Artificial intelligence technology, which is an important landmark in computer science, is a field of study which focuses on forming smart machines performing tasks that naturally necessitate human intelligence. AI, which models and mimics the human behaviour such as reasoning, learning, and planning as well as the tools humans handle, has been widely used in several fields of expertise such as economy, finance, education, agriculture, and healthcare. Coupled with deep learning, artificial intelligence turns out to be one of the most significant technological advancements of the 21st century as it pioneers the digital transformation of the modern society. As AI is drastically scientific and basically a preoccupation with numbers, literature, which is laden with figures of speech, has been regarded as the one and only branch that it can never master. The latest developments on computational literature indicate the opposite, though. Since the second half of the 20th century, there have been poems and novels written by AI. In this context, the aim of this study is to illustrate how artificial intelligence creates literary works based on the examples created using the online AI software programme – OpenAI. Within this framework, the study concludes that artificial intelligence technology has become prominent also in the literary field and that the death of the author might be sooner than it is actually expected.&nbsp

Amaç: KHDAK`de en sık K-RAS mutasyonu görülmekle birliktetedavi hedefi olduğundan EGFR, ALK, ROS-1 mutasyonları da değerlidir. Krizotinib bir ALK tirozin kinaz inhibitörüdür. ALK ve ROS-1mutasyonu olan hastalarda krizotinib tedavisinin etkili olduğu görülmektedir. Bu olgu takdiminde lokal ileri akciğer adenokarsinom tanılı,ROS-1 mutasyonu bulunan ve uzun sureli krizotinib tedavisi altındaprogresyon göstermeyen vakayı sunmayı amaçladık.Olgu: 57 yaşında, komorbiditesi bulunmayan kadın hastanın,2016 yılında öksürük ve balgam şikâyetiyle başvurusunda çekilenToraks BT`sinde sol akciğer üst lopta 25x22 mm kitle, sol hilusta enbüyüğü 4 cm LAP ve mediastinal patolojik lenf nodu saptandı. Alınandoku biyopsisi sonucu, adenokanseriyle uyumlu gelen hasta ROS-1pozitif lokal ileri KHDAK olarak değerlendirildi. 2 kür sisplastin ve pemetrekset tedavisi sonrası yanıt değerlendirmesinde hiler, sol parakardiak, prevasküler ve aorta-pulmoner yerleşimli en büyüğü 2 cm çaplılenfadenopatiler saptandı. Ardından krizotinib tedavisi başlandı. İyiyanıt elde edilmesi üzerine Radikal KRT (karboplatin ve taksol) uygulandı. Yeniden krizotinib tedavisi başlanan hasta 2017 yılından beritakiplerinde, krizotibin tedavisi altında progresyon göstermedi.Sonuç: Krizotinib gibi ROS-1’i hedefleyen tirozin kinaz inhibitörlerinin tümör büyümesini engelleyebildiği, hastalara klinik fayda sağladığı gösterilmiştir. 2016’dan beri krizotinib, tümörlerin üçte ikisinin yanıtvermesi ve ~20 ay süren progresyonsuz sağkalım ile birinci basamakreferans tedavi olmuştur.Bizim hastamızda 72 aylık bir progresyonsuzsağkalım elde edilmiştir. Moleküler hedefe yönelik tedaviler ile kansertedavsisnde uzun süreli sağkalım görülmesi mümkün olmakatadır

Amaç: Mukopolisakkaridozlar (MPS); glikozaminoglikanların (GAG) lizozom içerisinde yıkılamaması sonucu çoklu sistem tutulumlarına neden olan heterojen bir hastalık grubudur. MPS II ve III’ de santral sinir sistemi tutulumu daha ön plandadır. Erken dönemde bilişsel gerilik, davranış problemleri, ilerleyici davranış-uyku problemleri, gecikmiş konuşma, otizm benzeri bulgular, sebebi bilinmeyen zihinsel yetersizlik görülebilir. Yöntem: Mukopolisakkaridoz II ve III tanısı ile izlenen nöbet, gelişim basamaklarında gerilik, konuşma gecikmesi, nörolojik gelişim basamaklarında kayıp gibi nöropsikiyatrik semptomlarla başvuran hastaların sonuçları retrospektif olarak kaydedildi. Bulgular: Çalışmaya 11 olgu dahil edildi. 