【目的】评价3 种烤瓷贴面(PLV)牙体预备的近期临床效果。【方法】根据贴面形态和覆盖牙体范围将PLV 牙体 预备分为I 型、L 型和U 型3 种, 用于着色牙和前牙间隙、缺损等修复, PLV 采用长石质PLV 瓷系统制作, 双重固化树脂粘结 系统粘结, 观察6 ~ 22 个月, 考察3 种牙体预备类型的适应证、最终美学效果、粘接强度及牙本质敏感性等。【结果】35 例203 个瓷贴面修复后形态、颜色满意, 美学效果佳;无脱落, 仅局部崩折2 例, 近期粘接效果满意;备牙时牙本质暴露率达95.16 %, 粘固后牙本质过敏症状不同程度缓解。【结论】合理选择适应证、足够的牙体制备及高强度树脂粘结系统, 可保证3 种牙体预 备类型均能使PLV 获得良好的外形、满意的颜色及可靠的临床近期粘接强度;但牙本质过敏问题应予重视。

【目的】了解磁性固位的下颌种植覆盖义齿的应力状况及其分布规律, 为磁性固位体的临床应用提供生物力学 依据。【方法】采用二维光弹应力分析法, 分析圆柱形种植体在垂直向和侧向加载条件下种植体— 骨界面的应力状况。【结 果】种植体应力主要为均匀分布的压应力, 最大应力位于种植体根尖部, 颈部应力相对较小, 未见明显弯曲应力。【结论】磁 性固位种植义齿的种植体应力分布符合生物力学要求, 有利于种植体的长期成功。

BACKGROUND: Research has demonstrated a close association between ferroptosis and vascular dementia. Tongmai Kaiqiao Pill has a certain effect on improving the cognitive function of vascular dementia patients, but its mechanism is unclear. OBJECTIVE: To explore the interventional effects and molecular mechanisms of Tongmai Kaiqiao Pill for vascular dementia based on the regulation of ferroptosis by the nuclear factor erythroid-2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1)/glutathione peroxidase 4 (GPX4) signaling pathway. METHODS: Among eighty-four SD male rats, 12 rats were used as the sham-operated group, and the rest of them were prepared as a model of vascular dementia by the modified 2-VO method, and then randomly divided into the model group, the Tongmai Kaiqiao Pills high-, moderate-, and low-dosage (27. 6, 13. 8, and 6. 9 g/kg) groups, the combined group (Tongmai Kaiqiao Pill high-dosage+ML385, 20 mg/kg), and the donepezil hydrochloride group (0. 45 mg/kg). The drug was given once a day by intragastric administration. The combined group was also intraperitoneally injected Nrf2 inhibitor ML385, once a day, for 4 weeks. Morris water maze was used to detect the learning memory ability of rats. Hematoxylin-eosin staining was used to observe the histopathological changes in the hippocampus of rats in each group. Colorimetric assay was used to detect the content of reduced glutathione, ferrous ion (Fe2+), and malondialdehyde in the serum of rats. Prussian blue staining was used to detect the iron deposition in the hippocampal tissue of rats. Transmission electron microscopy was used to observe the ultrastructural changes of mitochondria in rat hippocampal tissues. Western blot assay was used to detect the protein expression levels of Nrf2, HO-1, GPX4, XCT, and ferritin heavy chain 1 (FTH1) in rat hippocampal tissues. RESULTS AND CONCLUSION: In comparison to the sham operation, rats in the model group exhibited a significantly prolonged latency period (P<0. 05) and a reduced number of platform crossings (P<0. 05). Additionally, the hippocampal tissues of these rats displayed loosely organized structure, deeply stained cell nuclei, and solidified or lysed chromatin. Ferri ions aggregated in CA1 region. There were atrophied mitochondria with dissolved cristae and thickened mitochondrial membranes. Fe2+, malondialdehyde, and reduced glutathione levels in rat serum were found to be elevated (P<0. 05). A significant reduction in the expression of GPX4, HO-1, XCT, Nrf2, and FTH1 proteins was detected in the hippocampus (P<0. 05). Compared to the model group, the average escape latency of the rats was significantly reduced following intervention with Tongmai Kaiqiao Pills and donepezil hydrochloride (P<0. 05), with an increased number of platform crossings (P<0. 05). Hippocampal neurons showed significant recovery. Notably, iron aggregation in the CA1 region was significantly reduced, and mitochondrial structure and function were improved. There were significant reductions in Fe2+and malondialdehyde levels, while the levels of GPX4, HO-1, XCT, Nrf2, and FTH1 in rat hippocampal tissues, and reduced glutathione in serum were significantly increased (P<0. 05). The high-dose Tongmai Kaiqiao Pills exhibited a treatment effect comparable to that of donepezil hydrochloride (P>0. 