For children with precursor B (pre-B) acute lymphoblastic leukemia (ALL) with TCF3-PBX1 fusion gene, their prognosis has been a controversial topic. From January 2008 to December 2012 in our hospital, 450 patients were diagnosed as ALL. Clinical characteristics of 20 patients with TCF3-PBX1 fusion gene were analyzed retrospectively, which were classified to the intermediate-risk (IR) group according to Chinese Children Leukemia Group-2008 (CCLG-2008) risk-stratification criteria and protocol based on the backbone of BFM 95 trails. Eighty five cases without TCF3-PBX1 in the same IR group were regarded as the comparison group. There were no differences in age, gender, initial white blood cell (WBC) count, status of central nerves system (CNS) at diagnosis and complete remission (CR) rates of bone marrow (BM) between the two groups ( P > .05 ). The 5-year probability of event-free survival (EFS) rates were 84.4 ± 15.6% and 73.5 ± 15.6% in the TCF3-PBX1 group and the comparison group ( P = .35), respectively. The 5-year probability of overall survival (OS) rates were 86.0 ± 17.6% and 81.8 ± 17.6% ( P = .46), respectively. Relapse rates were 10.5% and 12.9% ( P = 1.00), respectively. There were not cases with CNS relapse in the TCF3-PBX1 group. When intensive chemotherapy was used, the TCF3-PBX1 was associated with a favorable outcome in childhood pre-B ALL. ( E, E, E)-Geranylgeraniol (GGOH) is a valuable ingredient of many perfumes and a valuable precursor for synthesizing pharmaceuticals. In an attempt to increase the GGOH concentration in Coprinopsis cinerea, we demonstrated that the expression of geranylgeranyl diphosphate synthase ( ggpps) gene isolated from Taxus x media could promote GGOH production. Furthermore, the concentrations of squalene and ergosterol were measured in the engineered strains. Expectedly, significant decreases of squalene and ergosterol levels were observed in those strains transformed with ggpps gene. This could be explained by the partial redirection of metabolic flux from squalene to GGOH, whose biosynthesis competes for the same precursor with squalene. This work suggested that the expression of ggpps in higher fungi was an effective method for bio-production of GGOH. [ABSTRACT FROM AUTHOR]

Singlecrystal structures of blockbuster antihypertensive drug Valsartanare revealedthe first time in this report. Two new highly crystalline forms, namedform E and an ethanol solvate form F, were discovered and fully characterizedby PXRD, Raman, IR, TG, DSC, and DVS. Conformational flexibility andsingle crystal structures are discussed in detail. Physicochemicalproperties, such as hygroscopicity, chemical stability, crystallinity,and dissolution behaviors, are compared with the marketed solid-stateform (amorphous state). A varierty of previously unreported compounds, the cagelike hexacyclic schicagenin-type nortriterpenoid 1, three pre-schisanartane-type nortriterpenoids 2- 4, seven C29 nortriterpenoids 5- 11 possessing an uncommon pentacyclic carbon skeleton characterized by a 5/5/7/7/5 fused-ring motif bearing a C9 side chain, two highly oxygenated C28 nortriterpenoids 12 and 13 containing an unprecedented 5/5/7 ring nucleus bearing a C15 side chain, and the known C29 nortriterpenoid 14 were isolated from the leaves and stems of the Chinese medicinal plant Schisandra chinensis. The structures of these new metabolites were elucidated by means of detailed spectroscopic analysis. The absolute configurations of 4 and 5 were determined by time-dependent density functional theory electronic circular dichroism (TDDFT ECD) calculations, allowing the assignment of the absolute configurations of analogous compounds 1- 3 and 6- 13. Applying the emerging molecular networking strategy, an uncommon cembranoid orthoester, sarcotortin A (1), featuring a 3/14/8/5-fused scaffold, an unusual eunicellane-type diterpenoid, sarcotorolide A (2), and two new biscembranoids, ximaolides M and N (7 and 8), along with nine known terpenoids 3-6 and 9-13 were isolated from the Hainan soft coral Sarcophyton tortuosum. The structure and absolute configuration of all new compounds were established by a combination of spectroscopic data, X-ray diffraction analysis, and/or quantum chemical computational approaches. The plausible biogenetic relationship among these skeletally different terpenoids was proposed and discussed. In in vitro bioassay, new compound 7 exhibited a remarkable inhibitory activity against protein tyrosine phosphatases 1B (PTP1B) with the IC 50 value of 8.06 μM. In addition, compounds 4 and 10 displayed significant inhibitory effects on lipopolysaccharide (LPS)-induced inflammatory responses in RAW264.7 macrophages cells with the IC 50 values of 19.13 and 16.45 μM, respectively. A novel diterpenoid, sinueretone A ( 1 ), featuring an unprecedented tricyclo[12.1.0.0 5,9 ]pentadecane carbon framework, along with two new ( 2 and 3 ) and one known ( 4 ) casbane diterpenoids were isolated from the South China Sea soft coral Sinularia erecta . The structures of the new compounds, especially their absolute stereochemistry, were established by extensive spectroscopic analysis, various quantum chemical calculations, and/or X-ray diffraction analyses. A plausible biogenetic relationship of 1 - 4 was proposed, which gave an insight for future biomimetic synthesis of the novel compounds. In a bioassay, compounds 1 and 2 displayed interesting anti-inflammatory activity by the inhibition of lipopolysaccharide-induced tumor necrosis factor-α protein release.

