Elderly people in regions of China with hot summers and cold winters have significantly higher heat sensitivity than people in other regions and are ambiguous in their subjective perceptions of temperature, humidity, and solar radiation. This makes the elderly more vulnerable to the heat; consequently, when they engage in outdoor activities during the summer wearing light clothing, their diminished thermal perception increases the risk of heat stress injuries. Therefore, to more accurately evaluate the outdoor thermal comfort perception of the elderly in summer, this study used traditional field meteorological measurements, a questionnaire survey, physiological data, and machine learning prediction methods, to establish an outdoor thermal benchmark for retirement communities in hot summer and cold winter regions. Findings from the study reveal that the neutral universal thermal climate index (NUTCI) and the neutral universal thermal climate index range are 25.94°C and 22.23°C to 29.66°C respectively, and that the thermal comfort threshold is 35.39°C. It was also found that for 80% of elderly residents in the two retirement communities studied, the thermal acceptable range is from 19.41°C to 35.07°C. Using these findings as a guide, the thermal categories proposed are neutral 22.23°C to 33.08°C, slightly warm 33.08°C to 39.68°C, warm 39.68°C to 43.52°C, and hot above 43.52°C, with a preferred UTCI of 27.02°C. Background: Health and quality of life (QOL) are crucial constituents. In providing social safety for older people, the weakening social support system and rising burdens are having huge impact. Purpose: To assess the QOL of old age people and their relationship with availing of non-food-based social assistance schemes. Methods: This was a descriptive community-based study which was conducted in urban slums of Bankura district, West Bengal among all individuals aged ≥60 years fulfilling inclusion and exclusion criteria. WHOQOL-BREF questionnaire was used for assessing QOL. Results: A total of 107 participants were recruited for the study. Most common morbidities were hypertension, diabetes mellitus, chronic bronchitis, etc. The majority (80.5%) of participants were receiving non-food-based NSAS for less than 10 years. Overall QOL was ‘Good’ in 69.2% of study subjects. Most of the elderly had ‘Good’ QOL in Physical, Psychological, Social, and Environment domains. Participants availing nonfood-based NSAS had good physical, social, environmental health of QOL and overall QOL. Objective: The objective of this narrative review is to synthesize current research on the impact of fall-proof exercise programs on the quality of life of older adults. Methods and Materials: A comprehensive literature review was conducted using multiple electronic databases, including PubMed, Web of Science, and Google Scholar, covering the period from 2014 to 2024. Relevant studies on fallproof exercise programs targeting the elderly population were selected based on their focus on balance, strength training, and quality of life measures. Both quantitative and qualitative studies were included, and a snowballing technique was employed to expand the scope of the review. Findings: Fall-proof exercise programs demonstrated significant improvements in various aspects of quality of life for older adults. Physical benefits included enhanced balance, muscle strength, and functional mobility. Psychological improvements were observed in terms of reduced fear of falling, increased confidence, and improved cognitive function. Social engagement also improved, particularly in group-based programs, which fostered social interaction and peer support. The diversity in program settings, from group classes to home-based interventions, provided flexible options for elderly participants. Conclusion: Fall-proof exercise programs offer a holistic approach to fall prevention and overall well-being in older adults, addressing both physical and psychological aspects. These programs have the potential to significantly enhance quality of life, reduce fall risks, and promote independence among the elderly. Background and Purpose: Growth differentiation factor 11 (GDF11) and myostatin (growth differentiation factor 8) are related to most diseases and chronic disorders in elderly population. Even though these two factors have similar structural and signaling pathways, they can have different functions, and the available information about their effects on the regeneration of degenerated muscles during old age is contradictory. GDF11 plays a role in skeletal muscle regeneration, combating aging-induced muscle loss, and regulating metabolic homeostasis and energy balance. On the other hand, myostatin, mainly secreted by skeletal muscles, plays a role in inhibiting muscle hypertrophic response (negative regulation of muscle mass) and modulating metabolic processes. Also, its increased expression is related to obesity and insulin resistance. Considering the beneficial effect of resistance training on these variables, the present study aimed to investigate the effect of 12 weeks of elastic band resistance training on the serum levels of growth differentiation factor 11, myostatin, and insulin resistance in overweight elderly women. Materials and Methods: Twenty-eight overweight elderly women with an average age of 74.04 ± 4.69 years and a body mass index of 28.21 ± 3.35 kg/m2 were selected based on the inclusion criteria and then randomly divided into exercise (n = 14) and control (n = 14) groups. Subjects in training group performed 12 weeks of Elastic band resistance training, 3 days per week. Blood samples were taken 24 hours before the first training session and 48 hours after the last training session; and were analyzed for serum levels of GDF11, myostatin, insulin, and glucose. All data were analyzed by two-way repeated-measures of ANOVA and using SPSS version 25 software at a significance level of P<0.05. Results: After 12 weeks of elastic band resistance training, there were no significant differences in GDF11, BMI, and WHR variables between the two training and control groups (p>0.05), though, the serum levels of myostatin (P=0.027), insulin resistance index (P=0.045) and body fat percentage (P=0.023) decreased significantly. Conclusion: It seems that the decrease in the level of myostatin following elastic band training, which was associated with a reduction in insulin resistance and body fat percentage, can have a positive effect on the muscle mass and energy homeostasis of overweight elderly women. Also, in future studies related to GDF11, it is necessary to select the type, duration, and intensity of exercise as well as measurement methods with more scrutiny and accuracy. Aims: There is a lack of tools for accurately identifying the risk of readmission for heart failure in elderly patients with arrhythmia. The aim of this study was to establish and compare the performance of the LACE [length of stay ('L'), acute (emergent) admission ('A'), Charlson comorbidity index ('C') and visits to the emergency department during the previous 6 months ('E')] index and machine learning in predicting 1 year readmission for heart failure in elderly patients with arrhythmia. Methods: Elderly patients with arrhythmia who were hospitalized at Sichuan Provincial People's Hospital between 1 June 2018 and 31 May 2020 were enrolled. The LACE index was calculated for each patient, and the area under the receiver operating characteristic curve (AUROC) was calculated. Six machine learning algorithms, combined with three variable selection methods and clinically relevant features available at the time of hospital discharge, were used to develop machine learning models. AUROC and area under the precision–recall curve (AUPRC) were used to assess discrimination. Shapley additive explanations (SHAP) analysis was used to explain the contributions of the features. Results: A total of 523 patients were enrolled, and 108 patients experienced 1 year hospital readmission for heart failure. The