Objectives: To understand the influence of medical insurance policy reforms in Guangxi on the hospitalization expenses of breast cancer patients by analyzing the composition and changing trend in breast cancer diagnosis and treatment expenses in the Guangxi Medical University Cancer Hospital, and to provide the evidence for the improvement of medical insurance policy reform., Methods: A total of 3 950 breast cancer patients were collected from 2014 to 2017 and analyzed. Kruskal-Wallis test and multiple linear regression model were used to discuss the breast cancer related epidemiology and analyze the composition of hospitalization expenses and its influential factors., Results: The median hospitalization cost of breast cancer patients in our hospital from 2014 to 2017 was 29 266.94 Chinese Yuan. Single factor analysis showed that the impact of year, hospitalization days, age, payment method, tumor stage, and treatment method on hospitalization cost was significant (all P <0.01). Multiple linear regression analysis showed that the power of influential factors of hospitalization costs arranged descending from 2014 to 2017 as follows: hospitalization days, treatment methods, payment method, tumor staging, and age., Conclusions: Reasonably controlling hospitalization days and actively promoting the integration of urban and rural medical insurance can effectively reduce the economic burden for breast cancer patients.

Based on the statistical study of meteorological balloons equipped with thermosondes, a new model that estimates the profile of the refractive index structure constant ( C n 2) is proposed. Utilizing temperature, pressure, and wind shear as inputs, this new approach can estimate vertical profiles of C n 2 with 100 m vertical resolution. We used four outer scale models (Thorpe, HMNSP99, Dewan, and our proposed model) on data acquired from Rongcheng (Shandong Peninsula) and Maoming (Guangdong Province) to estimate the C n 2 profiles and compared the results with the measured C n 2 profile. The proposed method outperformed the other three models, yielding an estimation profile that matched well with the measured median C n 2 profiles, with an average relative error generally less than 3.5% and a mean correlation coefficient larger than 0.72 in Maoming, an average relative error generally less than 3.4% and a mean correlation coefficient larger than 0.84 in Rongcheng. The proposed outer scale model also shows good performance in estimating integrated atmospheric parameters.

Spectral bandwidth correction is an effective way to obtain the original spectrum. However, the correct selection of optimal parameters used to recover the distortion spectrum in bandwidth correction algorithms has always been an important problem. To overcome the shortcomings of traditional parameter selection methods and obtain the optimal parameter, we propose a new optimal parameter selection method based on deep learning (DL). First, the database and neural network were constructed, and then the optimal parameters of corresponding algorithms were obtained through the training of the neural network. In order to verify the superiority of the optimal parameter selection method based on DL, the Levenberg-Marquardt (L-M) and Richardson-Lucy (R-L) algorithms with corresponding optimal parameters were compared with the traditional L-M and R-L algorithms to recover the distortion white light-emitting diode, Raman spectrum, and compact fluorescent lamp spectrum. The type A uncertainty and root mean square error values of the different cases were calculated. The results proved that, compared with the traditional methods for obtaining the optimal parameters, the neural network was capable of obtaining parameters that can make the bandwidth correction algorithm more efficient at recovering the distorted spectrum.

The directional polarimetric camera (DPC) is a remote-sensing instrument for the characterization of atmospheric aerosols and clouds by simultaneously conducting spectral, angular, and polarimetric measurements. Polarization measurement accuracy is an important index to evaluate the performance of the DPC and mainly related to the calibration accuracy of instrumental parameters. In this paper, firstly, the relationship between the polarization measurement accuracy of DPC and the parameter calibration errors caused by the nonideality of the components of DPC are analyzed, and the maximum polarization measurement error of DPC in the central field of view and edge field of view after initial calibration is evaluated respectively. Secondly, on the basis of the radiometric calibration of the DPC onboard the GaoFen-5 satellite in an early companion paper [Opt. Express2813187 (2020)10.1364/OE.391078], a series of simple and practical methods are proposed to improve the calibration accuracy of the parameters-the diattenuation of the optics, absolute azimuth angle, and relative transmission corresponding to each pixel, thereby improving the polarization measurement accuracy of DPC. The calibration results show that, compared with the original methods, the accuracy of the diattenuation of the optics, relative azimuth angle, and relative transmission of three polarized channels obtained with the improved methods are improved from ±1%, 0.1 degree and ±2% to ±0.4%, 0.05 degree and ±0.2%, respectively. Finally, two verification experiments based on a non-polarized radiation source and a polarizing system were carried out in the laboratory respectively to verify the improvement of the parameters modified by the proposed methods on the polarization measurement accuracy of the DPC to be boarding the GaoFen-5 (02) satellite. The experimental results show that when the corrected parameters were employed, the average error in measuring the degree of linear polarization of non-polarized light source for all pixels in the three polarized bands and the maximum deviation of the degree of linear polarization between the values set by the polarizing system and the values measured by the DPC at several different field of view angles for each polarized spectral band are obviously reduced. Both the mean absolute errors and the root mean square errors of the degree of linear polarization obtained with the corrected parameters are much lower than those obtained with the original parameters. All of these prove the effectiveness of the proposed methods.

