As the most common cancer in the world, with the improvement of screening technology, the diagnosis rate of lung cancer is increasing year by year. Surgery is currently the preferred treatment for patients with early lung cancer, but postoperative symptom clusters cause certain difficulties for patients and affect their postoperative quality of life. Therefore, this article aims to review the concept of symptom clusters, symptom management theory, assessment tools, postoperative symptom clusters of lung cancer patients, and intervention strategies, and summarize the problems in existing research, in order to provide reference for clinical practice and nursing research. This article summarizes nursing practice of a patient with decompensated liver cirrhosis (liver depression and spleen deficiency type) who received comprehensive auricular acupoint therapy combined with guided music imagination technology to improve fatigue symptoms. Based on the comprehensive evaluation on primary symptoms of the patient, auricular acupoint massage, scraping, and pressing therapy at the appropriate time, combined with guided imagination techniques, were carried out to alleviate the severity of symptoms such as fatigue, sleep quality, and anxiety of the patient, improve overall quality of life and overall therapeutic effect, and provide clinical basis for clinical nursing. To construct a network-based model of symptom clusters and quality of life in perioperative rectal cancer patients, and to explore inter-symptoms and their association with quality of life, providing a theoretical basis for the development of targeted symptom management strategies. Methods A total of 255 patients with rectal cancer hospitalized in the department of gastrointestinal surgery of Special Medical Center of PLA Rocket Force from November 2021 to November 2023 were enrolled by the convenience sampling method. The general information questionnaire, M.D. Anderson symptom inventory (MDASI), European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) were used to investigate the socio-demographic and clinical data of rectal cancer patients as well as symptom severity and quality of life status at preoperatively(T1), 1 day postoperatively(T2), and 7 days postoperatively(T3), respectively. One-way ANOVA was used to assess the differences in symptoms and various domains of the quality of life in patients at each time point. The network models of symptom clusters and quality of life in the perioperative rectal cancer patients were constructed by a network-based analysis; core symptoms and node associations were identified by calculating node strength centrality and edge weight values. Results Except for the cognitive function domain of life quality (P=0.311), there were significant differences in patients' symptoms and life quality were statistically significant at T1, T2 and T3, respectively (all P<0.05). In the three network models, symptoms were negatively correlated with all the domains of life quality. At stage T1, the strongest correlation was between distress and social function (-0.22); at stage T2, the strongest correlation was between lack of appetite and physical function (-0.34); at stage T3, the strongest correlation was between difficulty remembering and cognitive function (-0.30). The perioperative symptom clusters of rectal cancer patients were mainly manifested as disease behavior symptoms (including shortness of breath, dry mouth, difficulty remembering, numbness), psychoneurotic symptoms (including sad, distress,drowsy, sleepy, pain, fatigue) and gastrointestinal symptoms (including lack of appetite, nausea, vomiting). According to the node strength values, shortness of breath (1.30), dry mouth (1.09), and difficulty remembering (1.03) were the core symptoms, and emotional function (1.16) was the core domain in stage T1; shortness of breath (1.42), sad (1.23), and distress (1.20) were the core symptoms, social function (1.24) was the core domain in stage T2; sad (1.45), lack of appetite (1.28 ), and drowsy (1.26) were core symptoms, while emotional function (1.25) was the core domain in stage T3. The three network models were highly stable, with network connection strength stability coefficients of 0.518, 0.561, and 0.672, respectively. Conclusions Patients with rectal cancer experience a stable cluster of disease symptoms during the perioperative period, which are strongly associated with the quality of life. Crohn's disease is a type of inflammatory bowel disease (IBD). Its symptom cluster can damage patients’ psychical and psychological health. It is required to enhance the symptom cluster management of patients with Crohn's disease. In this paper, the concept of symptom cluster, assessment tools of symptom cluster and intervention of symptom cluster were summarized to enlighten the future research direction and clinical nursing practice, aiming to provide reference for symptom cluster management and nursing of patients with Crohn's disease. In this paper, the category composition, evaluation tools, influencing factors and intervention methods of symptom clusters of patients with gastric cancer were reviewed, and the problems existing in the existing research were summarized, in order to provide reference for clinical practice and nursing research. Objective To investigate the related symptoms, symptom cluster and influencing factors among patients with gynecological cancer during chemotherapy. Methods A total of 430 inpatients with gynecological cancer treated by chemotherapy were investigated by using the General Information Questionnaire and Chinese version of M. D. Anderson Symptom Inventory (MDASI). Results There were many symptoms in gynecological cancer patients during chemotherapy. The top three symptom incidence were vomiting (91. 