6 (%54.5)’sı erkek, 5 (%45.5)’i kadındı. MPS III olgularının 4 (%50)’ü MPS 3A, 3 (% 37.5)’ü MPS IIIB, 1 (%12.5)’i MPS IIIC tanılıdır. Başvruda 6 (%54.5)’sinde konuşma geriliği, 2 (%18.1)’ i bilişsel gerilik, 3 (%27.2)’ sinin ise hiperaktivite varken ek olarak 9 (%81.8)’unda motor gerilik de eşlik etmekteydi. Başvuru yaş ortalamaları 8.4 (±5.2 SDS) yıldı. 6 (%54.5)’sı fizik tedavi rehabilitasyon ve özel eğitim desteği almaktaydı. Sonuç: Nöropsikiyatrik semptomlarla başvuran hastalarda mukopolisakkaridozların da düşünülerek, İdrar GAG, enzimatik ve gereğinde genetik analizlerin yapılması, erken teşhis ve tedavi açısından önemlidir

The purpose of this study is to search the mediator roles of unique product development and global technological competence in the effect of international entrepreneurial orientation on company performance in international markets for born global companies. The study was conducted on born global companies which started its international activities in its first 7 years and operate in technoparks in Turkey. Data was collected by a questionnaire from 158 born global companies. According to the method proposed by Baron and Kenny, three models were created and compared with each other. The Structural Equation Modeling (SEM) method was applied because it is more appropriate for complex models. According to the findings of the study, unique product development and global technological competence do not have mediator roles in the effect of international entrepreneurial orientation on company performance in international markets. However, there is a direct effect of international entrepreneurial orientation on company performance in international markets. Global technological competence has a direct effect on performance in international markets. On the other hand, unique product development does not have a direct effect on performance in international markets but unique product development has a direct effect on global technological competence.

1980’li yıllardan itibaren küreselleşme ve neo liberal politikaların etkisi ile Dünya’da çok uluslu şirketlerin yaygınlaşmaya başlaması ve bu yatırımları ülkelerine çekmek amacıyla özellikle gelişmekte olan ülkelerin emek piyasasına yönelik yapmış olduğu düzenlemeler, emeğin sendikal hak ve özgürlüklerinin gerilemesine, ücretlerin düşmesi, işsizliğin artması ve güvencesiz çalışmanın yayınlaşması gibi olumsuz sonuçların ortaya çıkmasına neden olmuştur. Bu nedenle reformist uluslararası sendikalar tarafından küresel çerçeve sözleşmeleri alternatif bir araç olarak geliştirilmiş olup ulusal sendikaların uluslararası düzeyde örgütlenmesini güçlendirmek ve uluslararası toplu pazarlık mekanizması aracı olarak kullanılmaya başlanmıştır. Bu çalışmanın amacı, uluslararası üretim halkasında önemli bir tedarik zinciri olan Türkiye’de küresel çerçeve sözleşmelerinin ulusal sendikaların toplu pazarlık sürecine olan etkisini araştırmaktır. Bu bağlamda Türkiye’de en geniş işkolunu kapsayan IndustriALL’a üye ve küresel çerçeve sözleşmesi imzalamış olan farklı işkollarındaki sendika uzmanları ve IndustriALL uzman ve yöneticileri ile yarı-yapılandırılmış soru formu üzerinden derilenmesine görüşme tekniği kullanılarak nitel araştırma yöntemi uygulanmış ve aynı zamanda farklı işkolu ve sektör temelinde karşılaştırmalar yapılmıştır. Alan araştırmasından elde edilen bulgular IndustriALL’un taraf olduğu Küresel Çerçeve Sözleşmelerinin Türkiye’de üye sendikaların toplu pazarlık sürecine sınırlı etkili olduğu tespit edilmiştir. Bunun yanında Küresel Çerçeve Sözleşme’lerin tekstil sektöründe örgütlü üye sendikaların toplu pazarlık sürecine etkisinin bulunmasına rağmen yerel yasa nedeniyle sürecin