05), with a significant prolongation of water maze escape latency (P<0. 05), a reduced number of platform crossings (P<0. 05), and insignificant neuronal pathological changes in the CA1 area. However, the combined group showed increased iron deposition, elevated malondialdehyde and Fe2+levels in blood serum (P<0. 05), reduced glutathione content (P<0. 05), hippocampal tissue mitochondrial atrophy, and reduced expression of Nrf2, XCT, HO-1, GPX4, and FTH1 proteins (P<0. 05). Within a certain range, higher doses of Tongmai Kaiqiao Pills demonstrated a more pronounced effect, comparable to the efficacy of high-dose donepezil hydrochloride. It is concluded that Tongmai Kaiqiao Pills have been shown to mitigate histopathological changes in the rat hippocampus and enhance cognitive function in rats with vascular dementia. The mechanism of action is likely associated with the suppression of ferroptosis through the activation of the Nrf2/HO-1/GPX4 signaling pathway. [ABSTRACT FROM AUTHOR]

14. 中药单体治疗脊髓损伤后神经炎症: 核转录因子 κB 信号通路的作用.

- Abstract
BACKGROUND: Targeted therapy based on nuclear transcription factor kappa B signaling pathway to explore neuroinflammation is increasingly worth exploring, and the advantages of Chinese medicine such as many targets, wide range, rich mechanisms, and few side effects have great potential in the treatment of various diseases. OBJECTIVE: Based on the nuclear transcription factor kappa B signaling pathway, this paper systematically expounded and summarized the research progress of kaempferol, safflower yellow, baicalin, and triptolide in the treatment of neuroinflammation after spinal cord injury. METHODS: Search terms “spinal cord injury, inflammation, anti-inflammatory, traditional Chinese medicine monomer, monomeric compound, NF-κB signaling pathway, flavonoids, glycosides, phenols, esters, alkaloids” were searched in CNKI and PubMed databases. Totally 67 articles were finally included. RESULTS AND CONCLUSION: (1) The role of nuclear transcription factor kappa B signaling pathway in the nervous system is complex and diverse, which can regulate neutrophils, microglia, astrocytes, and macrophages, and mediate the occurrence and development of inflammation after injury. (2) The effects of traditional Chinese medicine monomers such as baicalin on the degradation of nuclear transcription factor kappa B inhibitory protein, the inhibition of phosphorylation process by safflowerin on nuclear transcription factor kappa B signaling pathway, and the inhibition of kaempferol on nuclear transcription factor kappa B signaling pathway p65 nuclear translocation can reduce the impact of inflammatory response on the body, thereby promoting the recovery of neurological function. (3) The nuclear transcription factor kappa B signaling pathway can promote inflammation and immune cell migration and activation in the early stage of injury, and can promote the repair of injury site and the occurrence of fibrosis in the middle and late stages of injury. Appropriate activation of the nuclear transcription factor kappa B signaling pathway can promote the release of inflammatory factors, improve the antioxidant capacity of cells, and promote the activation of immune cells, but the over-activated nuclear transcription factor kappa B signaling pathway can easily lead to the occurrence and continuation of chronic inflammation and the inhibition of apoptosis. (4) Future research can further explore how to accurately regulate the activation level of nuclear transcription factor kappa B signaling pathway, how to achieve precise intervention for nervous system inflammation and injury, and can also focus on the preparation of traditional Chinese medicine monomers and the mechanism of action of traditional Chinese medicine monomers on signaling pathways, in order to provide more effective treatment strategies for the rehabilitation and functional recovery of neurological diseases. 15. 加味春泽汤调控内质网应激凋亡治疗脊髓损伤大鼠的尿潴留.

