Epidermolysis bullosa (EB) is an inherited blistering disorder characterized by the fragility of the skin and mucous membranes [1–4]. The disease can be traumatic and lethal especially for newborns and infants however non-lethal variants were reported [5]. Epidermolysis bullosa is categorized in three major groups: EB simplex(EBS), junctional EB (JEB) and dysthrophic EB (DEB) [3, 4, 6] . EB with pyloric atresia (EBPA) is classified as a form of JEB. EBPA; which is inherited in an autosomal recessive manner is characterized by fragile skin and congenital atresia of the gastrointestinal tract (especially pylorus) [7–9]. α6β4 integrin levels are shown to be reduced or absent in EBPA patients. α6β4 of hemidesmosomes attaches the keratinocyte to the basal membrane and is encoded by ITG A6 and ITGB4 genes [8, 10–13] Sequencing analysis shows that ITGB4 accounts for%80, ITGA6 accounts for%5 of EBPA patients [14]. Another binding protein of hemidesmosomes, Plectin also interacts with α6β4. Plectin is encoded by PLEC1 gene and as sequencing analysis shows; accounts for the%15 of EBPA patients [15, 16]. Recently, more than a hundred disease causing mutations for EBPA have been described. [1, 10, 11, 17–23]. In this case study we report an in vitro fertilization (IVF)- preimplantation genetic diagnosis (PGD) cycle with a novel mutation of ITGB4 (c.4505-4508insACTC) to a family with a history of 3 deceased children diagnosed as EBPA. A singleton pregnancy was confirmed at 7-week gestation by ultrasonography. After an uneventful pregnancy period, a healthy baby was born.

Üreme biyolojisinde maternal immün sistemin gelisen fetusu nasıl reddetmedigi gizeminikoruyan bir konudur. Bir seri çalısmanın sonucunda sınırlı sayıda polimorfizmi saptanmıs olanHLA-G antijeninin insan preimplantasyon embriyosunda eksprese olabildigi ve immündüzenleyici rolü ile tolerogenik fonksiyonunun oldugu ortaya koyulmustur. HLA-G molekülününimplantasyonun kontrolünde rol oynadıgı artık bilinmektedir. Bu tez çalısmasında HLA-Ggeninde tanımlanmıs olan 14 bç'lik delesyon polimorfizminin implantasyon basarısı ve gebeliküzerine etkileri degerlendirildi. 14 bç'lik delesyon polimorfizmini tarama yöntemi olarak PCR(Polimerase Chain Reaction) yöntemi kullanıldı. Çalısmaya dahil edilen hastalar üç grup altındatoplandı: Birinci grup (Grup 1); en az üç defa IVF denenmesine ragmen gebe kalamamıskadınlardan (n1=127), ikinci grup (Grup 2); IVF ile gebe kalabilmis kadınlardan (n2=76), Üçüncügrup (Grup 3) ise kontrol grubu olup tıbbi bir destek olmadan gebe kalabilmis fertil kadınlardanolusturuldu (n3=80). hla-g geninde tanımlanmıs olan 14 bç'lik delesyon polimorfizminin her birgrup için dagılımları tespit edilerek degerlendirilmistir. Homozigot yapıda delesyon, heterozigotyapıda delesyon ve hiç delesyon bulunmaması durumlarına ait sıklıklar bakımından gruplararasında oransal olarak farklılıklar gözlenmesine ragmen istatistiksel olarak anlamlı fark ortayakoyulamamıstır (p>0.05). Sadece heterozigot bireylerin her üç grup için dagılımına baktıgımızdaheterozigotluk oranları arasında net bir farklılık belirlenmistir. The series of experiments presented here test the hypothesis thatHLA-G antigen with limited polymorphism may be expressed in the human preimplantationembryos and immun regulation role and tolerogenic function of this gene was obtained. It isknown that HLA-G molecule plays a crucial role in the implantation. In this study the efect of 14bp deletion polymorphism of HLA-G gene on succesful implantation and pregnancy wasexamined. PCR method was preferred to detect this polymorphism. Patients, attending this study,were grouped into three: The first group (Group 1) as no pregnancy although at least three ormore IVF treatment (n1=127), the second group (Group 2) succesful pregnancy after IVFtreatment (n2=76) and the third group, fertil control group (n3=80) was described. Although therewere differences between frequency of homozygote deletion, heterozygote deletion and nondeleted conditions, no statistically significant difference was obtained (p>0.05). If