Objective: To compare the effect of electroacupuncture (EA) of acupoint group for "reinforcing the kidney and regulating Governor Vessel" and acopoint group for "reinforcing the kidney and lung and regulating Governor Vessel" on lear-ning-memory ability and expression of tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) proteins in the hippocampus and prefrontal cortex (PFC) in Alzheimer's disease （AD） rats, so as to explore the efficacy of the two acupoint groups and mechanisms underlying improvement of AD., Methods: Forty male SD rats were randomly divided into control, sham operation, model, "Baihui" + "Shenshu" (GV20+BL23, for "reinforcing the kidney and regulating Governor Vessel") EA and GV20+BL23+ "Feishu" (BL13, GV20+BL23+BL13, for "reinforcing the kidney and lung and regulating Governor Vessel") EA groups ( n =8 rats in each group). The AD model was established by bilateral injection of amyloid β peptide （Aβ 25-35 ，10 μL) into bilateral hippocampus, and rats of the sham operation group received injection of normal saline. After successful establishment of the model，EA (2 Hz, 2 mA) was applied to these acupoints for 15 min, once daily for 10 days. Then, the learning-memory ability was assessed by using Morris water maze tests, and the expression levels of TNF-α and IL-1β proteins in the PFC and hippocampus tissues were detected by using Western blot., Results: Following modeling, the average escape latency of place navigation test were significantly increased ( P <0.05) and the platform crossing times of spatial probe test was significantly decreased in the model group than in the control and sham operation groups ( P <0.05). The expression levels of IL-1β and TNF-α proteins in the PFC and hippocampus were apparently up-regulated in the model group than in the control and the sham operation groups ( P <0.000 1, P <0.001, P <0.01). After the intervention, the increase of the average escape latency and expression of IL-1β and TNF-α in the PFC and hippocampus, and the decrease of space exploration test were revised in both GV20+BL23 EA and GV20+BL23+BL13 EA groups ( P <0.05, P <0.01). No significant differences were found between the GV20+BL23 and GV20+BL23+BL13 EA groups in the above mentioned indexes ( P >0.05)., Conclusion: EA of both GV20+BL23 and GV20+BL23+BL13 acupoint can improve learning-memory ability of AD rats, which is associated with their effects in down-regulating the expression of IL-1β and TNF-α in the PFC and hippocampus to reduce inflammatory reaction. There were no significant differences between the two acupoint groups in the therapeutic effects.

Awareness of prevention is enhanced to reduce the rate of infection by media coverage, which plays an important role in preventing and controlling infectious diseases. Based on epidemic situation of the Corona Virus Disease 2019 (COVID-19) in Hubei, an SIHRS epidemic model with media coverage was proposed. Firstly, by the basic reproduction number R0, the globally asymptotically stable of the disease-free equilibrium and the endemic equilibrium were proved. Then, based on the reported epidemic data of Hubei Province from January 26 to February 13, numerical simulations are used to verify the analysis results, and the impact of peak time and the scale of disease transmission were mainly considered with different information implementation rate and the contact rate. It was shown that with the decrease of information implementation rate, the peak of confirmed cases would be delayed to reach, and would increase significantly. Therefore, in order to do a better prevention measures after resumption of work, it is very necessary to maintain the amount of information and implementation rate of media coverage.

