Abstract

Photoelectrochemical (PEC) fixation of carbon dioxide (CO2) into intermediates of Profen drugs, a kind of nonsteroidal antiinflammatory drug, is a useful and important technology for the utilization of CO2. In this work, we successfully designed and prepared a silver nanoparticles (AgNPs)-embedded silicon photocathode for PEC-fixating CO2 to synthesize intermediates of Profen drugs with high efficiency and stability. AgNPs endow the photocathode with extremely high catalytic activity, the Schottky junction between Ag and Si promotes the separation and transfer of photogenerated carriers, the AgNPs-embedded structure greatly improves the stability of the photocathode, and the nanohole structure improves light absorption. 228. EARLY INITIATION OF RENAL REPLACEMENT THERAPY IN INTENSIVE CARE UNIT PATIENTS WITH BOTH ACUTE RESPIRATORY DISTRESS SYNDROME AND SEPSIS WITH OR WITHOUT RENAL FAILURE: A RETROSPECTIVE COHORT STUDY BASED ON PROPENSITY SCORE MATCHING

Abstract

Background:It is unknown whether early renal replacement therapy (RRT) initiation strategy in intensive care unit (ICU) patients with both acute respiratory distress syndrome (ARDS) and sepsis with or without renal failure is clinically beneficial. Patients and methods:A total of 818 patients with both ARDS and sepsis admitted to the ICU of Tianjin Medical University General Hospital were included in the analysis. Early RRT was defined as initiating the RRT strategy within 24 h of admission. The relationship between early RRT and clinical outcomes, including primary (30-day mortality) and secondary (90-day mortality, serum creatinine, Pao2/Fio2, duration of invasive mechanical ventilation, cumulative fluid output, and cumulative fluid balance) outcomes, was compared using propensity score matching (PSM). Results:A total of 277 patients (33.9% of the total population) underwent an early RRT initiation strategy before PSM. After PSM, a cohort of 147 patients with early RRT and 147 patients without early RRT with matched baseline characteristics (including serum creatinine at admission) were constructed. Early RRT was not significantly associated with 30- (hazard ratio [HR], 1.25; 95% confidence interval [CI], 0.85-1.85; P= 0.258) or 90-day mortality (HR, 1.30; 95% CI, 0.91–1.87, P= 0.150). At each time point within 72 h after admission, there was no significant difference in serum creatinine, Pao2/Fio2and duration of mechanical ventilation between the early and the no early RRT groups. Early RRT significantly increased total output at all time points within 72 h of admission and reached a statistically significant negative fluid balance at 48 h. Conclusions:Early RRT initiation strategies had no statistically significant survival benefit in ICU patients with both ARDS and sepsis, with or without renal failure, nor did they significantly improve serum creatinine and oxygenation or shorten the duration of mechanical ventilation. The use and timing of RRT in such patients should be thoroughly investigated.

Abstract

Background: No consensus has been reached on an optimal blood lactate evaluation system although several approaches have been reported in the literature in recent years. A group-based trajectory modeling (GBTM) method could better stratify patients with acute respiratory distress syndrome (ARDS) complicated with sepsis in the intensive care unit (ICU).Patients and Methods: 760 patients from the comprehensive ICU of Tianjin Medical University General Hospital with ARDS complicated with sepsis were eligible for analysis. Serial serum lactate levels were measured within 48 h of admission. In addition to the GBTM lactate groups, the initial lactate, peak lactate level, the area under the curve of serial lactate (lactate AUC), and lactate clearance were also considered for comparison. The short- and long-term outcomes were the 30- and 90-day mortality, respectively.Results: Three lactate groups were identified based on GBTM, with group 3 exhibiting the worse short- [hazard ratio (HR) for 30-day mortality: 2.96, 95% confidence interval (CI) 1.79-4.87, P < 0.001] and long term (HR for 90-day mortality: 3.49, 95% CI 2.06-5.89, P < 0.001) outcomes followed by group 2 (HR for 30-day mortality: 2.05, 95% CI 1.48-2.84, P < 0.001 and HR for 90-day mortality: 1.99, 95% CI 1.48-2.67, P < 0.001). GBTM lactate groups exhibited significantly improved diagnostic performance of initial lactate + SOFA scores/APACHE II scores models. Based on the multivariable fractional polynomial interaction (MFPI) approach, GBTM lactate groups could better differentiate high-risk patients than the initial lactate groups in short- and long-term outcomes.Conclusions: To the best of our knowledge, this is the first report that GBTM-based serial blood lactate evaluations significantly improve the diagnostic capacity of traditional critical care evaluation systems and bring many advantages over previously documented lactate evaluation systems. Abstract

