Abstract: Zygomaticomaxillary complex fractures are common in midface trauma, with treatment often involving repair using titanium mini plates. However, the need for plate fixation along the zygomaticomaxillary suture on the infraorbital rim remains controversial. This study utilized a previously reported bite force simulator to investigate craniofacial strain patterns following zygomaticomaxillary complex fracture repairs with and without plating of the infraorbital rim. Osteotomies were made to 6 fresh-frozen cadaveric heads to simulate 2 types of zygomatic complex fractures: a dipod fracture with osteotomies at the zygomaticofrontal and zygomaticomaxillary sutures, and a tripod fracture with an additional osteotomies at the zygomaticotemporal suture. Repairs with and without the use of a titanium mini plate across the infraorbital rim were compared in both dipod and tripod fractures. Physiologically proportional masticatory loads were applied using the bite force simulator by actuating intrinsic muscle lines of action. The outcome metric was facial bone strains measured using uniaxial strain gauges. Mixed-effects linear models did not find a significant main effect on the overall strain pattern with the use of an infraorbital rim plate in both dipod (P = 0.198) and tripod (P = 0.117) fracture repairs. However, statistically significant differences were found locally at the zygomatic buttress (P = 0.019) and the zygomatic arch (P = 0.027) on the fractured side in dipod fractures. This is the first known study that successfully utilized a mechanical simulator to reproduce physiological intrinsic masticatory loads in a fracture fixation study. Purpose: Normal digital flexion relies on flexor tendon pulleys to convert linear muscular force to angular digital motion. However, there is a growing trend to vent them partially during flexor tendon repair. The objective of this study was to examine the effects of a thermoplastic ring, acting as an external pulley, on flexor tendon biomechanics and finger range of motion (ROM) after pulley venting., Methods: We tested 15 cadaveric digits using an in vitro active finger motion simulator. We measured loads induced by flexor digitorum superficialis (FDS) and flexor digitorum profundus (FDP) as well as joint ROM with sequential sectioning of the A2, A3, and A4 pulleys compared with an intact pulley condition. At each stage, external thermoplastic pulley rings were applied snugly over the proximal and middle phalanges to recreate A2 and A4 function, respectively., Results: After complete venting of the A2, A3, and A4 pulleys, proximal interphalangeal joint ROM significantly decreased by 13.4° ± 2.7° and distal interphalangeal joint ROM decreased by 15.8° ± 2.1°. Application of external rings over the proximal and middle phalanx resulted in a residual ROM decrease of 8.3° ± 1.9° at the proximal interphalangeal joint and 7.9° ± 2.1° at the distal interphalangeal joint, nearly restoring ROM. Similarly, complete pulley venting resulted in reduced FDS load by 37% and FDP load by 50% compared with intact pulleys. After application of external rings, loads were restored almost to normal, with a 9% reduction for FDS load and 9% reduction for FDP load compared with intact pulleys., Conclusions: The application of thermoplastic rings acting as external pulleys is an effective, noninvasive, and reproducible approach to restore flexor tendon biomechanics and digit ROM after pulley venting., Clinical Relevance: Thermoplastic rings may be a useful therapeutic adjunct in restoring joint ROM and flexor tendon loads after surgical venting of the pulleys., (Copyright © 2021 American Society for Surgery of the Hand. Objectives: The objective of this survey was to investigate the barriers, facilitators, expectations and patient preferences regarding joint protection (JP) programmes in people with hand arthritis., Design: Cross-sectional survey., Setting: Tertiary clinic., Participants: Patients with hand arthritis: osteoarthritis, rheumatoid arthritis, psoriatic arthritis and other forms of arthritis., Primary and Secondary Outcome Measures: This study used a survey among people with hand arthritis. Descriptive statistics and percentages were reported for all the data about the barriers, facilitators and preferences around JP., Results: A total of 192 patients consented to participate. Most of the patients (82%) were unaware of JP. Factors that may act as barriers to participation and were regarded as 'a very big concern' were: cost of the programme (44%), time of offering the programme (39%), work commitments (36%) and having a centre/clinic close to the house (28%). Factors that may act as facilitators and rated as 'extremely helpful' were: research that shows that JP works (26%) and having the centre/clinic close to the house (25%). An online format for JP was the most preferred option (54%). Half (46%) preferred a timeframe of 1 hour, three times per week and 44% preferred a 2-hour programme, for three times per week., Conclusions: Awareness of the potential benefits of JP, and prior experience with JP programme were very low. Common potentially modifiable patient-reported barriers to participate in future JP interventions, included: cost, work commitments, distance from home to clinic and times that the intervention were provided. Continuum-level finite element models (FEMs) of the humerus offer the ability to evaluate joint replacement designs preclinically; however, experimental validation of these models is critical to ensure accuracy. The objective of the current study was to quantify experimental full-field strain magnitudes within osteoarthritic (OA) humeral heads by combining mechanical loading with volumetric microCT imaging and digital volume correlation (DVC). The experimental data was used to evaluate the accuracy of corresponding FEMs. Six OA humeral head osteotomies were harvested from patients being treated with total shoulder arthroplasty and mechanical testing was performed within a microCT scanner. MicroCT images (33.5 µm isotropic voxels) were obtained in a pre- and post-loaded state and BoneDVC was used to quantify full-field experimental strains (≈ 1 mm nodal spacing, accuracy = 351 µstrain, precision = 518 µstrain). Continuum-level FEMs with two types of boundary conditions (BCs) were simulated: DVC-driven and force-driven. Accuracy of the FEMs was found to be sensitive to the BC simulated with better agreement found with the use of DVC-driven BCs (slope = 0.83, r 2 = 0.80) compared to force-driven BCs (slope = 0.22, r 2 = 0.12). This study quantified mechanical strain distributions within OA trabecular bone and demonstrated the importance of BCs to ensure the accuracy of predictions generated by corresponding FEMs.