Objective: To investigate the effect of electroacupuncture on the P35/P25-cyclin-dependent kinase 5 (CDK5)-Tau pathway in rats with Alzheimer's disease (AD), as well as the mechanism of electroacupuncture in the prevention and treatment of AD., Methods: Sprague-Dawley rats were randomly divided into control group, sham-operation group, model group, and electroacupuncture treatment group, with 12 rats in each group. A rat model of AD was established by injection of Aβ 25-35 into the bilateral hippocampus. The rats in the electroacupuncture treatment group were given electroacupuncture at "Baihui" (GV20) and "Shenshu" (BL23) once a day, 15 min each time, for 10 days. Morris water maze was used to evaluate learning and memory abilities, immunohistochemistry was used to measure the distribution and expression of P35/P25, CDK5, and Tau5 in the hippocampus, and Western blot was used to measure the expression of the above mentioned proteins, phosphory-lated Tau(Ser199, Ser202)., Results: In the visual platform test, there were no significant differences in escape latency and search path between groups ( P >0.05). In the hidden platform test, there were no significant differences in escape latency and search path between the control group and the sham-operation group ( P >0.05); the model group had significantly longer escape latency and search path than the control group and the sham-operation group ( P <0.05). Compared with the model group, the electroacupuncture treatment group had significantly shorter escape latency and search path ( P <0.05). In the spatial exploration test, there was no significant difference in the number of platform crossings between the control group and the sham-operation group ( P <0.05). The model group had a significantly lower number of platform crossings than the control group and the sham-operation group ( P <0.01, P <0.05). The electroacupuncture treatment group had a significantly higher number of platform crossings than the model group ( P <0.05). Compared with the control group and the sham-operation group, the model group had significant increases in the protein expression of P35/P25 and CDK5 ( P <0.001), and the electroacupuncture treatment group had significant reductions compared with the model group ( P <0.001). There was no significant difference in the protein expression of Tau5 between groups ( P >0.05). The model group had significantly higher protein expression of phosphorylated Tau(Ser199, Ser202) in the hippocampus than the control group and the sham-operation group ( P <0.01, P <0.05). The electroacupuncture treatment group had significantly lower protein expression of phosphorylated Tau(Ser199，Ser202) than the model group ( P <0.05, P <0.01)., Conclusion: Electroacupuncture may delay the progression of AD by affecting the expression of proteins involved in the P35/P25-CDK5-Tau pathway in the hippocampus of rats.

Research on new supramolecular synthons facilitates the progress of materials design. Herein, the ability of sp 2 carbonyl oxygen atoms to act as halogen-bond acceptors was established through cocrystallization. Four sets of carbonyl compounds, including aldehydes, ketones, esters, and amides, were selected as halogen-bond acceptors. In the absence of strong hydrogen bonds, 14 out of 16 combinations of halogen-bond donors and acceptors could form cocrystals, whereby the supramolecular synthon C=O⋅⋅⋅X acts as the main interaction. Further, the geometric parameters of the C=O⋅⋅⋅X interaction were statistically revealed on the basis of the crystallographic database. The bifurcated interaction mode that has been observed in other halogen-bond synthons rarely occurs in the case of C=O⋅⋅⋅X. The robustness of C=O⋅⋅⋅X makes its application in crystal engineering possible and opens up new opportunities in designing multicomponent fluorescent materials, as indicated by multicolor emission of cocrystals D through C=O⋅⋅⋅X interactions., (© 2019 Wiley-VCH Verlag GmbH & Co. A novel diterpenoid, sarcomililate A (1), possessing a previously undescribed tricyclo[11.3.0.0 2,16 ]hexadecane scaffold, along with two new cembranoids, sarcomililatols A and B (2 and 3), and two known related diterpenoids (4 and 5), was isolated from the Hainan soft coral Sarcophyton mililatensis. The complete chemical structure including absolute configuration (AC) of 1 was unambiguously determined by a combination of residual dipolar coupling (RDC)-based NMR analysis, TDDFT-ECD (ECD = electronic circular dichroism) calculation, and Snatzke's method. The AC of 1 was further confirmed by a comparison with 2, whose AC was determined by anomalous X-ray diffraction. A plausible biogenetic relationship of 1-3 was further proposed. All the reported compounds exhibited interesting inhibitory effects on the ConA-induced T lymphocytes and/or lipopolysaccharide (LPS)-induced B lymphocytes proliferation.