işveren güdümlü ilerlediği görülmektedir. Since the 1980s, the spread of multinational companies has risen worldwide due to globalization and neo-liberal policies Especially to attract these investments, developing countries started to regulate their labour markets, which has led to cause negative results such as the decline of the union rights and freedoms of the labour, the decrease in wages, and the increase in unemployment and precarious work. For this reason, global framework agreements had enhanced by reformist international trade unions as an alternative tool. It has started to use as a tool to strengthen the organization of national unions at the international level and as the international collective bargaining mechanism. This study aims to find the effects of global framework agreements on the collective bargaining process of national trade unions in Turkey, which is the prominent supply chain in the international production ring. In this context, a qualitative research method was applied by using a semi-structured interview technique with experts and managers of IndustriALL and the experts of national trade unions that are members of IndustriALL (the largest business line in Turkey) and have signed a global framework agreement. At the same time, a comparative analysis was made in different sectors and business lines. Findings from the field research It has been determined that the Global Framework Agreements, to which IndustriALL is a party, have a limited effect on the collective bargaining process of the member unions in Turkey. In addition, although the Global Framework Agreements have an effect on the collective bargaining process of the member unions organized in the textile sector, it is seen that the process is employer-driven due to the local law.

Araştırma, yaşlı hipertansiyon hastalarında kırılganlık ve öz-bakım gücü arasındaki ilişkinin belirlenmesi amacıyla yapılmış tanımlayıcı bir çalışmadır. Araştırmanın örneklemini evreni belli olmayan örnekleme yöntemine göre, %5 örneklem hatası ve %5 anlamlılık düzeyinde Ekim 2019- Ekim 2020 tarihleri arasında Düzce Atatürk Devlet Hastanesi dahili yataklı servisleri ve dahili polikliniklerine başvuran 65 yaş ve üzeri 299 hipertansiyon hastası oluşturmuştur. Araştırmanın verileri; "Hasta Tanıtıcı Bilgi Formu", "Edmonton Kırılganlık Ölçeği" ve "Öz-bakım Gücü Ölçeği" kullanılarak toplanmıştır. Verilerin değerlendirilmesinde tanımlayıcı istatistiksel metodlar, paremetrik karşılaştırma testleri ve korelasyon testleri kullanılmıştır. Hastaların yaş ortalaması 71.40±6.597'dir. Edmonton Kırılganlık Ölçeği toplam puan ortalaması 5.71±2.87, Öz-bakım Gücü Ölçeği puan ortalaması ise 90.19±12.06 olduğu saptanmıştır. Bu çalışmada, çevrimiçi platformlarda yer alan ziyaretçi yorumları hatırlanabilir turizm deneyimi boyutlarına göre incelenmiştir. Yapılan araştırmada, "Rize'nin Çamlıhemşin ilçe sınırları içerisinde yer alan yaylalarda, hatırlanabilir turizm deneyimi hangi boyutlarda gerçekleşmektedir?" sorusuna cevap aranmıştır. Nitel araştırma yöntemi kullanılarak gerçekleştirilen çalışma, Rize Çamlıhemşin ilçe sınırları içerisinde bulunan on iki yaylaya (Gito, Elevit, Palovit, Amlakit, Tirovit, Aşağı ve Yukarı Kavrun, Huser, Sal, Badara, Avusor, Pokut ve Ayder yaylası) ait yorumlar üzerinden gerçekleştirilmiştir. Veriler 2020-2021 yıllarına ait Tripadvisor ve Google haritalar 'da yer alan Türkçe 1015 yorumdan oluşmaktadır. Toplanan veriler MAXQDA nitel veri analiz programı kullanılarak analiz edilmiştir. Yapılan analiz sonucunda hatırlanabilir turizm deneyimi kapsamına hazcılık, bilgi, katılım, yerel kültür, yenilik ve tazelenme boyutlarına ilişkin verilere rastlanmıştır. Bu boyutlar arasında en çok hazcılık boyutuyla ilgili ifadelerin yer aldığı görülmüştür. Bu da