86%), drowsiness (90. 70%) and fatigue (90. 00%). The top three incidence of symptomatic distress were walking (76. 74%), interpersonal (76. 30%) and emotion (72. 79%). Four symptom groups were extracted by exploratory factor analysis, namely emotional symptom cluster, disease symptom cluster, fatigue symptom cluster and digestive tract symptom cluster. Multivariate stepwise regression analysis showed that metastasis, family monthly income, recurrence, expected hospital stay, number of chemotherapy, chemotherapy regimen, tumor staging and work status were the influencing factors of symptoms. Conclusion There are many symptoms in gynecological cancer patients during chemotherapy. Clinical nurses should start from the point of view of symptom cluster, and provide effective symptoms management measures for patients, to improve their quality of life. Objective: To explore the influencing factors of impaired dignity in patients with colorectal cancer, and analyze its relationship with symptom groups and quality of life. Methods: A total of 110 cases of patients with colorectal cancer who were admitted to our hospital from June 2018 to April 2020 were enrolled. The impaired dignity was assessed by the Patient Dignity Inventory (PDI), and they were divided into impaired group (PDI>50 scores) and unimpaired group (PDI≤ 50 scores). Univariate and logistic regression models were used to analyze the influencing factors of patients' dignity impairment. The M.D.Anderson Symptom Inventory was used to evaluate the symptom scores of the two groups of patients, the main factors were extracted to analyze the changes in the symptom groups, and the 36-item medical outcomes study short-form (SF-36) was used to evaluate and compare the quality of life of the two groups. Results: Among 110 patients, the incidence rate of impaired dignity was 29.09%. The independence, symptom distress, social support, survival distress, peace of mind score and PDI total score of the impaired group were higher than those of the unimpaired group (P<0.05). Logistic multiple regression analysis indicated that the patient's clinical stage was stage IV (OR=2.577, 95%CI: 1.385-4.795), elementary school education and below (OR=2.996, 95%CI: 1.395-6.434), family monthly income <1000 yuan (OR=2.068, 95%CI: 1.316-3.250), tumor metastasis (OR=3.412, 95%CI: 1.498-7.772), disease course ≥ 12 months (OR=3.898, 95%CI: 1.425-10.663) were the influencing factors of impaired dignity in patients with colorectal cancer (P＜0.05). The scores of core symptoms, the degree of disturbance of symptoms to normal life and the total scores of the impaired group were higher than those of the unimpaired group, but the scores of physical function, body pain, body role function, mental health, emotional role function and general health of the impaired group were lower than those of the unimpaired group (P<0.05). Conclusion: Patients with colorectal cancer have a higher rate of impaired dignity, and which is related to many factors. Objective: To explore the changes of symptom cluster in patients with heart failure, and to analyze its influence on the quality of life. Methods: 122 patients with heart failure who were admitted to our hospital from October 2018 to October 2020 were prospectively selected. Memorial Heart Failure Symptom Assessment Scale (MSAS-HF) and Minnesota Heart Failure Quality of Life Scale (MLHFQ) were scored of patients at admission, 3 months and 6 months after discharge, respectively. The correlation between MSAS-HF score and MLHFQ score, as well as the influencing factors of patients' quality of life 6 months after discharge were analyzed. Results: The scores of fatigue, discomfort, mood, congestion, ischemia and dyspnea of patients with heart failure at 3 and 6 months after discharge were lower than those at admission, and the scores of mood symptom cluster at 6 months after discharge were lower than those at 3 months after discharge (P<0.05). The scores and total scores of emotional field, physical field and other fields of patients with heart failure at 3 months and 6 months after discharge were lower than those at admission, and the scores and total scores of emotional field at 6 months after discharge were lower than those at 3 months after discharge (P< 0.05). At 6 months after discharge, the scores and total scores of MSAS-HF all dimensions were positively correlated with the total scores of MLHFQ (P<0.05). Univariate analysis showed that the quality of life at 6 months after discharge was related to the left ventricular ejection fraction (LVEF) at admission, the total score of MSAS-HF at admission, the New York heart Association (NYHA) at admission and the course of disease (P<0.05). Logistic