Vitamin B 9 gels were found to be pH-switchable by adding triethylamine or acetic acid. Such recyclable gels can be used as novel co-crystallization media, resulting in four stoichiometric vitamin C co-crystals. Under highly supersaturated conditions, uniform microspheres of (VC)·(NA) with different particle sizes were obtained through a spherical crystallization process.

A series of chiral oxazino-indoles have been synthesized via a key intermolecular oxa-Pictet-Spengler reaction. These compounds exhibited significant and selective neuroprotective effects against Aβ25-35-induced neuronal damage. This is the first report of evaluating the influence of chiral diversity of oxazino-indoles on their neuroprotective activities, with the structure-activity relationship been analyzed. Aim: To investigate the changes of retinal function in non-pathological myopic subjects using multifocal electroretinography (mfERG) and to correlate the data with the central macular thickness obtained using optical coherence tomography (OCT)., Methods: One hundred and thirteen subjects (113 eyes) with age range from 18 to 35y were enrolled in the study. The subjects were divided into four groups according to spherical equivalent (SE) and axial length (AL): emmetropia group (EG, n=31; SE: +0.75 to -0.50 D; AL: 22 to 24 mm), low and medium myopia group (LMMG, n=26; SE: >-0.50 to -6.00 D; AL: >24 to 26 mm), high myopia group (HMG, n=34; SE: >-6.00 to -10.00 D; AL: >26 to 28 mm) and super high myopia group (SHMG, n=22; SE: >-10.00 D; AL:>28 mm). The P1 amplitude density, P1 amplitude, and P1 implicit time of the first-order kernel mfERG responses were obtained and grouped into five rings. The central subfield macular thickness (CST) was obtained using macular cube 512×218 scan of Cirrus HD-OCT., Results: With the increasing of eccentricity, the first positive peak (P1) amplitude density (P=0.0000, 0.0001, 0.0021 in ring 1-3 respectively) and P1 amplitude (all P=0.0000 in ring 1-5) of each group decreased. With the increasing of myopia, P1 implicit time gradually extended (all P=0.0000 in ring 1-3). The average CST in four diagnostic groups was 241.56±12.72 µm, 244.56±12.19 µm, 254.33±11.61 µm, 261.75±11.83 µm respectively. With the increasing of myopia, CST increased (P<0.001). There was negative relationship between CST and P1 amplitude, P1 amplitude density (r=-0.402, P<0.001; r=-0.261, P=0.003). There was positive relationship between CST and P1 implicit time (r=0.34, P<0.001)., Conclusion: With the increasing of myopia, P1 amplitude density and P1 amplitude of the first-order reaction gradually reduced. This showed potential decline in retinal function in myopia. To some extent it may reflect the functional disorder or depression of the visual cells. The exact mechanism needs further study.

Objective: To study the correlations of IL-4R gene polymorphism and serum IgE levels with asthma predictive index (API) in children., Methods: One hundred and sixty-seven children with positive API, 187 children with negative API and 203 healthy children (control group) were enrolled. PCR and DNA sequencing were used to identify genotypes of the Arg551Gln locus in IL-4R gene. Serum IgE levels were measured using ELISA., Results: There was no significant difference in the genotype frequencies of the Arg551Gln locus in IL-4R gene among the positive API, negative API and control groups. Serum IgE levels in the positive API group were significantly higher than in the negative API and control groups (P<0.01). In the positive API group, the children aged less than 2 years had significantly lower serum IgE levels than those aged over 2 years (P<0.01)., Conclusions: There is no correlation between the Arg551Gln polymorphism in IL-4R gene and API results. API positivity is correlated with elevated serum IgE levels. An older age (>2 years) may be a risk factor for increased serum IgE levels in children with positive API.

