3. High-Grade Basal Cell Adenocarcinoma Arising from the Parotid Gland: A Case Report and Review of the Literature

Basal cell adenocarcinomas are exceptionally rare and were first included in the histopathological classification of salivary gland tumors by the World Health Organization in 1991. These tumors are generally described as a low-grade malignancy with rare regional or distant metastasis. However, 48 basal cell adenocarcinoma cases arising from the major salivary glands have been described as high-grade malignancy in the English literature. Surgical excision and possible postoperative radiotherapy is the treatment of choice. Close follow up is mandatory for early detection of possible local recurrence. A 66-year-old female with high-grade basal cell adenocarcinoma of the parotid gland was presented in this study. Total parotidectomy with protection of the facial nerve integrity and postoperative radiotherapy were performed. At the most recent follow-up examination, 24 months after the surgery, the patient had no complaints and there was no sign of disease recurrence.

Solitary fibrous tumors of the head and neck are extremely rare, slow-growing, and generally asymptomatic benign neoplasms. To the best of our knowledge, only 153 cases of solitary fibrous tumors in the head and neck have been previously reported in the English literature until 2010. However, none has been in the supraclavicular region. Herein, we report a case of solitary fibrous tumor arising from the supraclavicular region. Characteristics, differential diagnosis, and treatment of the disease were also summarized.

Dermoid cysts arising from the head and neck region are rare, slow-growing, and well-circumscribed neoplasms. Symptoms are non-specific and usually related to the size and the location of the lesion. A dermoid cyst of the epiglottis is extremely rare. To the best of our knowledge, only one case has been previously reported in the English literature. In this study, a case of a dermoid cyst arising from the epiglottis was presented as an extremely rare clinical condition. The characteristics, differential diagnosis, and treatment of the disease are summarized.

Bilateral sudden sensorineural hearing loss is a rare otologic emergency and constitutes 0.44%-4.9% of overall sudden sensorineural hearing loss cases. Microvascular dysfunction secondary to systemic cardiovascular diseases, such as embolism, vasospasm, and hypercoagulability, is the main cause of non-idiopathic bilateral sudden sensorineural hearing loss. In this study, a case of bilateral sudden sensorineural hearing that occurred after angiographic embolization for treatment of aneurysmal subarachnoid hemorrhage was presented as a rare clinical condition.

7. Our surgical approach and results for protruding ear deformity

Objectives:To evaluate the performed surgical technique for protruding ear deformity from the point of complications and aesthetic results.Methods:Seven patients who were operated with protruding ear deformity diagnosis between September 2006–September 2007 were included in this study. In all cases for anti-helix formation horizontal mattress sutures with Mustardé technique were used and reformation of the cartilage was maintained. Besides to prevent the protrusion effect of the conchal bowl, concha mastoid sutures were used. If it was required, in order to maintain a better aesthetic angle in the superior pole, additional fixation suture was inserted to the helix. Mastoid bandage was applied to patients in the first week and a sports headband was recommended in the second week. Cases were evaluated from the point of early / late complications and aesthetic results.Results:Surgery was applied to 13 ears of 7 patients. 3 of the patients were female and 4 were male. The average age was 18.1. Deformations in 6 cases were bilateral and unilateral in one. Surgery was performed under local anesthesia except for 2 patients. Horizontal mattress and concha mastoid sutures were used in all ears. Additional fixation suture was required between the helix and temporal bone periosteum in three ears. Average follow up period was 11.8 months. No complications were encountered in the early postoperative period. In one ear, due to suture failure in late period, partial asymmetry occurred. The problem was solved with limited revision surgery.Conclusion:We observed that in the cases with protruding ear deformity who operated with Mustarde technique, it was possible to obtain satisfactory results by performing only suture techniques without additional work on the cartilage, if careful preoperative evaluation was carried out.

Dermoid cysts are infrequently found in the head and neck. The most common location in this region is the external third of the eyebrow. Cysts of the floor of the mouth are rare. These lesions are slow growing and painless. They are most often seen in young adults. Diagnosis before histopathological evaluation may not be possible, because the clinical presentation is nonspecific. We report here a case of a 24-year-old male with a dermoid cyst on the floor of the mouth. The literature is reviewed and the characteristics of this lesion are discussed.

