27. Uluslararası Fiziksel Tıp ve Rehabilitasyon Kongresi’nde (17-21 Nisan 2019, Antalya) sözlü bildiri olarak sunulmuştur. Karpal tünel sendromu (KTS), en sık görülen tuzak nöropatisidir. Ellerde ağrı, uyuşma ve kuvvetsizlik gibi yakınmalarla karakterizedir. Fibromiyalji (FM) ise yaygın ağrının eşlik ettiği kronik kas iskelet sistemi hastalığıdır. Geniş semptom yelpazesi içinde parestezi yakınması sık görülmekte olup KTS ile karışabilmektedir. FM’li hastalarda KTS sıklığını araştıran az sayıda çalışma olup sonuçlar çelişkili bulunmuştur. Bu çalışmada amaç, klinik olarak KTS ile uyumlu yakınmaları olup sinir ileti çalışmalarında KTS saptanan ve saptanmayan iki grup arasında FM açısından farlılık olup olmadığını araştırmaktır. Fiziksel Tıp ve Rehabilitasyon Polikliniği’ne başvuran ve KTS ön tanısı ile elektrofizyoloji laboratuvarına yönlendirilen kadın hastalar çalışmaya dahil edilmiştir. Tüm hastalara FM 2016 ACR tanı kriterleri uygulanmış ve Yaygın Ağrı İndeksi (YAİ), Semptom Şiddet Skalası (SSS), Fibromiyalji Şiddet Skoru (FŞS) kaydedilmiştir. Hastalar elektrofizyolojik olarak KTS saptanan ve saptanmayan iki gruba ayrılarak sonuçlar karşılaştırılmıştır. Çalışmaya ellerde uyuşma yakınması olan ve sinir ileti çalışması sonucunda 30 KTS pozitif, 25 KTS negatif hasta alındı. İki grup arasında yaşa ve beden kitle indeksine göre farklılık saptanmadı. KTS saptananların %26,7’sinde, saptanmayanların %24’ünde ACR 2016 kriterlerine göre FM saptanmış olup, iki grup arasında istatistiksel olarak anlamlı fark bulunmamıştır. YAİ, SSS ve FŞS incelendiğinde de gruplar arasında istatistiksel fark saptanmamıştır. Diğer yandan FM saptanan ve saptanmayan iki grup karşılaştırıldığında FM saptanmayan grupta median sinir ileti hızları anlamlı olarak daha yavaş bulundu. Sonuç olarak, KTS hastalarında FM prevalansı yüksek bulunmuştur. FM olan ve olmayan KTS hastalarının median sinir iletim hızlarında anlamlı farklılıklar olduğundan, FM hastalarında KTS tanısı için elektrofizyolojik doğrulama önerilebilir. Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy. It is characterized by complaints such as pain, numbness and weakness in the hands. Fibromyalgia (FM) is a chronic musculoskeletal disease accompanied by widespread pain. In a wide range of symptoms, complaints of paresthesia are common and can be confused with CTS. There are few studies investigating the frequency of CTS in patients with FM, and the results have been contradictory. The aim of this study is to investigate whether there is a difference in terms of FM between the electrophysiologically confirmed and not confirmed CTS patients. Female patients who admitted to the Physical Medicine and Rehabilitation Outpatient Clinic and were referred to the electrophysiology laboratory with a preliminary diagnosis of CTS were included in the study. FM 2016 ACR diagnostic criteria were applied to all patients and the Widespread Pain Index (WPI), Symptom Severity Scale (SSS), Fibromyalgia Severity Score (FSS) were recorded.As a result of the nerve conduction study, 30 CTS positive and 25 CTS negative patients were included in the study. There was no difference between the two groups according to age and body mass index. FM was diagnosed according to ACR 2016 criteria in 26.7% of those with CTS, and 24% of those without and there was no statistically significant difference between the two groups. No statistical difference was found between the groups according to WPI, SSS and FSS. On the other hand, when the two groups with and without FM were compared, the median nerve conduction velocities were significantly slower in the group without FM. As a result, the prevalence of FM is high in CTS patients. Since there are significant differences in the median nerve conduction velocities of CTS patients with and without FM, electrophysiological confirmation may be recommended for CTS diagnosis in FM patients.