- Abstract
BACKGROUND: Preliminary clinical observations found that Jiawei Chunze Decoction is an effective formula for clinical treatment of urinary retention after spinal cord injury. Animal experiments have found that the phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) signaling pathway is closely related to the degree of bladder dysfunction. OBJECTIVE: To further investigate the effects of Jiawei Chunze Decoction on bladder function and PI3K/Akt signaling pathway in rats with urinary retention. METHODS: Sixty female Sprague-Dawley rats were randomly divided into sham operation group, model group, Jiawei Chunze Decoction low-dose group, Jiawei Chunze Decoction high-dose group and agonist group. In the sham operation group, the spinal cord was exposed but not transected. In the other groups, the modified Hassan Shaker spinal cord transection method was used to prepare the model of sacral medullary injury. At 24 hours after modeling, the sham operation group and model group were intragastrically given equal volume of normal saline, Jiawei Chunze Decoction low-dose and high-dose groups were given Jiawei Chunze Decoction granules containing 14.4 and 28.8 g/kg, respectively, via intragastric administration for 4 weeks, and the agonist group was treated with an intraperitoneal injection of PI3K/Akt signaling pathway agonist 740Y-P at a dose of 0.02 mg/kg. After 4 weeks of treatment, the maximum bladder capacity, leakage point pressure and bladder compliance of rats in each group were detected by urine flow dynamics. The minimum bladder contraction tension and frequency of rats in each group were detected by detrusor pull test. The pathological changes of the rat bladder in each group were observed by hematoxylin-eosin staining. The concentrations of GRP78, CHOP and Caspase-12 in serum were detected by ELISA, and the mRNA and protein expressions of PI3K, Akt, GRP78, CHOP and Caspase-12 in bladder tissues were detected by RT-PCR and western blot, respectively. RESULTS AND CONCLUSION: Compared with the sham operation group, the maximum bladder volume, bladder compliance and minimum systolic tension of rats in the model group were increased (P < 0.05), and the leakage point pressure and bladder contraction frequency were decreased (P < 0.05); serum GRP78, CHOP, and Caspase-12 levels were also increased (P < 0.05). The arrangement of bladder epithelial cells in the model group was disordered, and there was monocyte infiltration between cells, tissue edema, and detrusor tract atrophy. The mRNA and protein expressions of PI3K and Akt in bladder tissues were significantly decreased in the model group compared with the sham operation group, while those of GRP78, CHOP and Caspase-12 were increased (P < 0.05). Compared with the model group, the maximum bladder volume, bladder compliance and minimum systolic tension of rats were decreased in the Jiawei Chunze Decoction low-dose, high-dose and agonist groups after 4 weeks of intervention (P < 0.05), while the leakage point pressure and bladder contraction frequency were increased (P < 0.05); serum GRP78, CHOP, Caspase-12 levels were decreased (P < 0.05). The bladder epithelial cells in the three intervention groups were distributed evenly, arranged neatly, with less inflammatory cell infiltration and fuller detrusor muscle bundle. Compared with the model group, the mRNA and protein expressions of PI3K and Akt were increased in the three intervention groups, while those of GRP78, CHOP and Caspase-12 were decreased (P < 0.05). The Jiawei Chunze Decoction high-dose group was better than the Jiawei Chunze Decoction low-dose group and shared the similar results with the agonist group. To conclude, Jiawei Chunze Decoction can improve the bladder function of rats with urinary retention after spinal cord injury, and the mechanism may be related to reducing the occurrence of endoplasmic reticulum stress in bladder tissue through the PI3K/Akt signaling pathway, and then alleviating apoptosis. Objective Natural gas pressure differential power generation is a low-carbon energy innovation technology, which has no sewage and waste gas emissions in the power generation process, and is a clean and efficient distributed energy. However, some problems, such as fuzzy operation efficiencies and lack of application effects, need to be solved in the current pressure differential power generation system. Methods A calculation and analysis model for energy efficiency of natural gas pressure differential power generation operation is proposed, which combines the calculation of theoretical formula and simulation model. Considering the deviation from the measured flow in actual operation, the shaft power correction formula is established, the correction coefficient is obtained by means of optimization fitting, and the energy efficiency analysis is carried out by three-box analysis method. Results The exergy was applied to a pressure differential power generation project of a gas distribution station, and