Objective: To explore the mechanism of dendritic cells (DCs), novel regulatory B cells (B10 cells) and Th17/ Treg imbalance in the pathogenesis of patients with chronic obstructive pulmonary disease (COPD) and their correlation with lung function. Methods: According to the "Guidelines for the Diagnosis and Treatment of Chronic Obstructive Pulmonary Disease" a total of 93 COPD patients were prospectively selected from the Ninth People's Hospital of Suzhou from May 2019 to December 2021, and 50 healthy subjects were selected as the control group. The patients were followed up for 1 year to observe the occurrence of acute exacerbation COPD (AECOPD), and divided them into stable COPD group and AECOPD group. The course of disease, modified British Medical Research Society dyspnea index (mMRC) classification, COPD assessment test (CAT) score, BODE index score，6 min walking distance (6MWD), arterial partial pressure of oxygen (PaO2 ), arterial carbon dioxide Partial pressure (PaCO2 ) were compared between the two groups; compared the levels of FEV1, FVC, FEV1/FVC, and the percentage of FEV1 to predicted value (FEV1/Pred) in the three groups with peripheral blood DCs, B10 cells, Th17 cells, Treg cells and Th17/Treg, IL-12, IL-10, IL-17A and TGF-β1 levels. To analyze the correlation between peripheral blood DCs cells, B10 cells and Th17/Treg imbalance and pulmonary function indexes in AECOPD group. Logistic regression analysis of independent risk factors for AECOPD. Results: A total of COPD patients had AECOPD events (40.86%). The course of disease, mMRC grade, CAT score, BODE index score, and PaCO2 in AECOPD group were significantly higher than those in COPD stable group (P<0.05)，6MWD and PaO2 were significantly lower than those in COPD group. The levels of FEV1, FVC, FEV1/FVC and FEV1/Pred in the AECOPD group were significantly lower than those in the stable COPD group and control group (P<0.05); the levels of FEV1, FVC, FEV1/FVC and FEV1/Pred in the stable COPD group were significantly lower than those in control group (P<0.05). DCs, B10 cells and Treg cells in AECOPD group were significantly lower than those in stable COPD group and control group, while Th17 expression level and Th17/Treg were significantly higher than that in stable COPD group and control group (P<0.05). DCs, B10 cells and Treg cells in stable COPD phase were significantly lower than those in control group, while Th17 expression level and Th17/Treg were significantly higher than control group (P<0.05). The expression levels of IL-12, IL-10 and TGF-β1 in the AECOPD group were significantly lower than those in the stable COPD group and control group (P<0.05), while IL-17A was significantly higher than that in the stable COPD group and control group. The expression levels of IL-12, IL-10 and TGF-β1 in patients with stable COPD were significantly lower than control group, while IL-17A was significantly higher than control group (P<0.05). Pearson analysis showed that peripheral blood DCs, B10 cells were positively correlated with FEV1, FVC, FEV1/FVC and FEV1/Pred levels (P<0.05), while Th17/Treg was positively correlated with FEV1, FVC, FEV1/ FVC and FEV1/Pred levels all were negatively correlated (P<0.05). Logistic regression analysis found that mMRC grade and Th17/ Treg were independent risk factors of AECOPD (P<0.05). Conclusion: With the progression of COPD, DCs, B10 cells, Th17 cells, Treg cells and Th17/Treg gradually become unbalanced, resulting in disordered expression levels of pro-inflammatory and anti-inflammatory factors. Peripheral blood DCs and B10 cells were positively correlated with lung function levels, while Th17/Treg were negatively correlated with lung function levels. mMRC grade and Th17/Treg are independent risk factors of AECOPD. Therefore, actively intervening in the imbalanced state of immune function in patients has specific and important clinical significance in reducing the immune damage of lung tissue and promoting the improvement of lung function. Because the bilateral filtering algorithm is complicated and time-consuming, it is diffic ult to use in machine vision, a system based on FPGA is firstly designed for quickly solving bilateral filtering results. The system consists of an input buffer, a calculation module and an output buffer. First, the image acquisition terminal is used to input the image into the calculation module; then the image is re-decomposed by the input buffer, and the output data conforms to the format of the template calculation module; then the data output by the input