We present a case of ganglion cyst of the knee treated arthroscopically. Ganglion cysts typically arise from the anterior or the posterior cruciate ligament. In this case of an anomalous location, a 3.5-cm mass arose directly from the anterolateral articular capsule, lying over the lateral meniscus, displacing Hoffa's fat pad and joining the pretibial bursa. We removed it arthroscopically using only medial portals to avoid cystic wall damage and fluid leakage from the lateral portal. Arthroscopic evaluation was performed starting from the inferomedial portal to assess the cyst location, and shifting to the mid patellar portal. From that portal, we were able resect the cyst, opening the wall and shaving and aspirating the myxoid fluid.

A case is reported of fluid leakage per rectum after ejaculation. The patient was infertile and did not have antegrade or retrograde ejaculation. The cause of this problem is presented.

Avrim Eden, MD, William Lawson, MD, and Hugh Biller, MD, of Mt Sinai Medical Center, New York, speaking at the Eastern Section of the Triological Society, recommended staging the resection of large glomus tumors with posterior fossa extension, first performing suboccipital craniectomy and removing as much of the tumor as possible, and then closing the defect with a fascial graft. At an interval of three or more weeks, the temporal bone and cervical portions of the tumor are resected. The proposed advantages are (1) avoidance of subsequent surgery on those in whom intracranial extension proves to be nonresectable; (2) identification of resultant cranial nerve defects before the otolaryngologic phase of surgery, allowing the addition of rehabilitative measures such as cricopharyngeal myotomy with the second procedure; and (3) a decreased incidence of spinal fluid leakage and subsequent meningitis. The introductory chapters on pulmonary physiology and acid-base balance and the question-and-answer sections in the appendices are pitched more at the medical student level. Surgeons will find chapter 7, on the preoperative evaluation of pulmonary function, to be clearly written and well referenced; it is undoubtedly the best chapter in the book. Surgeons might take issue with the discussion of oxygen therapy and ventilation. There is insufficient emphasis on the deleterious effects of administering 100% oxygen, including the loss of the cushioning effect of residual nitrogen on alveolar collapse, the accelerated denaturation of surfactant at a high Po2, and progressive damage to endothelial cells with consequent fluid leakage and interstitial edema.