Bu çalışmanın amacı Bursa Uludağ Üniversitesi Tıp Fakültesi Hastanesinde çalışan hemşirelerde, kas-iskelet sistemi ağrılarının sıklığının belirlenmesi, omurga kaynaklı ağrılara yol açabilecek bireysel ve mesleksel risk faktörlerinin sorgulanması, omurga kaynaklı ağrıların depresyon ve anksiyete ile ilişkisinin belirlenmesi ve bu ağrılara eşlik eden fizik muayene bulgularının değerlendirilmesidir.Çalışmaya dahil edilen 239 hemşireye kişisel ve mesleksel faktörlerin sorgulandığı bir anket, İskandinav Kas İskelet Sistemi Sorgusu (İKİSS), Hastane Anksiyete ve Depresyon Ölçeği dolduruldu. İKİSS'ye göre boyun ağrısı olan hemşirelere Boyun Ağrısı Özürlülük İndeksi Anketi, bel ağrısı olan hemşirelere ise Oswestry Bel Ağrısı Engellilik Anketi dolduruldu. Katılımcıların ağrılı bölge muayeneleri yapıldı.Hemşirelerin en sık ağrı yaşadığı vücut bölgelerinin sırt (%72,8), bel (%72,4), boyun (%66,1) olduğu tespit edildi. Boyun ağrısı sıklığı ile ilişkili faktörler genel sağlık durumununun olumsuz olarak değerlendirilmesi (p 0,005), uykusu süresi (p 0,004), anksiyete (p 0,026), depresyon (p 0,010), servikal lordoz düzleşmesi (p 0,001), boyun eklem hareket açıklığı kısıtlılığı (p 0,001) ve paraspinal duyarlılık (p 0,001) olarak saptandı. Bel ağrısı sıklığı ile ilişkili faktörler kadın cinsiyet (p 0,020), genel sağlık durumunun olumsuz olarak değerlendirilmesi (p 0,001), anksiyete (p 0,003), depresyon (p 0,002), ağır yük kaldırma (p 0,027), ağır obje itme ya da çekme (p 0,041), Schober testinde kısıtlılık (p 0,012), lomber lordozda artma ya da azalma (p 0,001), paraspinal duyarlılık (p 0,001), Valsalva testi pozitifliği (p 0,040) olarak saptandı.Sonuç olarak çalışmamızda hemşirelerde kas iskelet sistemi ağrılarının yüksek oranda olduğu, ağrıların en sık omurga bölgesinden kaynaklandığı, bireysel ve mesleksel faktörlerin yanı sıra psikososyal etkilerin ağrı gelişiminde önemli rol oynadığı gösterilmiştir.Anahtar Kelimeler: İşle ilişkili kas iskelet sistemi ağrısı, boyun ağrısı, bel ağrısı, hemşire, risk faktörleri. The main objectives of this study are to determine the frequency of the musculoskeletal system pain experienced by the nurses working in Bursa Uludag University Medical Faculty Hospital and to investigate the relevant personal and occupational risk factors.239 nurses who participated in the study completed the survey including occupational and individual risk factors, the Nordic Musculoskeletal Questionnaire (NMQ) and the Hospital Anxiety and Depression Scale. Based on the answers to the NMQ, the nurses who had neck pain also completed the Neck Pain Disability Questionnaire. Likewise, nurses with low back pain completed the Oswestry Low Back Pain Disability Questionnaire. Additionally, physical examination was performed for the regional pain.This study showed that the most common pain areas of the nurses were upper back (72.8%), low back (72.4%) and neck (66.1%). Çalışmamızda akut ve subakut inmeli hastalarda uygulanan robotyardımlı yürüme tedavisinin fonksiyonel durum ve yaşam kalitesi üzerineetkisinin araştırılması amaçlanmıştır.Prospektif, randomize kontrollü olarak planlanan çalışmaya akut vesubakut dönemde toplam 28 inme hastası dahil edildi. Çalışma grubuna(n=14) 4 hafta boyunca, haftada 5 gün konvansiyonel rehabilitasyonyöntemleri ile birlikte günde 20 dk robot yardımlı yürüme tedavisiuygulanırken, kontrol grubuna (n=14) sadece konvansiyonel rehabilitasyonyöntemleri uygulandı. Her iki gruptaki hastalar tedavi öncesi ve tedavi sonrası1., 2. ve 3. aylarda 10 m yürüme testi (10 m YT), Fonksiyonel AmbulasyonSkalası (FAS), Fonksiyonel Bağımsızlık Ölçümü (FBÖ), Brunnstrom MotorEvrelemesi, Modifiye Ashworth Skalası (MAS), Konstipasyon Ciddiyet Ölçeği(KCÖ), Beck Depresyon Ölçeği (BDÖ) ile değerlendirildi.Her iki grupta da tedavi öncesine göre 1., 2. ve 3.ay izlemlerinde