Background: Over time, the integration of advancing technology into education gained speed due to the pandemic. Alternative education strategies to traditional education emerged. The study aims to examine whether web‐based asynchronous (WBA) education can be used as an alternative to traditional face-to-face (FTF) education if the views of post-graduate students are in line with it. Materials and Methods: The opinions of 19 students who enrolled in the post-graduate biostatistics course in the spring semester of 2019-2020 and participated in both FTF and WBA were consulted. Students were asked 17 questions about the distance education perception scale, educational strategies and 3 open-ended questions. Post-graduate students were grouped according to their answers to the question of which type of education would prefer if the students had to take the biostatistics course once again. Results: Thirteen of the students (68.4%) stated that if they would take this course once again, they would prefer WBA, 5 (26.3%) students FTF and only 1 (5.3%) student prefer online education. While FTF students were more satisfied with FTF training (p=0.014), WBA students stated that they were more satisfied with the WBA education (p=0.004). WBA students thought that WBA education provides more flexibility in terms of time (p=0.046) and resources (p=0.007). WBA students stated this education would be preferred more in the future (p=0.035). Conclusion: WBA education is a very good alternative for courses similar to biostatistics courses where computer programs are frequently used in practice. In case students and lecturers are provided with internet and necessary hardware and software support by the state and universities, the transition to WBA education may be permanent. &nbsp

Pediatric head injuries is one of the most important etiological causes of mortality and morbidity in childhood. The early decision for the prognosis due to the parameters in the emergency department admission can be warning for treatment and close follow-up. The aim of this study is to evaluate the effect of blood glucose level and body temperature on prognosis at the time of admission by using the Modified Rankin Score. 301 cases between the ages of 0-16 who applied to the Bursa Uludağ University Faculty of Medicine Emergency Department were included in this study. A negative correlation was found between the Glasgow Coma Score and the blood glucose level. In addition, a negative correlation was observed between Glasgow Coma Score and Modified Rankin Score, while a weak positive correlation was found between blood glucose level and Modified Rankin Score. It was observed that the hyperthermia detected at the time of admission had no effect on the prognosis. These results showed that, the high blood glucose level may be important in determining the prognosis in addition to the Glasgow Coma Score., Pediatrik kafa travması çocukluk çağının önemli mortalite ve morbidite sebepleri arasındadır. Acil servise başvuru anındaki parametrelere göre prognozun önceden bilinmesi tedavi ve yakın takip için uyarıcı olabilecektir. Bu çalışmada başvuru anındaki kan glukoz değerinin ve vücut sıcaklığının prognoz üzerine etkisinin Modifiye Rankin Skoru ile değerlendirilmesi planlanmıştır. Çalışmada Bursa Uludağ Üniversitesi Tıp Fakültesi Acil Servisi’ne başvuran 0-16 yaş aralığındaki 301 olgu incelenmiştir. Başvuru anındaki Glasgow Koma Skoru ile kan glukoz değeri arasında ters yönde korelasyon saptanmıştır. Ayrıca Glasgow Koma Skoru ile Modifiye Rankin Skoru arasında da ters yönde korelasyon gözlenirken, kan glukoz değeri ile Modifiye Rankin Skoru arasında pozitif yönde zayıf korelasyon saptanmıştır. Başvuru anında saptanan hiperterminin prognoz üzerine etkisinin olmadığı gözlenmiştir. Bu sonuçlar başvuru anındaki Glasgow Koma Skorunun yanı sıra kan glukoz değerinin yüksekliğinin prognoz tayininde önemli olabileceğini göstermiştir.