Background: The reconstructive options for instability-related anterior glenoid bone loss include iliac crest autograft, allograft, or coracoid transfer. The use of distal clavicle autograft (DCG) has also been described. The purpose of this imaging and cadaveric study was to examine the dimensions, morphology, and bone density of the DCG and compare it with the Latarjet procedure., Methods: We used 49 computed tomography scans from patients with anterior glenoid bone loss to measure the distal clavicle dimensions and bone density. Four glenoid reconstructions were simulated to compare techniques: DCG inferior surface toward glenoid (DCG inferior), DCG superior, classic Latarjet, and congruent-arc Latarjet. In addition, the morphology of the DCG was assessed on computed tomography and confirmed in 27 cadavers., Results: The mean width of the DCG (11 mm) was significantly greater (P < .001) than that of the classic Latarjet orientation (9 mm) but less (P = .002) than that of the congruent-arc orientation (12 mm). The DCG had a lower bone density than the coracoid (P < .001). The mean articular surface area of the DCG-inferior orientation was 208 mm 2 , which was greater (P = .013) than that of the DCG-superior orientation (195 mm 2 ) and not significantly different (P = .44) than that of the classic Latarjet orientation (214 mm 2 ). The surface area of the congruent-arc orientation was greater (285 mm 2 , P < .001) than that of all other graft orientations. The DCG-inferior orientation was able to reconstruct 22% of the glenoid articular surface; DCG-superior orientation, 21%; classic Latarjet orientation, 23%; and congruent-arc orientation, 30%. Three DCG morphologies were identified: square (34%), trapezoidal (53%), and rounded (13%)., Conclusions: The distal clavicle osteoarticular graft was able to reconstruct 22% of the glenoid face. Background: Joint registries provide invaluable data on primary arthroplasties with revision as the endpoint; however, the revision outcomes are often excluded. Therefore, a PROSPERO registered review (CRD42015032531) of all revision studies in North America and Europe was conducted to evaluate demographics, etiologies and indications, implant manufacturer, and complications by geographic region., Methods: The MEDLINE, EMBASE, and CENTRAL databases were searched for revision arthroplasty clinical studies with a minimum mean 24-month follow-up. There were no language exclusions. Articles published in German, French, and Italian were reviewed by research personnel proficient in each language., Results: The mean age at revision was 66 ± 5 years (male = 759, female = 1123). The male-female ratio in North American and Europeans studies was 43:57 and 34:66, respectively. The most common etiology for primary surgery in both regions was osteoarthritis or glenoid arthrosis (38%). The most common revision indication overall was rotator cuff tear, deficiency, or arthropathy (26%). The most common implant type used in revisions was a reverse shoulder arthroplasty (54%). The complication rate for all revisions was 17%. There were a total of 465 complications, and of those, 74% lead to a reoperation., Conclusion: Generally, shoulder arthroplasties are designed to last 10-15 years; however, revisions are being performed at a mean 3.9 years from the primary procedure, based on the published studies included in this systematic review. Additionally, of the complications, a large number (74%) went on to a reoperation. Existing in vitro simulators rely on external manipulation of the skull to replicate masticatory forces; however, external manipulations do not accurately represent internal loads as in physiological muscle forces.The purpose of the project is to develop an in vitro simulator that internally replicates the forces of mastication. The simulator has 3-dimensional-printed piston mounts that are reverse-engineered using a computed tomography scan of the specimen. The mounts are attached to the skull at muscle attachment sites using adhesive. The pneumatic pistons are sutured to muscle tendons; when the pistons are activated, they pull on the tendons which proportionally replicate muscle loads. The force output of the pistons can be individually modified by a custom software. Strain gauges are attached to craniofacial bones to measure deformation under replicated muscle loads. A 6 degrees-of freedom force sensor is placed intraorally to measure the generated bite force.The methodology was validated on 6 fresh-frozen cadaveric heads. Change in strain measurements was observed with change in simulated muscle loads. The simulator can validate computer simulation models and provide an experimental platform for craniofacial and dental implants. It sets the framework for a new, more physiologically consistent way of studying craniofacial stresses.