combined with typical field operating conditions, the calculation and analysis model was verified. Three types of operating conditions were analyzed, and the net power generation capacity was 1 612.8-2 697.5 kW-h in different working conditions, while the exergy efficiency of system was 43.5%-50.5%, Conclusions The intake air flow of the unit should be maintained in the range of 0.9-1.1 times measured inlet gas flow, and the inlet gas pressure or pressure ratio should be increased in the maximum power range to output more shaft work to drive the generator to generate electricity, so as to ensure the best system efficiency and stable operation. Objectives: Infrequent strong earthquakes which occur on subduction and intraplate faults worldwide can be extremely destructive. The exploration on the reliability and timeliness of high-rate global navigation satellite system(GNSS) continuous stations can help rapid inversion of slip distribution and rapid estimation of earthquake magnitude. Especially for the Anninghe fault with high potential for strong earthquakes, this research has important practical significance for early warning of strong earthquakes in this region. Methods: We adopt a kinematic simulation method to synthesize stochastic rupture scenarios by incorporating the geodetic fault coupling model as prior constraints. For the Anninghe fault, we synthesize 1 100 earthquake events from Mw 6.5 to 7.5 and rupture scenarios, and simulate displacement waveforms by using random noise signals from the existing and newly built GNSS stations. These waveforms are analyzed the reliability of fault slip distribution along the Anninghe fault in combination with the steepest descent method. Results: For these events along the Anninghe fault, the initial inversion magnitude of Mw 6.2 can be determined in about 8 s after the earthquake using the existing GNSS continuous stations. And the magnitude and slip distributions can be obtained within a short time or before the rupture completes. The GNSS continuous stations of the China earthquake science experimental field, which is about to be built, can not only effectively obtain the magnitudes and slip distributions, but also determine the dominant directions of fault rupture. Conclusions: The feasibility and usefulness of the high-rate GNSS data are confirmed in magnitude estimation and finite-fault slip inversion of strong earthquakes along the Anninghe fault. 28. 膝关节镜围手术期踝泵运动防治下肢深静脉血栓形成的疗效研究.

- Abstract
Objective To investigate the preventive effect of ankle pump exercise in the prevention and treatment of deep venous thrombosis of lower extremity during perioperative knee arthroscopy. Methods A total of 100 patients who underwent arthroscopic anterior cruciate ligament reconstruction in sports medicine department of our hospital from January 2021 to December 2022 were selected and divided into the whole course group, the postoperative group, the anticoagulation group and the control group by simple random sampling method, with 25 cases in each group. In the whole course group, patients received ankle pump exercise from admission to discharge. In the postoperative group, patients started ankle pump exercise immediately after awaking from surgical anesthesia until discharge. In the anticoagulation group. hypodermic injection of low molecular weight heparin sodium was given to prevent thrombus until discharge, and ankle pump exercise was not performed. The control group could not exercise ankle pump and prevent thrombus with drugs and the affected limb was moved regularly after surgery. D-dimer and lower limb blood vessel color Doppler ultrasound were performed before and on the 3rd day after operation to detect the occurrence of lower limb thrombosis in the four groups. Results A total of six cases of thrombus occurred in the control group (24.0%). The incidence of thrombus in the postoperative group, the whole course group and the anticoagulation group was 0. The incidence of thrombus in the postoperative group, the whole course group and the anticoagulation group was significantly different from that in the control group (X²=12.471.P<0.001). There was a statistically significant difference in BMI between the hypercoagulable group and the non hypercoagulable group (P< 0.05). The incidence of hypercoagulability in the control group [64.0% (16/25)] was significantly higher than that in the postoperative group [24.0% (6/25)], the whole course group [12.0% (3/25)], and the anticoagulation group [0(0/25)], with statistical significance (P<0.05). Conclusion Regular ankle pump exercise during perioperative operation under knee arthroscopy could effectively prevent deep venous thrombosis of lower extremity. - Abstract
