CRUSH syndrome ,CREATINE kinase ,ACUTE kidney failure ,SODIUM bicarbonate ,CITIES & towns - Abstract
Background Two earthquakes on 6 February 2023 destroyed 10 cities in Türkiye. We report our experience with pediatric victims during these catastrophes, with a focus on crush syndrome related–acute kidney injury (Crush-AKI) and death. Method Web-based software was prepared. Patient demographics, time under rubble (TUR), admission laboratory data, dialysis, and kidney and overall outcomes were recorded. Results A total of 903 injured children (median age 11.62 years) were evaluated. Mean TUR was 13 h (interquartile range 32.5, max 240 h). Thirty-one of 32 patients with a TUR of >120 h survived. The patient who was rescued after 10 days survived. Two-thirds of the patients were given 50 mEq/L sodium bicarbonate in 0.45% sodium chloride solution on admission day. Fifty-eight percent of patients were given intravenous fluid (IVF) at a volume of 2000–3000 mL/m
2 body surface area (BSA), 40% at 3000–4000 mL/m2 BSA and only 2% at >4000 mL/m2 BSA. A total of 425 patients had surgeries, and 48 suffered from major bleeding. Amputations were recorded in 96 patients. Eighty-two and 66 patients required ventilator and inotropic support, respectively. Crush-AKI developed in 314 patients (36% of all patients). In all, 189 patients were dialyzed. Age >15 years, creatine phosphokinase (CK) ≥20 950 U/L, TUR ≥10 h and the first-day IVF volume <3000–4000 mL/m2 BSA were associated with Crush-AKI development. Twenty-two deaths were recorded, 20 of 22 occurring in patients with Crush-AKI and within the first 4 days of admission. All patients admitted after 7 days survived. Conclusions These are the most extensive pediatric kidney disaster data obtained after an earthquake. Serum CK level was significantly associated with Crush-AKI at the levels of >20 950 U/L, but not with death. Adolescent age and initial IVF of less than 3000–4000 mL/m2 BSA were also associated with Crush-AKI. Given that mildly injured victims can survive longer periods in the disaster field, we suggest uninterrupted rescue activity for at least 10 days. [ABSTRACT FROM AUTHOR]- Published

Objective: Diabetic nephropathy is one of the most important complications in diabetes mellitus. We aimed to examine the influence of organ-specific antibody seropositivity in diabetic nephropathy. Methods: Patients with type 1 diabetes and no evidence of celiac disease, thyroid dysfunction, and other kidney diseases and with an annual average HbA1c level <10%, body mass index <25 kg/m2, and normal blood pressure were enrolled. Results: Eighty patients (39 boys and 41 girls) were evaluated. Twenty patients with moderately increased albuminuria (diabetic nephropathy group) and 60 patients without albuminuria (control group) showed no statistical difference in age, gender, diabetes duration, age at diagnosis, kidney function tests, and mean blood pressure measurements. Compared to control group, the mean anti-thyroglobulin level and anti-thyroxin peroxidase level were statistically higher in the diabetic nephropathy group, P = .004 and P = .045, respectively. However, the thyroid function tests were normal in either group. Conclusion: Determination of the impact of autoantibody seropositivity on the risk of diabetic nephropathy in type 1 diabetic children could be a non-traditional marker in the risk assessment of diabetic nephropathy [ABSTRACT FROM AUTHOR]

- Abstract
Background. Antiphospholipid syndrome (APS), particularly the catastrophic antiphospholipid syndrome (CAPS), is one of the rare causes of thrombotic microangiopathy (TMA). CAPS is the most severe form of APS, especially when accompanied by complement dysregulation, causes progressive microvascular thrombosis and failure in multiple organs. In this report, a case of CAPS with TMA accompanied by a genetic defect in the complement system is presented. Case. A 13-year-old girl was admitted to the hospital with oliguric acute kidney injury, nephrotic range proteinuria, Coombs positive hemolysis, refractory thrombocytopenia, a low serum complement C3 level and anti-nuclear antibody (ANA) positivity. The kidney biopsy was consistent with TMA. She was first diagnosed with primary APS with clinical and pathological findings and double antibody positivity. As initial treatments, plasmapheresis (PE) was performed and eculizumab was also administered following pulse-steroid and intravenous immunoglobulin treatments. Her renal functions recovered and she was followed up with mycophenolate mofetil, hydroxychloroquine, low dose prednisolone and low molecular weight heparin treatments. The patient presented with severe chest pain, vomiting and acute deterioration of renal functions a few months after the diagnosis of TMA. A CAPS attack was considered due to radiological findings consistent with multiple organ thrombosis and intravenous cyclophosphamide (CYC) was given subsequent to PE. After pulse CYC and PE treatments, her renal functions recovered, she is still being followed for stage-3 chronic kidney disease. Complement factor H-related protein I gene deletion was detected in the genetic study. Conclusions. The clinical course of complement mediated CAPS tends to be worse. Complement system dysregulation should be investigated in all CAPS patients, and eculizumab treatment should be kept in mind if detected. [ABSTRACT FROM AUTHOR]