Objectives: To compare the ability of resin-modified glass ionomer (RMGI) and bioactive cements to prevent root dentin demineralization., Materials and Methods: Fifty molars were prepared at the cementum-enamel junction (n = 10) and restored with three bioactive cements (Activa Bioactive Cement, ACT; Ceramir Crown and Bridge, CER; and Theracem, THE), a self-adhesive resin cement (Rely X Unicem 2, UNI), and a RMGI cement (Rely X Luting Plus, LUT). Specimens were cycled for 30 days between a demineralization solution (pH = 4) composed of 0.1 M lactic acid and 3 mM Ca 3 (PO 4 ) 2 for 4 hours and a remineralization solution (pH = 7.0) composed of 1.5 mM Ca, 0.9 mM P, and 20 mM Tris(hydroxymethyl)-aminomethane for 20 hours. Specimens were sectioned to 100 μm and evaluated with polarized light microscopy. A line was drawn parallel with the zone of demineralization for each tooth. The areas of "inhibition" (external to the line) were measured as positive values and "wall lesions" (pulpal to the line) were measured as negative areas., Results: Significant differences were found between materials for "inhibition/wall lesion" areas in root dentin (P < .001) and ranked as (μm 2 , mean ± SD): LUT (7700 ± 2500) > CER (3800 ± 1900), THE (2100 ± 2600), and ACT (1400 ± 700) > UNI (-2000 ± 1700)., Conclusions: Bioactive cements showed net areas of demineralization inhibition albeit at a lower level than a reference RMGI cement., Clinical Significance: RMGI or bioactive cements may be indicated for patients at risk of secondary caries around crown margins., (© 2020 Wiley Periodicals LLC.)

Background: Lead inhibits the enzymes in heme biosynthesis, mainly reducing δ-aminolevulinic acid dehydratase (ALAD) activity, which could be an available biomarker. The aim of this study was to detect the threshold of δ-aminolevulinic acid dehydratase activity reduced by lead exposure., Methods: We collected data on 121 lead workers and 117 non-exposed workers when annual health examinations were performed. ALAD activity was determined by the standardized method of the European Community. ALAD G177C (rs1800435) genotyping was conducted using the polymerase chain reaction and restricted fragment length polymorphism (PCR-RFLP) method. In order to find a threshold effect, we used generalized additive models (GAMs) and scatter plots with smoothing curves, in addition to multiple regression methods., Results: There were 229 ALAD1-1 homozygotes and 9 ALAD1-2 heterozygotes identified, and no ALAD2-2 homozygotes. Lead workers had significantly lower ALAD activity than non-exposed workers (41.6 ± 22.1 vs. 63.3 ± 14.0 U/L, p < 0.001). The results of multiple regressions showed that the blood lead level (BLL) was an important factor inversely associated with ALAD activity. The possible threshold of BLL affecting ALAD activity was around 5 μg/dL., Conclusions: ALAD activity was inhibited by blood lead at a possible threshold of 5 μg/dL, which suggests that ALAD activity could be used as an indicator for lead exposure regulation.

