Abstract

Introduction. The volumetric modulated arc therapy technique (VMAT) is now widely used in radiotherapy. Verification of the dose delivered to the patient is performed prior to the treatment (pre-treatment mode). However, during the therapeutic session, only the patient’s position is verified and monitored. AnEPID’s (electronic portal imaging device) matrices can measure the intensity of radiation passing through the patient, but the calculation of the dose distribution from this measurement is limited due to the lack of reliable algorithms and software. Therefore, it seems promising to develop a method to estimate the dose in the patient’s body based on the measured calibration units (CU) values. Material and methods. The material consists of 53 patients treated for prostate cancer with the VMAT technique. The CU signal is measured during the treatment and its value is then transformed according to the self-developed algorithm into a dose. This delivered dose is then compared with the planned dose in the target. Results. The performed measurements of the CU and preliminary calculations indicate that it is possible to estimate the dose that the patient receives during the therapeutic session. The mean difference between the prescribed and measured dose values is less than 1%, however, there are differences of 17%. Conclusions. The proposed method can be used in clinical practice for actual dose estimation. The uncertainty of the proposed method was estimated at 5%. In the event of differences above 10%, the treatment realization should be verified by additional tests including patient positioning and technical tests of accelerator, such as verification of kV and MV isocenter compatibility. Abstract

The accuracy of radiotherapy is the subject of continuous discussion, and dosimetry methods, particularly in dynamic techniques, are being developed. At the same time, many oncology centers develop quality procedures, including pretreatment and online dose verification and proper patient tracking methods. This work aims to present the possibility of using portal dosimetry in the assessment of radiotherapy repeatability. The analysis was conducted on 74 cases treated with dynamic techniques. Transit dosimetry was made for each collision‐free radiation beam. It allowed the comparison of summary fluence maps, obtained for fractions with the corresponding summary maps from all other treatment fractions. For evaluation of the compatibility in the fluence map pairs (6798), the gamma coefficient was calculated. The results were considered in four groups, depending on the used radiotherapy technique: stereotactic fractionated radiotherapy, breath‐hold, free‐breathing, and conventionally fractionated other cases. The chi2 or Fisher's exact test was made depending on the size of the analyzed set and also Mann–Whitney U‐test was used to compare treatment repeatability of different techniques. The aim was to test whether the null hypothesis of error‐free therapy was met. The patient is treated repeatedly if the P‐value in all the fluence maps sets is higher than the level of 0.01. The best compatibility between treatment fractions was obtained for the stereotactic technique. The technique with breath‐holding gave the lowest percentage of compliance of the analyzed fluence pairs. The results indicate that the repeatability of the treatment is associated with the radiotherapy technique. Treated volume location is also an essential factor found in the evaluation of treatment accuracy. The EPID device is a useful tool in assessing the repeatability of radiotherapy. The proposed method of fluence maps comparison also allows us to assess in which therapeutic session the patient was treated differently from the other fractions. [ABSTRACT FROM AUTHOR]

Abstract

Purpose In this study two different pre-planning methods (2D vs. 3D) were compared in respect to the implant quality as judged by volumetric and dose parameters of the treatment plans. The aim of this work was to evaluate the influence of the imaging modalities used for pre-planning purpose to the treatment plan quality. Material and methods Twenty-four patients treated with HDR multicatheter implants were randomly selected for experiment. All patients underwent breast conserving surgery. Flexible catheters were implanted into the breast through the template. Inter-catheter distance, number of planes and catheters were adjusted, in respect to the size and location of the target. Pre-planning was used to evaluate the implant geometry in respect to the target. Needles number and position were modified if necessary. There were two experimental subgroups consisted of 12 patients each. Different pre-planning procedure was employed in each group. In the first group 2D X-ray imaging system was used. In the second one the 3D pre-planning method based on CT was performed. Treatment plans were evaluated with parameters calculated based on dose-volume histograms (DVHs). Volumetric and dose parameters were used for comparison of the dose distribution between the two experimental subgroups. Results The mean value of target coverage VPTV100 is higher for 3D pre-planning than for 2D (91.7% vs. 86.1%). The dose that covers 90% of the PTV (D90) is also higher for 3D pre-planning than for 2D (4.2 Gy vs. 3.6 Gy). Similar relation can be observed for the values of dose homogeneity index where DHI obtained for 3D pre-planning is 0.60 and 0.53 for 2D. All differences were statistically significant with p < 0.05. Conclusions Analysis presented in this paper showed that 3D pre-planning method improves the geometrical quality of the implant.
