Objective: Circumstantial evidence indicates that, in the presence of a suitable substratum, sudden, behaviorally induced increases in sympathetic drive to the cardiovascular system might play an important physiopathological role in various conditions, ranging from arterial hypertension to sudden coronary death. Accordingly, it might be useful to study the effects of behavioral interventions, such as mental relaxation, that might be capable of blunting excitatory autonomic responses. It would also be preferable to study healthy subjects in whom autonomic control is not modified by the presence of disease, and to use noninvasive approaches to minimize the possible emotional impact produced by invasive recordings., Methods: We examined healthy subjects who were either subjected to relaxation training (N = 13) or sham relaxation (N = 12). An additional group, treated with beta-adrenergic blockade (N = 12), was also examined. Spectral and cross-spectral analysis of RR interval and systolic arterial pressure (SAP) variabilities provided quantitative markers of sympathovagal balance modulating the sinoatrial (SA) node, of sympathetic vasomotor modulation, and of the gain of the arterial pressure/heart period baroreflex (index alpha). Subjects were studied at rest, during standing, and during mental arithmetic., Results: Data indicate that both beta-adrenergic blockade and relaxation training significantly blunted the excitatory autonomic responses to standing and to mental arithmetic. Indices of sympathetic modulation also seemed reduced by beta blockade at rest. No changes were observed with sham training., Conclusions: Frequency domain analysis of cardiovascular variabilities, using a totally noninvasive approach, indicates that relaxation training significantly blunts the excitatory autonomic changes produced by standardized behavioral laboratory stimuli.

Delayed-type hypersensitivity (DTH) reactions to the experimental allergen diphenylcyclopropenone (DCP) were measured in four groups, which either trained (+) or did not train in relaxation (-) during the sensitization and/or the challenge phase. All groups consisted of high and low hypnotic susceptible subjects. While there were no differences in erythema, the mean induration of the group which trained in relaxation in both the sensitization and the challenge phase (+/+) was significantly greater than that of the group which trained in relaxation in the challenge phase only (-/+). Significant correlations were found between induration and hypnotic susceptibility scores, and between induration and degree of perceived relaxation during challenge. High hypnotic susceptible subjects experienced a higher degree of perceived relaxation and exhibited greater indurative and erythematous DTH reactions to DCP than low hypnotic susceptible subjects in all four experimental conditions. Though the mediating mechanisms remain unclear, our results suggest that relaxation may affect the DTH reaction, and support previous findings of higher psychophysiologic reactivity of high hypnotic susceptible subjects.

Alexithymia has been linked to various disorders, including compulsive behaviors, anxiety disorders, and physical conditions with or without symptoms. It has been hypothesized that these disorders result from the alexithymic inability to differentiate and elaborate affect, which gives rise to physiological arousal and a negative subjective state, which are not regulated by psychological strategies. We tested these hypothesized mechanisms by comparing 42 alexithymic subjects with 42 sex- and race-matched non-alexithymic subjects on physiological and subjective responses to an autogenic relaxation exercise and three different laboratory stressors. Alexithymic subjects had tonically greater electrodermal activity and reported more arousal and displeasure in general than nonalexithymic subjects. Groups did not differ in the degree to which they relaxed, but alexithymic subjects reported less enjoyment of, less involvement in, and poorer imagery during relaxation. All three stressors evoked reactivity, and alexithymic women had less heart rate change when viewing disgusting scenes than did nonalexithymic women; in general, however, groups did not differ in reactivity or recovery to the stressors. We find some support for the hypothesized mechanisms of alexithymia, and we suggest specific links between alexithymia and clinical disorders.