The directional polarimetric camera (DPC), developed by Anhui Institute of Optics and Fine Mechanics, Chinese Academy of Science, is a satellite sensor used to observe the polarization and directionality of the earth's reflectance. It acquires the two-dimensional image of the earth with a large field of view (118.74°) and a high spatial resolution (3.3 km) in 8 spectral bands. The first DPC was successfully launched onboard the GaoFen-5 satellite in May 2018, subject to the Chinese high-resolution earth observation program. In this paper, a set of systematic and complete pre-flight calibrations of the DPC are proposed to ensure the effective characterization for in-flight calibration, so as to ensure the accuracy of DPC measured radiation polarization data and the reliability of inversion results. Since the geometric calibration method of the DPC has been presented in an early companion paper [Appl. Opt. 59 226 (2020)], this paper will not introduce it in detail. Instead, the geometric calibration results of each spectral band together with a discussion on the origin of differences between spectral bands are analyzed, and the error analysis of the method is conducted. The results of the DPC geometric calibration is that the residuals of all spectral bands are less than 0.1 pixel. For radiometric calibration, the radiometric models of non-polarized bands and polarized bands are derived in detail, respectively, and the specific calibration methods with error analysis, equipment, and main results with their related accuracies for each parameter of the radiometric models are described. To verify the accuracy of calibration parameters, a series of polarization detection accuracy verification experiments based on a non-polarized radiation source, a polarizing system, and a natural scene were carried out. The experimental results show that the maximum deviation of degree of polarization between the set values of the polarizing system and measured values of the DPC at the corresponding positions of four field of view angles of 0, 15, 30, and 45 degrees of each polarized spectral band is 0.009, 0.004, and 0.003, respectively. The average error in measuring the degree of polarization of a non-polarized light source by all pixels in the three polarized bands is 0.0043, 0.0046, and 0.0037, respectively. And the relative deviations of each field of view are within 0.020 when the DPC and CE318N simultaneously measure the DoLP of sky. All of these prove the effectiveness of the pre-flight calibration.

The directional polarimetric camera is a polarization sensor that offers an ultrawide angle and low-distortion imaging. Stray light is one of the important factors affecting the accuracy of its polarization measurement. In this paper, the stray light of the directional polarimetric camera is first divided into local stray light and global stray light according to the characteristics of its optical system, and the causes and characteristics of the two kinds of stray light are analyzed. Second, a novel deconvolution method is proposed to correct the local stray light, and the matrix method is extended to a 2D form to correct the global stray light. Finally, image acquisition and stray light correction laboratory experiments of integrating a sphere light source were carried out. The experimental results show that the proposed correction methods can effectively suppress more than 94% of the stray light of the directional polarimetric camera.

Purpose: To determine if surface treatment and cement selection for traditional 3 mol% yttria partially stabilized zirconia (3Y-PSZ), "translucent" 5 mol% yttria-stabilized zirconia (5Y-Z), or lithium disilicate crowns affected their fracture load., Materials and Methods: Crowns with 0.8 mm uniform thickness (96, n = 8/group) were milled of 3Y-PSZ (Lava Plus), 5Y-Z (Lava Esthetic), or lithium disilicate (e.max CAD) and sintered/crystallized. Half the crowns were either particle-abraded with 30 µm alumina (zirconias) or etched with 5% hydrofluoric acid (lithium disilicate), and the other half received no surface treatment. Half the crowns from each group were luted with resin-modified glass ionomer (RMGI, RelyX Luting Plus) and half were luted with a resin cement (RelyX Unicem 2) to resin composite dies. Crowns were load cycled (100,000 cycles, 100 N force, 24°C water) and then loaded with a steel indenter until failure. A three-way ANOVA examined the effects of material, cement, and surface treatment on fracture load. Post-hoc comparisons were performed with the Tukey-Krammer method., Results: Fracture load was signficiantly different for materials and cements (p < 0.0001) but not surface treatments (p = 0.77). All lithium disilicate crowns luted with RMGI failed in fatigue loading cycling; 3Y-PSZ and 5Y-Z crowns luted with resin showed a higher fracture load compared with RMGI (p < 0.001). With resin cement, there was no signficant difference in fracture load between 5Y-Z and lithium disiliciate (p = 1) whereas 3Y-PSZ had a higher fracture load (p < 0.0001)., Conclusions: Cement type affected fracture load of crowns but surface treatment did not. The 0.8 mm uniform thick crowns tested benefited from using resin cement regardless of type of ceramic material. Spectroscopic data often suffers from spectral distortions due to the broadening effects of the spectrometer. In this paper, the issue of bandwidth correction is transformed into a multiparameter optimization problem, and an improved bandwidth correction method based on the Levenberg-Marquardt algorithm with improved Tikhonov regularization is presented. Example LED, Raman, and compact fluorescent lamp spectra were corrected with the proposed method, the Richardson-Lucy method, and the maximum a posterior method, and the correction errors and runtimes under the same stopping conditions of each method were compared. The experimental results show that the proposed method can be applied to different spectra with excellent correction.