AUROC of the LACE index was 0.5886. The complete machine learning model had the best predictive performance, with an AUROC of 0.7571 and an AUPRC of 0.4096. The most important predictors for 1 year readmission were educational level, total triiodothyronine (TT3), aspartate aminotransferase/alanine aminotransferase (AST/ALT), number of medications (NOM) and triglyceride (TG) level. Conclusions: Compared with the LACE index, the machine learning model can accurately identify the risk of 1 year readmission for heart failure in elderly patients with arrhythmia. Diabetes management applications effectively help patients monitor blood glucose levels and adjust diet and exercise plans. However, most diabetes management apps, including the SugarShift application developed in 2023, use generalized designs that often fail to consider the unique cognitive and physiological characteristics of elderly users, thereby impacting their acceptance and usage. This paper proposes an enhancement for the SugarShift application by introducing a mode specifically designed for elderly users. This proposed enhancement aims to directly involve elderly patients so as to meet their needs better through a participatory design approach. A heuristic evaluation conducted with six experts identified 126 usability issues and 179 heuristic violations in the current version, which has led to the recommendation to develop optimization strategies tailored for elderly users. Subsequent usability testing with 28 elderly patients and six experts emphasized the importance of interface simplicity, logical operation, and interaction quality in enhancing user experience. These factors improve usability, facilitate information processing, and reduce cognitive load. Despite the overall usability of SUS scores, significant challenges still need to be addressed in information display, system feedback, and user interaction. Recommendations for future enhancements include simplifying data entry and presentation, improving readability, and providing timely feedback to enhance usability and user satisfaction, ultimately increasing user retention. Background and aims: Injuries from Road Traffic Accidents (RTAs), particularly among vulnerable groups such as the elderly, are considered a significant public health concern. The aim of this study was to show epidemiology of RTIs among the elderly people in the Eastern Azerbaijan province of Iran. Method: This cross‐sectional study included all patients over 60 years old who admitted to the Shohada Hospital from 2006 to 2016. Data were extracted from the Hospital Information System. Injuries types were classified based on the International Classification of Diseases (ICD‐10). Descriptive statistics (Mean, median and frequency) and inferential statistics (The Pearson Chi‐square test) were used for data analysis through SPSS‐24 software. Results: A total of 3509 RTAs involving patients over 60 years old from 2006 to 2016. These traffic‐related injuries comprised 15% of all recorded injuries (23,321 cases). The mean age of the patients was 69.91 ± 7.61 years, with a predominant male representation (2324 patients, 64.74%). The median Length of Stay (LOS) in hospital was 3 days. Injuries most commonly affected the knee and lower leg regions (27.5%). The most frequent types of accidents were those involving car passengers (40.47%) and pedestrians (36.33%). The majority of RTAs occurred during the summer season. There was a significant difference in the rate of RTAs based on variables such as sex, age, season, and LOS (p < 0.001). Conclusion: The study highlights traffic‐related injuries among the elderly, mostly affecting the knee and lower leg regions, especially during the summer season. The efforts should include enhancing safety measures for car passengers and pedestrians, particularly during the high‐risk summer months. Background: Olfactory dysfunction was associated with poorer cognition. However, the association between transient receptor potential cation channel subfamily A member 1 (TRPA1) and cognitive function have not been studied. This study aimed to evaluate the mediation effect of TRPA1 on the association between olfactory and cognitive function among Chinese older adults. Methods: We recruited 121 participants with cognitive impairment (CI) and 135 participants with normal cognition (NC) from a memory clinic and the "Shanghai Aging Study." Olfactory identification of each participant was measured by the Sniffin' Sticks Screening Test 12 (SSST-12). Serum TRPA1 were quantified using the Enzyme-Linked Immunosorbent Assay. The mediation effects of TRPA1 on the association between olfactory function and cognitive function were explored using mediation analysis. Results: The CI group had a significantly higher proportion of the high level of serum TRPA1 (58.7%) than the NC group (42.2%) (p = 0.0086). After adjusted for gender, age, and years of education, mediation analysis verified that TRPA1 partially mediated the association between SSST-12 and Mini Mental State Examination (MMSE). It also verified that TRPA1 partially mediated the association between the identification of peppermint and MMSE. Conclusion: Our study emphasizes the mediation role of TRPA1 in the relationship between olfactory and cognitive function among older adults. Background: Untreated hearing loss has an effect on cognition. It is hypothesized that the additional processing required to compensate for the sensory loss affects the cognitive resources available for other tasks and that this could be mitigated by a hearing device. Methods: The impact on cognition of cochlear implants (CIs) was tested in 100 subjects, ≥60 years old, with bilateral moderately-severe to profound post linguistic deafness using hearing aids. Data was compared pre and 12 and 18 months after cochlear implantation for the speech spatial qualities questionnaire, Mini Mental State Examination (MMSE), Trail making test B (TMTB) and digit symbol coding (DSC) from the Wechsler Adult Intelligence Scale version IV and finally the timed up and go test (TUG). Subjects were divided into young old (60-64), middle old (65-75) and old old (75+) groups. Cognitive test scores and times were standardized according to available normative data. Results: Hearing significantly improved pre- to post-operatively across all age groups. There was no change post-implant in outcomes for TMTB, TUG or MMSE tests. Age-corrected values were within normal expectations for all age groups for the TUG and MMSE. However, DSC scores and TMTB times were worse than normal. There was a significant increase in DSC scores between baseline and 12-months for 60- to 64-year-olds (t[153] = 2.608, p = 0.027), which remained at 18 months (t[153] = 2.663, p = 0.023). Discussion: The improved attention and processing speed in the youngest age group may be a consequence of reallocation of cognitive resources away from auditory processing due to greatly improved hearing. The oldest age group of participants had cognition scores closest to normal values, suggesting that only the most able older seniors tend to come forward for a CI. Severe to profoundly deaf individuals with hearing aids or cochlear implants were still poorer than age-equivalent normally hearing individuals with respect to cognitive flexibility, attention, working memory, processing speed and visuoperceptual functions. Due to a lack of data for the TUG, TMTB and DSC in the literature for hearing impaired individuals, the results reported here provide an important set of reference data for use in future research. Background: Frailty is a common geriatric syndrome associated with adverse health outcomes in older adults. This study aimed to examine the association between frailty index and mortality in elderly residents of nursing homes in Iran. Methods: This historical cohort study used data extracted from the Sina Electronic Health Record System (SinaEHR®, Iran) and the national cause of death registry in Iran. The study population included 9199 adults aged =60 assessed for frailty in nursing homes across Iran from April 2021 to June 2022. They were followed up until August 2022. Frailty was defined as a score >3 on Fried's 5-item index. We used multivariable logistic regression to determine adjusted odds ratios (ORs) with 95% confidence intervals (CIs) for mortality-related factors. The chisquare test compared mortality across groups. Data were analyzed using Stata software, version 11 with P<0.05 as the significance level. Results: Of 9199 subjects, 3566(38.7%) were frail, and 5643(61.3%) were non-frail. During follow-up, 3354(36.5%) deaths occurred. Frailty was associated with 2.49 times higher odds of mortality (95% CI, 2.27%, 2.74%; P<0.001) compared to the non-frail state after adjustment for covariates. Older age (P<0.001), female sex (P<0.001), and comorbidities (P<0.001) were also significant predictors of mortality. Conclusion: Frailty index strongly predicts mortality risk in Iranian elderly nursing home residents. Assessing and managing frailty may help reduce adverse outcomes in this vulnerable population. Background: Dietary composition is one of the factors influencing the acid–base balance of the body by providing acid or base precursors. One of the methods for assessing the acid-forming potential of a diet is to calculate its potential renal acid load (PRAL). The aim of this study was to identify the sociodemographic, lifestyle, and health factors related to the PRAL. Methods: Dietary intake was assessed among 133 individuals aged 70+ years using the three-day record method. Results: The average PRAL value was 15.7 mEq/day (range from −42.4 to +101.7). The diets of a majority of the participants (71.4%) had acid-forming potential (PRAL > 0). From a univariate analysis, the acid-forming potential of the diets was linked mainly to women (65.3% in PRAL > 0 group vs. 10.5% in PRAL < 0 group), people using dietary supplements, those who consumed alcohol, those who assessed their health as being at least good, people with osteoporosis, those hospitalized during the previous year, and those with rather lower physical activity. Conclusions: From a multivariate analysis, gender was the strongest predictor of an acid-forming diet, but the following also contributed: an average self-rated health status (compared to good), a good health status (compared to poor), alcohol drinking, hospitalization, lack of nutritional knowledge, and, to a lesser extent, non-frail status (compared to pre-frail). Purpose: To assess whether the Modified 5 (mFI-5) and 11 (mFI-11) Factor Frailty Indices associate with postoperative mortality, complications, and functional benefit in supratentorial meningioma patients aged over 80 years. Methods: Baseline characteristics were collected from eight centers. Based on the patients' preoperative status and comorbidities, frailty was assessed by the mFI-5 and mFI-11. The collected scores were categorized as "robust (mFI=0)", "pre-frail (mFI=1)", "frail (mFI=2)", and "significantly frail (mFI≥3)". Outcome was assessed by the Karnofsky Performance Scale (KPS); functional benefit was defined as improved KPS score. Additionally, we evaluated the patients' functional independence (KPS≥70) after surgery. Results: The study population consisted of 262 patients (median age 83 years) with a median preoperative KPS of 70 (range 20 to 100). The 90-day and 1-year mortality were 9.0% and 13.2%; we recorded surgery-associated complications in 111 (42.4%) patients. At last follow-up within the postoperative first year, 101 (38.5%) patients showed an improved KPS, and 183 (69.8%) either gained or maintained functional independence. "Severely frail" patients were at an increased risk of death at 90 days (OR 16.3 (CI95% 1.7-158.7)) and one year (OR 11.7 (CI95% 1.9-71.7)); nine (42.9%) of severely frail patients died within the first year after surgery. The "severely frail" cohort had increased odds of suffering from surgery-associated complications (OR 3.9 (CI 95%) 1.3-11.3)), but also had a high chance for postoperative functional improvements by KPS≥20 (OR 6.6 (CI95% 1.2-36.2)). Conclusion: The mFI-5 and mFI-11 associate with postoperative mortality, complications, and functional benefit. As the proportion of elderly individuals in developed countries continues to rise globally, addressing their healthcare needs, particularly in preserving their autonomy, is of paramount concern. A growing body of research focuses on Ambient Assisted Living (AAL) systems, aimed at alleviating concerns related to the independent living of the elderly. This systematic review examines the literature pertaining to fall detection and Human Activity Recognition (HAR) for the elderly, two critical tasks for ensuring their safety when living alone. Specifically, this review emphasizes the utilization of Deep Learning (DL) approaches on computer vision data, reflecting current trends in the field. A comprehensive search yielded 2,616 works from five distinct sources, spanning the years 2019 to 2023 (inclusive). From this pool, 151 relevant works were selected for detailed analysis. The review scrutinizes the employed DL models, datasets, and hardware configurations, with particular emphasis on aspects such as privacy preservation and real-world deployment. The main contribution of this study lies in the synthesis of recent advancements in DL-based fall detection and HAR for the elderly, providing insights into the state-of-the-art techniques and identifying areas for further improvement. Given the increasing importance of AAL systems in enhancing the quality of life for the elderly, this review serves as a valuable resource for researchers, practitioners, and policymakers involved in developing and implementing such technologies. This study investigates the impact of the attitudes of the elderly on the acceptance of Thailand's high-speed rail technology according to the technology readiness index (TRI) and technology acceptance model (TAM) theories as guidelines for policies or strategies to enhance passengers' intentions to use high-speed rail. A self-administered questionnaire was used to collect data from 3200 elderly people aged over 60 years in the surveyed areas along high-speed rail routes in Thailand, before the use of statistical analysis and multigroup structural equation modeling (SEM) to analyze variations in the participants' attitudes toward urban and rural areas. The results that were thus obtained from both groups showed their differing attitudes toward the acceptance of technology. The TAM theory considers the attitude toward high-speed rail use in urban areas to be important, while, in rural areas, attitudes and perceived usefulness are important. With respect to the ease of use of high-speed rail, the most important factors were attitudes toward use and perceived usefulness. For the TRI theory, innovativeness features as the most positive influence on the perceived ease of high-speed rail use in both groups. Optimism and innovativeness were positive influences, but discomfort and insecurity carried a negative influence with respect to the perceived ease of use and usefulness. [ABSTRACT FROM AUTHOR]