A new intelligent diagnosis method based on a hybrid algorithm of multi-scale permutation entropy and multi-core limit learning machine was proposed to address the complex internal structure of reciprocating compressors, difficulties in extracting bearing clearance fault features, and low recognition accuracy. Firstly, a generalized multi-scale permutation entropy (GMPE) algorithm was proposed to solve the problem that the mean coarse-grained method of multi-scale permutation entropy in the multi-scale process “neutralized” the dynamic mutation behavior of the original signal to a certain extent and reduced the accuracy of entropy analysis. Then, in order to solve the limitations of kernel extreme learning machine in dealing with complex data sample classification, Gaussian kernel function, polynomial kernel function and perceptron kernel function were linearly superimposed to construct a hybrid kernel function, and a multiple kernel extreme learning machine (MKELM) model was proposed. The simulation results show that the fault diagnosis accuracy of the proposed method is as high as 98%, and the intelligent diagnosis of different types of bearing faults is realized efficiently. - Abstract
In order too effectively mitigate losses induced by loess collapse which is normally characterized by suddenness, concealment, and difficulty in capturing precursor information, the investigation into new methods for monitoring of loess collapse holds great engineering and scientific significance. There still remains a lack of a reliable theoretical and / or practical basis for the application of acoustic emission technology in monitoring and early-warning of loess collapse, while this technique has been found successful rock slopes. The single side connected loess slope model tests with water contents of 3%, 7% and 10% were conducted. The tests were complemented by employing active waveguide method for real-time acoustic emission monitoring in order to explore the acoustic emission response characteristics during the process of loess. The destructive form of loess collapse and the time-sequence evolution of acoustic emission counts in the deformation and destruction process were also analyzed. The results indicate that the failure pattern of loess slope model with 3% or 10% water content is dominated by local collapse of small pieces. On the other hand, the failure pattern of loess model with 7% water content is dominated by the overall collapse of large pieces. With deformation, the value of acoustic emission events also increased. As the acoustic emission counts increased rapidly before the overall failure, the initial point can be regarded as a precursor of loess collapse. [ABSTRACT FROM AUTHOR]

- Abstract
Objective To compare the safety and prognosis of drug-coated balloon (DCB) angioplasty and conventional balloon angioplasty in treating symptomatic intracranial atherosclerotic stenosis. Methods The patients with symptomatic severe intracranial atherosclerotic stenosis who were treated at The Affiliated Hospital of Qingdao University from March 2020 to April 2022 were retrospectively analyzed. Patients were divided into the DCB group and the conventional balloon group according to the treatment method. The general clinical data, efficacy, and safety endpoints, including perioperative complications, clinical outcomes, and imaging results during follow-up, were compared between the two groups. Results A total of 49 patients were included in the study, including 30 in the DCB group and 19 in the conventional balloon group. There were no significant differences in baseline data and perioperative complications, as well as stroke recurrence rate, restenosis rate and mortality within 6 months after surgery between the two groups (P>0.05). The DCB group exhibited a lower degree of restenosis at 14.50% (9.55%-23.42%), as compared to 30.00% (15.42%- 37.61%) in the conventional balloon group, with a statistically significant difference (P=0.027). Conclusions In comparison to conventional balloon treatment, DCB treatment is considered safe for symptomatic severe intracranial atherosclerotic stenosis and can effectively reduce the progress of target vessel restenosis. - Abstract
Vascular dementia is a cognitive impairment caused by cerebrovascular disease. Its pathogenesis is derived from a series of pathological processes such as low cerebral blood perfusion caused by cerebrovascular damage. Microglia is an important part of the human innate immune system. Under the stimulation of different microenvironmental signals, microglia can differentiate into M1 type with pro-inflammatory effects and M2 type with anti-inflammatory effects, and produce a variety of cytokines with neurotoxic or neuroprotective effects. In recent years, regulating the polarization of microglia and reducing excessive inflammatory response have become important strategies for the treatment of neurodegenerative diseases. By introducing the main signaling pathways related to microglia polarization and the role of microglia polarization-mediated neuroinflammation in vascular dementia, this paper clarified the important role of microglia polarization balance in the treatment of vascular dementia and provided new ideas for related clinical research.