buffer is calculated according to the template . Finally the data are outputted to the output module, and re-integrate the data are in the format of the image. This method enables bilateral filtering to be realized on the FPGA, and reduces the calculation time of the bilateral filtering, and increases the practicability of the bilateral filtering in mechanical image processing. Objective . To explore the effect on oxidative stress status of lithium treatment in bipolar disorder patients. Methods . This was a case-control study of 61 patients with bipolar disorder (8 manic patients and 53 depressed patients) matched with 49 healthy volunteers from Shanghai Mental Health Centbr. Shanghai Jiao Tong University School of Medicine. Patients with bipolar disorder were treated with lithium carbonate for 6 weeks. The 17 Hamilton Depression Rating Scale (HAMD-17), Young Mania Rating Scale (YMRS), Clinical Global Impression-Severity of Illness (CGI-SI) were used to assess the clinica outcomes at baseline and endpoint. The serum levels of superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GSH-Px) and malondialdehy&e (MDA) were measured at baseline and endpoint. The oxidative stress status of the patients and controls was compared, as well as its change after lithium seatment. Results * In the patients with bipolar mania or bipolar depression, the level of SOD was lower (f=5.403, P=0.000) and the levels of GSH-Px and^MDA were higher (r=8.371, i>=0.000; f=6.063, P=0.000) than those of the normal population, and the level of CAT had no difference in these two groups. There was no difference in the four oxidative stress indicators between the manic state and the depressive state. There were significant differences in plasma GSH-Px and MDA contents after lithium treatment (f=4.352, P=0.000; r=2.720, P=0.009), while there was no significant difference in plasma SOD and CAT levels after lithium treatment. After treatment with lithium, MDA content in bipolar mania and bipolar depression decreased significantly (f=3.072, P=0.018; f=3.532, P=0.005)y and that in the manic state decreased more. There was a significant decrease in GSH-Px level in bipolar depression (P=2.880, P=0.006). Conclusion . Oxidative stress injury exists in the patients with bipolar disorder. Residual volatiles in disposable syringe can be injected into human body, which is considered as a potential health risk. Traditional method like gas chromatography (GC), liquid chromatography (LC) and gas chromatography-mass spectrometry (GC/MS) are relative time consuming for detection. In this study, extractive electrospray -ionization quadrupole time of flight mass spectrometry (EESI-Q TOF MS) was used for rapid analysis of volatiles in disposable syringe. The EESI ion source allows direct sampling of mixture, while the Q TOF promises an accurate measurement of m/z value as a high resolution mass spectrometry. The mass spectra were recorded in the mass range of m/z 20-200 in positive ion mode. To assist identification of compounds, collision induced dissociation (CID) analysis was performed with collision energy of (20±10) eV. Analytes were sampled via a T-tube, and sucked into the source chamber from the bypass through an 80 cm long, 1.5 mm inner-diameter PTEF tube. The PTFE tube was heated to a constant temperature of 100 ℃ to avoid condensation which may lead to compound losses. The curtain gas was set at 4.0 L/min and 200 ℃. Ion source parameters were optimized using cyclohexanone, and the optimal sampling flow rate and capillary voltage were set at 3.5 L/min and 4 000 V, respectively. Various volatiles were detected from 9 syringes in 10 mL with 3 brands. Results showed that volatile compounds are similar in syringes with the same brands, but significantly different in species and content for syringes with different brands. Based on CID analysis, a total of 10 compounds with intensity over 5 000 are identified including dimethylformamide-(m/z 74.060 2), dimethyl sulfoxide (m/z 79.021 1), morpholine (m/z 88.076 5), cyclohexanone (m/z 99.080 2), ethylbenzene (m/z 107.085 9), 4-vinyl-1-cyclohexene (m/z 109.101 6), diethyl urea (m/z 117.102 8), 4-methyl-4-vinyl-1,4-butanolide (m/z 127.075 6), benzothiazole (m/z 136.021 9) and dibutylformamide (m/z 158.154 5). According to previous studies, some of these volatile compounds are toxic and harmful to human body. Detection of syringes in different capacity including 10 mL, 20 mL and 50 mL were also performed. And the result showed that syringes in larger volume would cause more