Objective: Epileptic seizures occurring in late adulthood often remain of unknown origin. Sporadic cerebral amyloid angiopathy (CAA) is a cerebral small vessel disease characterized by intracerebral hemorrhage, microhemorrhage and superficial siderosis, occurring mostly in elderly. This observational case–control study aimed to assess the occurrence of CAA in patients experiencing their first seizure in late adulthood. Methods: We enrolled consecutive patients aged ≥55 years presenting with late‐onset seizures (LOS) to the emergency departments or outpatient clinics of two Italian centers, from April 2021 to October 2022. Two age‐matched control subjects with neurological symptoms other than epileptic seizure were recruited for each enrolled case. All participants underwent brain MRI (1.5 Tesla) including blood‐sensitive sequences and were assessed for probable CAA diagnosis according to Boston criteria 2.0. Chi‐squared test was performed to evaluate group differences. Univariate logistic regression analysis tested the association between clinical variables and CAA. Results: We included 65 patients with LOS (27 females; mean age 72.2 ± 8.9 years) and 130 controls (49 females; mean age 70.3 ± 8.9 years). Diagnosis of probable CAA was achieved in 10.8% (7/65) of LOS patients and 2.3% (3/130) controls, with a statistically significant difference (p = 0.011). The OR for CAA in the LOS group was 5.2 as compared to the control group (95% CI = 1.3–20.6, p = 0.02). Significance: The frequency of CAA is significatively higher in patients with LOS as compared to other neurological diseases, suggesting that a portion of LOS of unknown or vascular origin are associated with CAA. Plain Language Summary: Late‐onset seizures (LOS) are very frequent in the elderly and often have no clear cause. Cerebral amyloid angiopathy (CAA) is a condition where amyloid proteins build up in the blood vessels of the brain, causing them to become weak and prone to bleeding. Background: Concurrent with population ageing, falls have become a significantly more challenging public health issue among older adults. Three years of data collected recently from a nursing home in northern Taiwan reveals an increasing trend in fall density that is primarily associated with aging, physiological decline, chronic diseases, polypharmacy, osteoporosis, and lack of exercise. The percentage of nursing home residents at high risk of falls is currently at 12.6%, and the fall rate has been reported as reaching as high as 30% annually. Purpose: A fall prevention program was implemented to reduce the fall incidence rate to 18%, with secondary goals of improving fall prevention awareness, behavior, self-efficacy, lower limb muscle strength, balance, and gait by 10% on average, respectively, between pre-test and post-test. Resolution: From September 30, 2023 to February 29, 2024, a health promotion activity and fall prevention exercise course were implemented using an interdisciplinary team collaboration model over a six-week period, providing individualized exercise for the participants. Result: The study included 20 older adults with an average age of 88 years. Most (90%; n = 18) had chronic diseases, 25% (n = 5) were on more than nine medications, 70% (n = 14) had reduced bone mass, and 40% (n = 8) were at high risk of falls, with a fall incidence rate of 30% during the immediately preceding year. Post-intervention, the fall incidence rate dropped to 5%, fall prevention awareness, behavior, and self-efficacy increased by 18.3%, and lower limb muscle strength, balance, and gait improved by 11.7%. The post-test results in fall prevention awareness, behavioral changes, self-efficacy, and lower limb strength, balance, and gait were all significantly better than pre-test results, with all results achieving statistical significance. This study evaluated the effect of ultrasound using papain and sodium bicarbonate (SC) on meat tenderness to achieve the desired texture for elderly individuals. Meats were immersed in distilled water (DW) or papain (PI), ultrasonically treated with papain (UPI), or ultrasonically treated with papain and SC (UPIS). Response surface methodology was used to optimize the processing conditions with the lowest hardness, and the optimal conditions were determined as follows: 400 U/mL papain, ultrasonic for 30 min, and 4% SC. Hardness, color, and myofibrillar fragmentation index (MFI) were investigated. The hardness followed the order of DW (22.50 N), PI (18.62 N), UPI (12.08 N), and UPIS (7.16 N), and UPIS showed the highest MFI. Papain and SC affected the color of the meat. Background: Thorough knowledge of the management of age-related hyperkyphosis is crucial to physiotherapists' effective handling of complex spinal deformities. Objective: This cross-sectional study investigated the knowledge, attitudes, and confidence of Thai physiotherapists regarding age-related hyperkyphosis. Methods: An online questionnaire with demographic, work-related, knowledge, attitude, and confidence questions was employed. The data analysis involved descriptive statistics, chi-square tests, and independent t-tests. Results: Complete responses were obtained from 314 physiotherapists. The correct responses amounted to 68.79% for the definition, 90.00% for causes, 14.97% for age of onset, 22.38% for prevalence, and 64.65% for the gold-standard diagnosis of hyperkyphosis. Most of the diagnostic methods involved visual examination. The respondents reported that hyperkyphosis disrupts respiratory function. The aim of treatment, according to 88.85%, was to increase spine mobility. Corrective posture exercises were the most common treatment strategy, but the range of treatments was diverse. Many cited undergraduate education as their primary evidence source. Respondents had conflicting attitudes towards "age-related hyperkyphosis is a normal aging process" but mostly positive attitudes towards the need for physiotherapy treatment. Approximately 22% reported fair confidence in treating hyperkyphosis. Conclusions: This study highlights the importance of raising awareness and enhancing knowledge, attitudes, and confidence among physiotherapists to improve care for older adults with hyperkyphosis. Background: Expanding dental care coverage for the elderly is globally recommended but not widely implemented due to its high costs and intangible benefits. Methods: This study examined the impact of such an expansion in Korea using the imputation-based method proposed by Borusyak et al. We analyzed data from the Korea National Health and Nutrition Examination Survey (2007–2019) on dental service utilization and chewing ability among older adults. Results: The policy resulted in a 13.5% increase in partial denture use and a 60.5% increase in dental implants among those aged 65 and above. These changes corresponded with reductions in severe chewing difficulty by 23.3% and 13.0%, respectively. No significant changes were observed in full denture use. The price elasticity of demand for partial dentures and dental implants was estimated to be −0.19 and −0.86, respectively. Conclusions: These findings underscore the critical role of affordability in enhancing the utilization of dental implants among the elderly and highlight the importance of appropriately expanding dental insurance coverage to improve oral health outcomes in this population. Introduction: This study aims to provide a safe, effective, and sustainable resistance training environment for the elderly by modifying chairs and movement systems used during training, particularly under unsupervised conditions. Materials and Methods: The research focused on investigating the effect of modified chair designs on enhancing physical stability during resistance training by involving 19 elderly participants (mean 72.1, SD 4.7). The study measured changes in the body's acceleration during movements to compare the effectiveness of the modified chairs with those commonly used in chair-based exercise (CBE) training in maintaining physical stability. A system was developed based on experimental video data, which leverages MediaPipe to analyze the videos and compute joint angles, identifying whether the actions are executed correctly. Results and Conclusions: Comparisons revealed that modified chairs offered better stability during sitting (p < 0.001) and stand-up (p < 0.001) resistance training. According to the questionnaire survey results, compared to the regular chair without an armrest, the modified chair provided a greater sense of security and a better user experience for the elderly. Video observations indicated that the correct completion rate for most exercises, except stand-up resistance training, was only 59.75%, highlighting the insufficiency of modified chairs alone in ensuring accurate movement execution. Consequently, the introduction of an automatic system to verify proper exercise