To summarize the Traditional Chinese Medicine rectal drip therapy and nursing management of an ovarian cancer patient with intestinal obstruction. Nurses had enhanced the maintenance of gastric tube and monitoring on drainage fluid during the gastrointestinal decompression treatment. In addition of intravenous nutrition therapy, the traditional Chinese Medicine rectal drip therapy was carried to relive the nausea and abdominal distension of the patient. Objective To investigate the efficacy of endoscopic ligation combined with traditional Chinese medicine (TCM) syndrome differentiation-based treatment in the secondary prevention of esophageal variceal bleeding (EVB) in patients with liver cirrhosis. Methods A total of 108 EVB patients who were admitted to Department of Liver Cirrhosis, Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from April 2015 to February 2021 and underwent endoscopic ligation were enrolled, among whom 59 patients with TCM treatment were enrolled as Chinese and Western medicine group, and 49 patients without TCM treatment were enrolled as Western medicine group. The two groups were compared in terms of the incidence rate of rebleeding, mortality rate, and the improvement rate of portal hypertensive gastropathy. The t-test was used for comparison of normally distributed continuous data between groups, and the Mann-Whitney U rank sum test was used for comparison of non-normally distributed continuous data between groups; the chi-square test or the Fisher's exact test was used for comparison of categorical data between two groups; a Cox regression analysis was used to evaluate the risk factors for rebleeding. Results Compared with the Western medicine group, the Chinese and Western medicine group had a significantly lower rebleeding rate within 13-24 months after ligation (2% vs 12%, P=0.045), a significantly lower mortality rate of rebleeding (2% vs 12%, P=0.045), and a significantly higher overall response rate of portal hypertensive gastropathy (90% vs 77%, P=0.04). Underlying diseases (mainly including diabetes, hypertension, and heart disease) and Child-Pugh class for liver function were significant risk factors for rebleeding (all P < 0.05). Conclusion Ligation combined with TCM treatment can significantly reduce the incidence rate of delayed rebleeding and the mortality rate of EVB and improve the degree of portal hypertensive gastropathy, which provides a new strategy for ligation combined with TCM treatment to improve the overall response of EVB secondary prevention. The attribute encryption scheme based on file hierarchy was efficient and low in the cloud storage environment, but the access structured itself contains sensitive information, there was the risk of user information leakage and easy to be stolen.Aiming at this problem, this paper proposed a file-level attribute encryption scheme with hidden access structure. The scheme improved the security of the encryption algorithm without affecting the encryption and decryption efficiency, and adopted a twofactor authentication mechanism to achieve a more secure and efficient access control. Endometrial polyps (EP) is a benign local lesion of the endometrium. It is common in women of childbearing age. Observational studies have shown that EP has an adverse effect on fertility, but its pathogenesis is not clear. Diagnosis methods of EP usually include two-dimensional transvaginal ultrasound and three-dimensional transvaginal ultrasound, saline infusion sonohysterography (SIS) and hysterosalpingography (HSG) and hysteroscopy. Hysteroscopy and biopsy is the gold standard for the diagnosis of EP. In most cases, small and asymptomatic polyps can adopt expectant therapy, however, in order to improve the natural conception or pregnancy rate of assisted reproduction, surgical resection of endometrial polyps is still the first choice for the treatment of infertility patients, and the curative effect is remarkable. ObjectiveTo investigate the clinical effect of 24-week tenofovir disoproxil fumarate (TDF) sequential therapy in HBeAg-positive chronic hepatitis B (CHB) patients with suboptimal serological response to 48-week pegylated interferon-α-2a (Peg-IFN-α-2a) monotherapy. MethodsA total of 21 patients who did not achieve HBeAg seroconversion after the 48-week Peg-IFN-α-2a monotherapy at a dose of 180 μg/week were enrolled. Among these patients, 11 were given TDF 300 mg/d in addition, the withdrawal of Peg-IFN-α-2a after 12 weeks, and TDF monotherapy for 12 weeks; 10 were given sequential adefovir dipivoxil (ADV) monotherapy at a dose of 10 mg/d for 24 weeks. The indicators including HBV DNA, liver and renal function, and HBV serological markers were measured at weeks 12 and 24 of treatment, and the clinical outcome was compared between the two groups. The independent samples t-test was used for comparison of normally distributed continuous data between groups and the paired t-test was used for comparison of normally distributed continuous data within one group; the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between groups. The Fisher's exact test was used for comparison of categorical data. ResultsAt weeks 12 and 24 of treatment, there were no significant differences in HBsAg and HBeAg levels between groups (both P>0.05). At week 24 of treatment, there was no significant difference in HBeAg seroconversion rate between the TDF group and the ADV group ［18.2% (2/11) vs 10.0% (1/10), P>0.05］. At weeks 12 and 24 of treatment, the TDF group had significant reductions in HBeAg compared with baseline (1.75±0.59 log10S/CO、1.61±0.70 log10S/CO vs 1.86±0.58 log10S/CO, P=0.017 and 0.027). At week 24 of treatment, the TDF group had a significantly lower alanine aminotransferase (ALT) level than the ADV group ［21.0 (15.0-27.0) U/L vs 33.0 (21.5-63.5) U/L, P=0.046］. There were no significant differences in estimated glomerular filtration rate at weeks 12 and 24 of treatment between the two groups (both P>0.05). ConclusionAs for CHB patients with suboptimal serological response to 48-week Peg-IFN-α-2a monotherapy, addition of TDF for 12 weeks and then TDF monotherapy for 12 weeks helps to achieve greater reductions in serum ALT and HBeAg levels compared with sequential ADV therapy.