Objective: We aimed to present the characteristics, genetic analysis results, long-term prognosis of our patients with distal kidney tubular acidosis, and the relationship between hyperammonemia and distal kidney tubular acidosis. Materials and Methods: Biochemical, clinical, and imaging findings were collected at presentation and the last clinic visit, and results of the genetic analysis were recorded. Results: Our study included 9 patients (3 female, 33%). The median age at diagnosis was 3 months, and the median follow-up period was 111 months. Height standard deviation scores were less than -2 in 4 (44%) patients at presentation and in 3 (33%) at the last clinic visit. The median estimated glomerular filtration rate was 98 mL/min/1.73 m² at presentation and 126 mL/min/1.73 m² at the last clinic visit. We have found 8 different types of mutations of 2 genes, including 6 in the ATP6V0A4 gene, 2 in the SLCA4A1 gene, and 2 of them were novel. At the time of presentation, nephrocalcinosis and hypercalciuria were present in all our patients, but at the last visit, only 1 patient had hypercalciuria. Sensorineural hearing loss was found in 4 of our patients with a mutation in the ATP6V0A4 gene. Serum ammonia levels were found to be high in 3 patients with mutations in the ATP6V0A4 gene. Conclusion: Adequate metabolic control is essential for optimal growth and preserved kidney function in distal kidney tubular acidosis patients. Distal kidney tubular acidosis may be associated with hyperammonemia. We recommend keeping potassium levels at high-normal levels to reduce ammonia levels, especially in the absence of acidosis. Objective: Idiopathic nephrotic syndrome (INS) is the most common glomerular disease in children. Approximately 80% of INS is steroid sensitive (SSNS), and half of SSNS patients are steroid dependent (SDNS). This study determines the risk factors that may predict steroid dependence in patients with SSNS. Material and Method: Patients with SSNS who followed in our Pediatric Nephrology clinic between September 2016 and September 2018 were retrospectively evaluated. The demographic characteristics, albumin, creatinine values, blood pressure, white blood cell, lymphocyte count, lymphocyte/white blood cell ratio, urine protein–creatinine ratio, time to remission, and triggers of the attack were recorded. Results: Twenty-three patients were diagnosed as SSNS and 16 as SDNS. The mean age at presentation was 77.34±32 months in the SSNS group and 73.62±27.32 months in the SDNS group (p=0.690). No significant difference was observed between the two groups in terms of blood albumin, creatinine, lymphocyte count, and lymphocyte/white blood cell ratio. Trigger initiating the attack was more common in the SDNS group (p=0.001). The most common trigger was upper respiratory tract infection. Urine protein/creatinine ratio and white blood cell levels were significantly higher in the SDNS group (p=0.014, p=0.004, respectively), and the time to remission was also longer in the SDNS group (p=0.001). Conclusion: Upper respiratory tract infection may be a trigger, and severe proteinuria and late remission time may be risk factors for the development of SDNS in INS. Early use of steroid-sparing agents in these patients can prevent long-term adverse effects of steroid therapy., Amaç: İdiyopatik nefrotik sendrom (İNS) çocuklarda en sık görülen glomerüler hastalık olup, %80’i steroide duyarlıdır.