A diminished mucosal concentration of secretory immunoglobulin A (sIgA) in the upper respiratory tract may increase susceptibility to colds and flu. The aim of the present study was to determine whether sIgA increases during relaxation in children aged between 8 and 12 years with recurrent upper respiratory tract infections. Forty-five healthy children and 45 children with 10 or more upper respiratory tract infections in the previous year were randomly assigned to one of three experimental conditions: relaxation with suggestions to increase immune system proteins, relaxation alone, or a control condition. Samples of saliva were obtained before and after each condition. The concentration of sIgA in the saliva samples was later determined by measuring the rate of precipitation of antigen-antibody complexes to known concentration of sIgA antigen. The concentration of sIgA increased in the relaxation conditions but not in the control condition. The sIgA/albumin ratio (a more specific measure of local mucosal immunity than concentration) increased during the relaxation-suggestion condition but not during the relaxation or control conditions; however, both the concentration of sIgA and the sIgA/albumin ratio increased in proportion to subjective relaxation ratings. Neither response differed between healthy children and children with recurrent infections. The findings indicate that a disturbance in mucosal immunity in children with recurrent colds and flu does not limit increases in sIgA during relaxation. Higher preinfection levels of sIgA correlate with resistance to upper respiratory tract infection, so enhancing the sIgA concentration with relaxation techniques may help children with recurrent infection problems.

The purposes of this research study were: (1) to determine whether changes in cardiac rate, skin temperature, and/or electrodermal activity occur as children change mental imagery and (2) to determine whether such changes are related to age, sex, or other variables. Children who were evaluated in this study had no previous experience with hypnosis or biofeedback training and were in good health with no learning disabilities. Thirty-eight boys and 38 girls ranging in age from 5 to 15 years were studied in a comfortable setting with a constant room temperature and biofeedback equipment. A Procomp 5DX computer software unit was used to measure autonomic reactivity during baseline and mental processing periods. After baseline monitoring indicated stabilization of autonomic measures, each child was asked to think about being in a quiet, pleasant place for 120 seconds. Pulse rate, skin temperature, and electrodermal activity were recorded. A resting period followed, and each child was then asked to think about an exciting activity, such as a preferred sports activity, for another 120 seconds. At the end of this monitoring, each child was asked to describe what had been his/her mental imagery during the two monitoring periods. Data analysis used paired t tests and repeated measures analysis of variance. For all children, the pulse rates showed significant decreases (p < .001) during quiet and relaxing imagery and significant increases (p < .001) during active imagery. Skin temperatures increased significantly (p < .001) during quiet imagery and active imagery, whereas electrodermal activity decreased (p < .001) during active imagery. Observed changes did not relate to age or sex. The results confirm our clinical observations that deliberate changing of mental imagery by children results in immediate autonomic changes. Questions evolving from this study and similar studies done in adults are: (1) Do average-thinking processes impact on autonomic changes over long periods of time and (2) do these changes ultimately impact on health, such as cardiovascular status?

This study examines the assumption that the dimensional structure of mood following strenuous physical exercise is unchanged by comparison with that beforehand. A specially-devised adjective checklist was completed by 165 regular runners 5-10 minutes before, and immediately after, a 3-mile run. Principal components analyses of the scores revealed independent dimensions of positive and negative affect which were similar on both occasions. There was no evidence of a separate major dimension of elation or well-being after running. The structure of mood differed before and after running only in minor components. Component-based scale scores were calculated for each subject, for each occasion. Positive mood was increased, and negative mood decreased, after running. The main changes in mood caused by running in these conditions are therefore quantitative rather than qualitative. Improvements in mood were greater in women than in men, largely because women experienced a worse mood state than did men before running.

Objective: To compare definitions of comfort held by a group of postpartum substance users with those of a group of nonusers., Setting: The postpartum unit of a large university hospital., Participants: The convenience sample constituted 36 postpartum women; half were substance users and half denied substance use. The nonusing participants were matched to the users according to age, race, type of delivery, and parity., Design: Qualitative, descriptive., Main Outcome Measures: Data were collected through semistructured, individual interviews. Patients defined comfort and described the characteristics and patterns of comfort during the postpartum period., Results: Data were analyzed into three categories of themes. Attributes of comfort were resolution of pain, resolution of fatigue, satiation of hunger, resolution of individual irritants, and relaxation. Interventions for comfort were standard therapy, supportive presence, and caring nursing approach. Modifiers of comfort were individualized comfort patterns, environmental ease, and maternal concern., Conclusions: Comfort is more than the absence of pain. A supportive presence and a caring nursing approach were the most helpful interventions.