Introduction: Measuring finger forces during activities of daily living and how these forces change for individuals with pathologies such as arthritis is valuable to our understanding of hand function., Purpose of the Study: The purpose of this study was to determine the forces of individual fingers during the performance of daily activities in healthy participants and determine the envelope of these applied forces., Methods: This is a cross-sectional study investigating twenty-five healthy participants (12 female: 22-65 years old and 13 male: 20-53 years old) and participants with osteoarthritis (12 female: 52-79 years old and 9 male: 64-79 years old) examined at one time point. The force sensors were calibrated for each individual using a load cell to provide force output in Newtons. Each participant performed 19 activities of daily living two times. Force was plotted over time for each task, and the maximum force in each finger during that task was evaluated., Results: The range of applied forces was 1.4 ± 0.6 N to 34.8 ± 1.6 N for healthy participants and 2.3 ± 1.0 N to 30.7 ± 3.7 N for those with osteoarthritis., Discussion: Sensors allowed for real-time monitoring of finger forces during tasks of daily life. This provides the opportunity to isolate hand grips based on finger recruitment and provide information about the magnitude of forces during the activity., Conclusion: Measurement of individual finger forces can provide more accurate biomechanical models of the hand and determine the effect of disease on hand functions., (Copyright © 2020. Subject- and site-specific modeling techniques greatly improve finite element models (FEMs) derived from clinical-resolution CT data. A variety of density-modulus relationships are used in scapula FEMs, but the sensitivity to selection of relationships has yet to be experimentally evaluated. The objectives of this study were to compare quantitative-CT (QCT) derived FEMs mapped with different density-modulus relationships and material mapping strategies to experimentally loaded cadaveric scapular specimens. Six specimens were loaded within a micro-CT (33.5 μm isotropic voxels) using a custom-hexapod loading device. Digital volume correlation (DVC) was used to estimate full-field displacements by registering images in pre- and post-loaded states. Experimental loads were measured using a 6-DOF load cell. QCT-FEMs replicated the experimental setup using DVC-driven boundary conditions (BCs) and were mapped with one of fifteen density-modulus relationships using elemental or nodal material mapping strategies. Models were compared based on predicted QCT-FEM nodal reaction forces compared to experimental load cell measurements and linear regression of the full-field nodal displacements compared to the DVC full-field displacements. Comparing full-field displacements, linear regression showed slopes ranging from 0.86 to 1.06, r-squared values of 0.82-1.00, and max errors of 0.039 mm for all three Cartesian directions. Nearly identical linear regression results occurred for both elemental and nodal material mapping strategies. Comparing QCT-FEM to experimental reaction forces, errors ranged from - 46 to 965% for all specimens, with specimen-specific errors as low as 3%. This study utilized volumetric imaging combined with mechanical loading to derive full-field experimental measurements to evaluate various density-modulus relationships required for QCT-FEMs applied to whole-bone scapular loading. The results suggest that elemental and nodal material mapping strategies are both able to simultaneously replicate experimental full-field displacements and reactions forces dependent on the density-modulus relationship used.

Background: The Walch type B glenoid has the hallmark features of retroversion, joint subluxation, and bony erosion. Although the type B glenoid has been well described, the morphology of the corresponding type B humerus is poorly understood. As such, the aim of this imaging-based anthropometric study was to investigate humeral torsion in Walch type B shoulders., Methods: Three-dimensional models of the full-length humerus were generated from computed tomography data for the Walch type B group (n = 59) and for a control group of normal nonarthritic shoulders (n = 59). An anatomic humeral head-neck plane was created and used to determine humeral torsion relative to the epicondylar axis. Measurements were repeated, and intraclass correlation coefficients were calculated., Results: The type B humeri had significantly (P < .001) less retrotorsion (14° ± 9°) than the control group (36° ± 12°) relative to the epicondylar axis. Male and female individuals within the control group showed statistically significant differences in humeral torsion (P = .043), which were not found in the type B group. Inter-rater reliability showed excellent agreement for humeral torsion (intraclass correlation coefficient, 0.962). A subgroup analysis between Walch type B2 and B3 shoulders showed no significant differences in any of the humeral or glenoid parameters., Conclusion: The Walch type B humerus has significantly less retrotorsion than non-osteoarthritic shoulders. At present, it is unknown whether the altered humeral retrotorsion is a cause or effect of the type B glenoid. Subject-specific finite element models (FEMs) of the shoulder complex are commonly used to predict differences in internal load distribution due to injury, treatment or disease. However, these models rely on various underlying assumptions, and although experimental validation is warranted, it is difficult to obtain and often not performed. The goal of the current study was to quantify the accuracy of local