yayla ziyaretçilerinin, hatırlanabilir turizm deneyimini daha çok hazcılık boyutunda gerçekleştirdiğini göstermektedir. Ayrıca birlikte hatırlanabilir turizm deneyimine ilişkin verilerin sadece olumlu deneyimlerden oluşmadığı, bunun yanı sıra bireylerin karşılaştıkları olumsuzlukların da bir deneyimi hatırlanabilir kılabileceği görülmüştür. In this study, visitor reviews on online platforms were examined according to the dimensions of a memorable tourism experience. In the research, the answer to the question, "which dimensions of memorable tourism experience can occur in the highlands within the boundaries of the Çamlıhemşin region of Rize?", was searched. The study, which was conducted using the qualitative research method, was carried out through the reviews of twelve highlands (Gito, Elevit, Palovit, Amlakit, Tirovit, Aşağı ve Yukarı Kavrun, Huser, Sal, Badara, Avusor, Pokut ve Ayder Highlands) within the boundaries of Çamlıhemşin, Rize. The data consists of 1015 reviews written in the Turkish language on TripAdvisor and Google Maps for the years 2020 and 2021. The collected data were analyzed by using MAXQDA qualitative data analysis program. As a result of the analysis, data on the dimensions of hedonism, knowledge, participation, local culture, innovation and refreshment were found within the scope of the memorable tourism experience. Among these dimensions, it was seen that the expressions related to the hedonism dimension took place the most. This shows that plateau visitors realize their memorable tourism experience mostly in the dimension of hedonism. Moreover, it has been seen that the data on the memorable tourism experience can not only consist of positive experiences, as well as the negative experiences faced by people may also make an experience memorable.

Primer immün yetmezliklerin en yaygın komplikasyonlarından biri enfeksiyonlardır. “Caspase associated recruitment domain-9 (CARD9)” fungal immün yanıtta rol oynayan bir proteindir. CARD9 eksikliği, otozomal resesif kalıtım gösteren ve farklı klinik tablolara neden olabilen primer immün yetmezlik bozukluklarından biridir. Yüzeyel veya derin dermatofitoz, invaziv feohifomikoz, kutanöz mukormikoz, ekstrapulmoner aspergilloz, mukokutanöz veya invaziv kandidiyaz gibi çeşitli fungal enfeksiyonlar ile ilişkilendirilmiştir. CARD9 eksikliğinde en sık görülen enfeksiyonlar Candida spp. kaynaklıdır. Bu raporda CARD9 eksikliğine bağlı gelişen Candida albicans meningoensefaliti olgusu sunulmuştur. Otuz yedi yaşında erkek hastanın öyküsünde bilinen hastalığı ve ilaç kullanımı olmamakla birlikte çocukluğundan beri tekrarlayan oral kandidiyaz ve kutanöz fungal enfeksiyon geçirdiği, iki ay önce de C.albicans meningoensefaliti tanısıyla lipozomal amfoterisin B tedavisi aldığı öğrenilmiştir. Klinik iyileşme sonrasında vorikonazol tedavisiyle taburcu edildiği ancak taburcu olduktan sonra vorikanazol tedavisini kestiği ve kontrole gitmediği öğrenilmiştir. Taburcu edildikten 1.5 ay sonra baş ağrısı, kusma ve bilinç değişikliği şikayetleriyle tekrar başvuran hastanın ateşi ve ense sertliği olmayıp Kerning ve Bruzinski belirtisi negatif saptanmıştır. Beyin bilgisayarlı tomografisinde (BT) hidrosefali tespit edilmesi üzerine eksternal ventriküler drenaj (EVD) kateteri takılmıştır. Beyin omurilik sıvısı (BOS) incelemesinde eritrosit sayısı 340/mm3 olarak saptanmış, lökosit görülmemiştir. BOS protein düzeyi 28 mg/dl, BOS glukoz düzeyi 59 mg/dl (eş zamanlı kan glukoz düzeyi 104 mg/dl) idi. BOS’un Gram boyamasında maya ya da bakteri, Ziehl-Neelsen boyamada ise asite dirençli bakteri görülmemiştir. Hastanın önceki yatışında çalışılan BOS örneğinden yapılan Mycobacterium tuberculosis kültüründe üreme olmadığı öğrenilmiştir. Serumda insan bağışıklık yetmezlik virüs antikoru negatif