Objective: To identify the effect of the organizational culture on patient safety in the hospital context. Materials and methods: A systematic review, without meta-analysis, registered in PROSPERO with number CRD42020162981. Cross-sectional and observational studies were selected that assessed the safety environment and safety culture published between 2014 and 2020 in journals indexed in the EMBASE, Latin American and Caribbean Literature in Health Sciences (Literatura Latinoamericana e do Caribe em Ciências da Saúde, LILACS) via the Virtual Health Library (Biblioteca Virtual em Saúde, BVS), Medline (International Literature in Health Sciences) via PubMed, and Cumulative Index to Nursing and Allied Health Literature (CINAHL). Results: The findings show that a positive safety environment exerts a beneficial effect on the safety culture, favors the notification of events, and enables improvements in the quality of health care. Conclusions: The effective interaction between safety culture and organizational culture is still scarce in the literature. Most of the studies carried out investigate the situational diagnosis and little progress is made in terms of deepening the implications for the professional practice and the repercussions for the safety of hospitalized patients. Objective: To know the characteristics of the end-of-life process in a Regional Hospital, according to the actors involved, establishing convergence/divergence elements in care process, decision-making, information and knowledge. Method: Observational, cross-sectional, descriptive, convergent of mixed methods and data triangulation study. Carried out on a sample of 63 subjects, of which 25 were health professionals, 19 family caregivers and 19 medical records of patients who died in the course of a year. Results: Data triangulation shows discrepancies between family members and health professionals regarding care quality and the information provided. The lack of documentation in medical records of care aspects related to decision-making or terminal sedation stands out. Conclusion: This study provides an integrated view of end-of-life care provided in a Regional Hospital, identifying priority areas of intervention in order to improve quality of life in this process, such as patient involvement in decision-making, appropriate completion of medical records and health professional training. Preventive actions have reached a new height in the COVID -19 pandemic and since there is not a specific treatment yet, while the vaccines demonstrate their effectiveness and gradually are applied to the population, preventive measures are still recommended. Among these actions, occupational health faces challenges, since a significant part of the population is economically active and have had to suspend work, and in the current context, return-to-work measures have been implemented. The care and protection of healthcare workers is crucial, given that they are the specific population that takes care for patients infected with SARS-CoV-2 and this increases their exposure to the virus and expands their possibility of suffering from COVID-19, which must be recognized as an occupational disease which must be attended from the prevention of exposure to the care of sequelae and even compensation for death derived from this disease. Health Care Associated Infections (HAIs) are a mayor problem in hospitals around the World. The World Health Organization (WHO) estimates that 7% of patients hospitalized in developed countries and 10% in developing countries will develop a HAI. These infections are related with a significant rise in morbimortality and health care costs. Infections are mostly associated with prolonged length of stay, intravascular devices, permanent urinary catheters, orotracheal intubation and use of mechanical ventilation among others. In addition, in recent years Multi Drug Resistant (MDR) organisms are often isolated in HAIs, which contributes to the significantly rise in morbidity, mortality, use of broader spectrum antimicrobials and cost. Efforts around the world are destined to reduce HAIs by using specific isolation precautions regarding the type of microorganism, its transmission and its drug susceptibility. These are primarily based on a high percentage of health care workers correct hand-wash, fast and adequate identification of HAIs and subsequently, the specific isolation recommendation to further prevent outbreaks. These measures are based on the three main transmission