performance is essential. The model developed in this study for recognizing the correctness of movements achieved an accuracy rate of 97.68%. This study proposes a new chair design that enhances physical stability during resistance training and opens new avenues for utilizing advanced technology to assist the elderly in their training. Background: In both the elderly and children, pneumonia remains one of the leading causes of hospitalization. This study aimed to assess the impact of pneumonia-related hospitalizations in the population over 65 years of age in the Veneto Region. Methods: This retrospective study analyzed hospital discharge records for patients aged 65 and older who resided in the Veneto Region and had a diagnosis of pneumonia from 2007 to 2023. The hospitalizations were identified using specific ICD-9-CM codes for pneumonia as a discharge diagnosis. Hospitalization rates, mortality rates, the prevalence of complications and comorbidities, the length of stay, and associated costs were calculated by age and year. Results: From 2007 to 2023, there were 139,201 hospitalizations for pneumonia. Emergency admissions accounted for 92.1% of these cases, and only 2.0% had a specific diagnosis of pneumococcal pneumonia. The median length of stay was 10 days, and the median diagnosis-related group (DRG) tariff per hospitalization was EUR 3307. Excluding the pandemic years, the hospitalization rates remained stable at approximately 850 cases per 100,000 inhabitants before 2019. After 2022, the rates started to increase again. Overall, in the investigated period, the results showed a negative trend (average Annual Percentage Change (AAPC) of −1.931, p < 0.0001). However, when only considering the pre-pandemic years, the trend was stable, while a decline was observed starting in 2020 (AAPC of −19.697, p = 0.001). The overall discharge mortality rates ranged from 13% to 19.3% but were significantly higher in those over 85 years of age (20.6% compared with 6.5% and 12.0% in the 65–74 and 75–84 age groups, respectively). This paper addresses perceived age discrimination of older persons in European countries. This disturbing issue has become all the more prominent, having doubled over a relatively short period between 2018 and 2021. Strongly associated with bad health outcomes, low levels of wellbeing, and considerable social tensions, age-related discrimination against senior citizens is a sign of the lack of their social integration. Our study used secondary data analysis based on survey methodology. Statistical analysis was performed on data from the European Social Survey wave 10 (2021) subsample of people aged 65 and above to assess the senior population's age-based discrimination experiences and its individual and macro-level determinants. The results indicate that in all European countries, low socioeconomic status, older age, and living alone are strongly associated with the experience of age discrimination. Having bad health and being functionally limited in everyday activities results in a state of dependency that is connected to a higher risk of perceived exposure to age discrimination. Smaller populations, such as those in villages, are protective against discrimination at older age. Finally, compared to other welfare regimes, in social democratic welfare states with egalitarian redistribution systems and high levels of social benefits, there is a significantly larger share of old people who reported to have been victims of age discrimination. The process of aging leads to a progressive decline in the immune system function, known as immunosenescence, which compromises both innate and adaptive responses. This includes impairments in phagocytosis and decreased production, activation, and function of T- and B-lymphocytes, among other effects. Bacteria exploit immunosenescence by using various virulence factors to evade the host's defenses, leading to severe and often life-threatening infections. This manuscript explores the complex relationship between immunosenescence and bacterial virulence, focusing on the underlying mechanisms that increase vulnerability to bacterial infections in the elderly. Additionally, it discusses how machine learning methods can provide accurate modeling of interactions between the weakened immune system and bacterial virulence mechanisms, guiding the development of personalized interventions. The development of vaccines, novel antibiotics, and antivirulence therapies for multidrug-resistant bacteria, as well as the investigation of potential immune-boosting therapies, are promising strategies in this field. Antibiotic resistance presents a critical challenge in healthcare, particularly among the elderly, where multidrug-resistant organisms (MDROs) contribute to increased morbidity, mortality, and healthcare costs. This review focuses on the mechanisms underlying resistance in key bacterial pathogens and highlights how aging-related factors like immunosenescence, frailty, and multimorbidity increase the burden of infections from MDROs in this population. Novel strategies to mitigate resistance include the development of next-generation antibiotics like teixobactin and cefiderocol, innovative therapies such as bacteriophage therapy and antivirulence treatments, and the implementation of antimicrobial stewardship programs to optimize antibiotic use. Furthermore, advanced molecular diagnostic techniques, including nucleic acid amplification tests and next-generation sequencing, allow for faster and more precise identification of resistant pathogens. Background: Evidence regarding the relationship between the use of statins and cognitive outcomes presents varying findings. This study aims to analyze the relationship between sustained statin use and cognitive performance in dyslipidemia patients. Methods: This study presents findings from the Beijing Ageing Brain Rejuvenation Initiative (BABRI) study, in which a cohort of community-dwelling dyslipidemia patients (Entire sample, N = 1,062, aged 50-86) was recruited. Participants were divided into two groups based on their sustained use statins (Statins group, N = 677) or not use any lipid-lowering agents (Untreated group, N = 385). Furthermore, the entire sample was stratified by age into the middleaged sample (N = 451) and the older people sample (N = 611), following a similar categorization based on statin application. ANCOVA was used to evaluate the relationship between sustained statin use and cognitive function. Results: Overall, in the total sample, the statins group demonstrated better cognition in overall cognition, memory, visuospatial ability, attention, executive function, and language domains compared to the untreated group. Moreover, the statins group only showed better performance in attention among the middle-aged sample. In the older people sample, statins group exhibited superior cognitive performance across various cognitive domains compared to untreated group. Conclusion: Among dyslipidemia patients in Beijing community, sustained statin users exhibited superior cognitive function across all domains compared to untreated individuals, with particularly noticeable improvements among those aged 65 and above. Background and Objectives: Bullous pemphigoid (BP) is the most common autoimmune blistering disease affecting mainly elderly patients. Still, little is known about the pathogenesis of pruritus in BP or the factors that affect the clinical course of the disease. This study aimed to evaluate the factors influencing the clinical course of BP among older patients. Materials and Methods: A retrospective analysis of medical records of 55 patients with BP hospitalized in the dermatology department in 2015–2021 was conducted. The study focused on preliminary diagnosis, medical history, clinical examination (characteristics and location of cutaneous changes), laboratory investigation, and direct and indirect immunofluorescence. Results: Analysis of laboratory results in combination with the clinical course of BP showed that