TNF-α inhibitör (TNFi) tedavisi ile remisyon elde edilmiş, tedaviye ara verilmiş inflamatuar romatolojik hastalık (İRH) tanılı bireylerin seyrini ortaya koymayı amaçladık. Hastalar geriye dönük olarak incelendi. C-reaktif protein (CRP), eritrosit sedimantasyon hızı (ESR) yanı sıra hastalık aktivite ölçütleri göz önünde bulunduruldu. Çalışmamıza 64 İRH (22 Romatoid Artrit (RA), 37 Ankilozan Spondilit (AS), 5 Psöriatik Artrit (PsA)) tanılı hasta dahil edildi. TNFi kesilmesiyle toplam 58 hastada reaktivasyon gelişmişti. Reaktivasyona kadar geçen süre ortalama 6 aydı. Hastaların 28’ inde (%46) ilk 6 ay içinde reaktivasyon görülürken, 47’ sinde (%73) ilk yılda reaktivasyon görülmüştü. TNFi kullanım süreleri ile reaktivasyon arasındaki ilişki incelendiğinde RA hastalarında TNFi kullanım süresi arttıkça reaktivasyona kadar geçen sürenin azalmış olduğu görüldü (p, We aimed to observe the course of individuals with diagnosis of inflammatory rheumatic diseases (IRD) whose remission had been maintained with TNF-α inhibitor (TNFi) therapy, and medication had been suspended. Günümüzde, okullardaki öğretim uygulamalarında, öğrenme ortamını bozan öğrenci davranışlarının önemli bir problem olarak ortaya çıktığı ve öğretimi aksattığı yönündeki öğretmen görüşlerine sıklıkla rastlanmaktadır. Araştırmada, eğitim uygulamalarında ya da okulun sportif aktivite alanlarında öğrencilerle sık sık iletişimde bulunan beden eğitimi öğretmenlerinin, öğrenme ortamını bozan davranışlarla ne sıklıkta karşılaştığı, öğrenme ortamını bozan davranışların nelerden kaynaklandığı konusundaki görüş- lerini ve düzeni bozan davranışlarla karşılaştıklarında hangi davranış yöntemini kullandıklarını öğrenerek, öğretmenlere bu konularda olumlu davranış önerileri sunma amaçlanmıştır. Bu amaçla Bursa Büyükşehir Belediyesi sınırları içinde yer alan 3 belediyede mevcut özel ve devlet okullarından rasgele örnekleme ile seçilen 171 beden eğitimi öğretmeni araştırmaya katıldı. Yapılan anket uygulaması verilerinin istatistiksel analizi SPSS 13.0 istatistik paket programında yapılmıştır. Araştırma sonuçlarına göre, öğrencilerin öğretim ortamını bozan davranış- ları, izin istemeden konuşmak, ders akışını bozacak davranışlarda bulunmak ve aşırı ve rahatsız edici biçimde konuşmak şeklindedir. Bunların nedenlerini ise, aile içi problemler, televizyon ve diğer kitle iletişim araçlarında sergilenen şiddet olaylarının etkisi, ailelerin çocukların eğitimine olan ilgisizliği olarak belirtmişlerdir. Beden eğitimi öğretmenlerimiz, öğrenme ortamını bozan davranışlarla karşılaştıklarında, samimi bir sınıf ortamı oluşturma, bu tür davranış gösterenleri olumlu davranış gösteren öğrenciler ile diyalog kurmasını sağlama, gözlerinin içine bakma şeklinde davranışlarla bu sorunu çözme yolunu benimsemektedirler. bjective: Patients commonly take a combination of prescription drugs and herbal medicines. Often these alternative products have known pharmacological effects which may interact with drugs given perioperatively, resulting in adverse effects (1, 2). These herbal mixtures, which are known as natural herbal products, have been reported in cases as increasing morbidity and mortality. These herbal products must have effect on anesthetic and surgical method since they