17. Prevention of possible vancomycin-resistant Enterococcus outbreak caused by infants transferred due to earthquake trauma.

Abstract

Infection control measures may be disrupted by inter- or intra-hospital transfers after major disasters. We report the possible outbreak and prevention of vancomycin-resistant Enterococcus spp. (VRE) strains carried by infants transferred to our neonatal intensive care unit from another unit after the earthquake. Screening cultures (rectal swabs) were obtained from infants during hospitalisation and contact isolation was performed. We analysed the clonal analysis of VRE strains isolated from patients by pulse field gel electrophoresis. • Infection control measures may be disrupted in extra situations such as disasters. • Contact isolation precautions (CIP) in interhospital transfers protect from outbreaks. • CIP is important to prevent the spread of multidrug-resistant bacteria. • Screening cultures from high-risk patients help prevent potential outbreaks. Abstract

Due to the rapid spread of a new subtype, as well as the increasing number of monkeypox (Mpox) cases seen since 2022, the World Health Organization declared an emergency declaration. To prevent an Mpox outbreak, the risks of infection spread and protecting priority populations must be identified. Mpox virus is mainly transmitted sexually, via skin-to-skin contact, by touching contaminated objects and surfaces, and through airborne droplets and aerosols. Accordingly, men who have sex with men, sex workers, people who are HIV-positive, travelers, and healthcare workers are the main priority groups that are at risk of this virus. Two doses of vaccine are recommended for the risk groups. Moreover, travelers are recommended to receive two doses of vaccine prior to travel, unless contraindicated. Persons with symptoms and rashes suspicious of Mpox are not advised to travel. In healthcare facilities, patients with suspected Mpox should be isolated in single rooms, and healthcare workers should wear full protective equipment. Considering that transmission is possible through indirect contact, health workers should avoid direct contact with patients' belongings, bed linen, and so on. [ABSTRACT FROM AUTHOR]

31. Evaluation of vitamin D binding protein and 25-hydroxy vitamin D metabolites in COVID-19 patients

Abstract

INTRODUCTION: The immunomodulatory roles of vitamin D and vitamin D binding protein (VDBP) are in interest with incidence or outcome of coronavirus disease-2019 (COVID-19). This study aimed to investigate the association between the severity of COVID-19 with VDBP, total 25-hydroxy vitamin D (25(OH)D), and its metabolites free vitamin D (VDfree) and bioavailable vitamin D (VDbio).METHODS: Study group consisted of 68 COVID-19 patients and 20 healthy subjects. Patients were subgrouped as asymptotic, mild/moderately pneumonia, or severe pneumonia. Plasma total 25(OH)D was quantitated by liquid chromatography with mass spectrometry and serum VDBP by a polyclonal sandwich enzyme immunoassay. In addition, routinely used laboratory parameters in follow-up were recorded. VDfree and VDbio were calculated using total 25(OH)D, VDBP, and albumin levels.RESULTS: Plasma total 25(OH)D (13.3± 5.7 vs 30.3±13.3 ng/dL), VDfree [2.18 (1.52-3.44) vs 4.34 (3.74-6.48) pg/mL], and VDbio [1.86 (1.09-2.81) vs 4.28 (3.45-6.34) nmol/L)] levels were lower in COVID-19 patients (pDISCUSSION AND CONCLUSION: COVID-19 patients have lower plasma 25(OH)D levels and lower 25(OH)D metabolites VDfree, VDbio which are physiologically active. Abstract