FAS,10 m YT ve Brunnstrom evrelemesinde istatistiksel olarak anlamlı artışsaptandı. Gruplar arası karşılaştırma yapıldığında iki grup arasında anlamlıfarklılık bulunmadı. FBÖ, KCÖ ve BDÖ değerlerinde her iki grup arasındaistatistiksel olarak anlamlı farklılık bulunmadı. Ancak her iki grup kendiiçerisinde incelendiğinde konvansiyonel tedaviye ek olarak robot yardımlıyürüme tedavisi alan grupta BDÖ ve KCÖ değerlerinde tedavi öncesine göre1., 2. ve 3. aylarda anlamlı iyileşme görülürken; konvansiyonel tedavi alangrupta BDÖ'de sadece 1. ay, KCÖ'de 2 ve 3. ay izleminde iyileşme olduğusaptandı.Sonuç olarak akut ve subakut dönemdeki inmeli hastalardakonvansiyonel rehabilitasyon programına ek olarak uygulanan robot yardımlıyürüme tedavisinin hastaların yürüme fonksiyonu ve motor gelişimi üzerinekonvansiyonel tedaviye benzer etkisinin olduğu saptandı. The aim of this study was to evaluate the affect of robot-assisted gaittraining on functional status and quality of life in patients with acute andsubacute stroke.This was a prospective randomized controlled trial comprising 28patients with acute and subacute stroke. The invervention group (n=14) hadundergone 5 days a week conventional rehabilitation therapy along with daily20 minute robot-assisted gait training for 4 consecutive weeks. The controllgroup (n=14) had undergone the same 5 days a week conventionalrehabilitation therapy for 4 consecutive weeks. 10 metre walk test (10 m WT),Functional Ambulation Categories (FAC), Functional Independence Measure(FIM), Brunnstrom Stage of Stroke Recovery, Modified Ashworth Scale(MAS), Constipation Severity Scale (CSC), Beck's Depression Inventory(BDI) were performed prior to initiation of therapy and at 1st, 2nd and 3rdmonths following completion of therapy in both groups.In both groups, there were significant improvement in FAC, 10 m WT,Brunnstrom Stage of Stroke Recovery at the 1st, 2nd and 3rd month followupscompared to the period prior to initiation of therapy. No statisticalsignifiance was detected when the two groups were compared for thesevariables. Similarly; the two groups did not significantly differ in terms of FIM,CSC and BDI. When each group was assessed per se during the follow-upperiod; CSC and BDI showed significant improvement at 1st, 2nd and 3rdmonths when compared to the period before therapy in invervention group.Whereas for the control group, BDI showed significant improvement only for ivthe 1st month follow-up and CSC showed improvement only at 2nd and 3rdmonth follow-up.In conclusion, robot-assisted gait training in addition to conventionalrehabilitation showed similar improvement in gait-function and motorrecovery when compared to conventional rehabilitation alone in patients withacute and subacute stroke. 80

Bu çalışmada, lateral epikondilit (LE) tanılı hastalarda, radial sinirin elektrofizyolojik olarak incelenmesinin yanında, aktivite modifikasyonu ve ev egzersizi tedavisine yanıtın değerlendirilmesi amaçlanmıştır. Çalışmamız LE tanısı olan 56 hasta ile prospektif olarak biçimlendirildi. Başvuru anında yakınması olan tarafa radial, median ve ulnar, karşı tarafa yalnızca radial duysal ve motor sinir ileti çalışması yapıldı. Hastalara aktivite modifikasyonu eğitimi ile birlikte, önkol kasları için germe ve güçlendirme egzersizlerini içeren ev egzersiz programı verildi. Hastalar başlangıç, 1. ve 3. ayda radial sinir ileti çalışması, algometrik duyarlılık, kavrama gücü, Kol, Omuz, El Disabilite Anketi (DASH), Hasta Bazlı Lateral Epikondilit Değerlendirme Testi (PRTEE) ile değerlendirildi. Başlangıçta yakınmaların olduğu taraf ile sağlam tarafın sinir ileti çalışmaları karşılaştırıldığında, hasta taraf radial sinir dirsek ve dirsek üstü motor latanslarının sağlam tarafa göre anlamlı düzeyde uzun olduğu saptanmıştır. Tedavi sonrası değerlendirmelerde