9. Our Results for Pectoralis Major Myocutaneous Flap in Reconstructive Head and Neck Surgery

Objectives:To evaluate the efficiency of the pectoralis major myocutaneous flap for reconstruction of high-volume defects in the head and neck region.Materials and Methods:Between 1992-2000, 14 cases with head and neck cancers, who were applied pectoralis major myocutaneous flap for reconstruction, were included in this study. 3 of the cases were female and 11 of them were male. Their ages were between 26-73 (mean age: 56.25). All the cases were reconstructed with pectoralis major myocutaneous flap in the same operation after the tumor resection. In this study, pectoralis major myocutaneous flap was evaluated with literature for applying indications and post-operative complications. In this way, the reliability and functionality of the flap was searched.Results:In our series of 14 cases, partial necrosis was found in one case, fistula in another case, seroma and dehiscence were found at the other two. No complications were found at the rest of the ten cases.Conclusion:We think that pectoralis major myocutaneous flap is a functional and reliable choice for reconstruction of high-volume defects in the head and neck region.

10. The Surgical Treatment of Facial Paralysis due to Temporal Bone Fractures and Results

Objectives:The purpose of this study is to discuss the superiority of the approaches to each other by means of timing, and necessity of surgery in facial paralysis related with temporal bone fractures in the view of literature.Materials and Methods:Twenty-seven cases, who had an operation due to facial paralysis caused by temporal bone fractures, were reviewed retrospectively. Transmastoid approach was applied in 14 (52%) of these cases, transtemporal supralabyrinthine approach was applied in 10 (37%) cases and translabyrinthine was applied in 3 (11%) cases. The operation indication was determined with clinical examination, radiologic findings and electrodiagnostic tests.Results:There was interruption in the nerve in 9 (33.3%) of cases, bone fragments impinging on the nerve in 8 (29.6%) of cases and perineural hematoma in 10 (37%) of cases. Edema was found in the nerve of all the cases who had an operation within a month after the trauma. The site of the injury in all transverse fractures was in the labyrinthine segment. In longitudinal fractures neural injury in 90.4% of cases was situated in the perigeniculate region.Conclusion:The results were better in early operated cases and in the transtemporal supralabyrinthine approach.

Mastoid osteomlar nadir gelisen, yavas buyuyen ve genellikle asemptomatik benign tumorlerdir. Tedavi cerrahi eksizyon ile estetik kaygilar ve ilgili semptomlar icin yapilir. Osteosarkom, fibroz displazi, metastatik tumorler, dev hucreli tumorler ve multipl miyelom ayirici tanida dikkate alinmalidir. Yirmi alti yasinda kadin hasta, sol postaurikuler bolgede bes yildir yavas buyuyen agrisiz kitle ile klinigimize basvurdu. Hastanin ayni bolgede travma ve hematom olusumu oykusu vardi. Hasta asemptomatikti, ancak estetik goruntuden sikayetciydi. Fizik muayenede sol postroaurikuler bolgede, kemik yogunlugunda, yaklasik 3.5x3.0 cm boyutlarinda, agrisiz kitle belirlendi. Kitle agrisiz mastoid kemige fikse, sert ve duzgun yuzeyli idi. Uzerindeki cilt normaldi. Bilgisayarli tomografide, iyi havalanan sol temporal kemigin mastoid korteksinden kaynaklanan kemik yogunlugunda bir kitle saptandi. Kitle estetik nedenlerle retroaurikuler yaklasimla cerrahi olarak eksize edildi. Histopatolojik tani osteom olarak bildirildi. Ameliyat sonrasi donem sorunsuz gecti ve ameliyat sonrasi altinci ayda yapilan kontrol muayenesinde nuks izlenmedi. Anahtar Sozcukler: Mastoid; osteom; temporal kemik; tedavi. Mastoid osteomas are rare, slow-growing and usually asymptomatic benign neoplasms. Treatment is surgical excision for the aesthetic reasons and the related symptoms. Osteosarcoma, fibrous dysplasia, metastatic tumors, giant cell tumors, and multiple myeloma should be considered for the differential diagnosis. A 26-year-old woman was admitted to our clinic with gradually increasing painless mass on the left postauricular region for five years. She had a trauma history and hematoma formation on the same region. The patient was asymptomatic, however complaining about the aesthetic appearance. Physical examination revealed a painless mass with bony consistency, approximately 3.5x3.0 cm in size on the left postauricular region. It was non-tender, fixed to the mastoid, hard and had smooth surface. Overlying skin was normal. Computed tomography demonstrated a bony mass arising from the mastoid cortex of the well-pneumatized left temporal bone. The mass was surgically excised by retroauricular approach for aesthetic reasons. Histopathologic diagnosis was reported as an osteoma. Postoperative period was uneventful and there was no recurrence on six-month follow-up examination.