Objective: As increasing number of young people with renal diseases are reaching adulthood and issues related to their transition to adult units are getting more important. Failure to transfer adolescents under a well-designed transfer program can lead to a decline in attendance to an adult unit. We aim to share our experience and the views of the patients who went through a transition program. Materials and Methods: We conducted a telephone questionnaire with 88 patients, who were asked to evaluate their concerns before their transition and their experience during the visit in the adult nephrology unit after the transition process. Results: The mean age of participants was 19.21±0.97 years. Thirty patients (34%) had chronic kidney disease, and three of them were on dialysis. Three patients (3.4%) had a renal transplant at the time of transition. Fifty-one patients (58%) were felt anxious before the transition process. After their transfer to the adult unit, although 84 patients (95.5%) were satisfied to be in the adult unit, there were still 65 patients (74%) ready to return to the pediatric unit, if it was possible. Conclusion: Transition to the adult unit is still a cause of concern for adolescent patients, even if they are transferred under a well-designed program. It is important to raise awareness about transition programs among pediatricians and to develop a pediatric-adult transition program for the medical and psychological well-being of patients. Anahtar Kelimeler: Kadın, Euergesia, Pamphylia, Küçük Asya, Roma İmparatorluğu The suppressed position of women in ancient societies has drastically changed Since Roman Imperial period. Asia Minor has had the most important examples of those changes. In our study, we examined the social and cultural boundaries of the era in which the leading women lived in Pamphylia region of Asia Minor. By doing so, we have documented through inscriptions how they attempted to exist in the public sphere in the male-dominated ancient world. We identified these women through inscriptions from Aspendos, Attaleia, Perge, Side and Sillyon which are politically important cenetrs in the ancient world. In our study, which aims to investigate the prominent women from first centruy to third century, how the socio-cultural and economic understanding of the Roman Empire had an impact on the lives of women in Asia Minor. This study, which focuses on the verdicts on marriage in Zaydī and Hanafī jurisprudence, tries to determine the differences and similarities in the provisions of both madhabs. Chapters on marriage from Muayyad-Billâh ibn Husayn ibn Hârûn's Tacrīd is selected as an example of Zaydī jurisprudence and Ahmad al-Qudûrī's Mukhtasar as an example of Hanafī jurisprudence. The study consists of three chapters: the historical developments in both madhabs are discussed in the first chapter and in the second chapter, the selected texts are compared in the framework of kitabu'n-nikâh style in terms of format, method and tone. In the third chapter, the parts of these books on marriage is compared. It is seen that the texts are similar in terms of format and method, while Mukhtasar is more concise in terms of its tone. The verdicts on marriage show that both madhabs are close to each other, yet there is a considerable amount of differences. 136

Background/aim: Nephrocalcinosis (NC) is defined as calcium deposition in the kidney parenchyma and tubules. This study aims to determine the etiology, risk factors, and follow-up results of patients with NC in Turkey. Materials and methods: Patients diagnosed with NC in the pediatric nephrology Department Units of 19 centers from all geographical regions of Turkey over a 10-year period (2010–2019) were included in the study. The medical records from the centers were reviewed and demographic data, admission complaints, medical history, systemic and genetic disorders, risk factors for NC, treatment details, and presence of NC after one-year follow-up, were recorded retrospectively. Results: The study sample included 195 patients (88 females, 107 males). The mean age at diagnosis was 39.44 ± 47.25 (0.5–208) months; 82/190 patients (43.2%) were diagnosed incidentally; 46/195 patients (23.6%) had an underlying disease; idiopathic hypercalciuria was detected in 75/195 (38.4%) patients. The most common systemic diseases were distal renal tubular acidosis in 11/46 patients (23.9%), primary hyperoxaluria in 9/46 patients (19.6%) and Bartter syndrome in 7/46 patients (15.3%). After one year of follow-up, NC resolved in 56/159 patients (35.2%) and they all did not have an underlying systemic disease. Conclusion: The most common presentation of NC was incidental. Distal renal tubular acidosis and primary hyperoxaluria were the main systemic diseases leading to NC, while hypercalciuria was the most common metabolic risk factor. Nephrocalcinosis was found to remain in most of the patients at a one-year follow-up. It may resolve particularly in patients with no underlying systemic disease. [ABSTRACT FROM AUTHOR]