bulunmuştur. Hastanın kontrol BOS kültüründe flukonazole duyarlı C.albicans üremesinin öğrenilmesi üzerine EVD kateteri değiştirilerek lipozomal amfoterisin B tedavisi başlanmıştır. BOS kültürü tekrarlanmış, takiplerinde tekrar edilen BOS kültürlerinde flukonazole duyarlı C.albicans üremesi devam etmiştir. Yapılan beyin manyetik rezonans (MR) görüntülemede apse lehine patoloji saptanmamıştr. Mevcut lipozomal amfoterisin B tedavisine flukonazol eklenmiştir. Anne ve babası üçüncü dereceden akraba olan hastada ve kardeşlerinde tekrarlayan mukokutanöz fungal enfeksiyon öyküsü olması CARD9 eksikliğini düşündürmüştür. Kan örneklerinden çalışılan moleküler testte hastada ve kız kardeşinde CARD9 eksikliğine bağlı homozigot p.Q295X mutasyonu saptanmıştır. Ancak hasta, hastaneye yatışının 62. gününde gecikmiş tanı, tekrarlayan C.albicans meningoensefaliti nedeniyle gelişen serebral komplikasyonlar ve granülosit koloni uyarıcı faktör (G-CSF) tedavisini alamaması sonucu, yetersiz tedavi gibi nedenlerle yaşamını yitirmiştir. CARD9 eksikliğinde gelişen inatçı fungal enfeksiyonlar ciddi komplikasyonlara ve mortaliteye sebep olmaktadır. CARD9 eksikliğinin Türk popülasyonundaki sıklığı ve ülkemizde tanı testinin yapılmaması göz önünde bulundurulduğunda tanı konulamayan, mortalite ya da morbiditeyle seyreden birçok hasta olabileceğini düşündürmektedir. Sonuç olarak bu olgu invaziv fungal enfeksiyonlar için bilinen risk faktörü taşımayan çocuk ve yetişkinlerde, etkili antifungal tedaviye rağmen tekrarlayan ya da devam eden inatçı invaziv fungal enfeksiyon durumunda CARD9 eksikliğinin göz önünde bulundurulmasını vurgulamak amacıyla sunulmuştur. Primary immunodeficiencies are disorders that cause clinical findings ranging from mild diseases to life-threatening diseases in a wide age range. Infections are the most common complications of primary immunodeficiencies. Caspase associated recruitment domain-9 (CARD9) is a protein that plays a role in fungal immune response. CARD9 deficiency is one of the primary immunodeficiency disorders that show autosomal recessive inheritance and can cause different clinical pictures. It has been associated with various fungal infections such as superficial or deep dermatophytosis, invasive pheohifomycosis, cutaneous mucormycosis, extrapulmonary aspergillosis, mucocutaneous or invasive candidiasis. The most common infections in CARD9 deficiency are caused by Candida spp. In this report, a case of Candida albicans meningoencephalitis due to CARD9 deficiency was presented. It was learned from the medical story that a 37 years old male patient had no known disease or drug use, but had recurrent oral candidiasis and cutaneous fungal infections since childhood, and received liposomal amphotericin B treatment with the diagnosis of C.albicans meningoencephalitis two months ago. It was learned that he was discharged with voriconazole treatment after clinical improvement, and he stopped the voriconazole treatment after discharge and did not go for follow-up. The patient, who reapplied 1.5 months after discharge with complaints of headache, vomiting and altered consciousness, did not have fever and neck stiffness, and Kerning and Bruzinski sign was negative. An external ventricular drainage (EVD) catheter was inserted after hydrocephalus was detected in the brain computerized tomography (CT). In the cerebrospinal fluid (CSF) examination, erythrocyte count was detected as 340/mm3, and no leukocytes were seen. CSF protein level was 28 mg/dl, CSF glucose level was 59 mg/dl (simultaneous blood glucose level was 104 mg/dl). There was no yeast or bacteria in CSF Gram staining and no acidfast bacteria in Ziehl-Neelsen staining. It was learned that there was no growth in the Mycobacterium tuberculosis culture made from