methods: contact precautions, droplet precautions and airborne precautions. Therefore, we present an actualization of the 2009 recommendations for prevention of HAIs according to our local epidemiology and to the latest WHO and the Centers for Disease Control (CDC) recommendations. Introducción: La seguridad del paciente es considerado como un patrón integrado de comportamiento individual y organizacional, basado en creencias y valores compartidos que continuamente busca minimizar el daño al paciente, que resulta de la atención. Objetivo: Analizar la correlación de percepción de cultura de seguridad del paciente, y grado de seguridad percibido en funcionarios de una Institución de Salud de tercer nivel de atención (2015-2017-2019). Materiales y Métodos: Estudio cuantitativo correlacional, con una muestra intencional derivada del registro de una base de datos de cultura de seguridad del paciente, con un total de 402 registros. La medición se realizó mediante el cuestionario Hospital Survey on Patient Safety Culture. Resultados: En los resultados globales el mayor porcentaje de encuestados correspondió al personal asistencial (73,4%). El principal servicio asistencial es unidad de cuidado intensivo (18,2%) y la mayor participación correspondió al rol de enfermeras y auxiliares de enfermería (45,7%). Según las respuestas positivas para los datos globales se determina 2 de 12 dimensiones clasificadas como fortaleza en el componente de Aprendizaje organizacional (81,5%) y Trabajo en equipo (85,5%) y el grado de seguridad percibido con correlaciones (p: 0,000) que se identifican en seis dimensiones entre ellas notificación de eventos, y aprendizaje organizacional. Discusión: La correlación del grado de seguridad percibido con las dimensiones del cuestionario guarda similitud en especial la notificación de eventos. Conclusión: La seguridad del paciente y el grado de seguridad percibido guardan correspondencia en la medida que en las instituciones realizan intervenciones en diferentes componentes entre ellos la notificación de eventos y trabajo en equipo. Introduction: Human resources are a fundamental element in the health system, their equitable distribution is essential to guarantee the attention of the health needs of a population. Objective: To describe the changes in the availability of human resources in health of the Ministry of Health and regional governments of Peru, during the 2017-2019 triennium. Methods: Non-experimental and retrospective study, based on secondary sources, which corresponded to the latest reports of the National Registry of Health Personnel of the Ministry of Health from 2017 to 2019. The availability of doctors, obstetricians and nurses who worked in health facilities, who correspond to different professions, according to the Peruvian regulatory framework. Absolute and relative frequencies were estimated. Results: The availability of doctors increased by 11.4%, obstetricians by 9.8% and nurses by 15.8%, from 2017 to 2019. In the first level of care, the availability of human resources in health is increased by 7.7%; Furthermore, in extreme poverty areas there was a 6.8% reduction in the number of doctors, a 4.4% increase in obstetricians in rural areas. Nurses were the fastest growing in the three natural regions of Peru. Conclusions: The availability of human resources in health increases for the year 2019 compared to 2017, in addition, nurses are the ones that present a greater percentage increase in this period.

RESUMO Objetivo: analisar a assistência dos profissionais de saúde em situação de perda gestacional. Método: realizou-se revisão integrativa da literatura, nas bases Lilacs via BVS, CINAHL e Medline via PubMed no mês de outubro de 2020. Incluíram-se estudos primários indexados em bases de dados, publicados nos idiomas inglês, português e espanhol, relacionados à temática de investigação e sem delimitação de recorte temporal. Resultados: a revisão abrangeu sete estudos. A análise crítica da revisão levou à organização, à síntese e à discussão dos resultados com os achados da literatura. Conclusão: quanto à assistência dos profissionais de saúde em situação de perda gestacional, observou-se falta de preparo emocional e técnico na assistência prestada às mães, familiares e pessoas próximas envolvidas na perda gestacional, o que reflete falhas na formação profissional acerca da temática. A literatura sinaliza a necessidade da abordagem sobre o tema durante o ensino acadêmico e estímulo a cursos de capacitação.