displacements predicted by subject-specific QCT-based FEMs of the scapula, compared to experimental measurements obtained by combining digital volume correlation (DVC) and mechanical loading of cadaveric specimens within a microCT scanner. Four cadaveric specimens were loaded within a microCT scanner using a custom-designed six degree-of-freedom hexapod robot augmented with carbon fiber struts for radiolucency. BoneDVC software was used to quantify full-field experimental displacements between pre- and post-loaded scans. Corresponding scapula QCT-FEMs were generated and three types of boundary conditions (BC) (idealized-displacement, idealized-force, and DVC-derived) were simulated for each specimen. DVC-derived BCs resulted in the closest match to the experimental results for all specimens (best agreement: slope ranging from 0.87 to 1.09; highest correlation: r 2 ranging from 0.79 to 1.00). In addition, a two orders of magnitude decrease was observed in root-mean-square error when using QCT-FEMs with simulated DVC-derived BCs compared to idealized-displacement and idealized-force BCs. The results of this study demonstrate that scapula QCT-FEMs can accurately predict local experimental full-field displacements if the BCs are derived from DVC measurements., (Crown Copyright © 2019. Study Design: Systematic review with meta-analysis., Introduction: Joint protection (JP) has been developed as a self-management intervention to assist people with hand arthritis to improve occupational performance and minimize joint deterioration over time., Purpose of the Study: We examined the effectiveness between JP and usual care/control on pain, hand function, and grip strength levels for people with hand osteoarthritis and rheumatoid arthritis., Methods: A search was performed in 5 databases from January 1990 to February 2017. Two independent assessors applied Cochrane's risk of bias tool, and a Grading of Recommendations Assessement, Development and Evaluation (GRADE) approach was adopted., Results: For pain levels at short term, we found similar effects between JP and control standardized mean difference (SMD; -0.00, 95% confidence interval [CI]: -0.42 to 0.42, I 2  = 49%), and at midterm and long-term follow-up, JP was favored over usual care SMD (-0.32, 95% CI: -0.53 to -0.11, I 2  = 0) and SMD (-0.27, 95% CI: -0.41 to -0.12, I 2  = 9%), respectively. For function levels at midterm and long-term follow-up, JP was favored over usual care SMD (-0.49, 95% CI: -0.75 to -0.22, I 2  = 34%) and SMD (-0.31, 95% CI: -0.50 to -0.11, I 2  = 56%), respectively. For grip strength levels, at long term, JP was inferior over usual care mean difference (0.93, 95% CI: -0.74 to 2.61, I 2  = 0%)., Conclusions: Evidence of very low to low quality indicates that the effects of JP programs compared with usual care/control on pain and hand function are too small to be clinically important at short-, intermediate-, and long-term follow-ups for people with hand arthritis., (Copyright © 2018 Hanley & Belfus. Incorporating subject-specific mechanical properties derived from clinical-resolution computed tomography data increases the accuracy of finite element models. Site-specific relationships between density and modulus are required due to variations in trabecular architecture and tissue density by anatomic location. Equations have been developed for many anatomic locations and have been shown to have excellent statistical agreement with empirical results; however, a shoulder-specific density-modulus relationship does not currently exist. This study used micro-finite element cores of glenoid trabecular bone and co-registered quantitative computed tomography finite element models to develop a validated glenoid trabecular density-modulus relationship. Micro finite element model tissue density was considered as either homogeneous or heterogeneous, scaled by CT-intensity. When heterogeneous tissue density was considered, near absolute statistical agreement was predicted in the co-registered QCT-derived finite element models. The validated relationships have also been adapted for use in whole bone scapular models and have the potential to dramatically increase the accuracy of clinical-resolution CT-derived shoulder finite element studies., (Copyright © 2018 Elsevier Ltd. Preclinical and clinical bone strength predictions can be elucidated by understanding bone mechanics at a variety of hierarchical levels. As such, down-sampled micro-CT images are often used to make comparisons across image resolutions or used to reduce computational resources in micro finite element models (µFEMs). Therefore, the objectives of this study were to compare trabecular apparent modulus among (i) hexahedral and tetrahedral µFEMs, (ii) µFEMs generated from 32, 64, and 64 µm down-sampled from 32 µm µCT scans, and (iii) µFEMs with homogeneous and heterogeneous tissue moduli. Trabecular µFEMs were generated from scans at the three spatial resolutions taken from the glenoid vault of 14 cadaveric specimens. Simulated unconstrained compression was performed and used to calculate and compare the apparent modulus of each µFEM. It was found that models derived from high-resolution images that account for material heterogeneity are nearly equivalent whether hexahedral or tetrahedral elements are used. However, translation of stiffness from down-sampled scans are not equivalent to scans performed at the down-sampled resolution, or that account for trabecular material heterogeneity. Material heterogeneity is most representative of in vivo trabecular bone and to accurately model trabecular mechanical properties, material heterogeneity should be considered in future µFEM development.