red blood cell count, hemoglobin, and hematocrit values were negatively associated with a risk of erosions and erythema, while MCHC values were positively correlated with a risk of associated pruritus. A correlation was found between neurological diseases and an increased risk of erosions. Conclusions: We have shown that age and neurological conditions, including stroke, affect the clinical course of BP. Further studies on a larger group of patients should be conducted to investigate the different factors affecting the clinical aspect of BP and to understand the relationship between them. Background/Objectives: The treatment of periarticular distal radius fractures remains challenging. Different surgical treatment options have been proposed as alternatives to conservative treatment. This systematic review aims to compare the functional outcomes, radiological outcomes, and complications among volar locking plates (VLPs), Kirschner-wire fixations, and external fixations (EFs) for distal radius fractures in patients aged 60 years and older. Methods: We conducted a comprehensive search of PubMed, Cochrane, and Science Direct databases assessing the effects of VLP, EF, and K-wire treatments for distal radius fractures in patients aged 60 years and over. The primary outcome was the evaluation of the range of motion (ROM) degrees after three surgical procedures, trying to assess the best treatment option. The secondary outcome included evaluation of the Disabilities of the Arm, Shoulder, and Hand (DASH) score, quick-DASH score, Patient-Rated Wrist Evaluation (PRWE) score, Visual Analog Scale (VAS) score, grip strength, radiographic assessment, and complications comparing VLPs, EFs and K-wires. Results: A total of 23 studies were included, comparing VLP, EF, and K-wire fixation. The overall population comprised 5618 patients, with 4690 females and 1015 males, of which 4468 patients were treated with VLP, 503 with EF, and 647 with K-wire. The most common complications among the VLP group were complex regional pain syndrome (7.5%) and carpal tunnel syndrome (6.8%); for the EF group, infections (9.8%) and carpal tunnel syndrome (6.8%); and for the K-wire group, carpal tunnel syndrome (7.5%) and infections (6.9%). Conclusions: VLP showed better clinical outcomes in the first few months after treatment. However, these differences decreased over time and became similar after one year. Background and Objectives: Sex differences are unclear in geriatric sleep medicine, and most evidence comes from inference from preclinical bases or clinical studies conducted on younger people. The aim of this study is to explore sex differences in sleep quality and daytime sleepiness in a cohort of elderly people. Materials and Methods: This cross-sectional study involved subjects aged 65 years or older undergoing multidimensional evaluation, including sleep quality and daytime sleepiness assessment with validated tools. Results: This study included 226 subjects (69.5% women), the majority of whom showed poor sleep quality (64.2%). A logistic regression model put one before the other sleep quality and gender. It initially showed that men were about half likely as women to have poor sleep quality (OR 0.48, 95%CI 0.27–0.86). Nonetheless, after adjusting for cognitive status and mood, the difference became smaller and insignificant (OR 0.72, 95%CI 0.38–1.38). Gerstmann syndrome, alternatively recognized as angular gyrus syndrome, epitomizes a complex cognitive impairment that has ignited substantial scholarly discourse within the realms of neurology and neuropsychology. The syndrome's original portrayal was, however, changed. It was demonstrated that the manifestation of its symptomatic tetrad, consisting of four main neurological impairments, is not invariably complete and frequently occurs alongside additional cognitive deficits. Furthermore, the precise cerebral localization of Gerstmann syndrome was traditionally posited to be the left angular gyrus; however, studies mention the possible involvement of other eloquent cerebral areas being pathognomonic. This case report delves into the case of an 86-year-old subject who exhibited the quintessential quartet of symptoms initially delineated by Josef Gerstmann, proposing that elderly patients may manifest a predisposition towards presenting the fully characterized symptomatology initially outlined by Gerstmann. Background: Catheter ablation (CA) is a well-established therapy for patients with atrial fibrillation (AF); however, there is paucity of data for elderly patients. We aimed to assess long-term efficacy and safety of CA for elderly patients with AF. Methods: Medline, BVS, Cochrane, and Embase were searched through April 2023 to investigate comparative outcomes between elderly patients > 75 or 80 years, as per-study cutoff, and individuals ≤ 75/80 years, undergoing CA. Primary efficacy and safety endpoints (AF recurrence and procedure-related major complications) were pooled with the Comprehensive Meta-Analysis 3.0 software. Subgroup analyses were performed by age groups and type of procedure (radiofrequency vs. cryoballoon). Results: Four thousand eight hundred twenty-nine titles were screened, and 27 studies were included, being 26 observational and 1 randomized trial, comprising 117,869 patients, being 8714 (7.4%) elderly > 75/80 years, with follow-up from 11.7 to 72.3 months. In comparative studies (N = 17 studies), elderly > 75/80 years had a higher risk of AF recurrence compared to those ≤ 75/80: relative risk (RR) 1.16 (95% CI 1.05–1.27, p = 0.002. However, funnel plot indicated publication bias, and after imputation of 5 studies, the groups were similar (RR 1.07 (95% CI 0.97–1.19)). The rates of major complications (N = 14 studies) were higher in elderly > 75/80 years (RR 1.30 (95% CI 1.10–1.54), I2 = 0, p = 0.002), but were similar in cryoablation studies (N = 7) (RR 1.10, 95% CI 0.94–1.29, p = 0.24, I2 = 0.0). Results were similar when individual study arms (N = 27 studies) were pooled. This study aims to examine the relationship among key factors including health care, mental health, attitude, social support, quality of life, life satisfaction and active aging to investigate factors that relate to active aging in the Thai elderly and to develop a new model of Thai active aging. Data for the quantitative research were collected using questionnaires and analyzed using descriptive statistics, explanatory factor analysis, and multiple regression analysis. The qualitative research sample consisted of thirty elders. The data were analyzed using content analysis. The results of this study indicate that all important factors related to the active aging of Thai elderly are at a high level such as previous active aging, quality of life, life satisfaction and adjusted active aging. From exploratory factor analysis, it was found that nine elements could be separated that reflect the adjusted active aging of Thai elderly people. In addition, the result indicated four variables that affect the adjusted active aging of the Thai elderly, health care, mental health, attitude and social support. Mental disorders are among the leading causes of disability worldwide, disproportionately affecting older people. This study aims to assess the mental health of elderly individuals living in a deprived region of Hungary, and to identify and estimate the weight of different determinants of mental health across different age groups. A cross-sectional study was conducted with randomly selected samples of individuals (n = 860) aged 18 years and older in Northeast Hungary. The World Health Organization Well-Being Index (WHO-5), the single-item Life Satisfaction Scale, and the 12-item General Health Questionnaire (GHQ-12) were used to measure mental health of the participants. Multiple linear regression analysis was performed to measure the association between sociodemographic and health-related variables and mental health. Overall, the mean WHO-5 score was 69.2 ± 18.1 and it showed a significant decrease by