Single-crystal structures of fingolimod hydrochloride (FTY 720), a block-buster multiple sclerosis drug, were revealed for the first time in this study. Three different conformational polymorphs (designated as forms I-III) were characterized using a variety of analytical techniques, including single-crystal XRD, VT-PXRD, DSC, HSM, and VT-confocal Raman spectroscopy. A temperature-dependent solid-solid transformation between different conformational polymorphs was observed to be reversible, and the transition process was studied using both VT-PXRD and VT-Raman techniques. Structural analysis revealed that the FTY 720 molecules adopt distinctive conformations in different polymorphs. Creatine phosphate sodium (CPS) salt is a first-line cardiovascular drug for severe diastolic heart failure. The drug exists in different hydrate forms. The marketed drug form was determined as CPS·4.5H2 O (H1); however, the reference standard was supplied as CPS·6H2 O (H2). In this work, we present two newly identified hydrate forms: a thermodynamically stable low hydrate form, CPS·1.5H2 O (H3), and a pressure-sensitive transit form, CPS·7H2 O (H4). The hydrate forms were discovered through a comprehensive solid-state screening experiment and fully characterized using a range of analytical techniques including X-ray powder diffraction (XRPD), FTIR, Raman spectroscopy, hot-stage microscopy (HSM), thermogravimetric analysis, and differential scanning calorimetry. Stability tests revealed that H3 was the most stable hydrate under thermal stimulation. H4 is a pressure-sensitive hydrate and easily transforms to H2 and then H1 upon grinding. The form transformation process was closely monitored using the HSM, variable-temperature XRPD (VT-XRPD), and VT-Raman spectroscopy techniques. Specifically, the transformation of H4 to H1 is characterized in a single-crystal-to-single-crystal transformation process. Aim: To investigate the changes of retinal thickness in macula of high myopic eyes using spectral domain optical coherence tomography (OCT)., Methods: Middle-aged and young myopic patients were divided into three groups according to their refractive error/axial length: low and medium myopia group (LMMG), high myopia group (HMG) and super high myopia group (SHMG). Cirrus HD-OCT was used to evaluate total average macular thickness, central subfield thickness, inner/outer macular thickness and macular volume. The differences among experimental groups were analyzed by one-factor analysis of variance. Associations between macular thickness and refractive error/axial length were analyzed by Pearson correlation analysis., Results: There was no significant difference in age among the three groups (P=0.2789). The mean refraction error in the LMMG, HMG, and SHMG groups was -2.49±1.38D, -8.53±1.95D and -13.88±1.76D, respectively (P<0.001). The central subfield thickness of three groups was 244.56±12.19µm, 254.33±11.61µm and 261.75±11.83µm, respectively, and there were statistically significance between random two groups. The total average macular thickness, inner/outer macular thickness, and macular volume decreased with increased myopia/axial length. Average foveal thickness had negative correlations with refractive error (P<0.001), and positive correlations with axial length. The inferior and temporal inner macular thickness, all the quadrants of outer ring, total average macular thickness and macular volume featured positive correlations with refractive error, and negative correlations with axial length. Average foveal thickness, superior and temporal inner macular thicknesses, and temporal outer macular thickness was lower in females compared to males., Conclusion: With an increase in myopia degree/axial length, the average foveal thickness increased and the inner/outer macular thickness decreased. Females featured thicker average foveal thickness, and thinner macular thickness compared to males.

Squalene synthase (SQS) catalyzes the condensation of two molecules of farnesyl diphosphate to give presqualene diphosphate and the subsequent rearrangement to form squalene. The gene encoding squalene synthase was cloned from Poria cocos by degenerate PCR and inverse PCR. The open reading frame of the gene is 1,497 bp, which encodes 499 amino acid residues. A phylogenetic analysis revealed that P. cocos SQS belonged to the fungus group, and was more closely related to the SQS of Ganoderma lucidum than other fungi. The treatment of P. cocos with methyl jasmonate (MeJA) significantly enhanced the transcriptional level of P. cocos sqs gene and the content of squalene in P. cocos. The transcriptional level of sqs gene was approximately fourfold higher than the control sample and the squalene content reached 128.62 μg/g, when the concentration of MeJA was 300 μM after 72 h induction.

A four-membered square-shaped H-bonding structure was successfully designed to synthesize vitamin D3 co-crystals. The co-crystallization process was found to be conformationally selective to result in topochemically stable materials.

Objective: To study the liposoluble constituents from the flowers of Calotropis gigantea and their antimicrobial activities., Methods: The liposoluble constituents from the flowers of Calotropis gigantea were analyzed by GC-MS and their relative contents were determined by area normalization. Their antimicrobial activities against 2 pathogens were tested by agar disc diffusion method., Results: 31 compounds were separated and identified, amounting to 85.14% of the total constituents. The liposoluble constituents from the flowers of Calotropis gigantea exhibited weak antimicrobial activities against Staphylococcus aureus and Canidia albicans., Conclusion: The research result provides scientific foundation for exploitation and utilization of Calotropis gigantea.