residual volatiles. To estimate the content of residual volatiles, semi-quantitative analysis of cyclohexanone was carried out, and the limit of detection (LOD) is calculated to be 0.13 μg/L. Concentration of cyclohexanone in 10 mL syringe ranges from 0.39 to 0.64 μg/L. Maximum concentration of 2.52 μg/L is detected in 50 mL syringe. Objective To study the effect of oral administration of Jintiange capsule on bone mineral density，bone metabolism and sclerostin in elderly patients with hypertension and osteoporosis. Methods From July 2013 to September 2015，144 elderly patients with hypertension and osteoporosis were randomly divided into treatment group (n = 72) and control group (n = 72).The patients in the treatment group were treated with Jintiange capsule + calcium and the control group received calcium only. Serum levels of osteocalcin (OC)，serum type I collagen C-terminal peptide (s-CTX)，serum bone-derived alkaline phosphatase (BAP)， serum osteogenic alkaline phosphatase (BAP)，serum sclerostin (SCL) and bone mineral density ［lumbar spine anteroposterior， femoral neck，femur trochanter and forearm］ were measured at baseline and 12 months after treatment. Results At baseline， there were no significant differences between the two groups in bone mineral density，bone metabolic index and SCL (P>0.05). After 12 months of treatment，the levels of OC，BAP and BMD of lumbar spine，femoral neck，femur trochanter and forearm increased significantly，and the levels of s-CTX and SCL decreased significantly in both groups (P<0.05)，with the improvements in OC，BAP，s-CTX，SCL and BMD of lumbar spine，femoral neck，forearm and femur significantly better in the Jintiange capsule + calcium group compared with the control group (P<0.05).Conclusion Oral Jintiange Capsule improves bone mineral density in patients with hypertension by influencing bone metabolism and suppressing SCL expression. Objective: To evaluate the early clinical effect of femtosecond laser-assisted exposure Descemet's membrane keratoplasty in the treatment of patients with keratoconus. Methods: A retrospective analysis was performed for17 eyes of 15 patients who were diagnosed as keratoconus and performed femtosecond laser-assisted exposure Descemet's membrane keratoplasty. Before the surgery, average uncorrected visual acuity, best corrected visual acuity, corneal endothelial, central corneal thickness, corneal curvature1(K1), corneal curvature2(K2), corneal astigmatism degree and intraocular pressure was measured. Cornea from donors and receptors were cut by femtosecond laser. The follow-up results of all patient were recorded postoperative one week, the first month, the second months and the third month. Results: To the third month after the surgery, UCVA and BCVA were improved obviously. Central corneal thickness was(493.0±46.80)μm; the average of K1 and K2 were(44.56±4.86)D and(40.22±3.60)D, which were closed to normal levels, and the difference of all the data above was statistically significant when compared with the preoperative data(P<0.001). Corneal astigmatism degree measured by corneal topography reduced to(4.57±3.60) D(P=0.185, P>0.05). The loss rate of corneal endothelial cells was 14.3. The postoperative IOP were normal. Conclusions: The early clinical effect of femtosecond laser-assisted exposure Descemet's membrane keratoplasty in the treatment of patients with keratoconus was obvious, which has the accuracy, security and predictability. OBJECTIVE: To establish rapid identification on biogerm limit of diester-type alkaloids in radix aconiti preparata by near infrared diffuse spectroscopy combined with correlation coefficient method. METHODS: Samples of radix aconitum preparata and radix aconitum were selected to analyze the near-infrared characteristic range of diestertype alkaloids. The content of diester-type alkaloids in each batch was also determined. The correlation coefficient method was used to analyze the correlation coefficient between samples of each batch and standard control of radix aconiti,and the method threshold was determined. RESULTS: The correlation coefficient of fully processed samples were ≤ 25％;yet the correlation coefficient of not fully processed samples were 60％-70％. Samples could be accurately screened when the defined thresholds were set at 40％. CONCLUSIONS: Near-infrared diffuse reflectance spectroscopy combined with correlation coefficient method can be used to determine the limit of diester-type alkaloids in radix aconiti preparata for the quality control of drugs purchased in the hospital.