ise 1. ayda radial motor ve duyusal sinir iletilerinde anlamlı iyileşme izlenmezken, 3. ayda başlangıca göre radial motor sinir dirsek üstü latansında ve ileti hızında, radial duyusal sinirin tepe latansında anlamlı iyileşme izlemiştir. Hastaların PRTEE ve DASH skorlarında, el kavrama gücünde 1. ve 3. ayda, algometrik duyarlılıkta ise 3. ayda anlamlı iyileşme izlenmiştir. Sonuç olarak, LE semptomları olan hastalarda radial sinir tuzak nöropatisinin eşlik edebileceği göz önünde bulundurularak elektrofizyolojik incelemelerin yapılmasının uygun olacağı düşünülmektedir. Ayrıca bu çalışma ile aktivite modifikasyonu ve egzersiz tedavisinin semptomların azaltılmasında ve elektrofizyolojik parametreler üzerinde etkili olduğu gösterilmiştir.Anahtar kelimeler: lateral epikondilit, egzersiz, radial sinir, sinir ileti çalışması In this study, we aimed to evaluate radial nerve conduction studies and treatment response with activity modification and home exercise program in patients with lateral epicondylitis.56 patients with lateral epicondylitis were enrolled in this prospective study. At admission, medial, ulnar and radial nerve conduction studies of the symptomatic side and only radial nerve conduction study at the asymptomatic side were done. Patients were given home exercises program including stretching and strengthening exercises for the forearm muscles and activity modification training.Patients were evaluated at beginning, 1st and 3rd month with radial nerve conduction studies, algometric test, grip strength, Disabilities of The Arm, Shoulder and Hand Questionnaire (DASH), Patient-rated Forearm Evaluation Questionnaire (PRTEE). At the beginning, nerve conduction studies of symptomatic and asymptomatic side were compared. Significant lengthening of radial motor nerve latency from elbow and above elbow stimulations was observed at symptomatic side. At the 3rd month of the treatment, significant improvement of radial motor nerve latency and conduction velocity with above elbow stimulation and radial sensory peak latency was observed. Significant improvement was recorded in PRTEE and DASH scores and grip strength at 1st and 3rd month. Significant improvement was also recorded in algometric test at 3rd month assessment.In conclusion, while the symptoms may be associated with the radial nerve entrapment in patients with lateral epicondylitis, electrophysiological studies are considered to be appropriate. This study also shows that activity modification and exercise therapy can be effective on electrophysiological parameters and symptom management. Bu çalışmada, hafif-orta dereceli karpal tünel sendromunda, el ve el bileği antropometrik ölçümlerinin hastalık şiddeti ve tedavi sonuçları ile ilişkisinin araştırılması amaçlanmıştır.Çalışmamız, hafif-orta dereceli KTS olan 39 hasta ile prospektif olarak biçimlendirildi. Hastaların demografik verileri, yakınma süreleri ve baskın elleri belirlendikten sonra, boy, kilo, VKİ, el ve el bileği antropometrik ölçümleri, median sinir kesitsel alan ölçümü ve karpal açı ölçümü yapıldı. Hastalığın şiddetini belirlemek ve tedavi yanıtını takip etmek için, Boston Semptom Şiddet Skalası, Boston Fonksiyonel Durum Skalası, ağrı için Görsel Analog Skala ve uyuşma şiddeti için Likert Skalası'ndan faydalanıldı. Median duyusal ve motor sinir ileti çalışmaları yapıldı. Başlangıç ölçümleri yapıldıktan sonra hastalara splintleme tedavisi uygulandı. Tedaviye yanıtı değerlendirmek için 2., 4. ve 12. haftada, elektrofizyolojik değerlendirmeler için ise sadece 4. ve 12. haftada izlem vizitleri yapıldı. Antropometrik ölçümlerle başlangıç semptom şiddet ölçekleri arasında anlamlı bir ilişki saptanmamakla birlikte, tedaviye yanıt sonuçları ile arasındaki anlamlı korelasyon gösterilmiştir. Özellikle hastaların bilek oranındaki artış ile, 2. ve 4. haftadaki