Purpose: Glenoid reaming is a technically challenging step during shoulder arthroplasty that could possibly be learned during simulation training. Creation of a realistic simulation using vibration feedback in this context is innovative. Our study focused on the development and internal validation of a novel glenoid reaming simulator for potential use as a training tool., Methods: Vibration and force profiles associated with glenoid reaming were quantified during a cadaveric experiment. Subsequently, a simulator was fabricated utilizing a haptic vibration transducer with high- and low-fidelity amplifiers; system calibration was performed matching vibration peak-peak values for both amplifiers. Eight experts performed simulated reaming trials. The experts were asked to identify isolated layer profiles produced by the simulator. Additionally, experts' efficiency to successfully perform a simulated glenoid ream based solely on vibration feedback was recorded., Results: Cadaveric experimental cartilage reaming produced lower vibrations compared to subchondral and cancellous bones ([Formula: see text]). Gain calibration of a lower-fidelity (3.5 [Formula: see text] and higher-fidelity (3.4 [Formula: see text] amplifier resulted in values similar to the cadaveric experimental benchmark (3.5 [Formula: see text]. When identifying random tissue layer samples, experts were correct [Formula: see text] of the time and success rate varied with tissue type ([Formula: see text]). During simulated reaming, the experts stopped at the targeted subchondral bone with a success rate of [Formula: see text]. The fidelity of the simulation did not have an effect on accuracy, applied force, or reaming time ([Formula: see text]). However, the applied force tended to increase with trial number ([Formula: see text])., Conclusions: Development of the glenoid reaming simulator, coupled with expert evaluation furthered our understanding of the role of haptic vibration feedback during glenoid reaming. This study was the first to (1) propose, develop and examine simulated glenoid reaming, and (2) explore the use of haptic vibration feedback in the realm of shoulder arthroplasty.

The anterior bundle of the medial collateral ligament (AMCL) of the elbow is commonly injured in patients with elbow dislocations and in throwing athletes. This in-vitro study quantified tension in the native AMCL throughout elbow flexion for different arm positions. We conducted passive and simulated active elbow flexion in seven fresh-frozen cadaveric upper extremities using an established motion simulator. Motions were performed in the valgus and vertical positions from 20-120° while measuring AMCL tension using a custom transducer. Average AMCL tension was higher in the valgus compared to vertical position for both active (p = 0.03) and passive (p = 0.01) motion. Peak AMCL tension was higher in the valgus position for active (p = 0.02) and passive (p = 0.01) motion. There was no significant difference in AMCL tension between active and passive motion in the valgus (p = 0.15) or vertical (p = 0.39) positions. In the valgus position, tension increased with elbow flexion from 20-70° for both active (p = 0.04) and passive (p = 0.02) motion, but not from 70-120°. This in-vitro study demonstrated that AMCL tension increases with elbow flexion, and is greater in the valgus position relative to the vertical position. This information has important implications to the desired target strength of repair and reconstruction techniques.

Background: The type B3 glenoid is an addition to the Walch classification. A potential etiologic theory is that it is a progression of the B2. It is characterized by uniconcavity, absent paleoglenoid, medialization, retroversion, and subluxation. The purpose of this study was to describe the morphology of B3 glenoids., Methods: Fifty-two patients with B3 glenoids underwent 3-dimensional analysis of computed tomography data. Glenoid measurements (retroversion, inclination, medialization) and humeral head subluxation according to the scapular and glenoid planes were determined. The measured variables were compared between male and female patients., Results: The mean B3 retroversion, inclination, and medialization were 24° ± 7°, 8° ± 6° superior, and 14 ± 4 mm, respectively. The mean posterior subluxation was 80% ± 8% and 54% ± 6% according to the scapular and glenoid planes, respectively. There were no differences in B3 characteristics between sexes (P > .05). A significant correlation existed between glenoid retroversion and humeral head subluxation relative to the scapular plane, with every 1° increase in retroversion translating to a 1% increase in subluxation (P < .001). In contrast, when referencing the glenoid plane, the humeral head remained concentric to the erosion., Conclusions: The B3 is uniconcave and retroverted. As glenoid retroversion increases, posterior humeral head subluxation significantly increases as referenced to the scapular plane; however, when referenced to the glenoid plane, the head remains concentric to the erosion. This appearance of "concentricity" is