CSF sample taken at the first admission of the patient. Serum human immunodeficiency virus antibody was negative. Upon learning of fluconazole-susceptible C.albicans growth in the control CSF culture of the patient, the EVD catheter was changed, and liposomal amphotericin B treatment was started. CSF culture was repeated. Fluconazole-susceptible C.albicans continued to grow in CSF cultures repeated in the follow-ups. No pathology in favor of abscess was detected in the brain magnetic resonance imaging. Fluconazole was added to the current liposomal amphotericin B treatment. Having a history of recurrent mucocutaneous fungal infection in the patient and his siblings, whose parents were third-degree relatives, suggested CARD9 deficiency. In the molecular test studied from blood samples, homozygous p.Q295X mutation due to CARD9 deficiency was detected in the patient and his sister. However, the patient died on the 62nd day of hospitalization due to delayed diagnosis, cerebral complications due to recurrent C.albicans meningoencephalitis, and insufficient treatment as a result of failure to receive the granulocyte colony stimulating factor (G-CSF) treatment. Persistent fungal infections that develop in CARD9 deficiency cause serious complications and mortality. Considering the frequency of CARD9 deficiency in the Turkish population and the lack of diagnostic testing in our country, it is thought that there may be many patients who cannot be diagnosed and who progress with mortality or morbidity. In conclusion, this case was presented to emphasize the consideration of CARD9 deficiency in case of persistent invasive fungal infection or recurrent invasive fungal infection after treatment despite effective antifungal treatment in children and adults who do not have known risk factors for invasive fungal infections. https://pubmed.ncbi.nlm.nih.gov/34666665/

Artan savaşlar ve göçlerden en çok etkilenen kitlelerden biri çocuk ve gençlerdir. Savaş ve göç sonucu yoğun bir genç nüfusu ruh sağlığına ilişkin bazı duygusal sorunlar yaşamaktadır. Bu çalışmada, SCARE Öfke Müdahale Programı’nın, Suriyeli öğrencilerin öfke kontrolü ve psikolojik dayanıklılıkları üzerindeki etkisini incelemek amaçlanmıştır. Çalışma çerçevesinde, kontrol ve deney grubu olmak üzere deney grubunda olan Suriyeli bir grup lise öğrencisine SCARE Öfke Müdahale Programı uygulanmıştır. SPSS programı ile ön test ve son test ölçek sonuçları analiz edilmiştir. Wilcoxon ve Mann Whitney-U Testi analizleri sonucunda, müdahale grubu öğrencilerinin sürekli öfke puanlarında ve sosyal davranışlarında anlamlı bir azalışın olduğu görülmüştür. Bu çalışma çerçevesinde SCARE Öfke Müdahale Programı’nın Suriyeli öğrencilerin öfke yaşama sıklıklarında anlamlı bir düşüşe sebep olduğu söylenebilir. Children and youth are two of the most affected by the increasing wars and migration. As a result of war and migration, a dense young population experiences some emotional problems related to mental health. In this study, it was aimed to examine the effect of SCARE Anger Intervention Program on Syrian students' anger control and psychological resilience. Within the framework of the study including the control and experimental groups, the SCARE Anger Intervention Program was applied to a group of Syrian high school students in the experimental group. Pre-test and post-test scale results were analyzed with SPSS program. As a result of the Wilcoxon and Mann Whitney-U Test analyzes, it was seen that there was a significant decrease in the trait anger and prosocial behavior scores of the intervention group students. Within the framework of this study, it can be said that the SCARE Anger Intervention Program caused a significant decrease in the frequency of Syrian students experiencing anger.
