Toxic epidermal necrolysis (TEN) is a potentially life-threatening dermatologic disorder that erythema and exfoliation of the skin involve more than 30% of the body surface and usually drug related. A 68-year-old male patient who was admitted to the emergency department with the complaint of extensive bullous lesions on his skin was followed up in the intensive care unit (ICU) with the diagnosis of TEN. He had been on multiple anti-inflammatory, antibiotic and analgesic treatment for approximately 20 days due to respiratory tract infection and gout. Methylprednisolone 1 g day
-1 was started after the patient's previous treatments were discontinued. The patient was connected to the mechanical ventilator on 11th day due to sepsis and respiratory mucosal involvement. Regression and epithelialisation of skin lesions started after starting cytokine filter treatment on 14th day. The cytokine filter was applied with a renal replacement therapy machine in our patient. Withdrawal of suspected drugs, maintaining an optimal electrolyte balance, sterile care of skin lesions and management in the ICU of specialised centres are essential. Although agents, such as corticosteroids, intravenous immunoglobulins and cyclosporine, are used in the treatment, we think that the use of cytokine filters will contribute to recovery by stopping the cytokine storm in these cases. İşsizlik olgusu hem gelişmiş hem de gelişmekte olan ülkeler açısından önemli bir sorun haline gelmiştir. Ülkeler, işsizliği önlemek ve işsizliğin olumsuz sonuçlarını ortadan kaldırmak için çeşitli politikalar üretmekte ve uygulamaktadırlar. Başlangıçta işsizliğin doğurduğu problemler, çeşitli gelir destekleri ile yani pasif istihdam politikaları ile çözülmeye çalışılmış ancak pasif politikaların yetersiz kaldığı görülmesi üzerine ülkeler, aktif istihdam politikalarına yönelmişlerdir. Avrupa Birliği(AB) giderek artan işsizlik sorununu Avrupa İstihdam Stratejisi ile çözmeye çalışmaktadır. AB ile tam üyelik müzakerelerini sürdüren Türkiye de bu gelişmeleri yakından takip etmekte ve 2004 yılından itibaren çeşitli aktif istihdam politikaları uygulamaktadır. Her ülkenin gelişmişlik düzeyi ve işgücü piyasasının yapısı farklı olduğundan uygulanan istihdam politikaları da ülkeden ülkeye değişmektedir. Bu itibarla, AB ve Türkiye'de işgücü piyasası ve işsizliğin yapısı ile uygulanan istihdam politikaları araştırılmış, Türkiye'de istihdamın korunması, geliştirilmesi ve işsizlikle mücadeleye yardımcı olması ile görevli bulunan Türkiye İş Kurumu (İŞKUR)'nun uyguladığı aktif ve pasif istihdam politikalarının işgücü piyasasına etkileri İŞKUR'un faaliyet raporları ve resmi istatistik bültenleri üzerinden incelenmiştir. Aktif istihdam politikalarını, pasif istihdam politikaları ile birlikte uygulamanın, işgücünün niteliğini ve istihdam edilebilirliğini arttırdığı, bu nedenle işsizlikle mücadelede en önemli araçlardan biri olduğu sonucuna ulaşılmıştır. Unemployment has become a major problem for both developed and developing countries. Countries are producing and implementing various policies to prevent unemployment and to eliminate the negative consequences of unemployment. The problems endangered by unemployment have been initially tried to be solved with various income supports, i.e. passive employment policies, but countries have turned to active employment policies because of the insufficiency of passive policies.The European Union (EU) is trying to solve the growing problem of unemployment with the European Employment Strategy. Continuing its full membership negotiations with the EU, Turkey has been following these developments closely and implementing various active employment policies since 2004. Since the level of development of each country and the structure of the labor market are different, the applied employment policies vary from country to country.In this respect, the structure of the labor market and unemployment, and the applied employment policies have been investigated, the effects of the active and passive employment policies on labor market applied by the Turkish Employment Agency, responsible for protecting, developing and helping the struggle with unemployment in Turkey, have been examined. It has been found that implementing active employment policies together with passive employment policies has increased the quality and employability of the labor thus, is one of the most important tools for fighting against unemployment. 163

- Abstract
Ultrasound guided caudal epidural block (CEB) and transversus abdominis plane block (TAPB) are two techniques which are used for pain management after inguinal hernia surgeries (IHR). CEB is accepted as gold standard for lower abdominal surgeries while TAPB is more popular. It is aimed to compare ultrasound guided CEB and TAPB for postoperative pain scores, additional analgesic requirement and chronic pain development in pediatric bilateral open IHR. Seventy patients aged 1–7 years undergoing bilateral open IHR were included in this prospective, randomized, double-blinded study. Patients were randomized into group T (Bilateral TAPB) and group C (CEB). Postoperative FLACC (Face, Legs, Activity, Cry, Consolability) scores were evaluated for pain density. Additional analgesic requirement, length of hospital stay was also recorded. Chronic pain is evaluated within a subgroup by using "revised Bieri faces pain scale" in the postoperative second month. Postoperative 15th, 30th, 45th min, first, second hour FLACC scores were similar in both groups (P > 0.05). In group C, FLACC scores at postoperative sixth and 24th were significantly higher than group T (1 (0–5) vs 0 (0–2); 1 (0–3) vs 0 (0–2), respectively; P < 0.001). Additional analgesic requirement in the postoperative 24 h was found to be statistically higher in Group C (56.7%) than in Group T (20%) (P < 0.01). Chronic pain development did not differ between the groups (P > 0.05). Length of hospital stay was found to be shorter in patients in Group T compared to patients in Group C [10 (5–14) vs 16 (5–18) hours, respectively; P < 0.01)]. Our results exhibited that TAPB and CEB for bilateral IHR have similar early analgesic efficacy in children. However; CEB patients experienced higher FLACC scores at the postoperative sixth and 24th hours, and the need for additional analgesics was higher and the length of hospital stay was longer comparing to TAPB. Of note, chronic pain was not observed in any of our subgroup patients (n = 21) who are older than four years. For bilateral IHR, US-guided CEB and TAPB have similar analgesic efficacy in the first six hours, postoperatively, However, TAPB appears to be slightly superior than CEB in terms of subacute pain and length of stay which is still not clinically distinctive.