Boston Fonksiyonel Durum Skalası'ndaki iyileşme ve 12. haftadaki Görsel Analog Skala'daki iyileşme arasında anlamlı düzeyde negatif korelasyon saptanmıştır.Sonuç olarak el ve el bileği antropometrik ölçümleri, KTS'de splint tedavisine yanıtı öngörmede yardımcı olabilecek bir yöntem olarak önerilebilir.Anahtar kelimeler: antropometrik ölçümler, karpal tünel sendromu, sinir ileti çalışması, splint tedavisi This study aims to investigate the relationship between anthropometric measurements of hand and disease severity and treatment outcomes in patients with mild-moderate carpal tunnel syndrome (CTS). 39 patients with mild-modarate CTS were enrolled in this prospective study. After obtaining demographic information, symptom duration, dominant side, height, weight, BMI, anthropometric measurements of hand and wrist, cross sectional area of median nerve and carpal angle measurements were recorded. In order to determine the severity of disease and response to treatment, Boston Symptom Severity Scale, Boston Functional Status Scale were used. Pain severity and numbness were assessed with Visual Analogous Scale and Likert Scale respectively. Median sensory and motor conduction studies were performed. After getting baseline measurements patients were treated with wrist splints. Follow up visits were performed at 2nd, 4th and 12th weeks for respond to treatment and 4th and 12th week for electrophysiological studies.No significant correlation between anthropometric measurements and severity of symptoms at onset was found but evidence indicating a correlation to response to treatment was found. Especially, the increment in wrist ratio at 2th and 4th week correlated inversely with the improvement of Boston Functional Status Scale and at 12th week, correlated inversely with the improvement of Visual Analogous Scale.In conclusion, anthropometric measurements of hand and wrist can be considered as a useful adjunct to determine the effectiveness of splinting in CTS.Key words: anthropometric measurements, carpal tunnel syndrome, nerve conduction study, splinting 85

Akut demyelinizan inflamatuvar bir poliradikülonöropati olarak tanımlanan Guillain Barre Sendromu (GBS), yol açtığı komplikasyonlar, yetiyitimi ve bunlarla ilişkili psikososyal sorunlar nedeniyle rehabilitasyon açısından önemli bir klinik tablodur. Bu çalışmanın amacı yatarak rehabilite edilen GBS olgularını klinik özellikler, komplikasyonlar ve fonksiyonel durum açısından incelemektir. Bu retrospektif çalışmada GBS tanısı ile rehabilitasyon amacıyla yatarak tedavi edilen 18 olgunun dosya bilgilerinden klinik özellikleri ve fonksiyonel bağımsızlık ölçümü skoru, fonksiyonel ambulasyon skoru ve Hughes skoru ile yapılan fonksiyonel değerlendirme sonuçları incelenmiştir. Rehabilitasyon kliniğine yatışa kadar geçen süre ortalaması 57,5 + 20,5 (12-382) gün ve klinikte yatış süresi ortalaması 33,5 + 4,9 (4-77) gün olan olguların yarısında nöropatik ağrı saptanmıştır. Fonksiyonel bağımsızlık ölçümü skoru, fonksiyonel ambulasyon skoru ve Hughes skoru ile yapılan değerlendirmelerde rehabilitasyon sonrasında anlamlı düzelmeler saptanmıştır. Hughes skoru yüksek (daha kötü) olan olgularda yatış süresi daha uzun olmuştur. Bulgularımız ambulasyon sorunu olan GBS’de rehabilitasyon gereksiniminin sürdüğünü ve rehabilitasyon sonucunda fonksiyonel iyileşmeler sağlanabildiğini desteklemektedir. Guillain Barre Syndrome (GBS) is an acute demyelinating inflammatory polyradiculoneuropathy which may lead to significant complications, disability and psychosocial problems. This study was designed to evaluate clinical features, complications and functional outcome of patients with GBS following inpatient rehabilitation. Clinical features and functional outcome measures assessed by functional independence measure, functional ambulation