- Abstract
BACKGROUND: The maximum muscle strength is typically used for evaluating the recovery of muscle function after anterior cruciate ligament reconstruction. Recent studies have suggested that neuromuscular function should also be considered, such as rate of force development, which measures the slope of the force time curve at different time intervals under conditions of isometric muscle contraction. OBJECTIVE: To elaborate on the current research status and shortcomings of muscle isometric rate of force development in patients undergoing anterior cruciate ligament reconstruction surgery, and analyze the degree of defects in quadriceps femoris and hamstring isometric rate of force development at different times after surgery; to analyze the effect of isometric rate of force development on postoperative functional performance, thereby providing important information for optimizing postoperative rehabilitation following anterior cruciate ligament reconstruction, reducing secondary injury to patients, and reducing the incidence of knee osteoarthritis. METHODS: Literature retrieval of CNKI, VIP, WanFang and PubMed was performed using “anterior cruciate ligament, rate of force development” as Chinese search terms and “anterior cruciate ligament, rate of force development, rate of torque development” as English search terms. Finally, 69 articles were included according to inclusion and exclusion criteria. RESULTS AND CONCLUSION: Most studies have found defects in bilateral muscle isometric rate of force development in patients undergoing anterior cruciate ligament reconstruction within 6 months. Early isometric rate of force development of the bilateral hamstring muscles (i.e. the slope of the force time curve at any time interval during muscle contraction of 100 ms) showed significant improvement after 6 months. However, long-term defects in early isometric rate of force development of the bilateral quadriceps indicate long-term damage to the neuromuscular function of the quadriceps after surgery. There is limited research on late isometric rate of force development (the slope of the force time curve at any time interval after 100 ms of muscle contraction), and conclusions cannot be drawn. Regarding landing exercises (jumping to the ground, lateral cutting, etc.) and daily activities (walking, running), early isometric rate of force development of the quadriceps is more correlated with isometric peak torque. Abnormal biomechanical changes during exercise are considered an important risk factor for secondary injury and traumatic knee osteoarthritis in patients. Actively improving early isometric rate of force development of the quadriceps may reduce the incidence of secondary injury and traumatic knee osteoarthritis. Currently, there is limited evidence to suggest that wholebody vibration training can improve early isometric rate of force development of the quadriceps femoris in patients undergoing anterior cruciate ligament reconstruction. It is recommended to use neuromuscular electrical stimulation to intervene in the quadriceps and hamstring in the early postoperative stage and implement explosive force and high resistance training in the late postoperative stage, which may improve the isometric rate of force development in patients. Generating sufficient muscle strength in a short period of time is necessary to effectively protect the anterior cruciate ligament, while the relationship between isometric rate of force development in the hamstring muscle and functional performance is still unclear, which may provide information on preventing secondary injury in patients. It is recommended to use the isometric rate of force development as one of the evaluation indicators for guiding rehabilitation and restoring movement. In addition to focusing on improving symmetry and differences from normal individuals, the hamstring to quadriceps strength ratio should also be considered. An appropriate range of ratios can ensure the balance of muscles during rapid muscle exertion, which may reduce the occurrence of secondary injuries. However, the normal range of ratios is not yet clear. Future research should consider the effects of graft type and knee flexion angle on isometric rate of force development, in order to identify neuromuscular dysfunction in patients as much as possible and help them recover better. - Abstract
Objective To investigate the application value of serum amyloid β 1-42 (Aβ1-42) and phosphorylated tau-181 (P-tau-181) in the clinical diagnosis of cognitive dysfunction in cerebral small vessel disease (CSVD). Methods The patients with CSVD admitted to Nanjing Jiangbei Hospital from July 2019 to July 2020 were divided into the cognitive dysfunction group and normal cognitive function group. Aβ1-42 and P-tau-181 level in the serum of CSVD patients were detected by Enzyme linked immunosorbent assay (ELISA) method, and compared between the two groups. The sensitivity and specificity of the above indicators for the diagnosis of CSVD cognitive dysfunction were analyzed by receiver operating characteristic curve. Results Compared with the normal cognitive function group, the levels of Aβ1-42 and P-tau-181 of patients in the cognitive dysfunction group were significantly increased, the differences were statistically significant (P<0. 05). The sensitivity and specificity of Aβ1-42 for the diagnosis of CSVD cognitive dysfunction were 75. 51%, 73. 47%, respectively. The sensitivity and specificity of P-tau-181 for diagnosing cognitive impairment in CSVD were 72. 09% and 65. 45%, respectively. Conclusion Serum Aβ1-42 and P-tau-181 are associated with cognitive dysfunction in CSVD, and have certain predictive value for the diagnose of cognitive dysfunction in CSVD. [ABSTRACT FROM AUTHOR]