Objetivo: Describir los niveles de autopercepción del estrés en el personal de salud de atención en primera línea en la ciudad de Lima–Perú. Método: El instrumento se elaboró en función a las reacciones físicas, psicológicas y conductuales que presenta una persona frente a una situación de estrés. La fiabilidad del instrumento nos brindó un valor de 942, lo que nos indica, el alto nivel de confiabilidad que presenta. Resultados principales: Señalan que, la muestra estudiada presenta niveles altos de estrés, manifestado a través de las reacciones físicas, como son algunas de ellas, la alteración del apetito, sueño y agotamiento; siendo el personal de enfermería femenino y del sector público del Ministerio de Salud, quienes se ubican en los niveles más altos, al igual que los profesionales más jóvenes han demostrado indicadores altos de estrés. Conclusión general: El personal de salud en primera línea de atención presenta altos niveles de estrés afectando su salud psicológica; por ende, su adecuado desenvolvimiento.

Introducción: Abordar el tema de Promoción de la salud y particularmente sus antecedentes, es un asunto de permanente vigencia, dada su importancia a la luz de las políticas internacionales de salud pública planteadas desde el siglo XX. Para que la Promoción de la Salud se siga fortaleciendo, se requiere de la creación de condiciones favorables a la salud en lo cual los profesionales de la salud juegan un rol trascendental, que pasa por su formación académica, continuando con su desempeño profesional y participación ciudadana. Objetivo: Presentar una revisión crítica de los antecedentes de la promoción de la salud y del rol de los profesionales de la salud en su desarrollo. Método: Se realizó una revisión de artículos, libros y documentos oficiales de organismos internacionales de salud, publicados en español y en inglés. Se usaron en español e inglés, los términos “promoción de la salud”, “personal de salud” y “salud pública”. Las bases de datos consultadas fueron PubMed, SciELO y Science Direct. Conclusiones: Los hallazgos muestran cómo la Promoción de la salud ha estado enmarcada en críticas y debates desde su origen, lo que demuestra su importancia y vigencia en el tiempo; los profesionales de la salud como actores principales en la promoción de la salud, deben recibir desde su formación las bases necesarias para su aplicación profesional y personal.

This study aims to understand the experiences of obstetric violence experienced by Embera women during childbirth at healthcare facilities in the city of Medellin, Colombia. Employing a qualitative, ethnographic approach, semi-structured interviews were conducted with nine indigenous Embera women between February and March of 2020. The data indicate these women, members of the largest indigenous community in Medellín, experience similar forms of obstetric violence to non-indigenous women, which might be explained by the authoritarian medical habitus and a health system that prioritizes market forces. However, the data also reveal that indigenous women experience a specific form of this phenomenon: a lack of respect or cultural sensitivity, stemming from the process of colonialism to which indigenous peoples have been subjected. This dynamic continues to be expressed during delivery in healthcare childbirth settings via ignorance and disregard for indigenous ancestral knowledge, which can be understood as microaggressions that occur frequently but are difficult to identify. Objective: to analyze the lethality of COVID-19 by sex and age among health professionals in Pará, Brazil. Method: epidemiological and observational study, using public secondary data on cases and deaths accumulated by COVID-19 and demographic data, between March and October 2020. The number of cases and deaths by COVID-19 that occurred among health professionals were compared in relation to age and sex using the chi-square test, followed by logistic regression using Wald's Backward Stepwise method. Results: among the 15,332 confirmed cases of COVID-19, 70.3% were female and 61.3% aged between 30 and 49 years (39.2 ± 11.6 years). 97 deaths were recorded, with a fatality rate of 0.6%. The probability of death was 52.8 times (20.7-134.5) and 4.0 times (2.5-6.2) higher among young men and men when compared to other reports. Conclusion: the lethality rate among health professionals is high, especially among young men. This is an alert about the impacts of the disease among health workers and raises the public authorities, specifically the health sector, better working conditions and worker health policies. Objective: to verify the association between patterns of alcohol consumption and attitudes toward alcohol, alcoholism, and alcoholics in nurses. Method: this was a cross-sectional, correlational study with a non-probabilistic sample of professional nurses (n=163) who completed the scale of attitudes to alcohol, alcoholism and alcoholics and the Alcohol Use Disorders Identification Test. Results: attitudes were negative (cutoff point <2.4), and a negative relationship was identified between attitudes to alcohol, alcoholism and alcoholics score and alcohol consumption (r=−.013, p=.868). Conclusion: the consumption patterns for the nurses were sensible. In particular, the Mexican nurses showed negative attitudes towards alcohol-related problems, alcoholism, and alcoholics. Objective: to know the perception of the multi-professional team regarding the strategies for the safety of pediatric patients in critical areas. Method: a qualitative and exploratory-descriptive study carried out in pediatric intensive care and emergency units of three hospitals in the city of Porto Alegre, between November 2018 and March 2019, linked to the matrix project entitled "Patient safety in hospital care services for children and adolescents in the city of Porto Alegre". A total of 20 practitioners who worked in these areas and who had experience in pediatrics for at least one year were included. Five collective interviews were conducted, which were audio-recorded and later transcribed for content analysis. Results: three categories arose: Maintenance of adherence to the protocols/goals of pediatric patient safety, Teaching/education in patient safety, and Involvement of companions for the promotion of patient safety. Conclusion: the practitioners acknowledge that the strategies described demonstrate minimal conditions to reach care quality and incident prevention, essential for patient safety. To identify patient- and provider-related factors associated with the success of multidrug-resistant tuberculosis (MDR-TB) treatment in the six municipalities of Colombia with the highest number of MDR-TB cases. Methods. Bivariate and multivariate logistic regressions were used to analyze the association between treatment success (cure or treatment completion) and characteristics of the patients and physicians, nursing professionals, and psychologists involved in their treatment. The importance of knowledge in the management of MDR-TB cases was explored through focus groups with these providers. Results. Of 128 cases of TB-MDR, 63 (49.2%) experienced treatment success. Only 52.9% of the physicians and nursing professionals had satisfactory knowledge about MDR-TB. Logistic regression showed that being HIV negative, being affiliated with the contributory health insurance scheme, being cared for by a male physician, and being cared for by nursing professionals with sufficient knowledge were associated with a successful treatment outcome (p ≤ 0.05). Qualitative analysis showed the need for in-depth, systematic training of health personnel who care for patients with MDR-TB. Conclusions. Some characteristics of patients and healthcare providers influence treatment success in MDR-TB cases. Physicians' and nurses' knowledge about MDR-TB must be improved, and follow-up of MDR-TB patients who are living with HIV and of those affiliated with the subsidized health insurance scheme in Colombia must be strengthened, as these patients have a lower likelihood of a successful treatment outcome. The COVID-19 pandemic has weakened the mental health of the population, health personnel are possibly an affected group, an important fact considering that they are the main elements in the face of the greatest challenge that the modern world has faced so far, in relation to diseases infectious. The COVID-19 disease has produced anxiety, depression, nervousness, and wakefulness in health personnel, among others; a result of stress, long shifts, excessive workload, inadequate training, and insufficient personal protective equipment; making them prone to disorders like Burnout Syndrome or Post Traumatic Stress Syndrome. Mental health is fundamental for public health and that of health personnel is a determinant of the quality of care and health of the patient; therefore, the objective of this paper is to carry out a bibliographic review that analyzes the mental health of health workers in the face of the COVID-19 pandemic. Objective: to describe experiences of nurses and doctors of Emergency Care Units in coping with the Covid-19 pandemic. Method: descriptive-exploratory study with a qualitative approach, conducted with seven doctors and seven nurses working in two Emergency Care Units, reference for Covid-19. The interviews took place between September and November 2020 and were guided by a semi-structured questionnaire. The statements were recorded, transcribed and submitted to Content Analysis. Results: two categories of analysis emerged: "We feel exhausted": the experience of nurses and doctors and Strategies to face the mishaps in the pandemic context. Final considerations: the professionals experienced several challenges, such as lack of institutional protocol, lack of physical structure, material, human resources and training, difficulty in sensitizing the population and concern to contaminate oneself and the family. However, they relied on different strategies, such