have that much side effects in daily usage. The purpose of this study is to raise awareness of medical specialists of herbal product usage in preoperative stage. Method: The study was performed on voluntary residents and specialists practicing in the Ankara Numune Education and Research Hospital following the approval received from the Ethics Committee. Metformin is usually well-tolerated and commonly used oral-antidiabetic in type-2 diabetes and pre-diabetes. The most common side effects are gastrointestinal ones. It is proposed that extended-release form of metformin has less gastrointestinal side effects. We aimed to compare the extended-release and standard form metformin in type-2 diabetic and prediabetic patients for gastrointestinal tolerability, weight effect and glycemic control. Seventy-three newly diagnosed type-2 diabetic or prediabetic patients enrolled in the study. Standard form (group I) and extended-release (group II) metformin were started to patients with randomisation. Gastrointestinal symptom rating scale was used at the beginning and in the first month of treatment for evaluation. Gastrointestinal symptom scores were similar in both groups. At least one new symptom were found 20.5% of group I and 26.4% of group II. There was a slight increase in symptoms associated diarrhea in both groups and indigestion in the group II. No patients discontined treatment due to side effects. Weight loss was observed 4.0 and 3.0 kg for prediabetics in the group I and II, 2.5 kg for diabetic patients in the both groups. In diabetic patients, HbA1c reduced 1.28% and 1.24% in the group I and II. In conclusion, both forms are effective and similar in terms of gastrointestinal side effects., Metformin tip 2 diyabetik ve prediyabetik hastalarda yaygın olarak kullanılan genellikle iyi tolere edilen bir oral antidiyabetiktir. En sık görülen yan etkileri gastrointestinal sistem yakınmalarıdır. Uzatılmış salınımlı formunun gastrointestinal sistem yan etkilerinin daha az olduğu öne sürülmektedir. Çalışmamızda metforminin uzatılmış salınımlı ve standart formunun tip 2 diyabetik ve prediyabetik hastalarda kullanımının gastointestinal tolerabilite, kilo ve glisemik kontrol üzerine etkilerini karşılaştırmayı amaçladık. Çalışmaya yeni tanı almış tip 2 diyabetik ve prediyabetik 73 hasta alındı. Hastalara rastgele randomizasyonla standart metformin (grup I) ve uzatılmış salınımlı metformin (grup II) başlandı. Tedavi başlangıcı ve bir ay sonra yapılan değerlendirmede gastrointestinal semptom derecelendirme ölçeği kullanıldı. Her iki gurupta gastrointestinal semptom skorları benzer bulundu. En az bir yeni yakınma oranı grup I'de %20.5 grup II'de %26.4 bulundu. Her iki grupta diyare ve grup II'de ayrıca hazımsızlık ile ilişkili semptomlarda hafif artış mevcuttu. Yan etki nedeniyle tedaviyi bırakan hasta olmadı. Grup I ve grup II'de prediyabetik hastalarda medyan 4.0(min-maks) ve 3.0(min-maks), diyabetik hastalarda ise her iki grupta med-yan 2.5(min-maks) kg kilo kaybı gözlendi. Diyabetik hastalarda HbA1c değerlerinde grup I'de %1.1, grup II'de %1.0 medyan düşüş saptan-dı. Bu sonuçlarla çalışmamızda her iki formun diyabet ve prediyabetiklerde etkin ve gastrointestinal yan etkiler açısından benzer olduğu görülmüştür.