The present study aims to evaluate the effects of fluoride varnish (FV), bioactive glass varnish (BV), sodium-trimetaphosphate treated eggshell, and eggshell membrane containing varnish (EV) against dental erosion. In this study, 27 teeth were cut using a low-speed diamond separator. Two 2×3 mm windows were created on buccal surfaces, and the remaining area was covered with acid-resistant nail polish. Fluoride varnish, Bioactive glass varnish, Sodium-trimetaphosphate treated eggshell, and eggshell membrane containing varnish were applied to one of the windows, and the other window was used as a control. Using separate containers, samples were immersed in cola drink for two minutes and shaken gently. Then, the samples were kept in 0.4 ml artificial saliva for two minutes. This pH cycle was repeated four times. The surface properties of the samples were examined in a scanning electron microscope, the Ca, P, F, O, C, Mg and Na ratios were determined, and the Ca/P ratio was calculated. The Ca/P ratio in the area where FV is applied is 1.56, lower than 1.61, the stoichiometric ratio in human enamel. This ratio was 1.50 in the FV control group. The Ca/P ratio was 1.63 in the BV group and 1.51 in the BV control group, 1.63 in the EV group, and 1.52 in the EV control group. No statistical difference was found among the groups. On the enamel surface treated with BV and EV, a protective layer was observed. Against dental erosion, BV and EV can be used as an alternative to fluoride varnishes. [ABSTRACT FROM AUTHOR]

Abstract

Investigation of the leaves of Bauhinia thonningii Schum led to the isolation and identification of a new flavonol derivative, 6-C-methylquercetin-3,4'-dimethyl ether (1) together with eleven known compounds (2–13), with two of them (10 and 11) obtained as a mixture. Their structures were established by extensive spectroscopic analyses. Antibacterial activity of compound 1 as well as the reference antibiotic, ciprofloxacin was tested on Gram-negative multidrug-resistant bacteria overexpressing active efflux pumps, and against methicillin-resistant strains of Staphylococcus aureus (MRSA). Samples were tested alone and in combination with an efflux pump inhibitor (EPI), phenylalanine-arginine-β-naphthylamide (PAßN). Results show that when compound 1 was tested alone, its inhibitory effects were obtained on 7/10 tested bacteria with the highest MIC value of 128 µg/mL whilst in the presence of EPI, this activity significantly increase in all the 10 bacteria. An interesting antibacterial activity was obtained with compound 1 against Klebsiella pneumoniae ATCC11296 (MIC of 4 µg/mL), KP55 and Staphylococcus aureus MRSA6 in the presence of the PaßN. AASLD - TASL Connect Regional Meeting -- MAR 15-16, 2019 -- Istanbul, TURKEY WOS:000535263200004 PubMed: 32412901 Background/Aims: mbitasvir/paritaprevir/ritonavir (OMV/PTV/r) +/- dasabuvir (DSV) +/- ribavirin (RBV) combination has demonstrated excellent rates of sustained virologic response (SVR) and a very good safety profile in patients with the chronic hepatitis C virus (HCV) genotype 1 or 4 infections. We aimed to investigate the effectiveness and safety of OMV/PTV/r +/- DSV +/- RBV combination regimen in a real-world clinical practice. Materials and Methods: Data from HCV genotype 1 and 4 patients treated with OMV/PTV/r +/- DSV +/- RBV (n=862) in 34 centers across Turkey between April 1, 2017 and August 31, 2018 were recorded in a large national database. Demographic, clinical, and virologic data were analyzed. Results: The mean age of the patients was 55.63, and 430 patients (49.9%) were male. The majority had HCV genotype 1b infection (77.3%), and 66.2% were treatment-naive. Non-cirrhosis was present at baseline in 789 patients (91.5%). SVR12 rate was 99.1% in all patients. Seven patients had virologic failure. No significant differences were observed in SVR12 according to HCV genotypes. HCV RNA was undetectable at treatment week 4 in 90.9%, at treatment week 8 in 98.5%, and at the end of treatment (EOT) in 98.9%. SVR12 ratio was significantly higher in the non-cirrhotic patients compared to that in the compensated cirrhotic patients. Rates of adverse events (AEs) in the patients was 59.7%. Conclusion: The present real-life data of Turkey for the OBV/PTV/r +/- DSV +/- RBV treatment of patients with HCV genotype 1b, 1a, or 4 infection from 862 patients demonstrated high efficacy and a safety profile.