age (p < 0.001), with the lowest score observed in aged 75 years and above (p < 0.001). The mean life satisfaction score was 7.5 ± 1.9 and it showed a significant decreasing trend over the life course (p < 0.001). The highest level of psychological distress as assessed by GHQ-12 was observed in the group aged 75 years or older (11.5 ± 6.0, p < 0.001). Multiple linear regression indicated that self-reported financial status, social support, sense of control over their health, activity limitation and pain intensity were the most important determinants of mental health among older adults. Interventions to improve the mental health of older adults should focus on the positive impact of social support, the reduction of financial insecurity and the use of effective pain relief medications. Background and Aim Methods Results Conclusions The prevalence of metabolic dysfunction‐associated steatotic liver disease (MASLD) is increasing worldwide, coinciding with aging population. However, limited studies have evaluated its incidence and progression to advanced fibrosis in the elderly population. Therefore, our study aimed to investigate the incidence of MASLD and advanced fibrosis in this age group.We included 878 686 individuals aged ≥60 years from the Korea National Health Insurance Service‐Senior cohort. After excluding participants with preexisting MASLD, 329 388 individuals were finally analyzed. Participants were categorized into four groups based on the presence of overweight/obesity and additional risk factors (aRF) included in the cardiometabolic diagnostic criteria of MASLD.The overall incidence of MASLD was 1.94 per 100 person‐years, and the incidence of advanced fibrosis in MASLD patients was 1.78 per 100 person‐years. MASLD development was significantly higher in overweight/obese patients (2.65 per 100 person‐years) compared to lean patients (1.09 per 100 person‐years), and this trend persisted after stratification by the presence of aRF. Similarly, the incidence of advanced fibrosis among MASLD patients was higher in overweight/obese individuals (2.06 per 100 person‐years) compared to lean counterparts (0.87 per 100 person‐years), irrespective of aRF.The lower incidence of MASLD in the elderly population compared to the general population underscores the importance of identifying age‐specific risk factors. Overweight/obesity emerged as a robust predictor of MASLD development and advanced fibrosis. Background: Early identification of frail patients and early interventional treatment can minimize the frailty-related medical burden. This study investigated the use of machine learning (ML) to detect frailty in hospitalized older adults with acute illnesses. Methods: We enrolled inpatients of the geriatric medicine ward at Taichung veterans general hospital between 2012 and 2022. We compared four ML models including logistic regression, random forest (RF), extreme gradient boosting, and support vector machine (SVM) for the prediction of frailty. The feature window as well as the prediction window was set as half a year before admission. Furthermore, Shapley additive explanation plots and partial dependence plots were used to identify Fried's frailty phenotype for interpreting the model across various levels including domain, feature, and individual aspects. Results: We enrolled 3367 patients. Of these, 2843 were frail. We used 21 features to train the prediction model. Of the 4 tested algorithms, SVM yielded the highest AUROC, precision and F1-score (78.05%, 94.53% and 82.10%). Of the 21 features, age, gender, multimorbidity frailty index, triage, hemoglobin, neutrophil ratio, estimated glomerular filtration rate, blood urea nitrogen, and potassium were identified as more impactful due to their absolute values. Conclusions: Our results demonstrated that some easily accessed parameters from the hospital clinical data system can be used to predict frailty in older hospitalized patients using supervised ML methods. Purpose: Frailty is common in surgical patients and is closely associated with postoperative outcomes. Aims: This study employed bibliometric methods to summarize and analyze research related to frailty and surgery, comprehensively analyzing the research structure and providing visualized maps. Methods: This study analyzed the volume of publications, countries, institutions, authors, journals, references, and keywords related to perioperative frailty in the Web of Science Core Collection from 1978 to 2024. Visual bibliometric analyses were conducted from multiple perspectives, including collaboration networks, citation analysis, and keyword clustering. Results: From 1978 to 2024, 21,879 authors from 95 countries and regions published 4,119 papers on perioperative frailty in 973 journals worldwide. The United States has the most publications, while Italy has the highest degree of international collaboration. The University of California System has the highest number of publications. The University of Kansas Medical Center is the institution with the highest centrality. The top nine authors in terms of publication volume are all from the USA. Bowers Christian A. is the most prolific author. The Journal of Vascular Surgery is the journal with the most publications. Current research directions include preoperative risk assessment of frailty, the relationship between frailty and postoperative complications, elderly frailty, and the relationship between frailty and sarcopenia. Research hotspots include risk stratification, postoperative delirium, the elderly, and sarcopenia. Conclusion: This study has identified the research hotspots and trends in perioperative frailty. Background: Long-term, informal care for the elderly in many sub-Saharan African countries is the responsibility of the family. However, urbanization, migration and the gradual nucleation of families is threatening this responsibility. As the burden of long-term care increases, traditional family support may no longer suffice and as such formal systems of care needs to be explored. While formalized care comes at a cost, it is unclear whether households will be willing and able to pay for these services. This study therefore sought to assess households' willingness-to-pay (WTP) for formal residential care for the elderly in an urban setting in Ghana. Methods: This was a cross-sectional study, conducted at the La Nkwantanang-Madina Municipality in Accra among 460 households. The study used a mixed sampling design, combining purposive and stratified random sampling techniques. A structured questionnaire was employed to gather data on socio-economic, demographic, health characteristics of the elderly and caregivers. The Contingent Valuation Method (CVM) was used to assess households' WTP for formal residential care for the elderly, while multiple logistic regression was employed to identify factors influencing households' WTP. Results: The study showed that about 72% of households indicated their willingness to pay for formal residential care for the elderly, with an average amount of US$122.35 (US$27.3–US$272.7) per month for the holistic service. Age of household head (OR = − 0.034, p = 0.02), household income (OR = 1.60, p < 0.01), employment status of household head (OR = 2.75, p = 0.01), awareness of the existence of the service (OR = 2.07, p < 0.01), belonging to the richer household (OR = 2.56, p < 0.00) and presence of an elderly member in the household (OR = 2.84, p < 0.01) were all significantly associated with households' WTP for formal residential care for the elderly. Conclusion: The study revealed a significant proportion of households were willing to pay for formal residential care for the elderly. Factors such as age, income, employment status, presence of elderly and awareness of care options were significantly associated with the households' decision-making processes. This increased interest offers a potential market for provision of formalized elderly care homes services within the capital. Research Summary Policy Implications We