Purpose: Insomnia has become one of the foremost health concerns among workers. Despite a significant number of epidemiological studies have reported on the correlation between insomnia and job stress, comprehensive evidence remains insufficient. Therefore, this research seeks to provide evidence with greater reliability, through summarizing relevant contemporary literature via a meta-analysis., Methods: Literature from across Europe and Asia that was of both a prospective and cross-sectional design was included, if well-controlled odds ratios were available. The meta-analysis was undertaken in accordance with the guidelines devised by PRISMA, including tests for publication bias and heterogeneity., Results: High job stress was associated with a greater risk of suffering from insomnia (random OR = 1.73, 95% CI 1.46-2.05), and the correlation between effort-reward imbalance and insomnia was statistically significant (random OR = 2.63, 95% CI 1.22-5.69). Higher demand was correlated to a relatively greater risk of insomnia (random OR = 1.35, 95% CI 1.20-1.51), while the pooled effect of low control was not found to be statistically significant. The summary random odds ratio of heavy workload was 2.76, and a pooled odds ratio of 1.67 (fixed, 95% CI 1.11-2.52) was calculated in low social support. With regard to the overall population, work-family conflict was correlated with insomnia (random OR = 2.32, 95% CI 1.53-3.51). The subgroup analysis provided comparable outcomes, for both males (fixed OR = 1.97, 95% CI 1.50-2.57) and females (random OR = 2.80, 95% CI 1.30-6.05). Egger's regression indicated that publication bias may be apparent in the syntheses of effort-reward imbalance, low social support, and work-family conflict (p < 0.05). Heterogeneity was caused by design, measuring the exposure or outcome, in addition to the region where the research was conducted., Conclusions: The correlation between insomnia and higher levels of job stress, effort-reward imbalance, high demand, heavy workload, and low social support was determined. Publication bias and heterogeneity were partially observed. Furthermore, future studies with improved methodologies and a focus on mechanisms are anticipated.

Objective: To analyze the advantages of spatial measurement of anatomical parameters in a 3D model in surgical planning for laparoscopic partial nephrectomy (LPN)., Methods: From February, 2016 to October, 2017, 37 patients diagnosed with T1 renal mass underwent LPN based on 3D reconstruction after enhanced CT scanning using the Uromedix-3D system (group A), and another 38 patients received LPN with conventional CT planning (group B). The anatomical parameters were measured in the reconstructed 3D model and the demographic data, surgical outcome and postoperative data were compared between the two groups., Results: In group A, the average time for 3D model reconstruction was (29.3∓9.7) min; the length, width and depth of the renal defect in 3D model were 3.2∓1.1 cm, 2.6∓0.9 cm and 1.7∓0.7 cm, respectively; The distance of the tumor from the collecting system was 3.8∓2.2 mm; The mean R.E.N.A.L score of the patients was 7∓1.5, and 3 patients had accessory renal artery and 2 had early branching of the renal artery. LPNs were completed via the retroperitoneal approach in all the 75 patients without conversion to open or total nephrectomy. Group A and group B showed significant differences in warm ischemic time (26.7∓6.4 vs 31.9∓7.0 min), tumor-excision time (8.4∓2.6 vs 10.4∓2.8 min), renal defect suture time (18.3∓3.9 vs 21.5∓3.4 min), 24-h volume of retroperitoneal drainage (88.6∓40.2 vs 134.3∓58.3 mL) and 48-h volume of retroperitoneal drainage (127.9∓54.5 vs 198.1∓86.3 mL), but not in the demographic data, operation time, intraoperative blood loss or postoperative hospital stay., Conclusions: 3D reconstruction of the renal masses can be completed efficiently and accurately using this system. Compared with conventional CT-based measurement, 3D spatial measurement of the anatomical structures helps to increase the precision in the performance of LPN and reduce the warm ischemia time.