we consideronly the distribution of heterozygote women among each three group, an obvious difference forheterozygote rates was detected. Evli çiftlerin yaklaşık %15'inde infertilite problemi ortaya çıkmaktadır. İnfertilite (kısırlık) vakaları % 30-50 oranında erkeğe bağlı nedenlerden etkilenmektedirler. Erkek infertilitesinin başlıca nedenlerinden biri sperm motilitesindeki bozukluklardır. Sperm motilitesorunu olan erkeklerde sperm hareketliliğini arttırmaya yönelik çeşitli ajanlar tanımlanmıştır. Motilitenin başlatılmasında ve devam ettirilmesinde bu faktörler temel gereksinimdir. cAMP sperm fonksiyonlarının düzenlenmesinde anahtar role sahiptir. Sperm motilitesinin invitro şartlarda uyarılmasının esası da hücre içi cAMP seviyesini arttırarak sperm fonksiyonlarının uyarılmasına dayanmaktadır. Pentoksifilin gibi fosfodiesteraz inhibitörleri laboratuvarda semene eklendiklerinde, hücre içi cAMP düzeyini, glikozisi ve ATP yapımını arttırdığı görülmüştür. Bu maddeler motil sperm oranını yükseltmekte ve aynı zamanda canlı ama motil spermatozoalarda motiliteyi de tetiklemektedir. Dondurulup-çözünmüş örneklerde sperm motilitesini uyardığı da gösterilmiştir. Platelet Aktive Edici Faktör spermde mevcuttur ve endojen içeriğinin motilite ve gebelik oranı ile önemli ve olumlu ilişkisi vardır. PAF'ın eylem mekanizması tam olarak belli olmasa da, normal üreme fonksiyonuna dair önemi barizdir. Endojen PAF normal sperm fonksiyonu için bir biyobelirteç olarak da rol oynayabilir. PAF kullanmanın özel bir avantajı, sperm tarafından üretilen doğal bir madde olmasından ve pentoksifilin gibi diğer bazı hareketliliği uyarıcılardan farklı olarak toksik etkisinin olmamasındandır. Sperm motilitesinin geliştirilmesi yardımla üreme tekniği sonuçlarını etkili biçimde artırabilir. Özellikle total immotil olgularda canlı sperm seçiminde ve testis biyopsi örneklerinde kullanımı fertilizasyon oranlarını iyileştirici yönde etki göstermektedir. Bu projede Pentoksifilin ve Platelet Aktive Edici Faktör (PAF) uygulaması sonrasında Sperm Kromatin Dağılımı (SCD) metodu ile DNA hasarının incelenmesini ve sonuçların kıyaslanmasını amaçladık. Approximately 15 percent of married couples have infertility problems. 30-50% of infertility cases are due to the reasons related to men. One of the main causes of male infertility is the disorder in the sperm motility. A variety of agents have been identified to improve sperm motility's of men who have sperm motility problems. These factors are the basic requirements in the initiation and continuation of motility. cAMP plays a key role in the regulation of sperm function. The basis of the stimulation of sperm motility in vitro conditions is based on the stimulation of sperm functions by increasing intracellular cAMP levels. When phosphodiesterase inhibitors, such as pentoxifylline, are added to semen in the laboratory, they increase intracellular cAMP level, glucose and the production of ATP. These substances increase the rate of motile sperm and at the same time they start lively but nonmotilspermatozoa motility. It has also been observed that it stimulates frozen-dissolved sperm motility. PAF is present in human sperm and endogenous content has an important and positive relationship between motility and the rate of pregnancy. Although the exact mechanism of action of PAF isn't certain, its function about normal reproduction is obvious. Endogenous PAF may also play a role as a biomarker for normal sperm function. One of the special advantage of using PAF is because of the fact that it is a natural substance produced by sperm and it is non-toxic unlike some other motility stimulants such as pentoxifylline. Improving sperm motility can efficiently increase the results of assisted reproduction techniques. In this project, we aimed to analyse the DNA damage with Sperm Chromatin Distribution (SCD) method after the application of Pentoxifylline and Platelet Activating Factor(PAF). 68