It has been argued that surgery is a form of psychological stress, and that the response to it should therefore be modifiable by psychological means. To test this hypothesis, we assessed the effect of preoperative relaxation instructions on endocrine, cardiovascular, and subjective responses in 21 patients undergoing minor abdominal surgery. Controls (N = 19) received general procedural information. In the relaxation group only, there was a significant increase in cortisol and adrenaline concentration from immediately before induction of anesthesia to immediately after surgery. Noradrenaline values were unaffected by relaxation. In contrast, relaxation reduced state-anxiety on the preoperative day and the two postoperative days; maximal peroperative systolic and diastolic blood pressures were also reduced. The results are consistent with previous evidence that adrenaline and cortisol responses to surgery are greater in less anxious patients.

This study investigates the use of biofeedback, relaxation and psychotherapy on five patients with severe, vascular headaches that occurred during the course of pregnancy. The subjects received between four and twelve sessions of treatment overall. The subjects all showed a marked reduction or complete cessation of headaches during treatment, the term of pregnancy, and during a follow-up evaluation months after the birth of the child. Possible alternate explanations for improvement are discussed along with the study's limitations. This preliminary investigation strongly suggests that psychological treatment may be a particularly useful intervention for management of headaches that occur in pregnant women.

Immediately before they underwent femoral angiography, 45 patients were given one of three types of audiotapes: a relaxation response tape recorded for this study, a tape of contemporary instrumental music, or a blank tape. All patients were instructed to listen to their audiotape during the entire angiographic procedure. Each audiotape was played through earphones. Radiologists were not told the group assignment or tape contents. The patients given the audiotape with instructions to elicit the relaxation response (n = 15) experienced significantly less anxiety (P less than .05) and pain (P less than .001) during the procedure, were observed by radiology nurses to exhibit significantly less pain (P less than .001) and anxiety (P less than .001), and requested significantly less fentanyl citrate (P less than .01) and diazepam (P less than .01) than patients given either the music (n = 14) or the blank (n = 16) control audiotapes. Elicitation of the relaxation response is a simple, inexpensive, efficacious, and practical method to reduce pain, anxiety, and medication during femoral angiography and may be useful in other invasive procedures.

The purpose of this study was to construct and validate the prototype of a structured interview or questionnaire for use in the functional assessment of problem behaviour. A set of situation-response (S-R) questions was used to test hypotheses concerning discriminative stimuli SD, inhibitive stimuli (SI) and the function of a problem behaviour (R). The situations in S-R questions include stimuli which are expected (a) to set the occasion for, or (b) to inhibit the problem behaviour. The response part includes the problem behaviour as well as behaviour patterns with known functions. The problem behaviour is correlated with those behaviour patterns whose functions it shares. For the study of 26 children suffering from orthodontal problems due to excessive thumb sucking, a structured interview was developed and validated for identifying (a) stimuli (SD) that trigger excessive thumb sucking (problem behaviour), (b) stimuli (SI) that inhibit this response, and (c) the function of thumb sucking.

Schools of talking therapy generally consider the doctor-patient relationship to be essential to the therapeutic process. Yet studies reveal that the presence of a therapist can sometimes inhibit frank disclosure and that patients will speak alone, in the absence of a therapist, about matters of importance to them. We have programmed a computer interview to facilitate soliloquy and have studied its effectiveness. Encouraged by the computer, subjects talked into a microphone first about anxiety-provoking circumstances and then about relaxation. Both mean heart rate and State anxiety scores fell significantly between the beginning and the end of the interview.
