In order to evaluate the antibacterial effects of different types of acidifiers in vitro, six common types of complex acidifiers were selected. The bacteriostatic effects of six different types of acidifiers (formic acid type Ⅰ, phosphoric acid type, lactic acid type, fumaric acid type, citric acid type and formic acid type Ⅱ) on four harmful bacteria (Escherichia coli, Salmonella, Staphylococcus aureus and Clostridium perfringens) and four beneficial bacteria (Bifidobacterium longum subsp. Infantis, Clostridium butyricum, Lactic acid bacteria and Lactobacillus acidophilus) were observed by plate culture method. Thus, the formulation of compound acidifier with excellent antibacterial effect in vitro was determined. The results showed that formic acid type Ⅰ, citric acid and fumaric acid had strong inhibitory effects on both harmful and beneficial bacteria. However, lactic acid and phosphoric acid were relatively good, which could inhibit harmful bacteria to a large extent, while still ensuring the survival of a large number of beneficial bacteria. Objective: To explore the mechanism of dendritic cells (DCs), novel regulatory B cells (B10 cells) and Th17/ Treg imbalance in the pathogenesis of patients with chronic obstructive pulmonary disease (COPD) and their correlation with lung function. Methods: According to the "Guidelines for the Diagnosis and Treatment of Chronic Obstructive Pulmonary Disease" a total of 93 COPD patients were prospectively selected from the Ninth People's Hospital of Suzhou from May 2019 to December 2021, and 50 healthy subjects were selected as the control group. The patients were followed up for 1 year to observe the occurrence of acute exacerbation COPD (AECOPD), and divided them into stable COPD group and AECOPD group. The course of disease, modified British Medical Research Society dyspnea index (mMRC) classification, COPD assessment test (CAT) score, BODE index score，6 min walking distance (6MWD), arterial partial pressure of oxygen (PaO2 ), arterial carbon dioxide Partial pressure (PaCO2 ) were compared between the two groups; compared the levels of FEV1, FVC, FEV1/FVC, and the percentage of FEV1 to predicted value (FEV1/Pred) in the three groups with peripheral blood DCs, B10 cells, Th17 cells, Treg cells and Th17/Treg, IL-12, IL-10, IL-17A and TGF-β1 levels. To analyze the correlation between peripheral blood DCs cells, B10 cells and Th17/Treg imbalance and pulmonary function indexes in AECOPD group. Logistic regression analysis of independent risk factors for AECOPD. Results: A total of COPD patients had AECOPD events (40.86%). The course of disease, mMRC grade, CAT score, BODE index score, and PaCO2 in AECOPD group were significantly higher than those in COPD stable group (P<0.05)，6MWD and PaO2 were significantly lower than those in COPD group. The levels of FEV1, FVC, FEV1/FVC and FEV1/Pred in the AECOPD group were significantly lower than those in the stable COPD group and control group (P<0.05); the levels of FEV1, FVC, FEV1/FVC and FEV1/Pred in the stable COPD group were significantly lower than those in control group (P<0.05). DCs, B10 cells and Treg cells in AECOPD group were significantly lower than those in stable COPD group and control group, while Th17 expression level and Th17/Treg were significantly higher than that in stable COPD group and control group (P<0.05). DCs, B10 cells and Treg cells in stable COPD phase were significantly lower than those in control group, while Th17 expression level and Th17/Treg were significantly higher than control group (P<0.05). The expression levels of IL-12, IL-10 and TGF-β1 in the AECOPD group were significantly lower than those in the stable COPD group and control group (P<0.05), while IL-17A was significantly higher than that in the stable COPD group and control group. The expression levels of IL-12, IL-10 and TGF-β1 in patients with stable COPD were significantly lower than control group, while IL-17A was significantly higher than control group (P<0.05). Pearson analysis showed that peripheral blood DCs, B10 cells were positively correlated with FEV1, FVC, FEV1/FVC and FEV1/Pred levels (P<0.05), while Th17/Treg was positively correlated with FEV1, FVC, FEV1/ FVC and FEV1/Pred levels all were negatively correlated (P<0.05). Logistic regression analysis found that mMRC grade and Th17/ Treg were independent risk factors of AECOPD (P<0.05). Conclusion: With the progression of COPD, DCs, B10 cells, Th17 cells, Treg cells and Th17/Treg gradually become unbalanced, resulting in disordered expression levels of pro-inflammatory and anti-inflammatory factors. Peripheral blood DCs and B10 cells were positively correlated with lung function levels, while Th17/Treg were negatively correlated with lung function levels. mMRC grade and Th17/Treg are