Üreme; dünyadaki tüm canlı türlerinin genetik özelliklerini bir sonraki nesillerle aktarmaları ve kendi devamlılıklarını sağlamaları için gerekli vazgeçilmez bir olgudur. İnfertilite giderek yaygınlaşan önemli bir problem olarak pek çok çiftin karşısına çıkmaktadır. İnfertilitenin %50 oranında kadına bağlı problemlerden kaynaklandığı bilinmektedir. Başarılı tedavi olasılığını ve üreme üzerindeki temel çalışma olanaklarını arttıran in vitro fertilizasyon ve yardımcı üreme tekniklerinin gelişmesi ile infertilite alanında önemli yenilikler meydana gelmiştir. Oosit aktivasyonu problemi olan olgularda oosit aktivasyonunu sağlamaya yönelik çeşitli yöntemler geliştirilmiştir. Oosit aktivasyonunda en önemli olay hücre içi Ca^(+2) konsantrasyonundaki geçici artıştır. Ca^(+2) artışı olmaması aktivasyonu önlediği için infertiliteye sebep olabilmektedir. Bu yöntemler ile yapılan çalışmalar da oosit aktivasyonunun uyarılması ile elde edilen gebelikler rapor edilmiştir. Piezoelektrik ve stronsiyum muamelesi bu yöntemler arasında yer almakta olup çalışmamızda da bu iki yöntemin hücre üzerindeki etkilerini ortaya koymak adına ele aldık. Stronsiyum, fertilizasyon sonucu sperm ile uyarımı taklit ederek uzun süreli kalsiyum dalgalanmaları meydana getirmektedir. Çalışmalarda; sperm kaynaklı fosfolipaz C aktive edildiğinde, fosfotidilinositol 4,5-bifosfatı, ikinci mesajcı olan inositol 1, 4,5-trifosfata (InsP3) hidrolize ettiği görülmüştür. InsP3, endoplazmik retikulum üzerindeki, kalsiyum kanalı ihtiva eden InsP3 reseptörleri (InsP3R) ile etkileşime girerek oositin hücre içi depolarından kalsiyum salınımını uyarır ve oosit aktivasyonunu sağlar. Piezoelektrik metodunda, mikroenjeksiyondan sonra `Elektro Hücre Manipulatör` cihazı ile hücrelere belirli düzeyde elektrik verilir. Bu işlem ile oosit aktivasyonu sağlanır. Çalışmamızda oosit aktivasyonunda kullanılan stronsiyum ve piezoelektrik yöntemleri uygulanmış oositler ile herhangi bir uygulanma olmamış kontrol grubu oositlerin geçirimli elektron mikroskobu (TEM) düzeyinde morfolojik yapılarını karşılaştırmayı amaçladık. Reproduction is an indispensable phenomenon that is necessary for all species to ensure their continuity and transfer their genetic characteristics to the next generation. Many couples encounter infertility as an important problem that is becoming more and more common. In our study, we aimed to investigate the effects of piezoelectric and strontium treatment which are two of these methods. It's also known that 50% of infertile couples has female factor infertility. Development of in vitro fertilization and assisted reproductive techniques, which increases the successful treatment possibilities and opportunity to study on basic reproduction projects, causes innovation in infertility issue. The most important step in oocyte activation is temporary increase of intracellular Ca+2 concentrations. If this doesn't occur, activation process arrests and causes infertility. Some methods are identified to increase intracellular Ca+2 concentration in women who has activation problems. Pregnancy have been achieved by using this kind of methods on cases with fertilization failures. Piezoelectric and strontium methods are two of this kind of methods.Strontium simulates the calcium oscillation and fertilization. Studies have shown that when the soluble spermatozoo factor, phospholipase C is activated, it hydrolyses phosphatidylinositol 4,5-biphosphate into the second messenger inositol 1, 4,5-triphosphate (InsP3). InsP3 reacts with InsP3 receptors localized on endoplasmic reticulum which contains calcium channels. It stimulates calcium oscillation from intracellular-storage and causes oocyte activation.In piezoelectric methods, an electric shock given to cell with 'electro cell manipulator' equipment after microinjection and consequently oocyte activation occurs.The aim of our study is to compare ultrastructure of the oocytes the group of treated with strontium and piezoelectric methods and non-treated oocyte control group with transmission electron microscope (TEM). 68

