Abstract

We address the problem of predicting when a disease will develop, i.e., medical event time (MET), from a patient's electronic health record (EHR). The MET of non-communicable diseases like diabetes is highly correlated to cumulative health conditions, more specifically, how much time the patient spent with specific health conditions in the past. The common time-series representation is indirect in extracting such information from EHR because it focuses on detailed dependencies between values in successive observations, not cumulative information. We propose a novel data representation for EHR called cumulative stay-time representation (CTR), which directly models such cumulative health conditions. We derive a trainable construction of CTR based on neural networks that has the flexibility to fit the target data and scalability to handle high-dimensional EHR. Numerical experiments using synthetic and real-world datasets demonstrate that CTR alone achieves a high prediction performance, and it enhances the performance of existing models when combined with them., Comment: To be published in IJCAI-22

Abstract

Many deaths and victims caused by catastrophic natural hazard occurred in Indonesia has proven the weakness of government to create a community disaster resilience among society. Under this circumstance any efforts of government are expected to be solution to stop or at least reduce the nightmare of society when the disaster occurred. This paper elaborates the discussion about building community disaster resilience through collaborative governance performed by Banyuwangi local government in Indonesia. Furthermore, the discussion about community disaster resilience and collaborative governance will be tied up by institutional and socio-ecological context. Therefore, this paper will highlight the role of actors, collaborative process and organization performance rather than any technical attributes from disaster resilience. The finding of this study reveals that there are some inhibiting factors influencing collaboration among actor to build community disaster resilience. The failure to manage these factors has resulting lack of collaboration and lead to weak community resilience in Banyuwangi.

Abstract

Aims/Introduction: This study was designed and carried out to investigate the association of dipeptidyl peptidase‐4 inhibitor (DPP‐4i) use with pancreatic cancer (PC) in individuals with diabetes in Japan. Materials and Methods: The JMDC Claims Database, which contains the medical and prescription information of Japanese employment‐based health insurance programs, was used. The primary outcome was duration to the first occurrence of PC (International Classification of Diseases 10th Revision code C25), both all and hospitalized, from prescription of DPP‐4is or other oral glucose‐lowering agents (GLAs). Results: Individuals with diabetes who received DPP‐4is (n = 61,430) or other oral GLAs (n = 83,304) were analyzed. Follow‐up periods (median [interquartile range]) were 17 months (8–33) for DPP‐4is and 14 months (7–28) for other oral GLAs. Kaplan–Meier curve analysis to determine the duration of first use of DPP4i or other oral GLA to diagnosis of PC disclosed no differences between the two groups in duration to all or hospitalized PC (log‐rank test: all, P = 0.7140; hospitalized, P = 0.3446). Cox proportional hazards models showed that use of DPP‐4is did not affect the PC risk adjusted for medications, age, sex and risk comorbidities (all, hazard ratio 1.1, 95% confidence interval 0.8–1.3, P = 0.6518; hospitalized, hazard ratio 1.1, 95% confidence interval 0.8–1.4, P = 0.6662). Similar results were obtained when individuals with ≥2 years oral GLA treatment and those with medical checkup data (e.g., smoking or drinking habit) available were analyzed. Conclusion: This database study shows that there is not a significant PC risk due to DPP‐4i treatment in individuals with diabetes in Japan, but larger studies with longer follow up are required to confirm these findings. Abstract

Background: Appropriate implantable cardioverter‐defibrillator (ICD) shocks are associated with an increased risk of mortality and heart failure (HF) events. The first appropriate shock may occur late after implantation. However, whether the timing of the first appropriate shock influences prognosis is unknown. This study aimed to evaluate the clinical significance of the timing of the first appropriate shock in patients with ICD. Methods: This retrospective and observational study enrolled 565 consecutive ICD patients. Patients who received an appropriate shock were divided into the early group (first appropriate shock <1 year after ICD implantation) and late group (first appropriate shock ≥1 year after ICD implantation). All‐cause mortality was compared between the two groups. Results: Over a median follow‐up of 5.6 years, 112 (19.8%) patients received an appropriate shock, including 32 patients (28.6%) in the early group and 80 patients (71.4%) in the late group. Comparisons of baseline characteristics at ICD implantation revealed that the late group was more likely to receive cardiac resynchronization therapy (66.3% vs. 31.3%, p < 0.001), ICD for primary prevention (60.0% vs. 31.3%, p = 0.001), and angiotensin‐converting enzyme inhibitor or angiotensin‐receptor blocker treatment (88.8% vs. 71.9%, p = 0.028). Survival after shock was significantly worse in the late group than in the early group (p = 0.027). In multivariable Cox proportional hazards analysis, the late group had an increased risk of all‐cause mortality compared with the early group (HR: 2.22; 95% CI 1.01–4.53; p = 0.029). In both groups, the most common cause of death was HF. Conclusions: Late occurrence of the first appropriate ICD shock was associated with a worse prognosis compared with early occurrence of the first appropriate shock. Cardiac death was the most common cause of death in patients who experienced late occurrence of the first appropriate ICD shock, resulting from HF in most cases. [ABSTRACT FROM AUTHOR]

Abstract

Perovskite photovoltaic devices added with tin (Sn) dichloride and copper (Cu) bromide were fabricated and characterized. The thin film devices were prepared by an ordinary spin-coating technique using an air blowing method in ambient air. A decaphenylcyclopentasilane layer was coated at the surface of perovskite layer and annealed at a high temperature of 190 °C. Conversion efficiencies and short-circuit current densities were improved for devices added with Sn and Cu compared with the standard devices. The energy gap of the perovskite crystal decreased through the Sn addition, which was also confirmed by first-principles calculations. [ABSTRACT FROM AUTHOR]