Diamond has excellent material properties such as wide band gap (5. 47 eV), high carrier mobility (3 800 cm 2 / (V·s) for holes and 4 500 cm 2 / (V·s) for electrons), high thermal conductivity (22 W·cm - 1·K - 1 ), high critical breakdown field strength ( > 10 MV/ cm), and optimal Baliga’ s figure of merit, which make diamond semiconductor devices ideal choices for the applications in extreme conditions such as high temperature, high frequency, high power, and strong irradiation. With the breakthroughs in diamond crystal growth by CVD techniques and p-type doping of diamond crystal, research on Schottky barrier diode ( SBD) based on boron-doped diamond has been widely carried out. In this review, the working principles of the diamond SBD are introduced in detail. The growth processes of highly doped p-type thick films, ptype films of low doping drift region are investigated, and the conditions for the formation of ohmic contact and Schottky contact between different metals and diamond crystals are studied. Then the preparation processes of transverse, vertical and pseudovertical device structures and their effect on forward, reverse and breakdown characteristics of the SBDs are analyzed. The modulation of the internal electric field of SBDs by device structures like field plate, passivation layer and edge terminal, which strengthens the reverse breakdown voltage of the device, is illustrated. Among them, Raman spectroscopy can be used to detect the vibrational properties of molecules, due to the fast, non-invasive, small sample treatment, and free interference from water in various fields. Nevertheless, incident light is also required to induce the Raman scattering, in order to compensate for the information with infrared spectroscopy. Therefore, the Raman scattering measurement can be widely used in the composition and dynamic process of soil minerals, bacteria colonies, and humic fraction in soil sensing. However, it is still lacking on the fluorescence effect of soil in the sensing of soil nutrients, which can weaken the Raman signals and the information extraction for the quantitative analysis. Raman spectroscopy has been introduced into some novel measurement techniques to improve the signal-to-noise ratio in recent years, including the shifted excitation Raman difference spectroscopy (SERDS), confocal Raman microscope, and surface-enhanced Raman spectroscopy (SERS). Meanwhile, both aliphatic and aromatic compounds are Raman active in the quantitative and qualitative detection of soil organic matter (SOM). Moreover, many phosphorous compounds in soil are also Raman active, leading to low prediction accuracy using visible-near infrared spectroscopy. In this review, the research progress was proposed on the Raman spectroscopy in the rapid detection of soil nutrients, together with the technical means in suppressing soil fluorescence interference to obtain high-resolution Raman signals. In the detection of SOM, the fusion of infrared and Raman spectral data significantly improved the prediction accuracy of 43% in the root mean square error (RMSE). In addition, the SERDS technique was used to detect the SOM of 33 soil samples. The reconstructed Raman peaks of soil minerals and organic materials obtained an excellent prediction accuracy of the SOM, with the determination of coefficient R 2=0.82, and the residual prediction deviation RPD=1.81. Raman spectroscopy was used to detect the water-soluble nitrogen in the soil solution, whereas, the SERS was to enhance the Raman peaks. An excellent correlation was obtained between the concentration of water-soluble nitrogen and the SERS data (R²=0.91). Research showed that Raman spectroscopy can be an effective tool to identify the different phosphate species in soil. Wavelet packet decomposition of Raman spectra was used to predict the phosphorus concentration in the phosphate-mixed soil with the SOM leached, where the accuracy of the regression model reached R²=0.94. Since the measurement area of Raman spectroscopy depended mainly on the spot size of the laser that irradiated on the sample surface, the spatial variability of the soil sample can be difficult to focus on the target substance. The effective Raman signal of soil nutrients can be obtained with high spatial resolution while suppressing the interferences from the background light. The combination of SERDS and micro-Raman technologies can be expected to serve as an in-situ measurement of soil nutrients. Reducing the redundant variables is crucial when applying spectral fusion since interpretability and reproducibility of the prediction model are paramount in the sensor development using Raman spectroscopy. BACKGROUND: Current research generally believes that the abnormal changes of lower limb gait biomechanics in patients with anterior cruciate ligament injury and reconstruction are an important risk factor for the high incidence of knee osteoarthritis in patients. As a kind of quantitative gait analysis, plantar pressure analysis has a very important clinical value for early identification of abnormal changes in gait and timely correction. OBJECTIVE: To review the application of plantar pressure in gait analysis after anterior cruciate ligament injury and reconstruction, to describe the abnormal gait characteristics of current patients from the aspect of plantar pressure, to understand the current research status and deficiencies so as to provide some ideas for the formulation of rehabilitation programs and research design in future research. METHODS: CNKI, VIP, Wanfang and PubMed databases were searched using “anterior cruciate ligament, plantar pressure, gait analysis” as Chinese and English search terms. Finally, 41 articles were included according to the inclusion and exclusion criteria.RESULTS AND CONCLUSION: (1) Plantar pressure can evaluate the gait pattern of patients with anterior cruciate ligament injury and reconstruction, which will provide information about avoiding pain and knee instability during walking. (2) The size and symmetry of foot load and the symmetry of support phase characteristics in patients with anterior cruciate ligament injury were significantly improved more than 1 year after operation. However, the excessive pronation of the affected foot observed after operation may be related to anterior cruciate ligament injury. (3) Therefore, gait analysis based on plantar pressure can help researchers better understand the gait characteristics of patients and guide the progress of postoperative rehabilitation, which may help to reduce the occurrence of secondary injury and knee osteoarthritis.