Erkek infertilitesi ve sebebi bilinmeyen infertilite, infertilite olguları arasında en sık görülenleridir.Mikroenjeksiyon yöntemi ile erkek infertilitesinin çözümünde çok önemli bir yol katedilmiş olsa da mikroenjeksiyon sonrası görülen fertilizasyon problemleri üzerinde hala yoğun çalışmalar yapılmaktadır. Mikroenjeksiyon sonrası fertilizasyonun olmayışı büyük ölçüde sperm hücresinin oosit aktivasyonunu sağlayamamasına bağlanmaktadır. Fosfolipaz C Zeta (PLC?); ayırt edici özellikleriyle sperm hücresi kaynaklı bir fosfolipaz olan çözünür 'sperm hücresi faktörü'dür ve oosit aktivasyonunu sağlayan en önemli faktörlerdendir.Fertilizasyon esnasında; sperm hücresi, oosit aktivasyonu için gerekli olan sürekli ve tekrarlayan intrasellüler serbest kalsiyum (Ca+2) salınımını tetikler. Sperm hücresi ekstraktlarındaki Fosfolipaz C Zeta eksikliği ise kalsiyum salınım yeteneğini inhibe eder. Oosit aktivasyon bozuklukları (OAD), Fosfolipaz C Zeta'nın anormal ekspresyonu, lokalizasyonu ve protein yapısına bağlanmıştır. Çalışmamızda; düşük fertilizasyona neden olan sperm örneklerini (globozoospermik, total fertilizasyon başarısızlığı (TFF), nükleer anomali, total immotil) normospermik örneklerle karşılaştırarak, Fosfolipaz C Zeta molekülünün lokalizasyonunu immunfloresan yöntemiyle belirlemeyi ve bu olgularda deoksiribonükleik asit (DNA) dizi analizi yöntemiyle bilinen mutasyonların Fosfolipaz C Zeta'nın 15 ekzonunda taranmasını ve yeni mutasyon ve polimorfizmlerin belirlenmesini amaçladık. Bu çalışma sonucunda düşük fertilizasyon gösteren sperm örneklerinde Fosfolipaz C Zeta lokalizasyonu normospermi grubuna göre baş ve post-akrozomal bölgelerde azaldığı ya da hiç olmadığı belirlendi. DNA dizi analizi ile düşük fertilizasyona neden olan sperm örneklerinde pek çok değişim saptandı. Literatürde hiç yer almamış ve protein farklılaşmasına neden olan bazı değişikliklerin yanı sıra stop kodonuna dönüşen bir nükleotid değişimi de saptandı. Sonuç olarak, Fosfolipaz C Zeta molekülünün düşük fertilizasyon gösteren bireylerdeki fertilizasyonda oosit aktivasyonunda etkili rolünün olduğu ortaya koyulmaktadır. Immunoflourescence Determination and Mutation Analysis of Phospholipase C Zeta In The Sperm Specimen Give Rise to Low FertilizationMale and unexplained infertility are the frequent cases among the infertile patients. Although the solution of male infertility have been solved by microinjection, extensive studies about fertility problems after microinjection are still performing. Fertilization failure after microinjection is highly related with sperm cells which unable to provide oocyte activation. Phospholipase C Zeta (PLC?) is a soluable sperm factor with it's unique features and one of the most important oocyte activating factor.Upon fertilization, the sperm triggers repetitive and persistent changes in the intracellular concentration of free calcium, also referred to as oscillations, that are required for egg activation. PLC? deficiency in the sperm extracts inhibits the ability of calcium oscillation. Oocyte activation deficiencies was interested with abnormal protein structure, expression, and localization of the PLC?. In our study we aimed the determination of immunoflourescence PLC? in the sperm specimens that give rise to low fertility rate (globozoospermia, total fertilization failure, nuclear anomaly, total immotile) by comparing with normospermic specimens and determination of the novel mutations and polymorphisms and also screening of the known mutations at 15 exons via DNA sequencing.As a result of this study, the localization of PLC? in the sperm specimens that give rise to low fertility rate compared with normospermic specimens was determined decrease or absence in the head and post-acrosomal region. Many variations were detected in sperm specimens that give rise to fertility with the anaylsis of deoxyribonucleic acid (DNA) sequencing. As well as some variations which didn't never take place in literature, and caused protein differentiation, a nucleotide variation, which turned into the stop codon, was determined. In conclusion, it was proved that PLC? has a significant role of oocyte activation in whom has shown low fertility individuals. 110