Patients with advanced pancreatic cancer have a dismal prognosis, and their management is still an important problem in medical oncology practice. So,in this study, we aimed to investigate the clinical features and survival outcomes in patients with locally advanced or metastatic pancreatic cancer. A total of 183 pancreatic cancer patients treated between January 1995 and May 2012 at Medical Oncology Department of Uludag University Faculty of Medicine were retrospectively analyzed. Demographics, clinical features, treatments and outcome were detect-ed There were 123 male and 60 female patients. The median age of patients was 60 years (range 33-84). One hundred five patients had metastatic disease at the time of diagnosis, and the most common metastatic site was liver (90%). Seventy-five patients received gemcita-bine+5-fluorouracil, 42 received gemcitabine, 24 received gemcitabine+cisplatin, and 3 received UFT+cisplatin. Eleven (%6) patients had complete response, 16 (%8.7) had partial response and 18(%9.8) had stable disease. Weight gain was observed in 34 of 145 (%18.6) patients. Bu nedenle bu çalışmada metastatik veya lokal ileri pankreas kanseri tanısıyla tedavi edilen hastalarımızın klinik özelliklerini ve tedavi sonuçlarını retrospektif olarak değerlendirmeyi amaçladık. Bu çalışma için, Ocak 1995-Mayıs 2012 tarihleri arasında tedavi edilen ve klinik bilgileri elde edilebilen toplam 183 olgunun dosyasını retrospektif olarak inceledik. Hastaların demografik özelliklerini, almış oldukları tedavileri ve sağkalım sonuçlarını belirledik. Olguların 123'ü erkek, 60'ı kadın olup ortanca yaş 60 (33-84) idi. Tanı anında 105 olgu metastatikti ve en sık metastaz yeri karaciğerdi. Yetmiş beş hasta gemsitabin + 5-Fluorourasil, 42 hasta gemsitabin, 24 hasta gemsita-bin+sisplatin ve 3 hasta UFT+sisplatin ile tedavi edilmişti. The aim of this study is to compare the effect of off-pump CABG technique obtained by eliminating cardiopulmonary bypass and conven-tional techniques of on-pump CABG surgery on development of acute renal injury postoperatively. Forty patients for elective coronary artery bypass operation were involved in the study and divided into two groups equally. First group underwent to off-pump and the second group underwent to on-pump CABG surgery. The peak serum creatinine values of the patients were taken preoperatively and postoperatively. All preoperative and intraoperative values except operation time (p=0.001) and number of distal anostomosis (p=0.02) were found to be identical for both. No statistical difference were observed between two groups in respect to development of acute renal injury postoperatively. In both groups, only grade 1 acute renal injury was developed (10% in group 1 and 25% in group 2) (p=0.204). Left main coronary artery lesion was found in 2 of 7 patients developing acute renal injury. Patients in group 2 is observed to stay longer in ICU (2.40±0.25 days), compared to group 1 (1.45±0.15 days) (p=0.006). This study showed that, there was no significant difference between off-pump and on-pump CABG surgery tecniques in development of acute renal injury postoperatively. It was shown that left main coronary artery lesion was found to be an important factor causing acute renal injury., Çalışan kalpte koroner baypas tekniğiyle pompalı koroner baypas tekniğinin postoperatif akut böbrek hasarı gelişimine etkisini karşılaştırdık. Kliniğimizde elektif koroner baypas yapılmak üzere 40 olgu, eşit iki gruba ayrılarak çalışmaya alındı. Birinci gruba çalışan kalpte koroner baypas (grup 1), ikinci gruba pompalı koroner baypas (grup 2) uygulandı. Preoperatif ve postoperatif en yüksek serum kreatinin değerleri alındı. İki grup arasında preoperatif ve intraoperatif değerler operasyon süresi (p=0.001) ve distal anastomoz sayıları (p=0.02) hariç benzerdi. Postoperatif akut böbrek hasarı gelişimi açısından iki grup arasında istatistiksel fark bulunmadı. İki grupta da sadece evre 1 akut böbrek hasarı gelişti (grup 1'de %10, grup 2'de %25) (p=0.204). Akut böbrek hasarı gelişen 7 hastanın 2'sinde sol ana koroner arter lezyonu saptan-dı. Grup 2'de yoğunbakımda kalış süresi 2.40±0.25 gün, grup 1'de 1.45±0.15 gün olarak bulundu (p=0.006). Postoperatif akut böbrek hasarı gelişimi açısından çalışan kalpte ve pompalı koroner baypas tekniklerinin birbirlerine üstünlük sağlamadığı, sol ana koroner arter lezyonunun akut böbrek hasarı gelişiminde önemli bir etken olduğu görüldü.