acquired secondary to the wear pattern, creating a uniconcave glenoid. Therefore, surgeons should be aware that the visualized concentricity is a product of the erosion pattern and thus may conceal a greater amount of subluxation potential., (Copyright © 2017 Journal of Shoulder and Elbow Surgery Board of Trustees. Background: Finite element modeling of human bone provides a powerful tool to evaluate a wide variety of outcomes in a highly repeatable and parametric manner. These models are most often derived from computed tomography data, with mechanical properties related to bone mineral density (BMD) from the x-ray energy attenuation provided from this data. To increase accuracy, many researchers report the use of quantitative computed tomography (QCT), in which a calibration phantom is used during image acquisition to improve the estimation of BMD. Since model accuracy is dependent on the methods used in the calculation of BMD and density-mechanical property relationships, it is important to use relationships developed for the same anatomical location and using the same scanner settings, as these may impact model accuracy. The purpose of this literature review is to report the relationships used in the conversion of QCT equivalent density measures to ash, apparent, and/or tissue densities in recent finite element (FE) studies used in common density-modulus relationships. For studies reporting experimental validation, the validation metrics and results are presented., Results: Of the studies reviewed, 29% reported the use of a dipotassium phosphate (K 2 HPO 4 ) phantom, 47% a hydroxyapatite (HA) phantom, 13% did not report phantom type, 7% reported use of both K 2 HPO 4 and HA phantoms, and 4% alternate phantom types. Scanner type and/or settings were omitted or partially reported in 31% of studies. The majority of studies used densitometric and/or density-modulus relationships derived from different anatomical locations scanned in different scanners with different scanner settings. The methods used to derive various densitometric relationships are reported and recommendations are provided toward the standardization of reporting metrics., Conclusions: This review assessed the current state of QCT-based FE modeling with use of clinical scanners. It was found that previously developed densitometric relationships vary by anatomical location, scanner type and settings. Reporting of all parameters used when referring to previously developed relationships, or in the development of new relationships, may increase the accuracy and repeatability of future FE models.

Background: Posterior glenoid erosion is thought to be initiated by humeral head subluxation. However, it is unknown whether subluxation is entirely caused by soft-tissue instability and unbalanced muscle activity or whether osseous morphology is a contributing factor. We hypothesized that patients with posterior erosion may exhibit premorbid glenoid morphology that is inherently retroverted and inferiorly inclined compared with age-matched normal glenoids., Methods: This study examined 80 scapulae, evenly distributed between 2 groups: osteoarthritic with type B2 glenoids and age-matched normal glenoids. From 3-dimensional computed tomography reconstructions, version and inclination were measured from the anterior paleoglenoid region of the B2 glenoids, which is representative of the premorbid glenoid, and compared with measurements obtained from similar regions in the normal cohort., Results: The anterior paleoglenoid region of B2 glenoids was significantly (P < .001) more retroverted (-14° ± 6°) compared with similar regions in nonarthritic normal glenoids (-5° ± 5°). There were no significant differences (P = .166) in the glenoid inclination angle between type B2 glenoids (0° ± 6°) and nonarthritic normal glenoids (2° ± 5°). Negative values represent retroverted and inferiorly inclined glenoids., Discussion: Understanding premorbid glenoid morphologic variations may provide insight into the pathoanatomy of humeral head subluxation, osteoarthritis, and posterior glenoid erosion. The results of this study indicate that patients with type B2 osteoarthritic glenoids have significantly greater premorbid glenoid retroversion compared with nonarthritic normal glenoids, suggesting that this premorbid morphologic variation may be one contributing factor to posterior erosion., (Copyright © 2016 Journal of Shoulder and Elbow Surgery Board of Trustees. The number of shoulder arthroplasty procedures has increased dramatically in recent years, with the primary indication being osteoarthritis (OA). Thus, morphology and subchondral bone changes associated with OA may be important factors to consider when choosing a replacement component. For surgical treatment, many implant options exist and survivability is often dependent on patient age, activity level, and progression of OA. In the placement of these replacement components, patient-specific guides now exist to improve component positioning, with the goal to improve long-term survivability by ensuring that intra-operative placement meets component design.