score and Hughes score of 18 patients with GBS who have undergone inpatient rehabilitation were evaluated in this retrospective study. Mean duration until admission to rehabilitation clinic was 57.5 + 20.5 (12-382) days and mean length of stay was 33.5 + 4.9 (4-77) days and neuropathic pain was noted in half of the patients. Significant improvements were noted in all functional outcome measures following inpatient rehabilitation. Length of stay was significantly longer in patients with higher (worse) baseline Hughes score. Our findings suggest that there is ongoing need for rehabilitation and significant improvements can be obtained by inpatient rehabilitation in GBS patients with residual ambulation problems.

This study was designed to investigate the relationship between radiological severity and clinical and psychological factors in patients with knee osteoarthritis (OA). Patients with knee OA (n = 100) were included in the study. Radiographic staging was done using Kellgren–Lawrence (KL) radiological rating scale. Stage 1–2 changes according to KL were grouped as ‘early’ and stage 3–4 as ‘late’ radiological OA. Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) was used for self-reported disability. Performance-based functional assessments were measured as time needed to walk 15 m and time needed to climb ten steps. Health-related quality of life was assessed by Nottingham Health Profile (NHP). Beck Depression Inventory (BDI) was used to assess the presence and severity of depression. Social isolation was assessed by a related NHP subscale. Significantly higher scores were found in the late stage group for NHP pain, physical mobility and social isolation subscales and all WOMAC subscales. Bu randomize, tek kör, plasebo kontrollü çalışmada, hemiplejik hastalarda alt ekstremiteye EMG biyogeribildirm (EMG BG) ile uygulanan egzersiz programının etkinliğini araştırmak amaçlanmıştır.Beyin damar hastalığı sonrasında hemipleji gelişen 34 hasta iki gruba randomize edilerek çalışmaya alındı. Her iki grupta da hastanın kas gücüne uygun olarak düzenlenen güçlendirme egzersizleri EMG BG yardımıyla uygulandı. Birinci gruptaki hastalar (n=17) egzersiz sırasında ekrandan görsel ve işitsel geri bildirim alırken, ikinci gruptaki hastaların (n=17) işitsel ve görsel geri bildirim alması engellenerek plasebo uygulama yapıldı. Hastaların tedavi öncesi, sonrası, 1. ve 3. ayda, aktif eklem hareket açıklığı, spastisite, kas gücü, fonksiyonel durum ve yürüme hızı değerlendirmeleri yapıldı.Çalışmaya alınan hastalarda yaş ortalaması 55,9±13,2 (18-78) yıl, inme sonrası geçen süre ortalama 133,6±132,2 (10-444) gündü. Modifiye Ashworth Skoru'nda (MAS) yalnızca Grup 1'de tüm vizitlerde anlamlı düzelme saptanırken eklem hareket açıklığı, kas gücü, FBÖ, Barthel ve 10 m yürüme süresinde her iki grupta da tedavi sonrası, 1. ve 3. ay vizitlerinde istatistiksel olarak anlamlı iyileşme kaydedildi. Gruplar arası karşılaştırmalarda tüm vizitlerde Grup 1 de MAS değerlerinde anlamlı farklılık vardı. Diğer tüm parametrelerde gruplar arasında istatistiksel olarak anlamlı fark saptanmadı.Bu bulgular, inme sonrası yapılan alt ekstremite güçlendirme egzersizleri ile kas gücü ve fonksiyonel değerlendirme parametrelerinde anlamlı iyileşmeler olduğunu göstermektedir. The aim of this randomized single blind placebo controlled study was to investigate the effectiveness of EMG biofeedback (EMG BF) assisted lower extremity exercise program in hemiplegic patients.Thirty-four patients with hemiplegia due to cerebrovascular disease were randomized into two groups. In both groups strengthening exercise program with EMG BF were applied according to the patient?s muscle strength. Patients in Group 1 (n=17) received visual and auditory feedback from the monitor during exercise while placebo treatment was given