The incidence of breast cancer currently ranks first among malignant tumors in women. Breast cancer exhibits high heterogeneity and can be classified into four molecular subtypes: luminal A, luminal B, human epidermal growth factor receptor 2 (HER2) overexpression and triple-negative. However, previous molecular subtype classifications have limited treatment options for patients with HER2 low expression. In recent years, with the rapid development of antibody-drug conjugates (ADCs), new treatment options have emerged for breast cancer patients with HER2 low expression. This has also led to updates in the criteria for determining HER2 expression status in both domestic and international guidelines, based on immunohistochemistry (IHC) and in situ hybridization (ISH) testing, categorizing HER2 expression as HER2-positive (IHC 3+ or IHC 2+/ISH+), HER2 low expression (IHC 1+ or IHC 2+/ISH-), and HER2-negative (IHC 0). ADCs are immunotherapeutics composed of a linker that conjugates a monoclonal antibody with a cytotoxic payload. In the field of breast cancer, several large clinical trials have demonstrated clinical benefits of ADCs targeting HER2, such as trastuzumab emtansine (T-DM1), trastuzumab deruxtecan (T-DXd) and sacituzumab govitecan targeting trophoblast cell surface antigen 2 (TROP2), in various molecular subtypes of breast cancer. With the phase Ⅲ DESTINY-Breast03 trial and others, T-DXd has been found to have superior efficacy compared to T-DM1 in advanced HER2- positive breast cancer patients (approximately two times higher complete response rate, and four times longer median progressionfree survival). T-DXd has now replaced T-DM1 as the recommended second-line therapy for HER2-positive breast cancer and as a second-line treatment option after local treatment for brain metastasis. The phase Ⅲ DESTINY-Breast04 trial confirmed that breast cancer patients with HER2 low expression can also benefit from T-DXd, further reshaping the treatment landscape for advanced breast cancer and supporting the need to redefine molecular subtypes of HER2-negative breast cancer. The phase Ⅲ ASCENT trial demonstrated that sacituzumab govitecan significantly improved survival and quality of life in triple-negative breast cancer (TNBC) patients, and the phase Ⅱ NeoSTAR study suggested its potential as neoadjuvant therapy in TNBC. Based on evidence, T-DM1, T-DXd and sacituzumab govitecan have been approved for marketing in both foreign and Chinese markets. Other ADC drugs, such as HER3- DXd, Dato-DXd and China-developed RC48, are also undergoing extensive clinical trials in the field of breast cancer and other tumors. Furthermore, there are several other ADCs targeting different molecular targets in active development. This article aimed to review the new advances related to ADCs therapy for breast cancer patients with different molecular subtypes and discuss the clinical application value of ADCs in breast cancer. Ectopia of intrauterine device (IUD) is a complication of contraception. The case was reported that a 33-year-old woman who had been placed with an IUD for 6 years and had a sudden acute abdominal pain after sex life. Acute pelvic inflammatory disease was initially considered. After 4 days of anti-infection treatment after admission, abdominal pain was not relieved. The imaging examination indicated the enlargement of pelvic mass, so exploratory laparotomy was performed. The final diagnosis was ectopic IUD, uterine perforation, intestinal perforation, and endometrioid cyst of ovarian. Combined with the literature review on ectopic IUD and gynecological acute abdominal pain, the clinical data of this case was presented in order to improve the attention and understanding of clinicians on acute abdominal pain and to treat timely these patients. In the era of sentinel lymph node biopsy (SLNB), the axillary management concept that 1-2 positive sentinel lymph node (SLN) could safely omit axillary lymph node dissection (ALND) are gradually being accepted by more and more patients. The omission of ALND is safe with no difference in regional recurrence in early breast cancer patients with limited SLN involvement. With the effective support and supplement of systemic therapy and radiotherapy, the management concept of axilla surgery is developing in a de-escalating trend. The combined application of multi-gene tests and ACSOG Z0011/AMAROS criteria could provide patients with a better strategy of dual de-escalation treatment, which includes the de-escalation of both axillary surgery and systemic treatment. In clinical practice, it is necessary to combine the residual tumor burden of regional lymph nodes to formulate the optimal irradiation fields in patients with 1-2 positive SLNs without ALND. In the era of SLNB, we can make full use of the benefits of systemic therapy and radiotherapy to reasonably reduce the scope of surgery and complications, therefore expand the "net benefit" of efficacy and quality of life. Regional lymph node status is one of the important prognostic indicators