Results: Among clinical, demographical, stress echocardiographic and angiographycal variables, factors strongly associated with high mortality rates were: myocardial jeopardy score >7 (p=0.0003), positive DobAtro (p=0.016), positive DipAtro (p=0.0215) and the number of diseased vessels (p=0.005), while other variables and the results ox Ex stress echocardiography didn’t show significant influence on long-term risk of cardiac death (p=ns). Variable most strongly associated with cardiac death was myocardial jeopardy score (RR 1.05; p=0.00054). Conclusion: Patients with high amount of potentially ischemic myocardium and positive DipAtro and DobAtro stress echocardiography are at high risk for subsequent cardiac death. The purpose of the study was to compare diagnostic value of low dose dipyridamole radionuclide ventriculography (DIPY-RNV) and low dose dipiyidamole echocardiography (DIPY-ECHO) for the prediction of functional recovery of viable myocardium in the medium term follow up. Twenty patients (18 male; 51±10 years) with previous myocardial infarction and resting wall motion dyssynergy were studied before angioplasty of infarct related artery (IRA), by RNV and ECHO at rest, as well as during dipyridamole infusion (0,28 mcg/kg/min over 2min). RNV as well as ECHO was repeated at rest, 12 weeks after successful angioplasty. Five percent increase of regional ejection fraction (REF) by RNV was used as criterion for functional improvement of infarcted regions. By ECHO, viability was defined as improvement of wall thickening or contractile improvement of grade one or more, utilizing wall motion score index (WMSI). Out of 180 examined (20x9) segments by RNV, 51 were dyssynergic and they had abnormal REF (29±10%). Out of these 51 segments functional improvement was documented in 33 on low DIPY. Sensitivity for predicting functional recovery after 12 weeks follow up was 63%, and specificity was 77%. Background —Patients undergoing major vascular surgery are at a relatively high risk of cardiac events, and pharmacological stress echocardiography is increasingly used for perioperative risk stratification. The aim of the current study was to evaluate the value of dipyridamole echocardiography test (up to 0.84 mg/kg over 10 minutes) in predicting cardiac events in a large-scale, multicenter, prospective, observational study design. Methods and Results —Five hundred nine patients (mean age 66±10 years) were studied before vascular surgery by dipyridamole stress echocardiography in 11 different centers. All patients underwent preoperative clinical risk assessment according to the American Heart Association guidelines. No major complications occurred during dipyridamole stress echocardiography. Technically adequate images were obtained in all patients; however, in 4 patients only the low dipyridamole dose (0.56 mg/kg over 4 minutes) was given for limiting side effects. Eighty-eight (17.3%) had a positive test. Perioperative events occurred in 31 (6.1%) patients: 6 deaths, 11 myocardial infarctions, and 14 episodes of unstable angina. Sensitivity and specificity of dipyridamole stress echocardiography for predicting spontaneous cardiac events were 81% and 87%, respectively, with a positive predictive value of 28% and negative predictive value of 99%. By multivariate analysis, the difference between wall motion score index at rest and peak stress (Δwall motion score index), test positivity, and ST-segment depression during dipyridamole infusion were independent predictors of any perioperative cardiac event. Conclusions —Dipyridamole stress echocardiography is safe and well tolerated in patients undergoing major vascular surgery and provides an effective preoperative screening test for the risk stratification of these patients, mainly because of the extremely high negative predictive value, which is a potent predictor of complication-free procedure.