for patients in Group 2 (n=17). Active range of motion, spasticity, muscle strength, functional level and walking speed were assessed before treatment, after treatment, at 1 month and 3 months.Mean age of the patients was 55.9±13.2 (18-78) years. Mean interval from stroke onset was 133.6±132.2 (10-444) days. Although statistically significant improvement was noted in Modified Ashworth Score (MAS) for all visits in only Group 1, statistically significant improvements were found for range of motion, muscle strength, FIM, Barthel and 10 m walking time for all visits in both groups. Between group analysis showed that improvement of MAS in Group 1 was significantly better for all visits.These findings show that strengthening exercises result in significant improvements in muscle strength and functional assessment parameters after stroke. As we have found no difference between two groups other than MAS, EMG BF assisted exercise program seems not to be superior to routine exercise program but can be suggested for hemiplegic patients with spasticity. 60

Study design Randomized single-blind controlled trial. Objective We aimed to compare the effects of epidural steroid injections and physical therapy program on pain and function in patients with lumbar spinal stenosis (LSS). Summary of background data LSS is one of the most common degenerative spinal disorders among elderly population. Initial treatment of this disabling painful condition is usually conservative including analgesics, nonsteroidal anti-inflammatory drugs, exercise, physical therapy, or epidural steroid injections. Owing to lack of sufficient data concerning the effectiveness of conservative treatment in LSS, we aimed to compare the effectiveness of epidural steroid injections and physical therapy program in a randomized controlled manner. Methods A total of 29 patients diagnosed as LSS were randomized into 3 groups. Group 1 (n = 10) received an inpatient physical therapy program for 2 weeks, group 2 (n = 10) received epidural steroid injections, and group 3 (n = 9) served as the controls. All study patients additionally received diclofenac and a home-based exercise program. The patients were evaluated at baseline, 2 weeks, 1 month, 3 months, and 6 months after treatment by finger floor distance, treadmill walk test, sit-to-stand test, weight carrying test, Roland Morris Disability Index, and Nottingham Health Profile. Results Both epidural steroid and physical therapy groups have demonstrated significant improvement in pain and functional parameters and no significant difference was noted between the 2 treatment groups. Significant improvements were also noted in the control group. Pain and functional assessment scores (RMDI, NHP physical activity subscore) were significantly more improved in group 2 compared with controls at the second week. Conclusion Epidural steroid injections and physical therapy both seem to be effective in LSS patients up to 6 months of follow-up.

Avascular necrosis of the hip is not a specific disease but a final common pathway of various pathological processes leading to vascular impairment of the femoral head. A number of factors including hip fractures, high dose corticosteroid administration and alcohol abuse have been implicated in the process. Clinical features are nonspecific including pain, restricted range of motion and limping. During the early stages plain radiographs show normal findings and pathologic condition can be detected by other techniques, especially Magnetic Resonance Imaging (MRI). Various classification systems have been developed particularly on the basis of radiological progression. Treatment of avascular necrosis is usually surgical, but in the early stages some pharmacological and biophysical methods have shown to be effective in recent studies.