Background: The purpose of this study was to evaluate and to compare the size and morphologic patterns among normal and osteoarthritic (OA) humeral heads., Methods: This comparative anatomic imaging study evaluated 150 humeral heads that were separated into 3 cohorts: normal, OA with symmetric glenoid erosion, and OA with asymmetric (type B2) glenoid erosion. Three-dimensional models were created of the humeral head from computed tomography data, and point coordinates were extracted for evaluation. Parameters measured were diameter (sphere fit and circle fit), chord distance (superoinferior and anteroposterior), and humeral head height., Results: The sphere-fit diameter of the humeral head for the entire OA cohort (100 patients; mean diameter, 59 ± 9 mm) was significantly greater (P < .001) than that of the normal cohort (50 patients; mean diameter, 49 ± 5 mm). Similarly, the humeral head circle-fit diameters in the superoinferior and anteroposterior planes were significantly greater (P < .001) in the combined OA cohorts (59 ± 9 mm and 56 ± 10 mm, respectively) compared with the normal cohort (51 ± 5 mm and 47 ± 5 mm, respectively). However, there were no significant differences (P ≥ .099) between the symmetric and asymmetric OA cohorts in sphere-fit or circle-fit diameters. The mean values of humeral head heights were not significantly different (P = .382) between cohorts, 19 ± 2 mm, 18 ± 2 mm, and 18 ± 2 mm for the normal, symmetric, and asymmetric cohorts, respectively., Discussion: Although OA humeral head morphology varies significantly from normal, it does not vary as a function of the Walch classification between symmetric and asymmetric glenoids. Background: Distal humeral hemiarthroplasty is a treatment option for elbow joint disease that predominantly affects the distal humerus, including distal humerus fractures, nonunions, and avascular necrosis. The effect of hemiarthroplasty implants on joint contact has not been reported. The purpose of this in vitro study was to quantify the effects of hemiarthroplasty and implant size on ulnohumeral joint congruency., Methods: Five fresh frozen cadaveric upper extremities were mounted to a custom elbow testing system. Active and passive motion were performed in dependent, horizontal, varus, and valgus positions. A registration and interbone distance algorithm was used to quantify ulnohumeral joint congruency throughout elbow flexion., Results: The optimally sized hemiarthroplasty implant demonstrated the greatest joint congruency with the ulna, followed by the oversized implant, then the undersized implant. Joint congruency was greater during active vs. passive flexion, indicating that the elbow joint is more reduced in active flexion than in passive flexion., Conclusion: This study demonstrates that undersized distal humeral hemiarthroplasty implants have the lowest joint congruency compared with an optimally sized or oversized implant., (Copyright © 2016 Journal of Shoulder and Elbow Surgery Board of Trustees. Background: Accurate measurement of forces through the proximal radius can assess the effects of some surgical procedures on radioulnar load sharing, but is difficult to achieve given the redundant loading nature of the musculoskeletal system. Previously reported devices have relied on indirect measurements that may alter articular joint location and function. An axial load transducer interposed in the diaphysis of the radius may accurately quantify unknown axial loads of the proximal radius, and maintain articular location., Methods: An in-vitro radius model was developed by interposing an axial load transducer in the diaphysis of the proximal radius. Static loads of 20, 40, 60, 80, and 100 N were applied with a servo-hydraulic actuator to the native radial head at angles of 10°, 20°, 30°, and 40° in the anterior, posterior, medial and lateral directions., Findings: Linear regression of five repeatability trials showed excellent agreement between the transducer and applied loads (R (2) = 1 for all trials). For off-axis net joint loads, the majority of measured loading errors were within the inter-quartile range for mean loads up to 80 N. Loads below 80 N and outside the inter-quartile range had errors of less than 1 N., Conclusions: The repeatability and off-axis net joint load results of this study validate the effectiveness of the interposed axial load transducer to accurately quantify proximal radius loads. The surgical technique preserves the native articular location and soft-tissue constructs, like the annular ligament. The modular design allows for testing the effects of length-changing osteotomies in subsequent biomechanical studies.

Background: The purpose of this computational modeling study was to compare the volume of glenoid bone removal required to implant 3 augmented component designs for management of B2 erosions. In addition, we assessed bone quality of the supporting bone directly beneath the implants by measuring bone density and porosity., Methods: Three augmented component designs—full-wedge, posterior-wedge, and posterior-step—were studied by virtual implantation in a cohort of 16 patients with B2 glenoids. B2 retroversion was corrected to 0° and 10°. The outcome variables were the volume of glenoid bone removal required for implantation and the density and porosity of the bone immediately beneath the implant., Results: Implant design had a significant effect on the volume of bone removal (P < .001). When correcting to 0°, the posterior-wedge implant removed less bone than the posterior-step (P < .001) and the full-wedge (P = .004). At 10° retroversion, the posterior-wedge removed less bone (P = .029) than the posterior-step but was no different than the full-wedge (P = .143). The residual glenoid bone density with the posterior-wedge was significantly greater than with the posterior-step (P = .048), with no other significant differences (P > .05). Residual glenoid bone porosity was not significantly different between implants (P > .262)., Conclusions: Augmented components can provide a bone-preserving option for B2 glenoid management. Substantial variations in the volume of bone removal and the quality of the remaining glenoid bone were found between 