Abstract Background and Purpose: Serum cholesterol is positively associated with the risk of developing coronary heart disease. The aim of this study was to determine the relation between response of coronary arteries to ergonovine provocation and lipid profile in patients with nonsignificant coronary artery disease. Patients and Methods: 105 patients (46 male, 59 female, mean age 52 8 years) with chest pain syndrome and nonsignificant coronary artery stenosis ( Results: There was a significant negative correlation between resting MLD and LDL cholesterol (r = –0.215; p = 0.034), and a significant positive correlation between MLD decrease provoked by ergonovine and total cholesterol (r = 0.275; p = 0.006), as well as LDL cholesterol (r = 0.284; p = 0.004), but not for HDL cholesterol and triglycerides (p = NS [not significant]). Conclusion: In patients with nonsignificant coronary artery stenosis evaluated by ergonovine provocation, there was not only a significant negative correlation between MLD and LDL cholesterol, but also a positive correlation between coronary vasoconstriction induced by ergonovine provocation and both total and LDL cholesterol. Foods and food products that contain polyphenols are proposed to modulate risk of cardiovascular disease. The aim of this three-arm, crossover, randomized, double-blind, placebo-controlled intervention study was to examine the impact of Aronia melanocarpa juice (AMJ), high-polyphenol (AMJ treatment, 1.17 g/100 mL polyphenols) and low-polyphenol (dAMJ treatment, 0.29 g/100 mL polyphenols) dose, on the transcriptome in peripheral blood mononuclear cells (PBMC) of 19 subjects at cardiovascular risk. Transcriptome data were obtained by microarray. Bioinformatic functional annotation analysis was performed on both the whole transcriptome datasets and the differentially expressed genes (DEGs). Expression of selected DEGs was validated by RT-qPCR. Administration of AMJ and dAMJ treatments during the two consecutive four-week treatment periods had additive effects on PBMC transcriptome profiles, with the most pronounced and specific effect noticed for AMJ in the last treatment period (TP3) of the trial. Between the high-dose and low-dose treatments in TP3, there was a multitude of overlapping DEGs and DEG-enriched biological processes and pathways, which primarily included immunomodulation and regulation of cell proliferation/death. Increased expression of TNF, IL1B, IL8, RGS1, OSM, and DUSP2 in TP3 was confirmed by RT-qPCR. The results suggest the immunomodulatory effects of prolonged habitual consumption of polyphenol-rich aronia juice in individuals at cardiovascular risk.

Objectives. We sought to evaluate the effects of combined administration of infra-low dose dipyridamole and low dose dobutamine on assessment of myocardial viability. Background. Low dose pharmacologic stress echocardiography with either dobutamine or dipyridamolc infusion has been proposed for the recognition of myocardial viability. Methods. Thirty-four patients with rest wall motion dyssynergy by two-dimensional echocardiography and with angiographically proved coronary artery disease underwent in combination with two-dimensional echocardiographic monitoring: 1) low dose (5 to 10 μg/kg per min over 3 min) dobutamine infusion; 2) infra-low dose (0.28 mg/kg over 4 min) dipyridamole infusion; 3) combination of infra-low dose dipyridamole infusion immediately followed by low dose dobutamine infusion (combined dipyridamole-dobutamine). Results. Follow-up rest echocardiography was available in 30 patients. After revascularization, 82 segments showed a contractile improvement of ≥1 grade, whereas 63 segments remained unchanged. The sensitivity of dobutamine, dipyridamole and combined dipyridamole-dobutamine for predicting recovery was 72% (95% confidence interval [CI] 60.9% to 81.3%), 67% (CI 55.8% to 77%) and 94% (CI 86.3% to 97.9%), respectively. The specificity of dipyridamole, dobutamine and combined dipyridamole-dobutamine was 95% (CI 86.7% to 99%), 92% (CI 82.4% to 97.3%) and 89% (CI 78.4% to 95.4%), respectively. The accuracy of the dobutamine, dipyridamole and combined dipyridamole-dobutamine test was 80%, 79% and 92%, respectively (combined dipyridamole-dobutamine vs. dobutamine, p < 0.05; combined dipyridamole-dobutamine vs. dipyridamole, p < 0.01). Conclusions. Infra-low dose dipyridamole added to low dose dobutamine recruits an inotropic reserve in asynergic segments that were nonresponders after either dobutamine or dipyridamole alone and destined to recover after revascularization.