Diabetic foot infection is a chronic but a preventable complication of diabetes. Uncontrolled infections cause major socioeconomic problems because they can end up with extremity amputations. In this study we aimed to evaluate the relation between undergoing amputation and patients age, diabetic age, diabetic treatment, presence of micro and macrovascular complications, A1C level, history of diabetic foot and amputation, presence of osteomyelitis, number of causative microorganism and antibiotic treatment. We inspected files of 45 patients retrospectively who was treated for diabetic foot infection with positive bacterial culture between 2008-2011. Data was recorded in previously prepared patient evaluation forms. Relation between amputation and age of diabetes, A1C level, sex, treatment with insulin or oral antidiabetic agents, presence of microvascular complications, history of diabetic foot, history of amputation, number of causative microorganism was not statistically significant. However, relationship between amputation and older age, presence of macrovascular complications (coronary artery disease, peripheral artery disease), presence of osteomyelitis was statistically significant. Older age, presence of macrovascular complications and osteomyelitis seems to be an important risk factor for amputation in patients with diabetic foot ulcer. Diabetic foot ulcers should be promtly treated before the development of osteomyelitis and complicating infections. Considering these parameters in patients with diabetic foot, initiation of prompt and agressive treatment can reduce amputation rates., Diyabetik ayak enfeksiyonu diyabetin kronik fakat önlenebilir komplikasyonlarından biridir. Kontrol altına alınamayan enfeksiyonlar ekstremite amputasyonuyla sonuçlanabildiği için çok önemli sosyoekonomik sorunlara yol açmaktadır. Bu çalışmada diyabetik ayak gelişen diabetes mellituslu hastalarda mikro ve makrovasküler komplikasyonlar, A1C düzeyi, osteomiyelit varlığı, üreyen mikroorganizma sayısı ve verilen antibiyoterapi gibi parametrelerle amputasyona gidiş arasındaki ilişkiyi değerlendirmeyi amaçladık. 2008-2011 tarihleri arasında diyabetik ayak enfeksiyonu nedeniyle tedavi gören ve yara kültürlerinde üreme olan 45 hastanın verileri retrospektif olarak incelendi. Oluşturulan hasta değerlendirme formuna bilgiler kaydedildi. Hastalarda amputasyona gidiş ile diyabet yaşı, A1C düzeyleri, cinsiyet, oral antidiyabetik ilaç ya da insülin kullanımı, mikrovasküler komplikasyonların varlığı, diyabetik ayak öyküsü, amputasyon öyküsü, üreyen mikroorganizma sayısı gibi parametreler arasında anlamlı istatistiksel fark görülmedi ancak ileri hasta yaşı, makrovasküler komplikasyon varlığı ve osteomiyelit varlığı ile anlamlı istatistiksel ilişki tespit edildi. İleri hasta yaşı, makrovasküler komplikasyon ve osteomiyelit varlığı diyabetik ayak ülserli hastalarda amputasyona gidiş için önemli risk faktörleri olarak gözükmektedir. Diyabetiklerde, ayak ülserlerine enfeksiyon eklenmeden ve osteomiyelit gelişmeden erken tanı konulup tedavisi gerçekleştirilmelidir. Klinisyenlerin diyabetik ayaklı hastalarda bu parametreleri değerlendirerek, erken ve agresif tedaviye başlamalarının amputasyon oranlarında azalma sağlayacağı düşünülmektedir.