Higher-order chromatin structure regulates gene expression, and mutations in proteins mediating genome folding underlie developmental disorders known as cohesinopathies. However, the relationship between three-dimensional genome organization and embryonic development remains unclear. Here we define a role for bromodomain-containing protein 4 (BRD4) in genome folding, and leverage it to understand the importance of genome folding in neural crest progenitor differentiation. Brd4 deletion in neural crest results in cohesinopathy-like phenotypes. BRD4 interacts with NIPBL, a cohesin agonist, and BRD4 depletion or loss of the BRD4-NIPBL interaction reduces NIPBL occupancy, suggesting that BRD4 stabilizes NIPBL on chromatin. Chromatin interaction mapping and imaging experiments demonstrate that BRD4 depletion results in compromised genome folding and loop extrusion. Finally, mutation of individual BRD4 amino acids that mediate an interaction with NIPBL impedes neural crest differentiation into smooth muscle. Remarkably, loss of WAPL, a cohesin antagonist, rescues attenuated smooth muscle differentiation resulting from BRD4 loss. Collectively, our data reveal that BRD4 choreographs genome folding and illustrates the relevance of balancing cohesin activity for progenitor differentiation.

242. A Novel Diagnosis Method Based on the Feature Selection Algorithm and Convolutional Neural Network for SF6 Gas-Insulated Equipment

Abstract

SF6-decomposition-based insulation degradation of gas-insulated equipment has attracted considerable research attention in the power industry. However, because limited feature mining has been performed for SF6 decomposition components, diagnostic accuracy is reduced. Therefore, in this study, a novel diagnosis strategy was proposed based on various feature selection algorithms and convolutional neural network (CNN). In all, 19 characteristic products were obtained by combining original characteristic products. Various feature selection algorithms were used to mine feature information. A novel CNN structure was designed. The 1-D input layer was reorganized into the 2-D layer to improve the operation efficiency of the CNN. The robustness and generalization ability of the network were improved by adjusting the training step size and convolution kernel size or number. The diagnosis accuracy of the SF6 decomposition component was improved to 99%. Thus, a set of accurate and effective diagnosis schemes was proposed for the power industry.

Abstract

Transcription at most promoters is divergent, initiating at closely spaced oppositely oriented core promoters to produce sense transcripts along with often unstable upstream antisense transcripts (uasTrx). How antisense transcription is regulated and to what extent it is coordinated with sense transcription is not well understood. Here, by combining acute degradation of the multi-functional transcription factor CTCF and nascent transcription measurements, we find that CTCF specifically suppresses antisense but not sense transcription at hundreds of divergent promoters. Primary transcript RNA-FISH shows that CTCF lowers burst fraction but not burst intensity of uasTrx and that co-bursting of sense and antisense transcripts is disfavored. Genome editing, chromatin conformation studies and high-resolution transcript mapping revealed that precisely positioned CTCF directly suppresses the initiation of uasTrx, in a manner independent of its architectural function. In sum, CTCF shapes the transcriptional landscape in part by suppressing upstream antisense transcription.

244. Study on optimization of laying mode of two parallel high voltage cables in the same phase of two circuits

Abstract

In power lines, the proportion of parallel cables is gradually increasing, but the uneven current distribution and sheath circulation affect the safe and stable operation of parallel cables. Existing studies show that cable current and sheath circulation are obviously affected by cable laying mode. At present, the designed cable laying mode is as “pin” laying and horizontal “I” laying. Combined with the above engineering difficulties, this paper focuses on the parameter optimization scheme of 220 kV two circuit parallel cables in the same direction when the metal sheath is cross connected and grounded. Firstly, the operation model of 220kV two circuits in-phase two parallel cables is established. The numerical differences between the current distribution non-uniformity coefficient and sheath circulating current of the cables are analyzed and calculated under the condition of “pin” laying and horizontal “I” laying. Meanwhile, the influence of different phase sequence arrangement on the current distribution non-uniformity coefficient and sheath circulating current of the cables is compared. Secondly, the cable laying mode of “symmetrical pin” is designed, and the improvement effect of uneven coefficient of cable current distribution and metal sheath circulating current under “symmetrical pin” laying is compared and analyzed. Finally, the differences of the above parameters in different phase sequence arrangements are compared and analyzed. The research results show that the symmetrical pin laying can improve the uneven distribution of current and sheath circulating current, which has important engineering practical value.