Objective: Ectopic kidney (EK) is a congenital anomalies of kidney and urinary tract (CAKUT). In this study, we aimed to evaluate the demographic characteristics, follow-up results of the patients with EK and to determine the frequency of urinary and other system anomalies. Material and Methods: A total of 67 patients (24 girls, 43 boys) were evaluated retrospectively in our pediatric nephrology outpatient clinic between 2000-2019. Results: The median age at diagnosis of was 3 [interquartile range (IQR): 5.4] years and the median follow-up time was 3.8 (IQR: 5.6) years. Sixteen (23.9%) patients were diagnosed antenatally, ten (%14.9) patients had parental consanguinity and ten (%14.9) had a family history of CAKUT. Simple pelvic ectopia was present in 56 (83.6%) patients, and cross ectopia in 11 (%16.4) patients. Voiding cystourethrography (VCUG) was performed for 58 patients and vesicoureteral reflux (VUR) was detected in 14 kidney units in 11 (%19) patients. Other urinary system abnormalities were detected in seven (%10.5) patients. Sixteen (%23.9) patients had urinary tract infection (UTI), ten (%62.5) of whom had urinary system abnormalities. There was no difference between cross and simple EK in terms of UTI and VUR (p=0.29 and p=0.16), but the relative renal function in technetium-99m-dimercaptosuccinic acid (99mTc-DMSA) was significantly lower in cross EK (p=0.015). The frequency of UTI was significantly higher in patients with urinary system abnormalities (p=0.009), but there was no difference in terms of scarring in 99mTc DMSA (p=0.26). Four patients (%6) had hypertension. Conclusion:Ectopic kidney is not an innocent diagnosis defined as abnormal anatomical location. Patients with EK should be followed-up regularly for urinary and other system abnormalities, UTI and hypertension. [ABSTRACT FROM AUTHOR]

Hematuria, proteinuria, renal biopsy ,urologic and male genital diseases ,female genital diseases and pregnancy complications - Abstract
Background/aim: We analyzed the clinical and pathological features and prognosis of 106 children with persistent asymptomatic microscopic hematuria (PAMH) with or without mild proteinuria. Materials and methods: This was a retrospective study of 106 children who were referred to our clinics from 2000 to 2013 for evaluation of PAMH. Results: Among the 106 patients, 69 (65%) were female and 37 (35%) were male. The patients were divided into two groups: 101 patients with isolated microscopic hematuria (IMH) and 5 patients with asymptomatic microscopic hematuria and mild proteinuria (AMHP). Renal biopsy was performed in all 5 children with AHMP: 2 patients had hereditary nephropathy and 2 patients had focal segmental glomerulosclerosis (FSGS). One biopsy specimen revealed nonspecific findings. Renal biopsy was performed in 9 children with IMH: 4 patients had hereditary nephropathy and 5 patients had nonspecific findings. None of the patients received any specific treatment prior to renal biopsy. During the follow-up period, none of the patients developed impaired renal function. Among all the children, only one patient with AMHP developed hypertension and 2 patients with IMH developed proteinuria. Conclusion: Long-term follow-up must be done carefully for isolated microscopic hematuria and renal biopsy should be performed in selected cases.

In recent years semiconductor quantum dot nanocrystals (NC) have attracted significant interest and have found numerous important optoelectronic device applications mainly because of their highly tunable optical properties. For example, precisely tuning shades of color chromaticity is critically important in solid state lighting to achieve ultra-efficient, application-specific, spectrally-engineered illumination. To date such color tuning and control of NC emitters have been investigated and demonstrated only based on their composition, shape, and size (using the quantum confinement effect). All of these parameters are, however, limited to be controlled and set during the synthesis process. As a post-synthesis alternative, we proposed and demonstrated the precise and broad control and tuning of color chromaticity by strongly modifying photoluminescence decay kinetics of NC emitters solely based on nonradiative Förster resonance energy transfer (FRET) in layer-by-layer self-assembled NC composite structures. Locating NC emitters in such a layered architecture with a targeted gradient of bandgap in the close proximity (

- Abstract
This study was designed to determine the usability of lemon fiber (LF-2%, 4%, 6%) and carrot fiber (CF-2%, 4%, 6%) to produce low-fat beef hamburgers. To that end, a certain amount of fat was replaced with each fiber. The proximate composition, pH value, cholesterol content, cooking characteristics, color, texture profile, and sensory properties of low-fat beef hamburgers were investigated. LF increased moisture content and cooking yield due to its better water binding properties, while CF caused higher fat and cholesterol contents owing to its higher fat absorption capacity (p<0.05). LF resulted in a lighter, redder, and more yellow color (p<0.05). Hardness, gumminess, springiness, and chewiness parameters decreased when the usage level of both fibers increased (p<0.05). However, more tender, gummy, springy, and smoother hamburgers were produced by the addition of CF in comparison with LF (p<0.05). Moreover, hamburgers including CF were rated with higher sensory scores (p<0.05). In conclusion, LF demonstrated better technological results in terms of cooking yield, shrinkage, moisture retention, and fat retention. However it is suggested that CF produces better low-fat hamburgers since up to 2% CF presented sensory and textural properties similar to those of regular hamburgers.