Objective: To study the chemical constituents in the fruit of Harrisonia perforata., Methods: The chemical constituents were isolated and purified by means of chromatographic techniques and their structures were elucidated by analysis of spectroscopic data and physicochemical properties., Results: Eight compounds were isolated and identified as caryolane-1,9beta-diol (1), gallic acid (2), 4-O-ethylgallic acid (3), syringic acid ethyl ester (4), protocatechuic acid (5), stigmasterol (6), beta-daucoster (7), beta-sitosterol (8)., Conclusion: Compounds 1, 3 - 5 and 7 are isolated from this plant for the first time. Compounds 1, 3 and 4 are isolated from this genus for the first time.

Objective: To investigate the association between pulmonary vascular remodeling and expression of hypoxia-inducible factor-1α (HIF-1α), endothelin-1 (ET-1) and inducible nitric oxide synthase (iNOS) in pulmonary vessels in neonatal rats with hypoxic pulmonary hypertension (HPH)., Methods: A neonatal rat model of HPH was established as an HPH group, and normal neonatal rats were enrolled as a control group. The mean pulmonary arterial pressure (mPAP) was measured. The percentage of medial thickness to outer diameter of the small pulmonary arteries (MT%) and the percentage of medial cross-section area to total cross-section area of the pulmonary small arteries (MA%) were measured as the indicators for pulmonary vascular remodeling. The immunohistochemical reaction intensities for HIF-1α, ET-1 and iNOS and their mRNA expression in lung tissues of neonatal rats were measured. Correlation analysis was performed to determine the relationship between pulmonary vascular remodeling and mRNA expression of HIF-1α, ET-1 and iNOS., Results: The mPAP of the HPH group kept increasing on days 3, 5, 7, 10, 14, and 21 of hypoxia, with a significant difference compared with the control group (P<0.05). The HPH group had significantly higher MT% and MA% than the control group from day 7 of hypoxia (P<0.05). HIF-1α protein expression increased significantly on days 3, 5, 7 and 10 days of hypoxia, and HIF-1α mRNA expression increased significantly on days 3, 5 and 7 days of hypoxia in the HPH group compared with the control group (P<0.05). ET-1 protein expression increased significantly on days 3, 5 and 7 days of hypoxia and ET-1 mRNA expression increased significantly on day 3 of hypoxia in the HPH group compared with the control group (P<0.05). Both iNOS protein and mRNA expression were significantly higher on days 3, 5 and 7 days of hypoxia than the control group (P<0.05). Both MT% and MA% were positively correlated with HIF-1α mRNA expression (r=0.835 and 0.850 respectively; P<0.05)., Conclusions: Pulmonary vascular remodeling is developed on day 7 of hypoxia in neonatal rats. HIF-1α, ET-1 and iNOS are all involved in the occurrence and development of HPH in neonatal rats.

In order to not affect the image quality of interference fringes on the basis of the structure by increasing the structure angle of Wollaston prism to improve spectrum resolution, the authors optimized the structure of Wollaston prism. Calculating the function of the splitting angle and the structure angle, analysis indicated that taking the isosceles triangle prism with the same nature of the second wedge-shaped prism after the Wollaston prism, which makes the o and e light parallel to the optical axis, and alpha=0 degrees, the imaging interference fringes are no longer affected by changes in the splitting angle. Several optimized Wollaston prisms were made as an array to improve the spectral resolution. Experiments used traditional and optimized Wollaston prism array to detect the spectrum of the 980 nm laser. Experimental data showed that using optimized Wollaston prism array gets a clearer contrast of interference fringes, and the spectral data with Fourier transform are more accurate with DSP.

Objective: Massive hemorrhage is life-threatening during armed conflicts. Tourniquets are important medical devices used to reduce severe bleeding in trauma. The aim of this study was to empirically evaluate the current tourniquets used in China and provide information to emergency nurses in selecting the appropriate tourniquet., Methods: Five tourniquets were self-applied by 20 healthy participants. The blood flow distal to the tourniquet site was assessed using vascular Doppler ultrasound. Application time, pain, numbness, and other parameters were evaluated., Results: The bladder tourniquet and windlass tourniquet effectively occluded arterial blood flow with success rates higher than 75% in both the upper and lower extremities. The Cargo-strap was the fastest to apply, taking (7.22+/-2.30) s for the upper extremity and (6.48+/-2.40) s for the lower extremity. The rubber tube was the most painful, and the improvised tourniquet was the least efficient. The success rates were higher in the lower extremity than in the upper extremity (P less than 0.05, X(2) equal to 5.714)., Conclusions: The bladder tourniquet and the windlass tourniquet are efficient tourniquets, although the windlass is superior with respect to portability and pain. The Cargo-strap and rubber tourniquets have several disadvantages that reduce their suitability for field use. The improvised tourniquet is not recommended because of low efficiency and severe pain during implementation.