multiple regression analysis suggested: LVEF 5 40% at admission, NYHA IV at admission, and disease course > 5 years were risk factors for 6 months after discharge of patient of life quality (P< 0.05), while MSAS-HF total score < 40 scores at admission was protective factor for 6 months after discharge of patient of life quality (P<0.05). Conclusion: The symptoms of patients with heart failure are reduced at 3 and 6 months after discharge, and the quality of life is improved. Objective To investigate the symptoms of lung cancer patients after three months of surgery and to explore the composition of symptom cluster. Methods Using a cross-sectional survey.130 patients with lung cancer were selected from the thoracic surgery department of a Grade III A hospital in Hengyang City and were follow up at three months after surgery. Chinese Version of the M. D. Anderson Symptom Inventory and the revised lung cancer module were used to evaluate the occurrence of symptoms and the degree of disturbance to patients' life and exploratory factor analysis was used to extract symptom groups. Results Among the 19 symptoms. the higher incidence of symptoms in lung cancer patients 3 months after surgery were fatigue. pain and cough. The higher incidence of symptom disturbance were mood. relationship with others and walking. Exploratory factory analysis revealed 4 symptom cluster. The degree of education. lung cancer stage, lung cancer style and gender were the influence factor of the intensity of symptom clusters with statistical significance (P<0.05). Conclusion Patients with lung cancer experience various symptom clusters, and the symptoms are interrelated to form a symptom group and the severity of the symptom group is affected by a variety of factors. Objective To analyze the longitudinal changes of symptom groups in patients with urinary ureterostomy at different time points, and provide a theoretical basis for clinical medical staff to take prospective intervention measures. Methods Patients who underwent radical total urinary ureterostomy at the Second Affiliated Hospital of Kunming Medical University from December 2018 to June 2019 were selected and the first part of the Chinese version of M.D.Anderson Symptom Inventory (MDASI) was used to assess their symptom clusters at 1 week, 1 month and 3 months after the operation. Results A total of 59 patients completed the effective investigation. At three time points, each symptom group had specific symptoms. After exploratory factor analysis, the patient presented 4 symptom clusters 1 week after surgery, with a cumulative variance contribution rate of 57.75%, including ostomy-related symptom cluster, pain-related symptom cluster, gastrointestinal symptom cluster, and somatic symptom cluster; 1 month after surgery 3 symptom clusters were presented, with a cumulative variance contribution rate of 55.45%, including stoma-related symptom cluster, pain-related symptom cluster, and somatic symptom cluster. Analysis results at 3 months after surgery showed 3 symptom clusters with a cumulative variance contribution rate of 59.24%, including ostomy-related symptomcluster, pain-related symptom cluster, and somatic symptom cluster, among which the most severe symptom cluster was the ostomy-related symptom 1 month after surgery, with a score of 6.27±1.41. Except for the gastrointestinal symptom group, each symptom group has differences at three time nodes (P < 0.05) . Conclusion Nursing staff should keep paying attention to patients with urinary tract ostomy, especially during the period from the patient's discharge to 1 month after surgery. Objective: To understand the symptom cluster and its connection with sense of coherence in patients with digestive tract cancer during chemotherapy. Methods: A total of 212 patients with digestive tract cancer during chemotherapy were surveyed with the M. D. Anderson Symptom Inventory (MDASI) and the Sense of Coherence (SOC) scale in 2 hospitals in Anhui Province. Exploratory factor analysis were used to extract the symptom clusters. Spearman correlation analysis were used to determine the relationships between the symptom clusters and SOC. Two subgroups were classified based on the scores of symptom clusters by using cluster analysis, and two independent samples t-tests were used to compare the differences between the two groups. Results: According to the factor analysis, four symptom clusters were identified, including psychological symptom cluster, gastrointestinal symptom cluster, fatigue-pain symptom cluster and neurotoxic symptom cluster. The cumulative variance contribution rate was 64. 16%. The fatigue-pain symptom cluster was divided into fatigue symptom cluster and pain symptom cluster according to the correlation. Those 5 symptom clusters were negatively correlated with the SOC (r = - 0. 14 - -0 . 57, Ps < 0. 05). Two subgroups were classified based on the cluster analysis, patients in the high-score group n=81) had significantly lower SOC scores (P < 0. 001) than those in low-score group n=131). Conclusion: It suggests that digestive tract cancer patients during chemotherapy could experience several physiology and psychology symptom clusters, which are significantly negatively correlate with the sense of coherence.