Purpose: Work-related stress factors are commonly considered to be increasing the risk of insomnia. But the association between circadian-rhythm gene period2 (per2) and insomnia remains unknown. The paper aimed to examine the relationships among the polymorphism in rs7602358 of Per2 gene, work stress, and insomnia., Methods: In the cross-sectional study of a total of 364 Chinese workers recruited, 218 had insomnia tested by the Athens Insomnia Scale (AIS-5). Work stress was assessed using the General Job Stress Questionnaire (GJSQ). Per2 genotypes were determined by SNaPshot technology. Unconditional logistic regression analysis was used to examine the interaction between polymorphism of Per2 and work stress in insomnia., Results: The genotype AC or allele C of Per2 significantly increased the risk of insomnia (OR, 5.41; 95 % CI, 2.23-13.1) (OR, 3.29; 95 % CI, 1.55-6.98). Compared to low work stress, high work stress had a higher risk of insomnia (OR, 2.73; 95 % CI, 1.77-4.22). A significant interaction between Per2 and work stress in the risk of insomnia was found. Compared to workers with low work stress and genotype AA, those with high work stress and genotype AC had a higher risk of insomnia (OR, 15.12; 95 % CI, 3.46-66.09)., Conclusions: The effect of genotype AC or allele C of Per2 on insomnia was relatively stronger than that of high work stress, suggesting that individual's susceptibility should be taken into consideration when intervening and controlling insomnia of workers.