examined darknet user discussions on the Dread forum to identify key themes and emerging topics in fraud planning, focusing additionally on elderly victimization. Using the conceptual framework of criminogenic learning to study the process of fraud planning in darknet communities of practice, we analyzed the content of original user posts (n = 818) and comments (n = 1365) collected from the Dread forum. We identified nine unique fraud categories, of which payments‐related fraud was the most‐discussed fraud category and accounted for 25% of original user posts. We further found our sampled forum content to be dominated by the theme of learning or knowledge sharing in eight of the nine fraud categories, which appeared in more than 44% of original user posts. Our content analysis revealed specific insights into why and how darknet forum users may target people, including the elderly subpopulation, for different types of fraud. Overall, our research demonstrates the diversity of opinion and knowledge sharing among darknet forum users in relation to planning and committing fraud against the elderly, views on who is a suitable target and why, and what veteran and aspiring fraudsters consider to be important information for success in fraud.Utilized independently or as part of a multistage strategy of darknet data analysis, our research method can be used to monitor criminally active darknet forums for current and emerging themes in fraud against general populations and vulnerable subpopulations, such as the elderly, and to develop strategies to identify, disrupt, or destroy hubs of criminal planning and knowledge sharing. Our study additionally informs policy makers of certain elder‐specific vulnerabilities that might be addressed by more up‐to‐date elder cybercrime awareness campaigns and initiatives. Electrical heated clothing (EHC) is a revolutionary concept that combines garment with electrical technology to create garments capable of actively regulating and providing warmth to the wearer, especially for the elderly. To develop an EHC for the elderly with both aesthetics and functionality, the technique of knitted Jacquard is employed for fabrics and Bluetooth module is applied to realize user interaction (UI) design. Firstly, 16 different heated fabrics were fabricated by choosing different silver-coated yarns and knitted Jacquard patterns. The heating temperatures of the fabrics were simulated, and the actual heating performance was observed through infrared thermal image. Suitable materials and patterns were selected to produce the EHC. Secondly, in order to meet the demands for the elderly, a prototype was developed by combining the characteristics of the elderly clothing design and UI design. After the design of a Bluetooth module with a PCBA, the heating temperature and heating time of the clothing can be adjusted through the WeChat mini-program on the mobile phone. It has been verified that the developed prototype meets the heating requirements. This study can serve as an excellent guide for the advancement of EHC and electrical wearables. Background and purpose: Large symptomatic Vestibular Schwannoma (VS) often requires surgical resection, regardless the patient's age. The aim of this study was to assess the surgical outcomes of patients in their ninth decade of life. Methods: This monocenter retrospective observational study included patients aged 80 years or older who underwent VS surgery between 2009 and 2020. We retrospectively analyzed their immediate post-surgical and long-term outcomes and complications. Results: Thirteen octogenarians who underwent VS surgery were included, with average age of 83.2 ± 1.97 years old (median 83.5, range 80–86 years). One patient had a Koos-Grade II tumor, and 12 patients had a grade IV. All patients had a preoperative ASA score ≤ 3 and underwent surgery in the supine position. Twelve patients underwent a pre-planned partial resection (PR) and one had a gross-total resection (GTR). Good facial function (House-Brackmann grade ≤ 2) was achieved in 10 patients (77%). We reported three Clavien-Dindo grade ≤ 3 treatment-related complications and no life-threatening complication. Two patients experienced tumor recurrence after PR. Conclusion: In this series of patients who underwent VS surgery in their ninth decade of life, surgical outcomes were acceptable. Therefore, age alone should not serve as a contraindication for surgery. Preplanned PR is a reasonable attitude in elderly patients. [ABSTRACT FROM AUTHOR]

Background: This study explores COVID-19 emergency admission and length of hospital stay hospitalization outcomes for Long-Term Care Facility (LTCF) residents with dementia. Methods: Utilizing a cross-sectional case control design, we employed logistic regression to analyze Texas Inpatient Public Use Data File (PUDF) for 1,413 dementia patients and 1,674 non-dementia patients (>60 years) to predict emergency admission and length of hospital stay with mediation by pre-existing conditions. Results: LTCF residents with dementia have a higher likelihood of COVID-19 emergency admission and shorter hospital stays. Adjusting for confounders of demographics, health insurance, and lifestyle, dementia diagnosis remained significantly associated with emergency admission and shorter hospital stays with preexisting conditions. Conclusion: Findings underscore the heightened risk for adverse COVID-19 hospitalization care disparities with dementia. Targeted health support programs for LTCF residents with dementia should aim to improve their COVID19 hospitalization outcomes, treating pre-existing health conditions and reducing their risk for excess mortality. Background: This study aimed to identify prognostic factors for survival and develop a prognostic nomogram to predict the survival probability of elderly patients with lower-grade gliomas (LGGs). Methods: Elderly patients with histologically confirmed LGG were recruited from the Surveillance, Epidemiology, and End Results (SEER) database. These individuals were randomly allocated to the training and validation cohorts at a 2:1 ratio. First, Kaplan-Meier survival analysis and subgroup analysis were performed. Second, variable screening of all 13 variables and a comparison of predictive models based on full Cox regression and LASSO-Cox regression analyses were performed, and the key variables in the optimal model were selected to construct prognostic nomograms for OS and CSS. Finally, a risk stratification system and a web-based dynamic nomogram were constructed. Results: A total of 2307 elderly patients included 1220 males and 1087 females, with a median age of 72 years and a mean age of 73.30 ± 6.22 years. Among them, 520 patients (22.5%) had Grade 2 gliomas, and 1787 (77.5%) had Grade 3 gliomas. Multivariate Cox regression analysis revealed four independent prognostic factors (age, WHO grade, surgery, and chemotherapy) that were used to construct the full Cox model. In addition, LASSO-Cox regression analysis revealed five prognostic factors (age, WHO grade, surgery, radiotherapy, and chemotherapy), and a LASSO model was constructed. A comparison of the two models revealed that the LASSO model with five variables had better predictive performance than the full Cox model with four variables. Ultimately, five key variables based on LASSO-Cox regression were utilized to develop prognostic nomograms for predicting the 1-, 2-, and 5-year OS and CSS rates. The nomograms exhibited relatively good predictive ability and clinical utility. Moreover, the risk stratification system based on the nomograms effectively divided patients into low-risk and high-risk subgroups. Conclusion: Variable screening based on LASSO-Cox regression was used to determine the optimal prediction model in this study. Prognostic nomograms could serve as practical tools for predicting survival probabilities, categorizing these patients into different mortality risk subgroups, and developing personalized decision-making strategies for elderly patients with LGGs.