3 different designs of augmented implants. Background: The purpose of this computed tomography-based study was to quantify erosions in B2 glenoids. We hypothesized that erosions do not occur symmetrically and that they have a predictable orientation., Methods: This study evaluated 55 type B2 glenoid cases. Computed tomography data were used to create three-dimensional reconstructions, and point coordinates were extracted from each reconstruction for morphologic analysis of the osteoarthritic glenoid and humerus., Results: There was a significant difference between the mean orientation angle (28° ± 11°) of the posterior glenoid line of erosion and the superoinferior axis (P < .001), which resulted in the average erosion being directed toward the 8-o'clock position. On average, the erosion started 1.6 ± 3.4 mm posterior to the glenoid center point. In 35% of B2 cases, the line of erosion was curved. The mean surface area was 763 ± 296 mm(2) for the neoglenoid and 957 ± 276 mm(2) for the paleoglenoid, indicating that the neoglenoid occupied 44% ± 12% of the total glenoid area. In this cohort, the mean radius of the humeral head neoarticulation was 32 ± 6 mm, the neoglenoid radius was 37 ± 8 mm, and the paleoglenoid radius was 34 ± 7 mm. The radius of the humeral head was significantly less than that of the neoglenoid (P < .001) and the paleoglenoid (P = .009). In addition, the radius of the neoglenoid was significantly greater than the radius of the paleoglenoid (P = .012)., Discussion: Type B2 glenoids have a predictable wear pattern, which is not axisymmetric to the glenoid superoinferior axis. Background: Accurate characterization of regional variations in bone density in symmetric and asymmetric (B2) glenoid erosion patterns can assist with surgical planning, reaming, and component implantation. The purpose of this study was to characterize regional bone density and porosity in symmetric and asymmetric (B2) osteoarthritic glenoids., Methods: Symmetric (n = 25) and asymmetric (B2) (n = 25) erosion patterns were compared by computed tomography-based imaging software. An orthogonal coordinate system separated each glenoid into quadrants. In addition, a linear best-fit line defined the line-of-erosion between the paleoglenoid and neoglenoid in the asymmetric cohort. All glenoids were further divided into volumes at depths of 0 to 2.5 mm and 2.5 to 5 mm. Average bone density was measured in Hounsfield units. Bone voids or cysts were included to quantify regional porosity as the fraction of void volume to total glenoid volume., Results: For the symmetric cohort, there were no significant differences in bone density between quadrants at either depth (P ≥ .089). For the asymmetric cohort, bone density was significantly higher in the posterior quadrants compared with the anterior quadrants (P < .001), especially posteroinferiorly (P ≤ .007) at both depths. In addition, the neoglenoid had significantly higher density and lower void fraction compared with the paleoglenoid (P < .001). There were also significant differences in void fraction between quadrants for both cohorts, at both depths (P ≤ .004)., Conclusions: This study demonstrates that osteoarthritic glenoids with symmetric erosion have uniform subarticular bone density. However, asymmetric (B2) erosion patterns have potentially important regional variations in bone density and porosity, with the densest bone with the least porosity found posteroinferiorly or in the neoglenoid region., (Copyright © 2015 Journal of Shoulder and Elbow Surgery Board of Trustees. Background: Distal humeral hemiarthroplasty is a treatment option for distal humeral fractures, nonunions, and avascular necrosis. The biomechanical effects, however, have not been reported. The purpose of this in vitro study was to quantify the effects of hemiarthroplasty and implant size on elbow joint kinematics., Methods: Eight fresh-frozen cadaveric arms were mounted in an in vitro motion simulator. An electromagnetic tracking system quantified elbow kinematics. A custom distal humeral stem was implanted by use of navigation, and 3 humeral articular spools were evaluated: optimally sized, undersized, and oversized. Statistical analysis was performed with repeated-measures analysis of variance., Results: Distal humeral hemiarthroplasty altered elbow kinematics, regardless of implant size. In the valgus position, the optimally sized implant resulted in a mean increase in valgus angulation of 3° ± 1° (P = .003) as compared with the osteotomy control. In the varus position, the optimal and undersized implants both resulted in significant increases in varus angulation: 3° ± 1° (P = .01) and 3° ± 1° (P = .001), respectively. The undersized implant had the greatest alteration in kinematics, whereas the oversized implant best reproduced native elbow kinematics., Conclusion: This study showed a small but significant alteration in elbow joint kinematics with placement of a distal humeral hemiarthroplasty implant, regardless of implant size. This could be due to errors in implant positioning and/or differences in the shape of the humeral implant relative to the native elbow. This study was conducted to determine the effect of in vitro passive and active loading on humeral head translation during glenohumeral abduction. A shoulder simulator produced unconstrained active abduction of the humerus in 8 specimens. Loading of the supraspinatus, subscapularis, infraspinatus/teres minor, and anterior, middle, and posterior deltoid muscles was simulated by use of 4 different sets of loading ratios. Significantly greater translations of the humeral head occurred both in 3 dimensions (P < .001) and in the sagittal plane (P < .005) during passive motion when compared with active motion from 30 degrees to 70 degrees of abduction. In the sagittal plane, passive abduction experienced a resultant translation of 3.8 +/- 1.0 mm whereas the active loading ratios averaged 2.3 +/- 1.0 mm. There were no significant differences in the translations that were produced by the 4 sets of muscle-loading ratios used to achieve active motions. This study emphasizes the importance of the musculature in maintaining normal ball-and-socket kinematics of the shoulder.