Introduction: The aim of this study was to estimate the prevalence and risk factors of hypothermia under general anesthesia in a large domestic hospital., Method: All of the consecutive 1840 patients who underwent scheduled surgery between August and December 2013 were admitted to the study. The nasopharyngeal temperature was measured, and the following variables were also recorded: sex, age, type of surgery, duration of anesthesia, active warming devices and type of operating room. Univariate and multiple regression binary logistic analyses with odds ratios (ORs) and 95% confidence intervals (95% CIs) were used to assess the relationship between each clinical risk factor and hypothermia., Results: The prevalence of hypothermia under general anesthesia was 25.7%. In the univariate analysis, the risk factors of hypothermia were age, the duration of anesthesia, the type of operating room and the type of surgery. Sex was not included. In the multiple logistic regression analysis, the significant risk factors of hypothermia were advanced age, laminar airflow operating rooms and general surgeries., Conclusion: Intraoperative hypothermia is still common and should therefore receive serious attention. The substance di-2-ethylhexyl phthalate (DEHP) is widely used as a plasticizer in chemical protective gloves to improve their flexibility and workability. However, it is possible that workers using protective gloves to handle various solvents may be exposed to DEHP leached by the solvents. Using an ASTM F739 permeation cell, it was found that BTEX solvents permeating through the glove samples dissolved DEHP from the gloves. Even without continuously contacting the permeant, DEHP was released from the contaminated glove samples during the desorption experiments. The DEHP leaching amounts were found to be inversely correlated to the permeability coefficients of BTEX in the glove samples. This result implied that the larger the amount of DEHP released from the glove samples, the higher the permeation resistance of gloves. Although chemical protective gloves provide adequate skin exposure protection to workers, the dermal exposure model developed herein indicates that leaching of DEHP from the glove samples may pose a potential health risk to the workers who handle BTEX. This study suggests that the selection of protective gloves should not only be concerned with the chemical resistance of the gloves but also the health risk associated with leaching of chemicals, such as DEHP, used in the manufacturing of the gloves., (Copyright © 2014 Elsevier B.V. Objective: Insomnia, a widely occurring sleep disorder in modern society, has a large impact on life quality and work safety. A cross-sectional study was conducted to explore the possible link between serotonin transporter-linked polymorphic region (5-HTTLPR), job-related stress, and insomnia in West China., Methods: Of the total 462 workers recruited, 177 had insomnia according to the Athens Insomnia Scale (AIS-5). The 5-HTTLPR genotypes were determined by polymerase chain reaction. Job-related stress was assessed for each participant by the General Job Stress Questionnaire., Results: Unconditional logistic regression models showed that the 5-HTTLPR genotype was significantly associated with insomnia, and >80% increased risk per S allele was observed. High job-related stress had a higher risk for insomnia than low job-related stress (odds ratio [OR], 6.14; 95% confidence interval [CI], 3.94-9.59). Crossover analysis found significant job-related stress × 5-HTTLPR interaction. Compared to individuals with both low job-related stress and SL/LL genotype, those with both higher job-related stress and SS genotype had a higher risk of insomnia (OR, 5.16; 95% CI, 3.13-8.54), whereas those with both low job-related stress and SS genotype showed a lower risk of insomnia (OR, 0.26; 95% CI, 0.08-0.74). The interaction remained statistically significant after adjusting for potential confounding factors., Conclusions: The findings indicated that 5-HTTLPR could modify the effect of job-related stress on employees' insomnia, suggesting that a work environment-based personalized intervention may be applied to prevent employees' insomnia by alleviating job-related stress in the workplace., (Copyright © 2014. In an attempt to obtain bacterial cellulose (BC) with improved rehydration ability, Tween 80, urea, fluorescent brightener, hydroxypropylmethyl cellulose (HPMC) and carboxymethyl cellulose (CMC) were introduced into BC fermentation medium. Measurements of the mechanical strength of the resulting BCs (TBC, UBC, FBC, HBC and CBC) showed a decline except for UBC. SEM images showed that, although the cellulose bundle widths of FBC, HBC and CBC increase, the cellulose network void in FBC grew, while those in HBC and CBC shrank. X-ray diffraction and FT-IR analysis demonstrated that the addition of HPMC and CMC reduced the degree of crystallinity in their corresponding MBCs from 70.54% to 52.23% and 45.38%, respectively. HBC and CBC also exhibited the highest rehydration ability among all MBCs as well as the lowest crystallinity. The in situ modification with HPMC and CMC during fermentation can effectively improve rehydration ability of BC by altering its network structure., ((c) 2010 Elsevier Ltd. All rights reserved.)

Objective: To investigate the features of intraductal papilloma of the breast in mammography and magnetic resonance imaging (MRI) and assess the diagnostic values of the two imaging modalities., Methods: Fifteen patients with intraductal papilloma of the breast confirmed surgically and pathologically underwent X-ray examination of the breast, and 11 of them also received enhanced MRI. The imaging findings by mammography and MRI were compared., Results: Enhanced MRI clearly displayed the location and morphology of the intraductal papilloma, and 7 patients showed smooth tumor margins and 2 showed irregular margins. On T(1)WI, the lesions were isointense or slightly hypointense, and appeared isointense or slightly hyperintense on T(2)WI. Some of the intraductal papillomas were seen encapsulated in the dilated ductal. The varying enhancement features of the lesions increased the difficulty in distinguishing from carcinoma. Mammography identified intraductal papillomas only in 2 of the 15 cases (13%) with lesion feature similar to that found by MRI. Fine cluster calcification was found in 1 case., Conclusion: MRI can more accurately define the location of the lesion than X-ray. In spite of some resemblance in the MRI findings between intraductal papillomas and breast carcinoma, MRI still serves as a useful diagnostic modality for intraductal papilloma that shows some characteristic findings.

Objective: To investigate the features of gynecomastia displayed by mammography., Methods: Twelve patients with gynecomastia were examined with a high-performance GITTO-TECH mammograph (IMS Company, Italy), and the results were compared with those obtained from pathological examination., Results: The 12 cases were pathologically confirmed as gynecomastia, 10 of which were also identified by mammography while 2 misdiagnosed as male breast cancer., Conclusion: Diagnosis of gynecomastia can be established when typical features are presented in mammography, and fine needle aspiration biopsy can be performed when possible for discrimination from male breast cancer.