as preventive self-isolation, family support, exchange of experiences with other professionals and keeping up to date on the disease. Objective: describing the evaluative statements of escorts about the care provided to women in the process of birth. Method: a descriptive research with qualitative approach performed with ten escorts in a maternity school. The semi-structured interview was used as a data collection technique. For the treatment of the statements, the Thematic Content Analysis Categorial was used. Results: the content obtained was systematized into two categories: evaluative statements related to health services and evaluative statements related to the care provided to women in the process of birth. Final thoughts: the escorts enunciated and experienced the care provided many times as limited and associated with the workload of the professionals; however, they reinforced the reception and communication as important. Objective: to understand the intentionality of the actions of health professionals to promote female autonomy during assistance to labor and delivery. Method: qualitative research based on Alfred Schütz's social phenomenology, carried out with 17 professionals, in a university hospital in southern Brazil. The comprehensive analysis was based on the theoretical-methodological framework. Results: the intentionality of professional action was to ensure the best for the woman, the newborn, the professional and the health service during the process of labor and delivery; and to expect changes in professional performance towards a new paradigm. Conclusion: health professionals act towards achieving an outcome favorable to all involved and recognize the need for institutional and professional changes to qualify obstetric care, in order to promote women's autonomy in this context. Objective: to describe the psychosocial impact caused by the pandemic of the new coronavirus on health professionals. Method: this is a narrative review carried out on the Virtual Health Library platform using the following descriptors "Health Professionals" AND "COVID-19", from May to June 2020. Results: a chart was developed showing the characteristics of the six selected scientific articles, according to title, authors, year of publication, journal, type of study and psychosocial impact. Conclusion: in the midst of the pandemic, professionals may feel helpless, overloaded with activities and severely affected physically and psychologically, becoming more vulnerable. There is need to adopt protective measures and a psychosocial action plan to provide better working conditions, medical care, psychological and therapeutic care, promoting qualifications and the use of integrative and complementary health practices. Objective: to understand, based on the discourses of the health team, the development of care for the elderly aged over 65 years, in order to remain healthy, active, independent and autonomous. Method: qualitative research with interpretative approach, carried out in a Group of Health Centers in Northern Portugal. Data were collected by semistructured interviews and analyzed through content analysis, by categories, in a sample of physicians, nurses and social workers. Results: among the participants, 83% did not have a gerontology training. There were focuses of care evaluation, in which all professionals estimated the same data, but there were data necessary for the follow-up of the elderly that were not evaluated by any professional. The sharing of information for care, when it occurred, fell into situations of illness or changes in the social context. Given the importance of developing protective actions to mental health in pandemic contexts, this study aimed to gather evidence about coping mechanisms that health professionals use during the Covid-19 pandemic. This systematic review, developed from international and Brazilian databases between November/2019 and July/2020, identified 3821 potentially relevant articles published in English and Portuguese. Checking the articles' appropriateness to the objective of the review resulted in the selection of 23 articles that were read in full, eight of which were selected for this review. The main coping mechanisms described were psychological support from relatives, colleagues, institutions, and patients; availability of continuing education and Personal Protective Equipment (PPE); spirituality; and time for hobbies. Promoting effective and sustainable coping strategies is vital to enable health professionals to feel capable of facing one of the major contemporary humanitarian challenges - the Covid-19 pandemic. Introduction: Patient safety is defined as an integrated pattern of individual and organizational behavior based on shared beliefs and values that continuously seeks to minimize harm to patients. Objective: To analyze the correlation between the perception of patient safety culture and the degree of safety perceived by healthcare workers in a tertiary