Objective(s): In this study, it is aimed to investigate the potential protective effect of caffeic acid phenethyl ester (CAPE) on ototoxicity caused by gentamicin in a rat model.Materials and Methods: Thirty Wistar albino rats were divided into 3 groups. Group I was selected as the control group. Gentamicin was administered intraperitoneally in group II, gentamicin and CAPE in group III. Audiological assessment was performed by the distortion product otoacoustic emission (DPOAE) and auditory brainstem response (ABR) measurements before and after treatment of each group. At the end of the study all rats were decapitated, cochlea was removed and electron microscopic examination was performed. Results: In group II post-treatment DPOAE levels were found to be lower than pretreatment DPOAE levels (P0.05). Amaç: İzole alt konka hipertrofili hastalarda, radyofrekans termal ablasyonu RFTA tekniği etkinliğinin, bilgisayarlı tomografi BT ve görsel analog ölçeği GAÖ ile değerlendirilmesi amaçlandı.Hastalar ve Yöntemler: Mayıs 2009 - Aralık 2009 tarihleri arasında kliniğimizde izole alt konka hipertrofisi tanısı konulan 30 hasta 10 kadın, 20 erkek; ort. yaş 31.0±12.0 yıl; dağılım 18-61 yıl çalışmaya alındı. Tedavi olarak, alt konkaya üç farklı noktada RFTA uygulandı. Konka boyutu değerlendirilmesi, ameliyat öncesi ve sonrası dönemde BT ile yapıldı. Ayrıca, GAÖ ile ameliyat öncesi ve sonrası dönemde, farklı bir kulak burun boğaz KBB uzmanının konka boyutlarını ve hastanın burun tıkanıklığını değerlendirmesi istendi.Bulgular: Ameliyat öncesi ve sonrası BT incelemesinde sol ve sağ alt konka ortalama kesit alanı değerleri sırasıyla 112.42±28.41 / 81.53±22.57 ve 117.72±27.85 / 86.53±23.66 mm2 idi. Farklı bir KBB uzmanı tarafından sol ve sağ alt konka boyutlarının ameliyat öncesi ve sonrası ortalama GAÖ değerleri, sırasıyla 6.60±2.48 / 4.30±1.47 ve 7.60±1.67 / 4.63±1.40 olarak belirlendi. Burun tıkanıklığının ameliyat öncesi ve sonrası ortalama GAÖ değerleri ise sırasıyla 6.77±1.19 ve 3.57±1.46 idi.Sonuç: Alt konka hipertrofisi tedavisinde kullanılan RFTA tekniği, BT ve GAÖ değerlendirme sonuçları ışığında etkin bir yöntemdir, Objectives: This study aims to evaluate the efficacy of radiofrequency thermal ablation RFTA technique by computed tomography CT and visual analog scale VAS in patients with isolated inferior turbinate hypertrophy. Also, the evaluation of turbinate dimension by another otorhinolaryngologist and of the nasal obstruction by the patients in pre- and postoperative period through VAS were asked. Results: The mean dimensions of the left and right inferior turbinate on CT in pre- and postoperative period were 112.42±28.41 / 81.53±22.57 and 117.72±27.85 / 86.53±23.66 mm2 , respectively. The mean VAS values of the left and right turbinate dimensions were determined by another otorhinolaryngologist in pre- and postoperative period as 6.60±2.48 / 4.30±1.47 and 7.60±1.67 / 4.63±1.40, respectively. The mean VAS values of nasal obstruction in pre- and postoperative period were 6.77±1.19 and 3.57±1.46, respectively. Conclusion: Radiofrequency thermal ablation technique used for the treatment of the inferior turbinate hypertrophy is an effective procedure in the light of the CT and VAS evaluation results.