In this study, we evaluated the efficiency and safety of rituximab (RIT) treatment in patients with rheumatoid arthritis(RA) who were ina-dequate responders to anti tumor necrosis alpha (antiTNFα) agents or patients that can not tolerate antiTNFα agents because of adverse events. Twenty two patients with RA were enrolled to the study. Disease activity score(DAS 28) before and after (3. month) the RIT treat-ment, acute phase reactants, number of antiTNFα agents before RIT therapy and adverse events were analysed. Rheumatoid factor was positive in 18 of the 22 patients. Seventeen of the 22 patients were inadequate responders to antiTNFα agents, 2 patients developed malig-nancy and in 3 patients antiTNFα agents could not be used due to other adverse events. In cross sectional analysis of the 3. month, a signifi-cant decrease was seen in DAS28 from 5,9 to 4,8 (n=15, p, Bu çalışmada, anti tümör nekroz faktör alfa (antiTNFα) tedavilere yeterli yanıt alınamayan veya yan etki gelişen romatoid artrit (RA) olgula-rında rituksimab (RIT) kullanımının etkinlik ve güvenilirliği değerlendirilmiştir. Çalışmaya 22 RA'lı olgu alınmıştır. RIT öncesi ve sonrası (3. ay) hastalık aktivitesi (DAS 28 skoru), akut faz reaktanları ile RIT öncesi kullanılan antiTNFα tedavi sayısı ve RIT tedavisine bağlı yan etkiler incelenmiştir. Yirmiiki olgunun 18'i romatoid faktör pozitif idi. Onyedi hastada antiTNFα tedavi yanıtsızlığı, 2 olguda malignite gelişimi ve 3 olguda antiTNFα tedaviye bağlı diğer yan etkiler nedeniyle RIT uygulandı. Ortalama 3. ayda yapılan kesitsel değerlendirmede DAS 28 skorlarında anlamlı düşme saptandı. Birinci kür RIT tedavisi sonrası başlangıca göre DAS 28 skorlarında 5,8 den 4,8'e (n=15, p=0,007), 2. kür RIT tedavisi sonrası başlangıca göre DAS 28 skorlarında 5,9 dan 4,3'e (n=9, p=0,008) gerileme gözlendi. RIT tedavisi süresince hiçbir olguda ciddi, tedaviyi sonlandırmayı gerektiren bir yan etki gözlenmedi. Sonuç olarak, RA'da RIT deneyimimiz, antiTNFα tedavilere dirençli veya yan etki geliştiren olgularda, RIT'ın etkili ve güvenilir olduğunu göstermektedir.

Çoklu karşılaştırma testleri, varyans analizinin gerçekleştirilmesi sonrasında anlamlı olan grup veya grupları belirlemede kullanılan testlerdir. Literatürde birçok çoklu karşılaştırma testi yer almaktadır. Çalışmamızda yaygın olarak kullanılan ve istatistiksel yazılımlarda yer alan 12 çoklu karşılaştırma testi belirlenmiştir. Bu testlerin temellendiği istatistikler ve dayandığı varsayımlar farklılık göstermektedir. Farklı grup sayıları, homojen varyans yapısı, eşit ve farklı örneklem büyüklükleri için simulasyon çalışması gerçekleştirilmiştir. Testlerin birbiriyle karşılaştırılmasında deneysel hata oranı, karşılaştırma başına hata oranı, istatistiksel güç ve yanlış bulgu oranı dikkate alınmıştır. Simulasyon sonucunda LSD ve Duncan testlerinin hata oranları yüksek bulunurken Schefee testinin düşük bulunmuştur. Buna bağlı olarak LSD ve Duncan testlerinin gücü yüksek iken Scheffe testinin düşük bulunmuştur. Çalışma sonucunda LSD, Duncan ve Scheffe testleri önerilmemektedir. Multiple comparison tests are used after variance analysis to determine the different group or groups. Many multiple comparison tests are available in the literature. In our study we compare twelve multiple comparison tests which are so popular and available in statistical softwares. These tests are different according to statistical theory and assumptions. Simulation is performed according to different number of groups, homogeneity of variance, equal and different sample size. These tests are compared according to experimentwise error rate, comparisonwise error rate, statistical power and false discovery rate. In simulation result, the error rates of LSD and Duncan tests are higher than others. On the other hand the error rates of Scheffe is lower than others. Depending on this result, the power LSD and Duncan tests are higher than others. On the other hand the power of Scheffe is lower than others.