Stress echocardiography has become an accepted and cost-effective method for diagnosing coronary artery disease. However, as exercise stress echocardiography is a demanding technique, difficult to reproduce, pharmacological stress echocardiography has become very popular in recent years. The two most popular tests from the pharmacological stress echocardiography arena are dipyridamole and dobutamine. They have enabled the technical limitations inherent in exercise echocardiography to be overcome, and have provided the opportunity to obtain, during stress, images of unchanged quality in comparison to baseline. However, the sensitivity of both pharmacological stresses applied separately is less than ideal in patients with milder forms of coronary artery disease and in patients under therapy. To overcome this, a new generation of pharmacological stress tests, the combined dipyridamole-dobutamine tests were introduced. A combined dipyridamole-dobutamine echocardiography stress test should suggest that the agents are natural allies rather than competitors but in some instances they are applicable only in selected patients, and each pharmacological agent may be clinically contraindicated for administration. This is the message from a large scale study of recent years.

Background The risk stratification of patients with diabetes mellitus (DM) is a major objective for the clinicians, and it can be achieved by coronary flow velocity reserve (CFVR) or with coronary artery calcium score (CS). CS evaluates underlying coronary atherosclerotic plaque burden and CFVR estimates both presence of coronary artery stenosis and microvascular function. Consequently, CFVR may provide unique risk information beyond the extent of coronary atherosclerosis. Aim Our aim is to assess joint prognostic value of CFVR and CS in asymptomatic DM patients. Materials and methods We prospectively included 200 asymptomatic patients (45,5 % male, mean age 57,35 ± 11,25), out of which, there were 101 asymptomatic patients with DM and 99 asymptomatic patients without DM, but with one or more conventionally risk factors for coronary artery disease. We analyzed clinical, biochemical, metabolic, inflammatory parameters, CS by Agatston method, transthoracic Doppler echocardiography CFVR of left anterior descending artery and echocardiographic parameters. Results Total CS and CS LAD were significantly higher, while mean CFVR was lower in diabetics compared to the nondiabetics. During 1 year follow-up, 24 patients experienced cardio-vascular events (one cardiovascular death, two strokes, three myocardial infarctions, nine new onsets of unstable angina and nine myocardial revascularizations): 19 patients with DM and five non DM patients, (p = 0,003). Background B-lines by lung ultrasound (LUS) were added to stress echo (SE) as a direct sign of pulmonary congestion useful to establish an objective link between dyspnoea symptoms and acute heart failure. They are feasible with “kindergarten” training of few hours and pocket size instruments. Aim To assess the prognostic value of “kindergarten SE” only based on B-lines and imaging-independent heart rate reserve (HRR). Methods We enrolled 2,149 patients (age 63±16 yrs, 831 women, 39%) with known or suspected coronary artery diseasereferred for exercise (n=1,015), dipyridamole (n=1,039), adenosine (n=16) or dobutamine (n=79) SE. By LUS, we adopted the 4-site simplified scan, each site scored from 0=normal A-lines, to 10=coalescing B-lines. HRR was assessed as peak/rest ratio of heart rate. All patients were followed-up. Results Interpretable HRR and LUS data were obtained in all patients (feasibility=100%). Abnormal B-lines (≥2) at peak stress were present in 756 patients (35%). Abnormal HRR (≤1.80 for exercise and dobutamine and ≤1.22 for vasodilator) was found in 986 patients (46%), both positivity in 388 patients (18%). During a median follow-up time of 15 months, 137 spontaneous events occurred in 120 patients: 38 deaths, 28 myocardial infarctions, 60 acute heart failures, 11 strokes. B-lines ≥2 and/or reduced HRR were independently associated with adverse outcome (see figure). At multivariable analysis, a three-fold increased risk of death was observed when both