independent risk factors of AECOPD. Therefore, actively intervening in the imbalanced state of immune function in patients has specific and important clinical significance in reducing the immune damage of lung tissue and promoting the improvement of lung function. Objective To investigate the value of a risk assessment model in predicting venous thromboembolism（VTE） in patients with liver failure after artificial liver support therapy. Methods A retrospective analysis was performed for the clinical data of 124 patients with liver failure who received artificial liver support therapy in Affiliated Drum Tower Hospital of Nanjing University Medical School from March 2019 to December 2021, among whom there were 41 patients with VTE（observation group） and 143 patients without VTE（control group）. Related clinical data were compared between the two groups, and the Caprini risk assessment model was used for scoring and risk classification of the patients in both groups. The t-test was used for comparison of continuous data between two groups; the chi-square test was used for comparison of categorical data between two groups; the Mann-Whitney U rank sum test was used for comparison of ranked data between two groups. The logistic regression analysis was used to investigate the independent risk factors for VTE in patients with liver failure after artificial liver support therapy. The receiver operating characteristic（ROC） curve was used to investigate the value of Caprini score and the multivariate predictive model used alone or in combination in predicting VTE. Results The observation group had a significantly higher Caprini score than the control group（4.39±1.10 vs 3.12±1.04, t=6.805, P<0.001）. There was a significant difference between the two groups in risk classification based on Caprini scale（P<0.05), and the patients with high risk or extremely high risk accounted for a higher proportion among the patients with VTE. The univariate analysis showed that there were significant differences between the two groups in age（t=6.400, P<0.001), catheterization method（χ~2=14.413, P<0.001), number of times of artificial liver support therapy（Z=-4.720, P<0.001), activity（Z=-6.282, P<0.001), infection（χ~2=33.071, P<0.001), D-dimer（t=8.746, P<0.001), 28-day mortality rate（χ~2=5.524, P=0.022）. The multivariate analysis showed that number of times of artificial liver support therapy（X1）（odds ratio [OR]=0.251, 95% confidence interval [CI]: 0.111-0.566, P=0.001), activity（X2）（OR=0.122, 95%CI: 0.056-0.264, P<0.001), D-dimer（X3）（OR=2.921, 95%CI: 1.114-7.662, P=0.029） were independent risk factors for VTE in patients with liver failure after artificial liver support therapy. The equation for individual predicted probability was P=1/[1+e-（7.425-1.384X1-2.103X2+1.072X3）]. The ROC curve analysis showed that Caprini score had an area under the ROC curve of 0.802（95%CI: 0.721-0.882, P<0.001), and the multivariate model had an area under the ROC curve of 0.768（95%CI: 0.685-0.851, P<0.001), while the combination of Caprini score and the multivariate model had an area under the ROC curve of 0.957（95%CI: 0.930-0.984, P<0.001）. Conclusion The Caprini risk assessment model has a high predictive efficiency for the risk of VTE in patients with liver failure after artificial liver support therapy, and its combination with the multivariate predictive model can significantly improve the prediction of VTE. Objective To explore the clinical effect of traditional chinese medical(TCM) treatment and TCM nursing intervention in patients with influenza. Methods The clinical data of 200 cases of influenza patients admitted to the infectious diseases department of the hospital from January 2017 to April 2019 were collected. The patients were randomly divided into two groups according to the number. The control group was given conventional treatment and general nursing care, while the observation group was given traditional Chinese medicine treatment and traditional Chinese medicine nursing intervention. Observing and comparing the clinical efficacy of the two groups of patients, the mean time to reduce fever, the time to improve cold symptoms and the time to cure. Results After treatment and nursing intervention, the effective rate of treatment in the observation group was significantly higher than that in the control group, and the average time for fever relief, time for cold symptom improvement and cure time were significantly better than those in the control group, with statistically significant differences (P <0.05). Conclusion The application of traditional Chinese medicine in the treatment of influenza patients and the implementation of targeted traditional Chinese medicine nursing intervention has a significant effect, which is worthy of clinical promotion and application.