of breast cancer, which can guide the staging and adjuvant treatment strategy. Sentinel lymph node biopsy (SLNB) for breast cancer has developed rapidly over the past three decades, evidence-based medicine class Ⅰ evidence supports that SLNB is a safe and effective axillary staging technique for patients with clinically negative axillary lymph node in early breast cancer. The risk of axillary recurrence and complications in breast cancer patients with negative/few-positive sentinel lymph node omitting axillary lymph node dissection is very low. Locoregional management of breast cancer should consider the metastatic risk of the primary tumor, the efficacy and side effects of systemic therapy, and the efficacy and side effects of locoregional therapy (surgery/radiotherapy). This view has promoted the continuous expansion of the SLNB population. SLNB with neoadjuvant therapy and internal mammary SLNB will further promote the de)escalation therapy, and the ongoing studies of omission of axillary surgery should be anticipated. The era of precision medicine for regional lymph node management of breast cancer has gradually arrived with the development of SLNB. Early breast cancer can be cured if treated effectively upon diagnosis. Long-term disease-free survival should thus be the treatment goal for patients with early breast cancer. Hormone receptor (HR)-positive early breast cancer was believed to have the best prognosis among all the subtypes. However, as treatments for the human epidermal growth factor receptor 2 (HER2)- positive and triple-negative breast cancer advance, HR-positive breast cancer no longer exhibits an obvious prognostic advantage. At present, postoperative endocrine monotherapy with tamoxifen or aromatase inhibitors is the mainstay of adjuvant treatment for HR-positive early breast cancer. However, some patients are at a high risk of disease recurrence which would greatly affect their long-term survival. To improve the prognosis and quality of life for these patients, clinical research has been focusing on three main directions: ① Treatment escalation: patients at risk are recommended to escalate treatment by extending endocrine therapy or adding ovarian function suppression etc. Recent studies also explored the efficacy and safety of cyclin-dependent kinase 4 and 6 inhibitors as an option for treatment escalation in HR-positive early breast cancer. Accurate identification of high-risk patients suitable for treatment escalation is also a research hotspot where breast cancer index and STEPP analysis have already been recommended by authoritative guidelines to assist in the decision-making of treatment escalation. ② De-escalation or exemption of chemotherapy: to avoid excessive use of chemotherapy is a main trend in the area. Several international guidelines recommend using gene expression profiling tools such as MammaPrint®, Oncotype DX® and EndoPredict® to help predict the necessity of adjuvant chemotherapy. The clinical research and use of these tools are also increasing in China and may help bring more benefits to Chinese patients. ③ Preoperative endocrine therapy: although the beneficial effect of preoperative endocrine therapy remains to be further investigated, a number of studies have suggested that changes in certain biomarkers during preoperative endocrine therapy are closely related to patient prognosis and may play an important role in subsequent treatment decisions (e.g., exemption of chemotherapy, application of cyclin-dependent kinase 4 and 6 inhibitors). Objective To determine the clinical benefits of internal mammary sentinel lymph node biopsy (IM-SLNB) acquired by breast cancer patients with clinically positive axillary lymph node (ALN), and further optimize the IM-SLNB indications. Methods All primary breast cancer patients with clinically positive ALN from February 2014 to September 2017 were prospectively recruited in this study. IM-SLNB was performed under the guidance of the modified injection technique. The success rate and visualization rate of IM-SLNB, metastatic rate of internal mammary sentinel lymph node (IMSLN) and its related factors were analyzed, and the clinical benefits were accessed according to the current guidelines. Results Among 126 patients, all of 94 patients (74.6%) who showed internal mammary drainage successfully underwent IM-SLNB. The incidence of internal mammary artery bleeding and pleural lesion were 4.3%(4/94) and 9.6%(9/94), respectively. The metastatic rate of IMSLN was 38.3% (36/94), which was significantly associated with the number of positive ALN (P<0.001) and tumor size (P=0.024). The lymph node staging of 94 patients who underwent IM-SLNB was more accurate. Among them, 36 cases with positive IMSLN underwent internal mammary radiotherapy (IMRT), while the other 58 cases with negative IMSLN avoided radiotherapy. Conclusions IM-SLNB should be routinely performed in patients with positive ALN. IM-SLNB can provide more accurate staging and guide tailored IMRT to benefit more breast cancer patients.