Five new pregnane steroids, toonasterones A (1), B (2), (Z)-aglawone (3), (Z)-toonasterone C (4), and (E)-toonasterone C (5), were isolated from the stem bark of Toona ciliata var. pubescens. Their structures were elucidated by means of detailed spectroscopic (IR, MS, and 2D NMR) analysis, and the stereochemistry of 1 was secured by X-ray diffraction analysis. Objective: To explore if the new treatment with Lund concept could reduce the mortality of patients after severe brain injury., Methods: This study included 68 severe brain injury patients in whom Gloasgow coma score (GCS) was 3-8, and in 30 of them Lund concept was adopted, and the other 38 patients were taken care of by the conventional treatment in controlling intracranial pressure (ICP). Furthermore, in patients of the Lund group and control group ICP and cerebral perfusion pressure (CPP) were monitored continuously for 5 days., Results: The amount of mannitol (g) used was markedly smaller in Lund group than that in the control group (139.6±25.0 vs. 587.5±31.8, P<0.01). The 28-day mortality of Lund group was significantly lower than that in control group (30.0% vs. 57.9%, P<0.05). In Lund group, the incidence of ICP exceeding 25 mm Hg (1 mm Hg=0.133 kPa) or 35 mm Hg or lowering of CPP by 50 mm Hg observed in the non-survivors (n=9) was greater than that observed in the survivors [n=21, (45.0±23.2)% vs. (7.2±3.6)%, (40.2±18.6)% vs. (2.2±1.6)%, (35.9±12.9)% vs. (3.1±2.4)%, all P<0.05]., Conclusion: By adopting the Lund concept, it is possible to reduce postoperative mortality after severe head injury.

Objective: To extract and analyze the liposoluble chemical constituents of the petroleum ether extraction and alcohol extraction from fruits of Harrisonia perforata., Methods: To analyze the liposoluble components by GC-MS., Results: 29 compounds were identified from petroleum ether extraction, amounting to 69.67% of the total detected constituents and the ir relative contents were confirmed. From the alcohol extraction, 12 compounds were identified, amounting to 71.00% of the total detected constituents and the ir relative contents were also confirmed., Conclusion: Linoleic acid, Palmitic acid, beta-Stitosterol are common liposoluble components in two extractions.

Objective: To study the chemical constituents from the root of Millettia speciosa., Methods: To isolate and purify by silica gel, macroporous resin D-101 and Sephadex LH-20., Results: Five compounds were isolated from 95% EtOH extract of Millettia Speciosa and their structures were elucidated by physico-chemical properties and spectroscopic analysis as Isoliquiritigenin (I), Maackiain (II), Pterocarpin (III), Medicarpin (IV) and Homopterocarpin (V)., Conclusion: Compounds I, III are obtained from this plant for the first time, compounds V is isolated from the genus Millettia for the first time.

Objective: To investigate the effects of different temperatures of fluid infusion on diamine oxidase(DAO), MB isoenzyme of creatine kinase (CK-MB), creatine kinase (CK) and lactate dehydrogenase (LDH) in rabbit with hemorrhagic shock so as to provide a clue to improve the nursing care in patients with hemorrhagic shock., Methods: An animal model of controlled hemorrhagic shock in rabbits as previously reported was used in this randomized experiment. Twenty-three rabbits were randomized to a warm fluid infusion group (n=8), a ambient-temperature fluid infusion group (n=7), and a cold fluid infusion group (n=8). The rabbits were then resuscitated with Ringer's lactate solution three times of that of blood loss. DAO and values of myocardial enzyme were measured before shock, during shock and 1 hour, 2 hours and 4 hours after fluid infusion, respectively., Results: There was no significant difference in DAO and values of myocardial enzymes among groups, though there was a lower value in DAO and values of myocardial enzymes at 1 hour to 4 hours after warm fluid infusion., Conclusion: Our data indicate that warm fluid infusion in rabbit model of controlled hemorrhagic shock has some beneficial effect on some biochemical values. In view of this result, temperature of fluid infusion in nursing care for patients with hemorrhagic shock should pay more attention to.