B-lines and HRR were abnormal (Hazard ratio: 3.097, 95% Confidence Intervals 1.095–8.754, p=0.03). Conclusions A super-simplified stress test (“SE without SE”) with simple heart rate assessment by EKG and LUS for B-lines evaluates key variables such as chronotropic incompetence (due to reduced sympathetic reserve) and pulmonary congestion (due to backward acute heart failure) and allows an accurate prediction of outcome. Congenital leukemia is an extremely rare disorder in the newborn. The most common type is acute myeloid leukemia. Neonates may present with a broad variety of atypical symptoms (low-grade fever, diarrhea, easy bruising, failure to gain weight), simulating septicemia and congenital infections. We report a term male infant, born by cesarean section at 38 weeks gestation to a primigravida woman with pregnancy complicated by epilepsia and anemia. APGAR scores were 9 and 7 at first and fifth minutes, respectively. The amniotic fluid was meconial. He was hypotonic, cyanotic and transferred to the NICU (Neonatal intensive care unit). He required supplemental oxygen for 22 hours. Initially antibiotics were administered for empiric treatment of early-onset sepsis. Laboratory parameters revealed total leukocyte count 67 000/mm3, C-reactive protein (CRP) levels of 0,0 mg/l. At day two of life, he did not require supplemental oxygen, physical examination was normal. For next three days C-reactive protein increased to 30 mg/l, and broad-spectrum antibiotic was given. At two weeks of life we noticed swelling of left antebrachium and left lover leg. All bacterial cultures were negative. Serological tests for congenital viral infections were negative. On the fourteenth day of life white blood cell count is still very high 83 400 mm/3 and the baby was transferred to the Institute for Health Protection of Mother and Child of Serbia "Dr Vukan Cupic", for further diagnostic examinations. Congenital acute megakaryocityc leukemia was diagnosed. With this case report we would like to show that "suspected sepsis" is one of the most common diagnosis made in the NICU. The signs of sepsis are nonspecific and many noninfectious conditions mimic those of neonatal sepsis. Clinicians must keep a high index of suspicion of acute leukemia even in the neonatal period. 288. The Value of Stress Echocardiography Imaging and Functional Parameters in Patients with aVR Lead ST-Segment Elevation during an Exercise Stress Test to Detect Significant Left Main Stenosis.

Objective. To evaluate the role of functional and imaging parameters during exercise stress echocardiography (SE) in the presence of ST-segment elevation (ST-E) in aVR leads to predict significant left main/left main equivalent/or ostial left anterior descending (LAD) stenosis (LM+). Methods. The study population included 548 patients with ECG and echo markers of myocardial ischemia, in whom diagnostic coronary angiography was performed. We analyzed the patients' clinical characteristics, ECG changes, wall motion score index (WMSI) by stress echocardiography (SE), as well as functional capacity during exercise (METs) and Duke treadmill score. Results. aVR ST-segment elevation was found in 60/548 (11%) patients, whereas aVR ST-E was found in 23/57 patients with left main LM stenosis (Sn 40%, Sp 92%, PPV 38%, NPV 93%). When aVR ST-E was combined with other functional/imaging parameters, patients with aVR ST-E and LM+ had significantly worse functional capacity in METs (5.0±2.2 vs. 6.7±2.3, P=0.005), lower Duke score (-6.8±6.8 vs. -3.6±4.1, P=0.049), and higher deterioration of WMSI (0.51±0.24 vs. 0.39±0.24, P=0.046). Significant multivariable predictors of the left main (LM) stenosis were aVR ST-E and positive SE in LAD territory in the whole group of patients, and Delta WMSI, Duke score and METs achieved in patients presented with aVR ST-E during exercise. Conclusion. The aVR ST-segment alone has intermediate sensitivity in detecting significant LM stenosis in patients referred to SE testing for chest pain. When combined with other functional and imaging parameters, including poor exercise functional capacity in METs, lower Duke score or greater WMA in the territory of LAD, its diagnostic power to detect LM significantly increases.
