy (cN+), in order to define the best candidates for neoadjuvant chemotherapy (NAC). Patients and Methods: Using an international, multicentre database of 924 patients with pSCC, we identified 334 men who harboured cN+ with available clinical and follow-up data. Lymph node involvement was defined either by the presence of palpable inguinal node disease or by preoperative computed tomography (CT) assessment. Fluorine-18 fluorodeoxyglucose positron-emission tomography (18F-FDG-PET)/CT scan was performed based on clinical judgment of the treating physician. Regression-tree analysis generated a risk stratification tool for prediction of 24-month overall mortality (OM). Kaplan–Meier explored the OS benefit related to the use of NAC according to the regression-tree-stratified subgroups. Results: Overall, 120 (35.9%), 152 (45.5%), and 62 (18.6%) patients harboured cN1, cN2, and cN3 disease. 18F-FDG-PET/CT was performed in 48 (14.4%) patients, and 16 (4.8%) had inguinal and pelvic nodal PET detection. The median OS was 107months, with a 24-month OS of 66%. At regression-tree analysis (area under the curve = 70%), patients with cN3 and cN2 with PET/CT-detected inguinal and pelvic nodal activity had a higher risk of 24-month OM (>50%). NAC was associated with improved 24-month OS rates (54% vs 33%) only in this subgroup of patients (P=0.002), which was also confirmed after multivariable adjustment (hazard ratio 0.28, 95% confidence interval 0.13–0.62; P=0.002). Conclusion: Patients with pSCC with cN3 or cN2 and inguinal and pelvic 18F-FDG-PET/CT scan detected disease had higher 24-month OM rates according to our regression-tree model. NAC was associated with improved OS only in these subgroups of patients. Our novel decision model may help to stratify cN+ patients, and identify those who most likely will benefit from NAC prior to radical surgical resection.

Основна частина. Проведено огляд сучасної літератури по впровадженню симуляційних методів навчання в медичній освіті. Симуляційне навчання є одним з ефективних способів навчитися керувати помилками. Симуляційне навчання дозволяє навчатися працювати відповідно до сучасних алгоритмів діагностики та надання невідкладної допомоги, виробити командну взаємодію та координацію, підвищити рівень виконання складних медичних маніпуляцій та оцінити ефективність власних дій. Зокрема, в ДЗ “ДМА МОЗ України” на кафедрі внутрішньої медицини 3 розроблена методика імітаційного навчання із залученням співробітників ВНЗ в якості стандартизованих пацієнтів для моделювання клінічних ситуацій диференціації та надання допомоги при різних невідкладних станах. Висновки. На наш погляд, дана імплементація симуляційного тренінгу є цінним інструментом вдосконалення професійних навичок студентів медичних вузів. Елементи даної методики застосовні на різних етапах освітньої діяльності: не тільки при відпрацюванні навичок невідкладної допомоги, а також при проведенні підсумкової атестації студентів 6 курсу.

Pulmonary arterial hypertension (PAH) is a serious condi­tion caused by the damage of the small pulmonary vessels, leading to the increase of pulmonary artery pressure, pulmonary vascular resistance, development of right ventricular failure and death. PAH is one of the actual problems of modern medicine due to low survival rate, rapid disability of the patients and a high cost of treatment. PAH is one of the leading causes of death in systemic sclerodermia (SSD). PAH associated with scleroderma is a unique phenotype combining the manifestations of both SSD and PAH, the pathogenetic mechanisms of which modify the clinical picture and the course of these conditions. Timely diagnosis and treatment of PAH show significant effect on survival rates, however, early detection of PAH is still difficult in SSD due to several factors. The main causes are restriction of modern screening methods and polyorganic involvement in SSD. In comparison with other subgroups of PAH the patients with SSD-PAH poorly answer to the specific PAH therapy. SSD-PAH, along with idiopathic pulmonary hypertension (IPAH), belongs to group I of pulmonary hypertension classification and according to modern consensus has a similar pathogenesis and clinical picture, however, differences in the response to therapy are observed in these groups, that indicates the role of other pathobiological mechanisms. Recent investigations explain these differences by such factors as autoimmune and inflammatory responses, more severe vascular remodeling and direct myocardial damage in the SSD. Drug therapy of PH in SSD is similar to that in IPAH and includes prostaglandins, endothelin receptor antagonists, calcium channel blockers, which are prescribed in cases of a positive vasoreactive test, PDE-5 inhibitors. In this literature review we showed traditional and new methods of PAH treatment and their relevance to SSD-PAH in accordance with randomized clinical trials.

Adjuvant Nivolumab for Invasive Urothelial Carcinoma In a prospective, randomized trial involving patients with urothelial carcinoma who had undergone radical surgery, adjuvant nivolumab was compared with placebo. The median disease-free survival was 20.8 months with nivolumab and 10.8 months with placebo. Treatment-related adverse events of grade 3 or higher were noted in 17.9% of patients in the nivolumab group. Background The role of adjuvant treatment in high-risk muscle-invasive urothelial carcinoma after radical surgery is not clear. Methods In a phase 3, multicenter, double-blind, randomized, controlled trial, we assigned patients with muscle-invasive urothelial carcinoma who had undergone radical surgery to receive, in a 1:1 ratio, either nivolumab (240 mg intravenously) or placebo every 2 weeks for up to 1 year. Neoadjuvant cisplatin-based chemotherapy before trial entry was allowed. The primary end points were disease-free survival among all the patients (intention-to-treat population) and among patients with a tumor programmed death ligand 1 (PD-L1) expression level of 1% or more. Survival free from recurrence outside the urothelial tract was a secondary end point. Results A total of 353 patients were assigned to receive nivolumab and 356 to receive placebo. The median disease-free survival in the intention-to-treat population was 20.8 months (95% confidence interval [CI], 16.5 to 27.6) with nivolumab and 10.8 months (95% CI, 8.3 to 13.9) with placebo. Еру primary Sjogren’s Syndrome (PSS) is a chronic autoimmune disease of unknown etiology with the specific predisposition to the occurrence of exocrine gland inflammation. This disease is characterized by the presence of the sicca syndrome and systemic manifestations, including inflammatory arthritis, skin lesions, hematological disorders, neuropathies, interstitial lung disease. Systemic manifestations occur in 70 % of patients with PSS. In addition, such patients are characterized by a high risk of developing B-cell lymphoma, comprising 5 — 10 %. Over the past decade, a series of practical guidelines for managing patients with PSS have been published, based on the systematic reviews involving both experts and patients. This literature review on the treatment of PSS is based on an analysis of current recommendations, systematic reviews, and clinical studies. Treatment stratification aims to alleviate symptoms, stimulate secretion, prevent damage, and suppress the activity of a systemic disease. The first-line therapy for treating dry eye syndrome should be volume replacement and lubrication using artificial tear preparations and eye gels, the main components of which should be lubricants with a polymer base or agents. При вовлечении в патологический процесс внутренних органов глюкокортикоиды следует использовать в минимальной дозе и в течение периода, необходимого для контроля активного системного заболевания. При залученні у патологічний процес внутрішніх органів глюкокортикоїди слід використовувати в мінімальній дозі та протягом періоду, необхідного для конт­ролю активного системного захворювання.