hospital in 2015, 2017 and 2019. Materials and Methods: A quantitative correlational study was conducted with a purposive sample of 402 records obtained from a patient safety culture database. The Hospital Survey on Patient Safety Culture was used for assessment. Results: The global results show that the highest percentage of respondents corresponded to healthcare workers (73.4%). The main healthcare service provided is the intensive care unit (18.2%) in which most participants were nurses (45.7%). According to positive responses received for global data, 2 out of the 12 dimensions are classified as strengths in the Organizational Learning (81.5%) and Teamwork (85.5%) components, as well as the degree of safety perceived through correlations (p: 0.000) is found in five dimensions such as event notifications and organizational learning, among others. Discussion: The correlation between the perceived degree of safety and survey dimensions shows similarity, especially for event notifications. Conclusion: Patient safety and the perceived degree of safety are correlated as institutional interventions are made for different components, including organizational learning and teamwork. Objective: To report the experience of a health action of the Commission for Permanent Education in Nursing at the Hospital Geral de Fortaleza during the Covid-19 pandemic. Methods: Experience report of the actions developed by the Permanent Nursing Education Commission to train the nursing team to the suspected or confirmed patient with Covid-19 and self-protection measures, from March 2020 to March 2021. 406 professionals were trained in 81 workshops. Results: The professionals demonstrated insecurity, concern and anxiety in patient care with Covid-19, fear of own or family illness. We found inexperienced and recently graduated professionals, scarcity and inadequate use of PPE. It is a reflection to think about strategies that help in the work process of these professionals, mainly in adopting measures to support mental health. Objective: To analyze the demographic and clinical factors related to falls of older adults living at home in a five-year follow-up Materials and methods: Retrospective quantitative and longitudinal study conducted between 2007/2008 and 2013 with 262 Brazilians participants over 65 years old. We used the demographic profile, Mini-Mental State Examination, self-reported diseases, falls and their characteristics, Edmonton Frail Scale, Lawton and Brody Scale, Functional Independence Measure instruments. We performed descriptive analysis, Chi-squared, Wilcoxon test and mixed model test. Results: Most participants were women, aged between 65 to 79 years, schooling between 1 to 4 years and retired. The prevalence of falls was 21.8% and 37.8% in both evaluations. We verified that the chance of suffering a new fall increases for each additional disease of the older adult. In addition, for each point to more on the frail scale, the older adult has a greater chance of falling. On the other hand, for each medication withdrawn, the chance of falling decreases 10%. Conclusion: The fall was associated with clinical factors in older adults, therefore constant evaluations must be carried out to identify this event and its triggers. [ABSTRACT FROM AUTHOR]

However, in Spain, the circumstances suffered by the health professionals who have cared for pandemic patients from a clinical nutrition standpoint has remained unknown up to this moment. Objectives: the management and care changes made in clinical nutrition units in Spain, and their impact on clinical practice are described. Material and methods: a cross-sectional study was carried out using a survey directed at SENPE members (June 2020). Responses sent by health professionals in the field of clinical nutrition who had treated patients with COVID-19 in Spanish hospitals were included in the study. Resultados: a total of 116 survey forms were analyzed, mostly filled out by doctors (57.8 %) working at hospitals with more than 500 beds (56 %); 46 % of survey respondents were on telework. There was a nutritional care plan in 68 % of cases, such plan being present mainly in hospitals with more than 500 beds (p < 0.001). In these hospitals more specific diets for COVID-19 were implemented than in those under 500 beds: 18 (35.3 %) vs 44 (67.7 %), (p < 0.001). The use of recommendations issued by scientific societies was reported in 86 % of cases. Never or almost never could a satisfactory nutritional assessment be performed for 38.8 %. The prescription of nutritional supplements was not less than 50 %. Health workers rated their performance as satisfactory or very satisfactory (51.7 %), and this was not related to hospital size but to having implemented a COVID-19 diet (p < 0.05). Conclusions: clinical nutrition in Spain has responded to the COVID-19 pandemic with organizational and managerial changes and, although care has been clearly affected, some quality standards were ultimately maintained.
